OBJECTIVES: Although a number of human Legionnaires' disease in tourists are recorded annually in Europe, there are few cases where a direct link can be made between the infected person and the source of infection (hotel or other accommodation). We present a scheme followed in order to track down and identify the source of infection in a tourist suffering from L. pneumophila sg 5 infection, who was accommodated in seven different hotels during his holidays in the island of Crete, and we comment on various difficulties and draw-backs of the process. METHOD: Water samples were collected from the seven hotels where the patient had resided and analyzed at the regional public health laboratory using cultivation and molecular tests. RESULTS: Of 103 water samples analyzed, 19 (18.4%) were positive for Legionella non-pneumophila and 8 (7.8%) were positive for L. pneumophila. A successful L. pneumophila sg 5 match was found between the clinical and environmental sample, which led us to the final identification of the liable hotel. CONCLUSION: Timely notification of the case, within the the European Legionnaires' Disease Surveillance Network (ELDSNet) of the partners involved, is crucial during a course of travel associated with Legionella case investigation. Moreover, the urinary antigen test alone cannot provide sufficient information for the source identification. However, acquiring clinical as well as environmental isolates for serogroup and SBT identification is highly important for the successful matching.

Je popsán případ legionelózy u staršího muže, který začal chřipkovými symptomy s následným zhoršováním a hospitalizací a při další progresi pokračoval umístěním na ARO fakultní nemocnice. Až zde byla diagnostikována legionelóza. Epidemiologické šetření se zaměřilo na vodovodní vodu v bytě nemocného a odebrané vzorky potvrdily podezření na akviraci nákazy v místě bydliště. Izoláty z pitné a teplé vody obsahovaly bakterii Legionella pneumophila s denzitou 102–103 KTJ/100 ml. Za jednoznačnou příčinu kontaminace vody byl označen nedostatečný ohřev vody v nezaregulovaném teplovodním systému. Účinným opatřením k nápravě byla termodezinfekce domovního vodovodního systému a chemodezinfekce pomocí instalovaného generátoru chlordioxidu.
Described is a case of legionellosis in an elderly male, that began with flu symptoms with consequent deterioration and hospitalization progressing to the IC unit. Only then was legionellosis diagnosed. Epidemiological investigations were focused on tap water in the patient's flat; samples taken confirmed the suspicion. Isolates from drinking water as well as warm tap water contained Legionella pneumophila organisms at 102–103 CFU/100 ml. The unequivocal cause of water contamination was the insufficient heating of the water in the unregulated hot water meins system. An effective remedy was thermal disinfection of the water mains system in the building and chemical disinfection with the aid of an installed generator of chlorine dioxide.

Legionářská nemoc je akutní bakteriální plicní onemocnění se závažným průběhem a vysokou mortalitou. Z epidemiologického hlediska má původ v komunitním, cestovním, nemocničním nebo pracovním prostředí. Nejvýznamnější rezervoáry legionel pro expozici osob představují chladicí věže a odpařovací kondenzátory, dále centrálně řešené vytápění a chlazení moderních budov. Autoři prezentují výsledky mikrobiologických vyšetření vody na přítomnost legionel v technologiích průmyslu plastikářského, sklářského, z chladicích věží. Ve vazbě na tyto výsledky vyhodnocují možné zdravotní dopady na exponované pracovníky. Ve všech vyšetřovaných provozech byly legionely nalezeny, a to kmeny patogenní i saprofytické. Největší potenciál možného zdravotního rizika má Legionella pneumophila séroskupina 1, podskupina A – Pontiac. Riziko expozice pracovníků je relativně nízké při běžném provozu, významně narůstá při operacích údržby a oprav. Se zjištěnou kolonizací prostředí legionelami korespondují v České republice sporadické případy hlášených profesionálních onemocnění legionelózou. Posuzování profesionality tohoto onemocnění musí vycházet z kritérií epidemiologické surveillance s využitím moderních diagnostických a identifikačních metod.
Legionnaire's disease is an acute bacterial disease affecting the lungs, with severe course and high mortality rates. In epidemiological terms, the disease originates in communal, travel, hospital, or occupational environments. The most important potential reservoirs of Legionella causing human exposure are cooling towers and evaporative condensers, as well as central heating and cooling systems in modern buildings. The authors present the results of microbiological tests of water for the presence of Legionella in industrial technologies (plastic injection molding, glass and cooling towers) and their potential health effects on exposed workers. Pathogenic and saprophytic Legionella strains were found in all investigated plants. The highest potential health risks are connected to Legionella pneumophila serogroup 1, subgroup A – Pontiac. During normal operation the risk of occupational exposure is relatively low, but it significantly increases during repairs and maintenance operations. The environmental Legionella colonization found in the Czech Republic corresponds to sporadic cases of reported occupational cases of legionellosis. Assessment of whether the disease is to be defined as occupational or not has to be based on criteria of epidemiological surveillance using modern diagnostic and identification methods.

The postantibiotic effects (PAE) of azithromycin, clarithromycin, ciprofloxacin, and levofloxacin were investigated against Legionella pneumophila (L. pneumophila) strains isolated from several hot water systems of different buildings in Istanbul. Each strain in logarithmic phase of growth was exposed to concentrations of antibiotics equal to minimum inhibitory concentration (MIC) and 4× MIC for 1 h. Recovery periods of test cultures were evaluated after centrifugation using the viable counting method. The mean values of PAEs for the strains of L. pneumophila, azithromycin at a concentration equal to and 4 times of MIC values were found 1.75 ± 0.28 h and 4.06 ± 0.44 h, for clarithromycin 2.98 ± 0.70 h and 4.18 ± 0.95 h, for ciprofloxacin 2.97 ± 0.63 h and 4.70 ± 0.63 h, for levofloxacin 2.05 ± 0.33 h and 3.78 ± 0.46 h, respectively. All of the antibiotics showed increased PAE values in a concentration-dependent manner. The findings of our study may play useful role in selecting the appropriate timing of doses during therapy with antimicrobials to treat patients infected with L. pneumophila.

Legionella pneumophila is increasingly recognized as a significant cause of sporadic and epidemic community-acquired and nosocomial-acquired pneumonia. This review focuses on the latest literature concerning the epidemiology, pathogenesis, clinical presentation, diagnosis, and treatment of Legionnaires' disease. RECENT FINDINGS: A significant increase in the incidence of Legionnaires' disease in the United States has been documented over the last years. L. pneumophila has recently been found to be a leading cause of community-acquired pneumonia in hospitalized and ambulatory patients in Germany. Recent studies provide insight into the understanding of the pathogenesis of Legionnaires' disease and the relevance of the formation of biofilms. Clinical manifestations of Legionnaires' disease are not specific and current diagnostic scores are of limited use. Several recent studies offer useful information concerning Legionnaires' disease in immunosuppressed patients. A systematic review of English literature performed to assess test characteristics of Legionella urinary antigen has found that the pooled sensitivity of the test was 0.74 and specificity was 0.991. Improved clinical response has been observed for patients with Legionnaires' disease treated with highly active antimicrobial agents against Legionella. SUMMARY: Legionnaires' disease is a significant health problem in many countries. Clinical manifestations are unreliable in diagnosing Legionnaires' disease. Therefore, diagnostic laboratory tests for Legionella, including the urinary antigen test, should be applied to all patients with pneumonia. Levofloxacin (or other fluoroquinolone) or azithromycin are the current drugs of choice for treatment of Legionnaires' disease. Effective preventive strategies are needed.

This study surveyed Legionella in 805 samples taken from 18 hot water systems under operating conditions. The results were analyzed and discussed in relation to water temperature, legislative requirements and optimization of the systems. The temperature of most samples (71%) ranged from 45-60 degrees C. The highest levels of colonization by L. pneumophila were found at water temperatures from 30-35 degrees C. At temperatures above 50 degrees C there was a large decrease in the number of positive samples, as well as the number of Legionella in individual samples. However, L. pneumophila was found in some samples having a temperature of 55-60 degrees C. These results indicate that the legislative requisite temperature of 50 degrees C for hot water systems is insufficient. A system operating temperature of 55 degrees C might be a better optimum, given the economical and safety limitations of temperatures as high as 60 degrees C. If it is impossible or ineffective to use classical method superheating (70-80 degrees C) then it is necessary take into account the chemical decontamination of water.
