BACKGROUND: Implementation of adjuvant therapies in non-metastatic melanoma improved treatment outcomes in some patients; however, adjuvant therapy can be associated with significant cost and risk of toxicity. Therefore, there is an unmet need to better identify patients at high risk of recurrence. PATIENTS AND METHODS: We carried out an ultrasensitive droplet digital PCR (ddPCR)-based detection of BRAFV600E-mutated circulating tumor DNA (ctDNA) from blood samples prospectively collected before surgery, 1 hour after surgery, and then serially during follow-up. RESULTS: In 80 patients (stages ≤III), BRAFV600E mutations were detected in 47.2% of tissue, in 37.7% of ctDNA samples collected before surgery, and in 25.9% of ctDNA samples collected 1 hour after surgery. Patients with detected ctDNA in blood collected 1 hour after surgery compared to patients without detected ctDNA had higher likelihood of melanoma recurrence (P < 0.001) and shorter median disease-free survival (P = 0.001) and overall survival (P = 0.003). CONCLUSIONS: Ultrasensitive ddPCR can detect ctDNA in pre- and post-surgical blood samples from patients with resectable melanoma. Detection of ctDNA in post-surgical samples is associated with inferior treatment outcomes.

Recentně aktualizovaná klasifikace nádorů centrální nervové soustavy (CNS) zpracovaná Světovou zdravotnickou organizací (WHO) v roce 2016 využívá poprvé kromě histopatologických kritérií také molekulárně genetické charakteristiky (biomarkery) nádorových buněk a zavádí pojem tzv. integrované diagnostiky. Molekulárně genetické biomarkery mají mnohdy zásadní vliv na prognózu nemocných a/nebo výběr vhodné terapie u řady nádorových entit. Aktualizace tak představuje významný pokrok ve srovnání s klasifikací nádorů CNS z roku 2007. Přesnější zařazení nádorů za pomoci známých a nyní široce akceptovaných molekulárně genetických biomarkerů umožní také další výzkum protinádorové léčby a konzistentní vřazování nemocných do klinických studií. Hlavním nedostatkem nového přístupu je především přítomnost skupin nádorů blíže nespecifikovaných (Not Otherwise Specified; NOS). Nádory řazené v současné době k NOS entitám budou pravděpodobně přesněji charakterizovány s pokračujícím výzkumem molekulární genetiky v neuroonkologii. Rovněž budou překonány provozně-technické bariéry molekulárně genetických analýz, což dále umožní významně zmenšit skupinu NOS nádorů. Předpokládá se, že nová klasifikace usnadní klinické, experimentální i epidemiologické studie a povede tak ke zlepšení života nemocných s mozkovými nádory. Předložený přehled přináší nejdůležitější poznatky z nové klasifikace nádorů CNS a jejich klinické souvislosti.
Recently updated classification of the central nervous system (CNS) tumours prepared by the World Health Organization (WHO) in 2016 uses, in addition to the histopathological criteria, also molecular genetic characteristics (biomarkers) of tumour cells and introduces the so-called integrated diagnostics concept for the first time. Molecular genetic biomarkers often have a major impact on the patients´ prognosis and/or selection of an appropriate therapy in a variety of tumour entities. This update represents a significant progress compared to the 2007 classification of CNS tumours. The more precise classification of tumours using well-known and widely accepted molecular genetic biomarkers, will also facilitate further research in anticancer therapeutics and consistent inclusion of patients into clinical trials. The presence of a tumour group called „not otherwise specified“ (NOS) is the major drawback of this novel approach. Tumours currently classified as the NOS entities are likely to be more accurately characterized with an ongoing neurooncological research in molecular genetics. The operational and technical barriers of molecular genetic analyses will be also overcome and this will further enable significant reduction of the NOS tumour entities. It is assumed that the new classification will facilitate clinical, experimental as well as epidemiological studies and thereby will improve the life of patients with brain tumours. This review presents the key findings from the new classification of the CNS tumours together with their clinical sequelae.

Diagnóza gliomu centrálního nervového systému stupně malignity II (nízkostupňový gliom, Low-Grade Glioma; LGG) znamená vždy významný zásah do života nemocných, neboť i přes zřetelný pokrok v terapii se nadále jedná o nevyléčitelné onemocnění. Léčebné možnosti zahrnují neurochirurgický zásah, radioterapii a chemoterapii. Dosud však nejsou stanovena jednoznačná kritéria pro určení rozsahu jednotlivých léčebných metod, jejich kombinací nebo vhodného načasování. Teprve výsledky dlouhodobého sledování v klinické studii fáze III RTOG 9802 prokázaly příznivý účinek kombinované onkologické léčby radioterapie (54 Gy) a chemoterapie (režim prokarbazin, lomustin a vinkristin; PCV) u nemocných se zvýšeným rizikem (věk > 40 let s reziduálním pooperačním radiografickým nálezem nebo věk ≥ 40 let po jakémkoli operačním zásahu). Nezodpovězena prozatím zůstává otázka významu molekulárně-genetických biomarkerů (kodelece 1p/19q, mutace IDH1/2 a dalších) ve vztahu k predikování účinku kombinované léčby. Očekává se, že detailní molekulárně-genetická analýza nádoru bude součástí rutinní klinické péče o nemocné s gliomy všech stupňů malignity vč. LGG. Kombinovaná radioterapie a chemoterapie režimem PCV následující po neurochirurgickém zásahu by měla být v současné době preferovaným přístupem k léčbě nemocných s vysoce rizikovým LGG.
The diagnosis of grade II central nervous system glioma (Low-Grade Glioma; LGG) always significantly impacts on the lives of patients, as, despite clear progress in therapy, LGG continues to be an incurable disease. Treatment options include neurosurgical intervention, radiotherapy and chemotherapy. So far, however, no clear criteria have been set to determine the effect of individual treatments, their combinations or their timing. The results of a long-term follow up of the phase III RTOG 9802 trial demonstrated better effect of combined radiotherapy (54 Gy) and chemotherapy (procarbazine, lomustine and vincristine; PCV) treatment in patients with high risk disease (age > 40 years with postoperative radiographic residuum or age ≥ 40 years after any surgical intervention). The question of the role of molecular genetic biomarkers (co-deletion 1p/19q, IDH1/2 mutations and others) in predicting the effects of combined treatment remains unanswered. It is expected that detailed molecular genetic analysis of each tumor will become a part of routine clinical care of patients with gliomas of all stages of malignancy, including LGG. Combined radiotherapy and chemotherapy with PCV following neurosurgical intervention should be the preferred approach to treatment of patients with high-risk LGG.

PURPOSE: To assess the benefit of 4D-CT angiography (4D-CTA) in determination and precise measurement of middle cerebral artery (MCA) occlusion in comparison to CTA. Possible relationship of measured occlusion lengths with recanalization after intravenous thrombolysis was analysed as a second objective. METHODS: Detailed evaluation of complete MCA occlusions in 80 patients before intravenous thrombolysis using temporal maximum intensity projection (tMIP) dataset, calculated from 4D-CTA and conventional single-phase CTA was performed. Further, manual measurement technique was compared to results of semiautomatic procedure (vessel analysis) as reference. Statistical analysis of correlation between MCA occlusion length and IVT efficacy (24 h recanalization rate according modified Thrombolysis In Myocardial Infarction criteria-mTIMI) was performed. RESULTS: The distal end of occlusion was identified in all patients using tMIP, but only in 48 patients (60%) using CTA. The manual measurement method was not statistically different and well correlated with reference tMIP-vessel analysis. (15.4 vs. 16.3 mm; p = 0.434; r = 97). In measurable occlusions by CTA, no significant difference was proved in manually measured lengths using tMIP and CTA (14.5 vs. 13.3 mm; p = 0.089). Favorable recanalization (mTIMI 2-3) was achieved in 37 patients (47%). Length of occlusion in M1 segment (p = 0.002) and M2 segment involvement (p = 0.017) were proved as independent negative predictors of recanalization. Using receiver operating characteristics analysis, the cutoff length of the M1 segment occlusion for favorable recanalization was found to be 12 mm. CONCLUSION: The feasibility of MCA occlusion assessment using tMIP datasets and benefit over conventional CTA were confirmed. The manual measurement method was proved as feasible and simple with good correlation to reference semiautomatic analysis. The significant correlation of the MCA occlusion length and early recanalization was found. The length of 12 mm was recognized as cut-off length for favorable recanalization.
