Významný a rastúci podiel zhubných nádorov na celkovom počte úmrtí obyvateľov vyspelých a postupne i rozvojových krajín viedol už od polovice minulého storočia k formulovaniu preventívnych programov proti týmto ochoreniam. Deklaratívny charakter programov, ich zameranie na rozsiahle spektrum lokalizácií, nerešpektovanie lokálnych priorít, rýchle očakávanie pozitívnych výsledkov pri súčasnom nedostatku materiálnych a personálnych zdrojov viedlo k ich zlyhaniu. Naviac tvorba a implementácia onkologických programov nutne zaostávala za dramatickým a pritom dlhodobo podceňovaným vzostupom onkologických ochorení. Pri formulovaní onkologických programov v súčasnosti sa zdôrazňuje predovšetkým ich komplexný a reálny charakter. Vychádzajú zo všeobecne akceptovaného faktu poznania príčin zhruba 40 % všetkých nádorov, čím sú určené a ohraničené možnosti ich primárnej prevencie. Akceptované možnosti liečby asi 33 % sú východiskom pre aplikáciu metód sekundárnej prevencie, zlepšenia diagnostiky a následnej úspešnejšej liečby ranných štádií ochorenia. Všetky uvedené zásady splňuje metóda surveillance osvedčená pri prevencii infekčných chorôb. Ide o sústavný zber detailných údajov o aktuálnom a budúcom rozsahu a vývoji zhubných nádorov, ako aj diagnostike, liečbe a celkovej starostlivosti o onkologických pacientov. Takéto poznatky môžu poskytnúť iba populačné onkologické registre, a to nielen pri formulovaní a smerovaní komplexných programov, ale i hodnotení ich výsledkov.
Important and increasing contribution of malignant tumours on the total number of deaths in developed and later also in developing countries led in the middle of previous century to the formulation of preventive cancer control programs. Eroneus interpretation of epidemiological as well as results obtained on laboratory animals, the general acceptance that the etiology of nearly all human malignant tumours is known, declarative character of these programs, orientation on large scale of cancer sites, ignorance of local priorities, avaiting the positive results in short time together with limited material and personal resources led to the failure of majority of cancer control programs. Moreover.the formation and implementation of these programs remained all the time behind dramatic increase of cancer. Actually cancer control programs are based on generally accepted fact that the causes of about 40% of cancers are known and therefore preventable. Possibilities of treatement roughly 33% of cancer cases provide the aims of secondary prevention .The essential basis of the performance of cancer control programs is their surveillance which was succesfully used in prevention of infectious diseases. In oncology surveillance encompasses the systematic collection of detailed data on actual and future burden and development of cancer incicence and mortality in given population, as well as on diagnostic, treatment and overall care of cancer patients. Information of this kind could be derived only from population based cancer registries which are also valuable for the evaluation and monitoring of the results of national cancer control programs.
This paper describes results of two ecological studies design to analyze the incidence of selected malignancies in two populations exposed to polychlorinated hydrocarbons, mostly PCBs and TCDDs/Fs by comparing data available in the National Cancer Registry of the Slovak Republic and National Oncological Registry of the Czech Republic databases for the Slovak Republic (approximately 5M inhabitants) and the Czech Republic (10,3 M inhabitants) to the data relevant for the population of Michalovce District, the Slovak Republic (approximately 112,000 inhabitants) and Uherske Hradiste, the Czech Republic (146,000 inhabitants). Those districts are recognized as PCB-contaminated areas due to production and industrial use of PCBs. Data were analyzed for the 10-year period 1987-1996. The age adjusted world standard ratio (WSR) incidence of thyroid, pancreatic, breast, ovarian, bladder, and brain tumors in females and thyroid, pancreatic, breast, bladder, brain, prostate and testicular tumors in males were compared. Neither PCBs nor TCDDs/Fs appear to contribute to the observed significantly lower incidence of breast and prostate cancer in the Michalovce District and lower bladder cancer incidence in Uherske Hradiste District. However, anti-estrogenic and anti-androgenic properties have been described for hydroxylated and methylsulfonyl PCB metabolites. These properties could contribute to a mechanism through which these metabolites might modulate the development of breast, prostate and bladder cancer. The results of our analysis points to substantial potential problems of risk assessment for cancer incidence in populations exposed to xenobiotics, or more generally, as it relates to a wide spectrum of confoundings of cancer risk factors.

Zhubné nádory pečene sa 625 000 prípadmi ochorení v roku 2002 zaradili na 6. miesto a počtom 598 000 zomretých na tretie miesto v celkovom počte nádorových ochorení a úmrtí v celosvetovom meradle. Najvyšší výskyt sa zaznamenáva trvale v štátoch Juhovýchodnej Ázie a Subsaharskej Afriky, menší vo vyspelých štátoch Európy, Severnej a Južnej Ameriky a v Austrálii. Presné určenie primárnych nádorov pečene je vzhľadom na početné metastázy z nádorov lokalizovaných v iných orgánoch zložité a viazané na mikroskopické potvrdenie nálezu morfologických typov nádorov typických pre pečeň: hepatocelulárnych a cholangiocelulárnych karcinómov a zriedkavejších hepatoblastómov v detskom veku. Vysoký výskyt primárnych nádorov pečene v endemických oblastiach hepatitídy spôsobených vírusom typu B (HBV) a v menšej miere i typu C (HCV) viedol postupne k zisteniu, že až 70 % všetkých primárnych zhubných nádorov pečene má vzťah k chronickej infekcii s HBV a HCV. Významná je však skutočnosť, že tento nález viedol k vyhotoveniu a aplikácii vakcíny proti HBV s pozitívnymi výsledkami.
Cancer of liver with 625,000 cases was in 2002 the sixth most common type of cancer and with 598,000 deaths the third most common cause of deaths from cancer worldwide. The highest incidence rates were registered in southeastern Asia and sub-Saharian Africa, low in developed areas of Europe, Northern and Southern America and Australia. Exact identification of primary liver cancer is based on microscopic confirmation of typical morphologic types of liver malignancieshepatocellular and cholangiocellular carcinomas and of rare hepatoblastomas in children. The liver is a very frequent organ for metastases from other cancer sites. High occurrence of primary liver cancers in endemic areas of hepatitis caused by B-type virus (HBV) and in lesser extent by type C (HCV) led gradually to discovery that about 70 % of all primary liver cancers is related to the chronic infection with HBV or HCV. Of great importance is the fact that this discovery led to the preparation and application of anti HBV vaccine with positive results.
The objective of the study was to investigate the contribution of dietary factors and physical exercise to the variation in the risk of lung cancer and its major histological types among men and women in the Czech Republic, and reveal interactions between smoking and diet/physical exercise, if any. In a hospital based case-control study, data collected by in-person interviews from 1096 microscopically confirmed lung cancer cases (587 women, 509 men) and 2966 controls were analyzed using unconditional logistic regression stratified by appropriate factors. Among all nonsmoking women protective effects were observed for black tea (OR=0.69), among all smoking women for wine (OR=0.71), physical exercise (OR=0.64) and vitamin supplements (OR=0.71). Among all men, inverse associations were found in smokers between lung cancer risk and frequent intake of fruits (OR=0.69) or moderate intake of spirits (OR=0.64), and a direct association for fat foods (OR=1.68). Comparing the effects of diet/physical activity on lung cancer risk among nonsmokers versus smokers, interactions with smoking appeared for the intake of black tea and milk/dairy products among women, and for moderate intake of spirits in men. When the effects of diet/physical exercise on risk were analyzed by major cell types in women, the intake of wine and physical exercise were inversely associated with the risk of both adenocarcinoma and small cell cancer, the intakes of fruits and vitamin supplements were inversely associated with the risk of squamous cell cancer. In men, the intake of fat foods was directly associated with the risk of squamous cell cancer, while the frequent intake of apples was inversely associated with the risk of both squamous- and small cell cancers. In men an inverse association with the risk of squamous cell cancer was found for the intake of other fruits. These data suggest that diet/physical exercise may affect the risk of lung cancer and major cell types, and that interactions between some dietary items and smoking may occur. Lung cancer is a multifactorial disease, since smoking, its main determinant, and other environmental and lifestyle factors interact with one another and with genetic factors to cause the disease.

The objective of the study is to estimate the differences in the impact of diet and physical exercise on lung cancer risk in female nonsmokers vs. smokers, and reveal interactions, if any. In a hospital based case-control study, data collected by in-person interviews from 569 female lung cancer cases and 2120 controls were analyzed using unconditional logistic regression stratifying by appropriate factors. Protective effects were observed for intake of milk/dairy products (OR=0.57, 95%CI 0.35-0.94), vegetables (OR=0.60, 95%CI 0.40-0.91), apples (OR=0.69), wine (OR=0.77), and physical exercise (OR=0.59, 95%CI 0.42-0.83) among smokers only, while no similar effects were found among nonsmokers. In contrast, the intake of black tea was associated with a protective effect (OR=0.66, 95%CI 0.47-0.94) among nonsmokers only. Comparing the effects of dietary items and physical activity on lung cancer risk among nonsmokers versus smokers, statistically significant effect modifications were found for black tea (P 0.005), and milk/dairy products (P 0.047). Borderline effect modifications emerged for physical exercise (P 0.077). CONCLUSIONS: These results indicate protective effects of some components of healthful diet and physical exercise among smokers, and of the intake of black tea among nonsmokers. The observed interactions of the impact of black tea, milk/dairy products and physical activity upon lung cancer risk in women at different levels of the smoking habit deserve further studies.

Východisko. V práci autori podávajú súhrn údajov o incidencii primárnych nádorov štítnej žľazy (p.m.n. š.ž.) zo svojho pracoviska z rokov 1984–2005, výsledky zároveň interpretujú v kontexte dostupných údajov o incidencii p.m.n. š.ž. z celoslovenského onkologického registra z rokov 1996–2002. Metódy a výsledky. V rokoch 1984–2005 bolo na pracovisku autorov indikovaných 6434 operácií š.ž., pri ktorých sa histologicky potvrdilo 365 p.m.n. š.ž.., čo predstavuje 5,7 % všetkých histologických nálezov. V 295 prípadoch boli p.m.n. š.ž. zaznamenané u žien (80,8 %), v 70 prípadoch u mužov (19,2 %), s najčastejším výskytom v 5. dekáde a posunom výskytu k mladším vekovým skupinám v poslednom období. Zatiaľ čo v rokoch 1984–1989 prevažoval histologicky folikulárny karcinóm š.ž. (24 prípadov, t.j. 66,7 %), v rokoch 1990–1997 a 1998–2005 už dominuje papilárny karcinóm – 70 (61,4 %), resp. 160 (74,4 %) prípadov. Pri hodnotení celoslovenského výskytu p.m.n. š.ž. je zrejmé, že pracovisko autorov sa podieľa na diagnostike p.m.n. š.ž. na Slovensku stúpajúcim percentom (8,9 % v roku 1996 vs. 13,9 % v roku 2002). Zlepšenie diagnostických metodík a rozšírenie kapacity vyšetrovaných pacientov každopádne nevysvetľuje globálne stúpajúcu incidenciu p.m.n. š.ž. (3,9/106 v roku 1996 vs. 5,6/106 v roku 2002 u žien, resp. 1,3/106 v roku 1996 vs. 1,7/106 v roku 2002 u mužov). Závery. Úplne nezodpovedanou otázkou, vyžadujúcou ďalšie (rádio)epidemiologické, geografické a genetické zhodnotenie, sú výrazné rozdiely v incidencii p.m.n. š.ž. medzi jednotlivými krajmi (u mužov 0,6/106 v roku 1996 a 1,5/106 v roku 2002 v Trnavskom kraji vs. 1,2/106 v roku 1996 a 2,4/106 v roku 2002 v Prešovskom kraji, resp. u žien 3,6/106 v roku 1996 a 5,6/106 v roku 2002 v Trnavskom kraji vs. 5,4/106 v roku 1996 a 8,4/106 v roku 2002 v Prešovskom kraji), resp. okresmi Slovenska – u mužov nulová v 42 zo 79 okresov vs. najvyššia (15,9/106) incidencia v roku 2002 v okrese Svidník, u žien nulová v 15 okresoch vs. najvyššia (26,5/106) incidencia v roku 2002 v okrese Stropkov.
Background. In this paper the authors summarise the primary malignant thyroid tumors (p.m.t.t.) incidence data from their centre over the period 1984–2005. The results are explained in the context of the p.m.t.t. incidence data from the National Cancer Register (1996–2002). Methods and Results. Overall, 6434 thyroid operations were indicated and carried out during the sampling period at the authors’ institution, of which 365 cases were histologically confirmed p.m.t.t. (5,7 % of all histological findings). Specifically, p.m.t.t. were diagnosed in 295 women (80,8 %) and 70 men (19,2 %), with the most frequent occurrence in the 5th decade and a higher recent incidence in younger age groups. Whereas follicular thyroid carcinomas predominated in the 1984–1989 period (24 cases or 66,7 %), the papillary thyroid carcinoma was the most frequent type in 1990–1997 and 1998- 2005: 70 (61,4 %) and 160 (74,4 %) cases respectively. In the context of the national incidence, the authors’ institution increasingly contributes to the diagnosis of p.m.t.t. in Slovakia (8,9 % in 1996 vs. 13,9 % in 2002). However, neither the progress in diagnostic methods nor the capacity expansion for patient examination can explain the increasing incidence of the p.m.t.t. (3,9/106 in 1996 vs. 5,6/106 in 2002 in women and 1,3/106 in 1996 vs. 1,7/106 in 2002 in men, respectively). Conclusions: It remains to be explained why there are significant differences in p.m.t.t. incidence among individual counties. For example, in county Trnava 0,6/106 in men in 1996 vs. 1,5/106 in 2002; in county Prešov 1,2/106 in 1996 vs. 2,4/106 in 2002. Similarly, in women in county Trnava 3,6/106 in 1996 vs. 5,6/106 in 2002 and in county Prešov 5,4/106 in 1996 vs. 8,4/106 in 2002. As far as the districts of Slovakia are concerned, data relative to the male population in 2002 indicate zero incidence in 42 out of 79 districts, with the highest incidence (15,9/106) recorded in Svidník. Likewise, data relative to the female population in 2002 indicate zero incidence in 15 out of 79 districts, with the highest incidence (26,5/106) in Stropkov. These differences call for further (radio)epidemiological, geographical and genetic evaluation.

Zhubné nádory anusu a análneho kanála sú pomerne zriedkavé. Na celkových počtoch novozisťovaných nádorov ročne sa podieľajú v jednotlivých populačných, národných alebo regionálnych onkologických registroch zhruba jedným percentom. Štandardizované hodnoty incidencie medzi jednotlivými oblasťami, ale i kontinentmi sú, až na malé výnimky, nepatrné. Všeobecne sú vyššie u žien, pričom hlavne u nich dochádza v poslednom desaťročí k ich vzostupu. V etiológii týchto, hlavne skvamocelulárnych karcinómov, sa uplatňujú predovšetkým neobvyklé sexuálne praktiky, promiskuita, zápalové a benígne lézie ako aj vírusové infekcie. Rizikové faktory určujú orientovanie primárnej prevencie.
Malignant tumours of anus and anal canal are rare and comprise about 1 % of all malignancies notified yearly in population based, national or regional cancer registers worldwide. Age adjusted incidence rates among different countries and continents are, with the rare exceptions, small. Incidence rates are generally higher in women, and it shows the increase during the recent decade. In the aetiology of mostly squamous cell cancers the important role is played by unusual sexual intercourses, promiscuity, inflammatory and benign lesions as well as viral infections. The primary prevention should be oriented to the elimination of known risk or likely aetiologic factors of this cancer.

Kontinuálny nárast počtov ochorení a úmrtí na malígne ochorenia v uplynulom storočí postupne viedol k ich významnému postaveniu v celkovej úmrtnosti a chorobnosti populácií vo vyspelých štátoch sveta. Táto skutočnosť si vynútila aplikáciu epidemiologických prístupov a metód, ktoré sa v minulosti osvedčili pri štúdiu vývoja, rozsahu a prevencie infekčných chorôb aj na oblasť neinfekčných, najmä malígnych ochorení. Cieľom príspevku je poskytnúť urológom a odborníkom v iných klinických odboroch prehľad základných epidemiologických štatistických ukazovateľov a ich vzájomných vzťahov, spolu s ich adekvátnym použitím pri jednotlivých urologických malignitách.
Gradual increase of cases and deaths from malignant diseases during the previous century resulted in their dominant position in the overall mortality and morbidity of the populations of developed countries. This situation led to the use of epidemiological methods in the field of non-infectious, mainly malignant diseases, applied in the past with success in the study and control of infectious diseases. The aim of this paper is to provide the urologists and other clinicians with the most common epidemiological indicators as well as their adequate use in the study urological malignancies.
