Plasma cell leukaemia (PCL) is a rare and very aggressive plasma cell disorder. Preventing a dismal outcome of PCL requires early diagnosis with appropriate analytical tools. Therefore, the investigation of 33 patients with primary and secondary PCL was done when the quantity of circulating plasma cells (PCs) using flow cytometry (FC) and morphology assessment was evaluated. The phenotypic profile of the PCs was also analysed to determine if there is an association with clinical outcomes and to evaluate the prognostic value of analysed markers. Our results revealed that FC is an excellent method for identifying circulating PCs as a significantly higher number was identified by FC than by morphology (26·7% vs. 13·5%, P = 0·02). None of secondary PCL cases expressed CD19 or CD20. A low level of expression with similar positivity of CD27, CD28, CD81 and CD117 was found in both PCL groups. A decrease of CD44 expression was detected only in secondary PCL. Expression of CD56 was present in more than half of PCL cases as well as cytoplasmic nestin. A decreased level of platelets, Eastern Cooperative Oncology Group score of 2-3 and lack of CD20+ PC were associated with a higher risk of death. FC could be incorporated in PCL diagnostics not only to determine the number of circulating PCs, but also to assess their phenotype profile and this information should be useful in patients' diagnosis and possible prognosis.

AIM: To investigate potential associations between selected oncomarkers [carcinoembryonic antigen (CEA), C-terminus of cytokeratin 19 (CYFRA 21-1, CYFRA), and squamous cell carcinoma antigen (SCC)] and outcomes in patients with NSCLC treated with bevacizumab plus chemotherapy. PATIENTS AND METHODS: We retrospectively analysed 105 patients with NSCLC from the Czech TULUNG registry treated at University Hospital in Pilsen with bevacizumab plus chemotherapy. Response to therapy was tested by Fisher's exact test. Survival statistics were evaluated using the Kaplan-Meier method and Cox analysis. RESULTS: Only normal values of CYFRA (not CEA or SCC) were associated with significantly better overall and progression-free survival in univariate analysis. We also observed a trend for a better disease control rate in patients with normal levels of CYFRA. In a multivariate Cox model, only CYFRA was associated with significantly better overall but not progression-free survival. CONCLUSION: In our retrospective study, we point out the possibility of using CYFRA as a prognostic marker in patients with NSCLC treated with chemotherapy plus bevacizumab.

Východiska: Nádorové antigeny testis (CTA) jsou považovány za nádorové biomarkery z důvodu jejich vysoce specifické exprese u lidských malignit a jelikož se téměř nevyskytují v normálních somatických tkáních. Díky své specifické expresi umožňují v posledních letech lépe stanovit včasnou diagnózu, prognózu pacientů a léčbu rakoviny. Lactobacily jsou skupina probiotik s protinádorovými, imunomodulačními a dalšími prospěšnými vlastnostmi. Bylo prokázáno, že tyto bakterie mění expresi několika genů souvisejících s nádory. Cíl: Po synchronizaci buněk HeLa pomocí MTT [3-(4,5-dimethylthiazol-2-yl)-2,5-difenyltetrazoliumbromid] jsme pomocí kvantitativní polymerázové řetězové reakci v reálném čase zkoumali vliv supernatantu Lactobacillus rhamnosus GG (LRS) a supernatantu Lactobacillus crispatus SJ-3C-US (LCS) na expresi čtyř CTA (TSGA10, AURKC, OIP5 a AKAP4). Výsledky: LRS a LCS inhibovaly růst buněk HeLa po 24 hod, což bylo prokázáno pomocí MTT testu. Exprese všech CTA byly po léčbě oběma supernatanty nižší. Závěr: Tato studie prokázala úlohu laktobacilů při snížení exprese genů CTA. Taková změna exprese může být zapojena do protinádorových účinků těchto laktobacilů. Základní mechanismy těchto pozorování nejsou jasné, ale v tomto procesu se mohou účastnit epigenetické modulační mechanismy. K posouzení funkčních rolí laktobacilů v modulaci jiných genů souvisejících s nádory je třeba dalších studií.
Background: Cancer testis antigens (CTAs) are considered cancer biomarkers due to their highly specific expression pattern in human malignancies and near absence from normal somatic tissues. Their specifi c expression has made them potential targets for early diagnosis, assessment of patients' prognosis and treatment of cancer in recent years. Lactobacilli are a group of probiotics with anti-cancer, immunomodulatory and other beneficial features. These bacteria have been shown to alter expression of several cancer-related genes. Aim: We investigated the eff ect of Lactobacillus rhamnosus GG supernatant (LRS) and Lactobacillus crispatus SJ-3C- -US supernatant (LCS) on expression of four CTAs (TSGA10, AURKC, OIP5 and AKAP4) in HeLa cell line after synchronization using MTT [3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide] assay and quantitative real-time polymerase chain reaction. Results: LRS and LCS inhibited HeLa cell growth after 24 h as demonstrated by MTT assay. Expressions of all CTAs were down-regulated after treatment with both supernatants. Conclusion: This study showed the role of Lactobacilli in down-regulation of CTAs genes. Such expression change might be involved in the anticancer effects of these Lactobacilli. The underlying mechanisms of these observations are not clear but epigenetic modulatory mechanisms may participate in this process. Future studies are needed to assess functional roles of Lactobacilli in modulation of other cancer-related genes.

In order to select a suitable combination of cancer cell lines as an appropriate source of antigens for dendritic cell-based immunotherapy of ovarian cancer, we analyzed the expression level of 21 tumor associated antigens (BIRC5, CA125, CEA, DDX43, EPCAM, FOLR1, Her-2/neu, MAGE-A1, MAGE-A2, MAGE-A3, MAGE-A4, MAGE-A6, MAGE-A10, MAGE-A12, MUC-1, NY-ESO-1, PRAME, p53, TPBG, TRT, WT1) in 4 established ovarian cancer cell lines and in primary tumor cells isolated from the high-grade serous epithelial ovarian cancer tissue. More than 90% of tumor samples expressed very high levels of CA125, FOLR1, EPCAM and MUC-1 and elevated levels of Her-2/neu, similarly to OVCAR-3 cell line. The combination of OV-90 and OVCAR-3 cell lines showed the highest overlap with patients' samples in the TAA expression profile.

Tradiční histopatologická diagnostika karcinomu prsu se v posledních letech rozšířila o výsledky dalších metod doplňujících morfologické zhodnocení nádoru. Nedílnou součástí histopatologické diagnózy jsou tak dnes výsledky vyšetření exprese hormonálních receptorů, proteinu HER-2/neu a proliferačního antigenu Ki67. Kritickým momentem pro korektní využití těchto vyšetření je dodržení podmínek preanalytické fáze - tj. optimální způsob fixace a dále zohlednění heterogenního charakteru nádorové populace. Kromě této dnes již rutinní praxe se v posledních letech rozšiřují snahy o zohlednění molekulárních charakteristik nádorů a jejich využití v klasifikaci nádorů a v predikci úspěšnosti protinádorové léčby. Většina prací se opírá o stanovení tzv. genových expresních profilů. Na základě průkazu zvýšené či snížené exprese velkého počtu genů je možné najít sadu několika genů, která nejlépe koreluje s biologickými vlastnostmi nádoru. Takto bylo možné identifikovat 4 základní skupiny karcinomů - luminální, bazální (basal-like), HER-2 pozitivní (HER-2 enriched) a podobné normální prsní žláze (normal-like). Počet kategorií se následně rozšířil - původně jednotná skupina luminálních karcinomů se rozpadla na luminální A a B, případně i luminální C. Také v rámci bazálních karcinomů byly nalezeny další podskupiny. Výsledky studií zabývajících se analýzou genových expresních profilů nádorů ale ukazují, že naše porozumění biologii karcinomu prsu je teprve na začátku. V různých publikacích jsou jednotlivé kategorie různě definovány, a proto je srovnávání jejich výsledků velmi problematické. Další možností molekulární klasifikace karcinomů prsu je imunohistochemická analýza exprese jednotlivých proteinů, používaných jako náhradní parametr (surrogate marker) místo detekce mRNA jednotlivých genů. Výhodou je možnost využití archivního parafinového materiálu a řádově nižší cena. Limitací je naopak nemožnost paralelní detekce tisíců markerů, jak je tomu u genomického profilování. Výsledky molekulárně klasifikačních studií však nejsou zcela zásadně překvapivé, již z minulosti je známo, že nádorová kmenová buňka u karcinomu prsu se může diferencovat směrem myoepiteliálním (neboli bazálním) a luminálním. Oba směry se liší mj. rozdílnou expresí cytoskeletálních proteinů a dalších molekul. Molekulární klasifikace karcinomů prsu - ať opírající se o různé sady genových expresních profilů nebo o extenzivní imunohistochemické vyšetření řady markerů - výsledky těchto prvních morfologických studií potvrdily. Výsledky výzkumu v oblasti genových expresních profilů se nicméně poměrně rychle promítly do klinické praxe. V současné době je komerčně dostupných několik certifikovaných testů sloužících jako doplňková informace při klinickém rozhodování o léčbě.
Traditional histopathological diagnosis of breast cancer has been extended in recent years through the results of additional methods. Today, the results of the detection of hormone receptors, HER-2/neu, and Ki67 antigen are thus an integral part of the histopathological diagnosis. A critical factor for the success of these tests is the fulfillment of pre-analytical phase conditions - i.e. optimal fixation, as well as taking into account the heterogeneous nature of the neoplastic population. In addition to the above-mentioned markers - which have become a routine practice in recent years, there are many efforts to include the molecular characteristics of tumors both in tumor classification as well as in the prediction of results of cancer treatment. Most of the work is based on the use of gene expression profiles. On the basis of the detection of increased or decreased expression of a large number of genes, it is possible to find a set of multiple genes correlating with the biological behavior of the tumor. Using this approach, four basic subgroups of breast cancer have been identified - luminal, basal-like, HER-2 enriched and normal gland-like. Over the course of time, the number of molecular categories has expanded - originally a homogenous group of luminal cancers has been subclassified into the luminal A, B and C. Also within basal-like carcinomas additional subgroups have been identified. However, the results of studies dealing with the analysis of gene expression profiles suggest that our understanding of the biology of breast cancer is far from being complete. The individual categories are defined differently in various publications and thus the comparison of the results of these studies is very difficult. Another approach for the molecular classification of breast cancer is the immunohistochemical detection of various proteins used as a surrogate marker instead of the detection of the mRNA of individual genes. The advantage of this approach is the possibility to use even archive material, as well as much lower costs. On the other hand, its main limitation is the inability of parallel detection of thousands of markers, unlike in genomic profiling. The results of molecular classification are, however, not fundamentally surprising. The fact that breast cancer tumor stem cells can differentiate towards myoepithelial (or basal) and luminal cells has been known for a long time. These two lines of differentiation are - among others - characterized by differential expression of cytoskeletal proteins as well as of other molecules. These findings have been confirmed by the results of molecular studies - either those based on gene expression profiling or immunohistochemical ones. Research results in gene expression profiling have relatively quickly translated into clinical practice. At present, several commercially available certified tests serve as a complementary source of information for decisions about clinical treatment.

BACKGROUND: In recent years different IRT/PAP protocols have been evaluated, but the individual performance remains unclear. To optimize the IRT/PAP strategy we compared protocols from three regional CF newborn screening centers (Heidelberg, Dresden, and Prague). METHODS: We evaluated the effect of elevating the IRT-cut-off from 50 to 65 μg/l (~97.5th to ~99.0th percentile), the need of a failsafe protocol (FS, IRT ≥ 99.9th percentile) and the relative performance using either two IRT-dependent PAP-cut-offs or one PAP-cut-off. FINDINGS: Elevation of the IRT cut-off to 65 μg/l (~99.0th percentile) increased the PPV significantly (Dresden: 0.065 vs. 0.080, p < 0.0001, Prague: 0.052 vs. 0.074, p < 0.0001) without reducing sensitivity. All three IRT/PAP protocols showed a trend towards a higher sensitivity with FS than without and when using one PAP-cut-off instead of two IRT-dependent PAP-cut-offs. CONCLUSIONS: For best performance we suggest an IRT/PAP protocol with an IRT-cut-off close to the 99.0th percentile, FS, and a single PAP-cut-off.

Průtoková cytometrie je moderní analytická metoda používaná v klinických i výzkumných laboratořích k imunofenotypizaci vybraných buněčných subpopulací v suspenzi (periferní krve, aspirátu kostní dřeně, mozkomíšního moku atd., ale i buněčné suspenzi získané z nefixovaného solidního tumoru). Poskytuje současně informace o četných povrchových nebo intracelulárních znacích analyzovaných elementů. V klinické praxi k tomu využívá fluorescenčních monoklonálních protilátek proti povrchovým nebo intracelulárním antigenům, které jsou asociovány s určitým typem buněk (T-lymfocyty, B-lymfocyty, apod.), jejím vývojovým stádiem nebo monoklonalitou (např. lehké řetězce povrchových imunoglobulinů). Pravděpodobně nejširšího využití dosáhla metoda v diagnostice hematologických malignit, kde právě rychlá a přesná diagnostika dává lékaři stěžejní informace ke správnému stanovení diagnózy a k výběru optimální léčby, následně lze takřka v reálném čase sledovat efekt léčby. Flow cytometry represents a modern analytical method useful for an assessment of selected cellular subpopulations in suspension (peripheral blood, aspirate of bone marrow, liquid fluid etc. and also in suspensions prepared from non-fixed solid tumors) in clinical and research laboratories. The method provides information on numerous surface or intracellular markers of the analyzed elements at the same time. The usage of fluorescent monoclonal antibodies against surface or intracellular antigens associated with specific type of cells (T-lymphocytes, B-lymphocytes etc.), with their developmental stage, or with monoclonality (for example antibodies against light chains of immunoglobulins), are the most valuable for the clinical practice. The most important application of flow cytometry in pathology has remained in hematologic malignancies, where this fast and exact method provides, practically in a real time, crucial information for correct diagnosis as well as for the choice of optimal therapy, and subsequently for assessing the effect of the therapy. Multiple myeloma (MM) is a haematological malignancy characterized by the accumulation of clonal plasma cells (PCs) in the bone marrow (BM). Although novel therapeutic strategies have prolonged survival of patients, the disease remains difficult to treat with a high risk of relapse. The failure of therapy is thought to be associated with a persistent population of the so-called MM stem cells or myeloma initiating cells (MIC) that exhibit tumour-initiating potential, self-renewal and resistance to chemotherapy. However, the population responsible for the origin and sustainability of tumour mass has not been clearly characterized so far. This review summarizes current myeloma stem cell concepts and suggests that high phenotypic and intra-clonal heterogeneity, together with plasticity potential of MM might be other contributing factors explaining discrepancies among particular concepts and contributing to the treatment failure.
