Mumps outbreaks, especially in adolescents and young adults, have been reported in the Czech Republic. The aim of the presented study was to determine the seroprevalence of specific IgG antibodies against mumps in the adult population of the Czech Republic. The study was designed as a multicenter serological survey of adults aged 18 years and over. Specific IgG antibodies against mumps were detected in blood samples using an enzyme-linked immunosorbent assay (ELISA). A total of 1,911 serum samples were examined. The overall seropositivity reached 55.3%. In individual age groups, the highest seropositivity 63% (63.5-65.2%) was recorded in adults aged 40 years and over; the lowest seropositivity was found in adults aged 18-29 years (27.4%). The difference in seropositivity rate between the 18-29 years age group and the 40 years and over age groups was statistically significant (p < 0.001). Only the 18-29 years age group included both vaccinated and unvaccinated (born in the pre-vaccine era) individuals. In vaccinated individuals, seropositivity was reported in only 19.1% of persons; in unvaccinated individuals, seropositivity reached 48.2%. Our results demonstrate the long-term persistence of antibodies following natural infection and the decrease in seropositivity that occurs after vaccination over time. This immunity waning may account for the higher susceptibility of adolescents and young adults to mumps. Therefore, the current vaccination program in the Czech Republic could be considered as less effective. It will be modified with the shifting of the second dose of vaccine from two years of age to the preschool age.

Although mumps vaccine is part of the routine childhood immunization schedule, mumps cases or outbreaks have been reported repeatedly. The reasons behind the re-emergence of the disease are not clear. One of the factors may be MMR vaccine failure against mumps, waning protective antibodies, or lack of protection as a result of low vaccine coverage of the currently circulating genotypes. Mapping the genotype-specific prevalence of the mumps virus in the Czech Republic and detailed analysis of the antibody response could provide clues to mumps outbreaks in the vaccinated population and would be helpful in updating the vaccination strategy against mumps in the Czech Republic. The results obtained will make it possible to generate an effective algorithm for the laboratory and clinical investigations and to analyze the correlation, if any, between clinical symptoms and causative genotypes of the mumps virus.
Ačkoliv je očkování proti příušnicím (epidemické parotitidě) součástí povinného očkování dětí, dochází k opakovaným výskytům nemoci až lokálním epidemiím. Příčiny nejsou zcela jasné. Může se jednat o selhání parotitické složky vakcíny nebo tzv. vyvanutí ochranných protilátek, případně o nedostatečnou ochranu danou rozdílností aktuálně cirkulujících genotypů a vakcinálního kmene. Zmapování genotypů vyskytujících se v České republice a detailní analýza protilátkové odpovědi by mohly vysvětlit výskyt epidemické parotitidy v očkované populaci a v konečném důsledku i ovlivnit strategii očkování proti parotitidě v ČR. Získané výsledky dále umožní vytvořit efektivní algoritmus laboratorního a klinického vyšetření a zhodnotit vztah závažnosti klinických projevů a daného genotypu.

AIM: The aim of the study was to map the incidence of mumps in the Czech Republic in terms of clinical symptoms, epidemiological links, and characteristics of circulating genotypes. METHODS: Patients with suspected mumps examined in the Infectious Diseases Clinic of the Na Bulovce Hospital in 2013 were enrolled in the study. Buccal swab specimens were tested by means of nucleic acid detection (RT-qPCR) and when positive, they were cultured in tissue culture. Sequencing was carried out using the BigDye Terminator v3.1 Cycle Sequencing Kit and Genetic Analyzer 3500. The SeqScape software was used for the analysis of sequencing data and filtering out low quality reads. The phylogenetic analysis and genotyping were performed using the Mega 6 software. To generate the phylogenetic tree, all sequences were aligned by the MAFFT tool and the alignment obtained was edited using the BioEdit software. In all patients, selected biochemical markers (C-reactive protein, white blood cell count and serum amylase) were measured. The EPIDAT system used for reporting infectious diseases, record keeping, and data analysis in the Czech Republic was the source of statistical data. RESULTS: Eighty-nine patients with suspected mumps were examined in the Na Bulovce Hospital and 65 of them were laboratory confirmed with mumps: 40 males (61.5%) and 25 females (38.5%). The mean age of the study cohort was 25.9 years (median age of 23 years, age range from 10 to 73 years) and 14 patients were under 18 years of age. Thirty-four (52.3%) patients were vaccinated in childhood, 28 (43.1%) were unvaccinated, and for three persons, vaccination data were not available. A severe course of the disease was reported in 15 (23.1%) patients. Fourteen of them needed hospitalization because of orchitis (9 males) and meningitis (5 patients). One patient with orchitis was treated on an outpatient basis. The need for hospitalization tended to be lower in the unvaccinated patients (14.7% vs. 35.7%, p=0.076). In 2013, 1,553 cases of mumps were reported to the EPIDAT system. Of these, 640 were laboratory confirmed. The most often reported complications were orchitis (90 cases, i.e. 10.3%) and meningitis (21 cases, i.e. 1.4%). Orchitis was diagnosed in 30.3% of the unvaccinated and in 6.4% of the vaccinated males. Meningitis occurred in 3.1% of the unvaccinated and in 1.0% of the vaccinated patients. CONCLUSION: Despite the emergence of mumps among the vaccinated population, the present study has confirmed a positive effect of the vaccine, particularly on the incidence of complications and inflammatory markers. All 30 sequenced mumps virus strains were assigned to group G. A secondary vaccine failure due to waning immunity seems to be a plausible explanation for the rise in mumps cases.

Mumps vaccines are live attenuated viruses. They are known to vary in effectiveness, degree of attenuation and adverse event profile. However, the underlying reasons are poorly understood. We studied two closely related mumps vaccines which originate from the same attenuated Jeryl Lynn-5 strain but have different efficacies. Jeryl Lynn-Canine Kidney (JL-CK), produced on primary canine kidney cells, is less effective than RIT4385, which is produced on chicken embryo fibroblasts. JL-CK and RIT4385 could be distinguished by a number of in vitro and in vivo properties. JL-CK produced heterogeneous, generally smaller plaques than RIT4385, but gave 100-fold higher titres when grown in cells and showed a higher degree of hydrocephalus formation in neonatal rat brains. Sanger sequencing of JL-CK identified 14 regions of heterogeneity throughout the genome. Plaque purification of JL-CK demonstrated the presence of five different Jeryl Lynn-5 variants encompassing the 14 mutations. One JL-CK mutation was associated with a small plaque phenotype, the effects of the others in vitro or in vivo were less clear. Only 4% of the JL-CK population corresponded to the parental Jeryl Lynn-5 strain. Next generation sequencing of JL-CK and virus before and after growth in cell lines or neonatal rat brains showed that propagation in vitro or in vivo altered the population dramatically. Our findings indicate that growth of JL-CK in primary canine kidney cells resulted in the selection of a mixture of mumps virus variants that have different biological properties compared to the parent Jeryl Lynn-5 virus. We also report three previously unknown heterogenic regions within the N gene of the RIT4385 vaccine.

Cíl práce: Cílem práce bylo rozšíření současné rutinní sérologické laboratorní diagnostiky příušnic o metody přímého průkazu infekčního vyvolavatele a následná genotypizace izolovaných virů, ve snaze získat podrobnější informace o recentních virech příušnic cirkulujících na území České republiky. Dílčím cílem pak bylo poukázat na specifika laboratorního vyšetření u vysoce proočkované populace a zhodnocení současné epidemiologické situace. Materiál a metody: Do studie bylo zahrnuto 47 stěrů z bukálních sliznic od pacientů s podezřením na onemocnění příušnicemi. Klinické materiály byly odebrány v počátku klinické manifestace. Sběr vzorků probíhal od února do prosince 2012 a zahrnoval pět krajů ČR. Pro izolaci viru byly použity buněčné kultury Vero buněk, izoláty byly identifikovány pomocí RT-qPCR. Genotypizaci provedla WHO Regionální referenční laboratoř pro spalničky, zarděnky a příušnice, Institut Roberta Kocha v Berlíně (RRL MMR). Jako zdroj epidemiologických údajů byl využit systém EPIDAT. Výsledky: Ze 47 bukálních stěrů byl ve 20 případech na Vero buňkách izolován virus příušnic, v dalších sedmi vzorcích byla metodou RT-qPCR prokázána pouze přítomnost virové RNA bez pozitivní izolace. Ke genotypizaci bylo zasláno 19 izolátů, genetickou analýzou SH genu byla prokázána jejich fylogenetická příslušnost ke genotypu G, a to ve čtyřech variantách. V roce 2011 i 2012 tvořili převážnou část nemocných očkovaní jedinci (80 %); nejpostiženější věkovou skupinou byli dospívající ve věku 15–19 let. Nejčastější komplikací byla orchitida a druhou nejčastější komplikací byla meningitida. Komplikacemi byli více postiženi neočkovaní jedinci. Závěry: Na zvýšeném výskytu příušnic v České republice v roce 2012 se významně podílel genotyp G, který je v největší míře zodpovědný za zvýšenou nemocnost ve většině evropských zemí již od roku 2005. U nás byl výskyt genotypu G poprvé prokázán v roce 2006. S ohledem na nepříznivý epidemiologický vývoj se provádění molekulárně epidemiologických studií zahrnujících genotypizaci recentních příušnicových kmenů jeví jako nezbytné. Detailní monitorování by mohlo napomoci k objasnění vzorce cirkulace viru a k vytvoření strategie kontroly nad vznikajícími epidemiemi. Účinnost vakcinace ve vztahu ke genotypu, ale i možnost přispění vyvanutí imunity ke zvýšenému výskytu onemocnění jsou otázkami, které by měly být dořešeny.
Goal: To extend the present routine serological diagnosis of mumps with the methods of direct detection of the pathogen and subsequent genotyping of the isolated viruses in an attempt to obtain more detailed data on recent mumps viruses circulating in the Czech Republic. Sub-goals were to point out the particularities of the laboratory examination in the population with a high vaccine coverage and to evaluate the current epidemiological situation. Material and methods: Altogether 47 buccal swabs from patients with suspected mumps were included in the study. Clinical specimens collected at the onset of clinical symptoms were obtained from five administrative regions of the Czech Republic from February 2012 to December 2012. Vero cell cultures were used for virus isolation and isolates were identified using real-time quantitative polymerase chain reaction (RT-qPCR). Genotyping was performed by the WHO Regional Reference Laboratory for Measles, Mumps, and Rubella (RRL MMR), Robert Koch Institute, Berlin. The EPIDAT system was used as a source of epidemiological data. Results: From 47 buccal swabs, 20 mumps viruses were isolated on Vero cells and in seven other specimens, the presence of viral RNA without positive isolation was only detected by RT-qPCR. Nineteen isolates were referred for genotyping. The phylogenetic analysis of the SH gene classified them into genotype G, as four variants. In both 2011 and 2012, most cases occurred in vaccinated patients (80%), with 15–19-year-olds being the most affected age group. The leading complication was orchitis, followed by meningitis. More complications were reported in non-vaccinated individuals. Conclusions: The increased incidence of mumps cases in the Czech Republic in 2012 was due primarily to genotype G, the leading cause of mumps in most European countries since 2005. The presence of genotype G was first reported in the Czech Republic in 2006. In the context of the unfavourable epidemiological trend, molecular epidemiological studies including genotyping of recent mumps virus strains appear to be necessary. A detailed monitoring could be helpful in elucidating the pattern of virus circulation and in designing strategies to control emerging outbreaks. The vaccination efficacy in relation to the causative genotype and possible role of waning immunity in mumps outbreaks are the issues that need to be addressed.

Mumps outbreaks have recently been recorded in a number of highly vaccinated populations. We related seroprevalence, epidemiological and vaccination data from 18 European countries participating in The European Sero-Epidemiology Network (ESEN) to their risk of mumps outbreaks in order to inform vaccination strategies. Samples from national population serum banks were collected, tested for mumps IgG antibodies and standardized for international comparisons. A comparative analysis between countries was undertaken using age-specific mumps seroprevalence data and information on reported mumps incidence, vaccine strains, vaccination programmes and vaccine coverage 5-12 years after sera collection. Mean geometric mumps antibody titres were lower in mumps outbreak countries [odds ratio (OR) 0·09, 95% confidence interval (CI) 0·01-0·71)]. MMR1 vaccine coverage ⩾95% remained protective in a multivariable model (P < 0·001), as did an interval of 4-8 years between doses (OR 0·08, 95% CI 0·01-0·85). Preventing outbreaks and controlling mumps probably requires several elements, including high-coverage vaccination programmes with MMR vaccine with 4-8 years between doses.
