AIMS: Pulmonary vein isolation (PVI) is a well-established strategy for the treatment of paroxysmal atrial fibrillation (PAF). Despite randomized controlled trials and real-world data showing the promise of pulsed-field ablation (PFA) for this treatment, long-term efficacy and safety data demonstrating single-procedure outcomes off antiarrhythmic drugs remain limited. The aim of the FARA-Freedom Study was to evaluate the long-term efficacy and safety of PFA using the pentaspline catheter for PAF. METHODS AND RESULTS: FARA-Freedom, a prospective, non-randomized, multicentre study, enrolled patients with PAF undergoing de novo PVI with PFA, who were followed for 12 months with weekly transtelephonic monitoring and a 72-h Holter ECG at 6 and 12 months. The primary safety endpoint was a composite of device- or procedure-related serious adverse events out to 7 days post-ablation and PV stenosis or atrioesophageal (AE) fistula out to 12 months. Treatment success is a composite of acute PVI and chronic success, which includes freedom from any documented atrial tachyarrhythmia longer than 30 s, use of antiarrhythmic drugs or cardioversion after a 3-month blanking period, or use of amiodarone or repeat ablation at any time. The study enrolled 179 PAF patients (62 ± 10 years, 39% female) at 13 centres. At the index procedure, all PVs were successfully isolated with the pentaspline PFA catheter. Procedure and left atrial dwell times, with a 20-min waiting period, were 71.9 ± 17.6 and 41.0 ± 13.3 min, respectively. Fluoroscopy time was 11.5 ± 7.4 min. Notably, monitoring compliance was high, with 88.4 and 90.3% with weekly events and 72-h Holter monitors, respectively. Freedom from the composite primary effectiveness endpoint was 66.6%, and 41 patients had atrial tachyarrhythmia recurrence, mostly recurrent atrial fibrillation (31 patients). The composite safety endpoint occurred in two patients (1.1%), one tamponade and one transient ischaemic attack. There was no coronary spasm, PV stenosis, or AE fistula. There were four cases of transient phrenic nerve palsy, but all resolved during the index procedure. CONCLUSION: In this prospective, non-randomized, multicentre study, PVI using a pentaspline PFA catheter was effective in treating PAF patients despite rigourous endpoint definitions and high monitoring compliance and demonstrated favourable safety. BACKGROUND: Heart failure is a common complication after myocardial infarction (MI) and is associated with increased mortality. Whether remote heart failure symptoms assessment after MI can improve risk stratification is unknown. The authors evaluated the association of the 23-item Kansas City Cardiomyopathy Questionnaire (KCCQ) with all-cause mortality after MI. METHODS AND RESULTS: Prospectively collected data from consecutive patients hospitalized for MI at a large tertiary heart center between June 2017 and September 2022 were used. Patients remotely completed the KCCQ 1 month after discharge. A total of 1135 (aged 64±12 years, 26.7% women) of 1721 eligible patients completed the KCCQ. Ranges of KCCQ scores revealed that 30 (2.6%), 114 (10.0%), 274 (24.1%), and 717 (63.2%) had scores <25, 25 to 49, 50 to 74, and ≥75, respectively. During a mean follow-up of 46 months (interquartile range, 29-61), 146 (12.9%) died. In a fully adjusted analysis, KCCQ scores <50 were independently associated with mortality (hazard ratio [HR], 6.05 for KCCQ <25, HR, 2.66 for KCCQ 25-49 versus KCCQ ≥50; both P<0.001). Adding the 30-day KCCQ to clinical risk factors improved risk stratification: change in area under the curve of 2.6 (95% CI, 0.3-5.0), Brier score of -0.6 (95% CI, -1.0 to -0.2), and net reclassification improvement of 0.71 (95% CI, 0.45-1.04). KCCQ items most strongly associated with mortality were walking impairment, leg swelling, and change in symptoms. CONCLUSIONS: Remote evaluation of heart failure symptoms using the KCCQ among patients recently discharged for MI identifies patients at risk for mortality. Whether closer follow-up and targeted therapy can reduce mortality in high-risk patients warrants further study.

BACKGROUND: Clinical parameters linked to a low benefit of ICD implantation and increased mortality risks are needed for an individualized assessment of potential benefits and risks of ICD implantation. METHODS: Analysis of a prospective registry of all patients hospitalized from 2009 to 2019 in a single centre for a first implantation of any type of ICD. RESULTS: A total of 2,681 patients were included in the registry. Until the end of follow-up (38.4 ± 29.1 months), 682 (25.4%) patients died. The one-year mortality in all patients, the one-year CV mortality, the three-year mortality in all patients, and the three-year CV mortality were 7.8%, 5.7%, 20.6%, and 14.8%, respectively. There was a statistically significant difference when the subgroups were compared according to the type of cardiomyopathy. No significant difference was found between primary and secondary prevention and between the types of devices. Male gender, age ≥ 75 years, diabetes mellitus, and atrial fibrillation were associated with a significantly increased mortality risk. CONCLUSION: In an analysis of a long-term follow-up of 2,681 ICD patients, we found no mortality difference between patients with ischemic or non-ischemic cardiomyopathy and in the device type. A higher mortality risk was found in men, patients older than 75 years, diabetics, and those with atrial fibrillation.

Implantace srdečních implantabilních elektronických zařízení (CIED – cardiac implantable electronic device) je považována za menší chirurgický výkon. Přesto popisuje část pacientů v různém časovém odstupu od implantace bolest v oblasti přístroje a pletence ramenního. Ta často omezuje provádění všedních denních činností, snižuje kvalitu života a může být dokonce indikací k extrakci stimulačního systému. V kazuistice uvádíme případ pacientky po implantaci dvoudutinového implantabilního kardioverter-defibrilátoru (ICD) v sekundární prevenci náhlé srdeční smrti, u které se v průběhu několika měsíců od výkonu postupně zvyšovala bolest v oblasti kolem ICD a vyzařovala do ipsilaterální paže. Vyšetření vyloučila infekční, mechanickou i cévní příčinu. Následná fyzioterapie vedla k významnému snížení bolesti a k rychlému ústupu potíží spojených s bolestí. Bolest v oblasti implantovaného CIED a ipsilaterálního ramene a paže je známou komplikací. Po vyloučení jiných příčin lze cílenou fyzioterapií dosáhnout významné a úplné úlevy od bolesti nejen v místě implantovaného přístroje, ale i ve vzdálených segmentech horní končetiny.
Implantation of Cardiac Implantable Electronic Devices (CIED) is considered a minor surgical procedure. In spite of this, a portion of patients with a varying delay from the implantation itself describe pain in the region of the implanted device and the shoulder girdle which negatively affects their daily activities. This negatively affects their quality of life and it can become an indication for device removal. In a case study, we present a case of a patient implanted with a two-chamber implantable cardioverter-defibrillator (ICD) for secondary prevention of sudden cardiac death, who, after several months, developed pain in the area surrounding the ICD pocket, with the pain irradiating to the ipsilateral arm. Tests and examinations excluded infection, mechanical or vascular causes. The following physiotherapy led to a significant reduction in perceived pain, and reduction in difficulties connected with the pain. Pain in the area of the implanted CIED and ipsilateral shoulder and arm is a known complication. After excluding other causes, it is possible to use targeted physiotherapy to significantly or completely relieve pain in the area of device implant, as well as in more remote segments of the arm.

Stimulace srdečního převodního systému (conduction system pacing, CSP) se "vynořila" jako fyziologičtější alternativa stimulace pravé komory a ve vybraných případech se používá i pro srdeční resynchronizační terapii. Stimulace Hisova svazku byla zavedena před více než dvěma desítkami let a v posledních pěti letech se začala používat ve větší míře poté, co se na trhu objevilo instrumentarium usnadňující implantaci elektrod. Stimulace oblasti levého Tawarova raménka je novější strategií, která získává vzhledem k větší cílové oblasti a vynikajícím elektrickým parametrům rychle oblibu. Nicméně stejně jako v případě jakékoli intervence je předpokladem bezpečné a účinné léčby její správné provádění. Cílem tohoto dokumentu je standardizovat způsob provádění daného výkonu a nabídnout jistý rámcový přehled lékařům, kteří by rádi implantovali elektrody pro CSP, případně kteří si přejí zdokonalit svoji techniku implantace.
Conduction system pacing (CSP) has emerged as a more physiological alternative to right ventricular pacing and is also being used in selected cases for cardiac resynchronization therapy. His bundle pacing was first introduced over two decades ago and its use has risen over the last five years with the advent of tools which have facilitated implantation. Left bundle branch area pacing is more recent but its adoption is growing fast due to a wider target area and excellent electrical parameters. Nevertheless, as with any intervention, proper technique is a prerequisite for safe and effective delivery of therapy. This document aims to stan- dardize the procedure and to provide a framework for physicians who wish to start CSP implantation, or who wish to improve their technique.
