Telemedicína v současné době představuje především pasivní transfer dat od pacienta směrem k lékaři, zatímco telerehabilitace je aktivním terapeutickým nástrojem. Telerehabilitace a její aplikace jsou především doménou fyzioterapeutů, ergoterapeutů, klinických logopedů, klinických psychologů a dalších nelékařských profesí. Case modelem telerehabilitace v ČR je model Virtuální ambulance pro distanční terapii v RÚ Kladruby, který zahrnuje všechny základní oblasti rehabilitace (fyzioterapie, ergoterapie, psychologie, logopedie). Využívá v Česku vyvinutých platforem, které splňují základní kyberbezpečnostní nároky na zabezpečenou komunikaci s pacientem.
Currently, Telemedicine mainly represents the passive transfer of data from the patient to the doctor, while telerehabilitation is an active therapeutic tool. Telerehabilitation and its applications are mainly the domain of physiotherapists, occupational therapists, clinical speech therapists, clinical psychologists, and other non-medical professionals. Case model of telerehabilitation in the Czech Republic is the model of the Virtual Ambulance for distance therapy in the Rehabilitation Centre Kladruby, which covers all basic areas of rehabilitation (physiotherapy, occupational therapy, psychology, speech therapy). It uses platforms developed in the Czech Republic that meet the basic cyber security requirements for secure communication with the patient.

Telerehabilitace je v některých zemích běžnou součástí zdravotní péče již několik let. V České republice se do povědomí širší odborné veřejnosti dostala až ve spojitosti s pandemií COVID-19, kdy vznikla akutní potřeba zajistit péči nejen v oblasti klinické logopedie. Prudký rozvoj telerehabilitace v posledních dvou letech s sebou nese také rizika spojená s využíváním různých prostředků, které nesplňují požadavky na kybernetickou bezpečnost a ochranu dat pacienta. Předkládaný text se zabývá vyjasněním užívaných pojmů telehealth, telemedicína, telerehabilitace a jejích forem. Informuje odbornou veřejnost nejen o celosvětové potřebě vypracování Standardů telerehabilitační péče, ale také o legislativních požadavcích na bezpečnost, ochraně dat a technickém vývoji telerehabilitace v České republice a možnostech jejího dalšího využití a směřován
In some countries, telerehabilitation has been a common part of healthcare for several years. In the Czech Republic, telerehabilitation became known to the professional public in connection with the COVID-19 pandemic, when there was also an acute need to provide care in the field of clinical Speech Therapy. In the last two years, the rapid development of telerehabilitation also carries the risks associated with the use of methods that do not meet the requirements for cyber security and general data protection regulations. This article deals with the clarification of the terms “telehealth”, “telemedicine”, “telerehabilitation” and their forms. The article informs clinicians about the global need to develop telerehabilitation guidelines, the legislative requirements for cyber security, patient personal data protection and the technical advances in telerehabilitation in the Czech Republic, as well as future trends.

Východisko: Studie se zaměřuje na pacienty, kteří byli propuštěni z lůžkové rehabilitační péče do domácího prostředí, ale v důsledku nedostupnosti či nemožnosti dopravy na ambulantní rehabilitaci nemají možnost navazující ergoterapie pro horní končetiny (HK). Situace je ještě více akcentovaná v oblasti ambulantní ergoterapie, která je minimálně dostupná ve všech regionech České republiky. Cíl: Cílem studie bylo zhodnotit aplikaci distanční terapie (DT) u pacientů se získaným poškozením mozku se zaměřením na terapii HK a ověřit, zda dojde k udržení či významnému zlepšení jemné motoriky paretické HK. Metody: Studie zahrnuje sedm pacientů v subakutním stadiu zařazených do distanční terapie v domácím prostředí (pět žen, průměrný věk 40 ± 15,66 let, čtyři s hemoragickou cévní mozkovou příhodou, tři levostranné hemiparézy, sedm dominantních pravých HK), kteří dokončili terapii pomocí senzoru Leap motion a softwaru Rehamza. Pacienti byli hodnoceni na začátku (T1) a po ukončení DT (T2) funkčními testy pro HK Motor Assessment Scale (MAS), testy koordinace a dexterity 9 Hole Peg Test (9HPT) a testem Box and Block (BBT), Motricity indexem (MI) pro svalovou sílu HK, modifikovanou Ashworthovou škálou (mAS) pro svalový tonus. Soběstačnost byla hodnocena indexem Barthelové (BI) a byla monitorována i změna kvality života první měsíc po propuštění z hospitalizace dotazníkem kvality života Short Form-36. Výsledky: Výsledky motorických testů ukazují, že došlo ke statisticky významnému zlepšení jemné motoriky, koordinace a motorického tempa dle 9HPT (p = 0,05) a ke statisticky významnému zlepšení funkce HK dle MAS (p = 0,05). V testu BBT bylo pozorováno klinické zlepšení v průměru o 4,28 kostek. U všech sedmi pacientů došlo ke zvýšení svalové síly ruky na plný počet (33 b.) dle MI. Adherence všech pacientů k distanční ergoterapii byla 100 %. Závěr: Výsledky studie ukazují, že pacienti s lehčím motorickým deficitem na HK po získaném poškození mozku mohou pozitivně profitovat z distanční ergoterapie.
Background: The study focuses on patients who were discharged from inpatient rehabilitation care to their home environment, but due to the unavailability or impossibility of transport to outpatient rehabilitation, the patients did not have the possibility of follow-up occupational therapy for the upper limbs. The situation is even more strongly emphasised in the field of outpatient occupational therapy, which is minimally available in all regions of the Czech Republic. Aim: The aim of the study was to evaluate the application of distance therapy in patients with acquired brain injury with the therapy focusing on the upper limbs and to verify whether the fine motor skills of the paretic upper limb are maintained or significantly improved after distance occupational therapy. Methods: The study included seven subacute patients enrolled in distance therapy at home (five females, mean age 40 ± 15.66 years, four haemorrhagic stroke, three left-sided hemiparesis, seven right handedness) who completed therapy with Leap motion sensor and Rehamza software. The patients were evaluated at baseline (T1) and at the end of distance therapy (T2) by the Motor Assessment Scale (MAS) functional test, the 9 Hole Peg Test (9HPT) and Box and Block test (BBT) coordination and dexterity tests, the Motricity Index (MI) for the upper extremities for muscle strength, the modified Ashworth scale (mAS) for muscle tone, self-sufficiency was assessed by the Barthel Index (BI) and the change in the quality of life was monitored with the SF-36 questionnaire in the first month after discharge from hospitalisation. Results: The results of the motor tests show that there was a statistically significant improvement in fine motor skills, coordination and motor pace according to 9HPT (p = 0.05) and a statistically significant improvement in the upper limb function according to MAS (p = 0.05). A clinical improvement of 4.28 cubes in average was observed in BBT. In all seven patients, the muscle strength of the hand increased according to MI to the full score (33 b.) The adherence of seven patients to distance occupational therapy was 100%. Conclusion: The results of the study show that patients with a mild motor deficit on the upper limb after acquired brain damage can positively benefit from distance occupational therapy.

Základními cíli fyzioterapie u kriticky nemocných s onemocněním SARS-CoV-2 je snaha o zlepšení klinického stavu pacienta, zkrácení délky jeho pobytu na jednotce intenzivní péče a zabránění dlouhodobému nebo trvalému poškození, s co nejrychlejším návratem do běžného života. V akutní fázi onemocnění k tomu využíváme pasivních a asistovaných technik. Po stabilizaci kardiopulmonálních funkcí terapeutické spektrum rozšiřujeme o cílenou respirační terapii, včasnou mobilizaci a o individuálně zacílený rehabilitační program. Rehabilitace těchto pacientů by měla dále pokračovat i v domácím prostředí, a to i s využitím nových technologií distanční terapie a telerehabilitace.
The basic goals of physiotherapy in critically ill patients with SARS-CoV-2 infection are to improve the patients' clinical condition, shorten their stay in the intensive care unit and prevent long-term or permanent damage, with an early return to normal life. In the acute phase of the disease, we use passive and assistive techniques. After stabilization of cardiopulmonary functions, therapeutic spectrum can be expanded with targeted respiratory therapy, mobilization and individually tailored reconditioning training. Physiotherapy should continue in the home environment, using telerehabilitation and distance therapy technologies.
Pro zajištění následné péče o pacienty se získaným poškozením mozku je důležité garantovat specializovanou komplexní rehabilitační péči. Kromě fyzioterapie a ergoterapie je nutné zajistit také logopedickou, dysfagickou a psychologickou péči. Rehabilitační ústav Kladruby (RÚ) spustil pilotní komplexní neurorehabilitační program - kranioprogram, s garancí dostupnosti logopedické a psychologické péče na základě indikačních kritérií, v roce 2015. Na základě zkušenosti s neodhalenými tichými aspiracemi z primární péče byl také zaveden sekundární dysfagický screening (n = 124), který pomáhá identifikovat rizikové pacienty (n = 27). Na základě těchto zkušeností byl sestaven dysfagiologický tým. V současné době je kranioprogram v RÚ plně hrazený zdravotními pojišťovnami a cílem je rozšíření tohoto programu i do dalších rehabilitačních zařízení.
Specialized complex neurorehabilitation care is important part of the complex neurology care for survivors with acquired brain injury. In addition to physiotherapy and occupational therapy, it is necessary to provide a speech pathology and dysphagia care and as well psychological care. The Rehabilitation Centre Kladruby (RÚ) started a pilot complex neurorehabilitation program - Kranioprogram in 2015, with a guarantee of availability of speech and psychological care, based on the indicative criteria. A secondary dysphagia screening (n = 124) has also been introduced to help identify risk in patients with silent aspiration (n = 27) based on the experience of undetected silent aspiration. Based on these experiences, a dysphagia team was assembled. Currently, the Kranioprogram in the RÚ is fully funded by health insurance companies, with the aim of extending this programme to other rehabilitation facilities.

Úvod: Výkonová indukční stimulace je metodou volby v léčbě algických stavů muskuloskeletálního aparátu v oblasti neurologie, ortopedie, rehabilitace a fyzikálního lékařství. Princip terapie spočívá v neinvazivním průniku a působení nestacionárního elektromagnetického pole na nervosvalovou tkáň, v níž indukuje elektrický proud. Působením pole na nervosvalovou tkáň se ovlivňuje neurofyziologický akční potenciál, vedoucí ke svalové kontrakci. Cíl: Cílem pilotní studie bylo ověřit bezprostřední analgetický účinek výkonové indukční stimulace přístroje BTL-6000 Super Inductive System (BTL Industries Ltd.) u algických stavů muskuloskeletálního aparátu. Metody: Pilotní studie se zúčastnilo 31 pacientů z Rehabilitačního ústavu Kladruby, kteří jednotlivě absolvovali v průměru 7 terapií. V terapii byl ruční aplikátor typu „focus field“. Pro dosažení analgetického účinku byly aplikovány terapeutické parametry opakovací frekvence odpovídající vrátkové a kódové teorii tlumení bolesti. Pro objektivizaci hodnocení bolesti byla použita 10stupňová Visual Analog Scale (VAS), na které pacienti zaznamenali bolest před a po skončení každé terapie. Výsledky: Bezprostředně po terapii nastal analgetický účinek u 62 % ošetřených pacientů. Závěr: I při malém počtu probandů se podařilo zlepšit subjektivní vnímání bolesti díky bezprostřednímu analgetickému účinku výkonové indukční stimulace. I když design experimentu nedovoluje porovnání efektu terapie vůči placebu, lze na základě provedených analýz očekávat (α = 0,2), že bezprostředně po terapii nastane analgetický účinek u 50–74 % pacientů.
Background: Repetitive peripheral inductive stimulation is a solution in musculoskeletal pain management in medical branches such as neurology, orthopaedics, rehabilitation and physical medicine. Therapy is based on the principle of time-varying electromagnetic field passing through neural and muscular tissue, in which electric currents are induced. By affecting conductive tissue, a chain reaction including changes in action potential and leading to muscle contraction is performed. Aim: Aim of the pilot study was to investigate immediate pain relief effect of the repetitive peripheral inductive stimulation device BTL-6000 Super Inductive System (BTL Industries Ltd.) in musculoskeletal diseases. Methods: 31 subjects from Rehabilitation Center Kladruby were comprised in the pilot study. Subjects underwent approx. 7 therapies individually. A hand-held applicator type „focus field“ was used. Pulse repetition rates matching with gate theory and peripheral pattern theory were applied to achieve pain relief effect. A Visual Analog Scale (VAS) was used to evaluate pain before and after each therapy. Results: Immediate pain relief effect of the repetitive peripheral inductive stimulation in 62% patients with musculoskeletal diseases was observed. Conclusion: Despite small number of subjects, decrease of painful perception and pain relief effect were achieved. Although, the study design does not allow comparison with placebo effect, statistically significant (α = 0.2) immediate pain relief effect in 50 - 74 % of subjects might be expected.
