Chondrální defekty jsou na kolenním kloubu poměrně častým nálezem a jejich terapie je obtížná. Dobrému hojení těchto defektů brání nízký hojivý potenciál hyalinní chrupavky. Metod chirurgického ošetření těchto defektů bylo popsáno mnoho. Na našem pracovišti používáme od roku 2008 kombinaci heterologních implantátů (scaffoldů) s abrazivními metodami. V roce 2008 jsme poprvé použili implantát Chondrotissue. Roku 2013 jsme následně publikovali dobré výsledky u 52 pacientů léčených touto metodou. V roce 2016 jsme poprvé implantovali implantát Biomatrix CRD – bifázický scaffold. Mezi lety 2016 a 2019 jsme tento implantát použili u 10 pacientů. Výsledky jsme hodnotili s minimálním odstupem šesti měsíců od operace. Jako hodnotící skórovací systém jsme používali Lysholmovo skóre. S výsledkem 91,2 dle Lysholma se jedná o pooperační výsledky hodnocené jako dobré. Ve srovnání s dříve hodnocenou skupinou ošetřenou implantátem Chondrotissue jsme po implantaci Biomatrix CRD dosahovali lepších výsledků.
Chondral defects of the knee are a fairly common phenomenon and their therapy is difficult. Their healing is prevented by a low healing potential of hyaline cartilage. Many methods of surgical treatment have been described for these defects. In our facility we have been using a combination of heterologous implants (scaffolds) and abrasive methods since 2008. In 2008, we used the Chondrotissue implant for the first time. We then published favourable outcomes in 52 patients treated using this method in 2013. In 2016, we implanted the Biomatrix CRD, a biphasic scaffold, for the first time. Between 2016 and 2019 we used this implant in 10 patients. We evaluated the results no sooner than after six months from the surgery. We used the Lysholm scoring scale on which we achieved a score of 91.2, i.e. good postsurgical results. In comparison to the previously evaluated Chondrotissue implant group, we achieved better results after implanting the Biomatrix CRD..

Úvod: Autoři v práci předkládají výsledky klinických, morfologických a biochemických vyšetření po léčbě defektů kloubní chrupavky kolenního kloubu léčených metodou implantace solidního chondrograftu v porovnání s léčbou prostými návrty spodiny defektu. V experimentální části se zaměřují na stanovení typu novotvořené tkáně vzniklé v místě defektu pomocí imunohistochemického a molekulárněbiologického průkazu přítomnosti kolagenu I. a II. typu a stanovení přítomnosti proteolytických enzymů (matrix metaloproteináz), které se podílí na reparativních procesech v novotvořené chrupavce. Na základě těchto vyšetření poté formulují doporučení pro klinickou praxi. Metoda a materiál: Sledovaný soubor tvoří celkem 87 pacientů s chondrálním nebo osteochondrálním defektem v oblasti kolenního kloubu, kteří podstoupili metodu implantace solidního chondrograftu. Mezi pacienty je 52 mužů a 35 žen ve věkovém rozmezí 16-50 let, průměrný věk 29 let. Interval pooperačního sledování činí 12-173 měsíců, průměrná doba sledování je 48 měsíců. U 27 pacientů byl při reartroskopické operaci odebrán vzorek chrupavky z místa implantace solidního chondrograftu a tyto vzorky byly dále histologicky a imunohistochemicky vyšetřeny. Ke klinickému zhodnocení výsledků operace bylo použito klasifikačního skóre podle Lysholma před operací a v odstupu jednoho roku, dvou let a pěti let po operaci. U tří pacientů nebyl odebraný vzorek hodnotitelný. Jako kontrolní soubor byla stejným způsobem hodnocena skupina osmi pacientů (pět mužů, tři ženy ve věku 17-42 let, průměrný věk 30 let) ošetřených klasickými návrty osteochondrálních defektů podle Pridieho. V jednom případě byl odebraný vzorek nehodnotitelný. Výsledky: Průměrné předoperační skóre podle Lysholma v našem souboru 67 pacientů s chondrálním defektem v oblasti kondylů femuru či tibie činí 39,5. V jednom roce po operaci dochází ke zlepšení na 81,6. S odstupem dvou let po operaci činí u 52 dále sledovaných pacientů skóre 84,3. U 31 pacientů po pěti letech od operace činí průměrné skóre 84,8. U sledovaných pacientů s defektem na patele (20 operovaných) bylo předoperační skóre podle Lysholma průměrně 38,5, v jednom roce 80,4, po dvou letech 82,5 a u 13 pacientů sledovaných pět let po operaci vyšlo skóre 80,1. V souboru 24 vzorků tkáně odebrané z místa implantace solidního chondrograftu byl imunohistochemickým vyšetřením prokázán kolagen typu II ve všech 24 případech, kolagen typu I byl u tří vzorků z této skupiny. Histologickým vyšetřením byla hyalinní chrupavka potvrzena ve všech 24 vzorcích. Skupina sedmi kontrolních vzorků po návrtech vykazovala rozmanitý patomorfologický obraz a expresi kolagenu I. a II. typu. Ve dvou případech byla nalezena vazivová chrupavka, v níž Western blotting odhalil přítomnost kolagenu I. i II. typu, ve třech případech chrupavku hyalinní bez obsahu kolagenu I. typu, a ve dvou případech byla zastižena pouze vazivová tkáň, která nevykazovala imunohistochemickou pozitivitu s protilátkou proti kolagenu II. typu. U žádného vzorku z obou skupin nebyla imunohistochemicky prokázána exprese matrix metaloproteináz (MMP-1, MMP-3, MMP-13). Závěr: Na základě těchto klinických, morfologických, imunohistochemických a molekulárněgenetických vyšetření jednoznačně vyplývá vhodnost aplikace chondrograftu při defektech kloubních chrupavek u pacientů všech věkových skupin. Pouze u mladých pacientů (1. a 2. decennium) s malým defektem kloubní chrupavky nezasahující subchondrální kost je možné zvolit ošetření metodou prostých návrtů spodiny defektu.
Introduction: The authors present the results of clinical, morphological and biochemical examinations after treating joint knee joint cartilage defects treated with the solid chondrograph implantation method as compared to simple prosthesis recovery. In the experimental part, they focus on the determination of the type of newly formed tissue produced at the defect site by immunohistochemical and molecular-biological evidence of the presence of collagen types I and II and the presence of proteolytic enzymes (matrix-metalloproteinases) involved in reparative processes in newly formed cartilage. Based on these examinations, they then formulate recommendations for clinical practice. Method and Material: A total of 87 patients with a chondral or osteochondral defect in the knee joint area who underwent a solid chondrograph implantation method were studied. There were 52 men and 35 women in the age range of 16-50 years, with an average age of 29 years. The postoperative follow-up interval was 12-173 months, with an average follow-up time of 48 months. In 27 patients, a cartilage sample from the solid chondrograph implantation was taken during a rearthroscopic operation and these samples were further examined histologically and immunohistochemically. Clinical evaluation of the results of the operation was based on the Lysholma classification score before and at 1 year, 2 years and 5 years after surgery. In 3 patients, the sample was not evaluated. A group of 8 patients (5 males, 3 females aged 17-42 years, mean age 30 years) treated with classical osteochondral defects recovered by Pridie were evaluated as the control group. In 1 case, the sample was unobservable. Results: The average preoperative score according to Lysholm in our group of 67 patients with chondral defect in the area of the femur or tibia is 39.5. One year after surgery, the improvement reached 81.6. Two after the surgery, the score for the 52 followed-up patients was 84.3. In 31 patients at 5 years after surgery, the average score was 84.8. In the patients with a defect of the patella (20 patients) the preoperative Lysholm scores went from 38.5 on average, to 80.4 one year after the surgery, 82.5, after two years, and 13 patients studied at 5 years after surgery had the published score of 80.1. In a set of 24 tissue samples collected from the implant site of solid chondrografts, collagen type II was demonstrated by immunohistochemistry, in all 24 cases, type I collagen in 3 samples from this group. By histological examination, hyaline cartilage was confirmed in all 24 samples. The group of 7 control samples showed a diverse patomorphological image and expression of collagen type I and II. In 2 cases, connective tissue cartilage was found and Western blotting revealed the presence of collagen type I and II in the hyaline cartilage 3 cases without collagen type I, and in 2 cases were encountered only fibrous tissue, which did not show immunohistochemical positivity with anti-collagen type II. Mammalian metalloproteinases (MMP-1, MMP-3, MMP-13) were not immunohistochemically demonstrated in any of the two groups. Conclusion: Based on these clinical, morphological, immunohistochemical and molecular-genetic examinations, the suitability of chondrography for articular cartilage defects in patients of all age groups is clearly demonstrated. Only young patients (1st and 2nd decennium) with a small defect of articular cartilage, non-subchondral bone may be treated with the simple back deflection method.

Autoři v návaznosti na práci o konzervativní léčbě artrózy shrnují možnosti operační léčby poúrazových a degenerativních chondrálních defektů. Abrazivní metody popsané již v 60. letech minulého století jsou využívány dodnes, zlepšení klinického stavu postiženého kolenního kloubu je jen krátkodobé. Uplatňují se i jako kostní dřeň stimulující výkon i u metod transplantačních. Transplantace jsou preferovány v případě fokálních poúrazových chrupavčitých plošných defektů nebo u lehkých až středně těžkých artrotických postižení nosných kloubů dolní končetiny u mladších jedinců. Přehojený chrupavčitý defekt obsahuje vazivovou tkáň, hyalinní chrupavce podobnou tkáň a v případě mozaikové plastiky kombinaci obou uvedených s hyalinní chrupavkou na povrchu transplantovaných bločků. Při správné kloubní biomechanice je klinické zlepšení pacientů střednědobé ve srovnání s abrazivní artroplastikou. Implantace umělé kloubní náhrady je indikována u těžkých destrukcí kloubního povrchu.
In relation to the article about conservative treatment of arthrosis, the authors describe the possibilities of surgical treatment ofpost-traumatic and degenerative chondral defects. Abrasive methods used since the 1960s are still being used to the present timewith a short-term improvement in a clinical condition. This methods additionally function as bone marrow stimulating performanceagents when used for transplantations. Transplantation is preferred in focal post-traumatic cartilage flat defects or in mildto moderate arthrosis of lower limb joints in younger individuals. The transplanted cartilaginous defect contains connective tissue,hyaline cartilage-like tissue and, in the case of mosaicplasty, a combination of both of these with hyaline cartilage on the surface ofthe transplanted blocks. In case of correct joint biomechanics there is a mid-term clinical improvement in patients in comparisonwith abrasive arthroplasty. Implantation of artificial joint replacements is preferred for severe destruction of the joint surface.

UNLABELLED: PUPOSE OF THE STUDY The treatment of osteochondral lesions of weight-bearing joints remains a serious therapeutic challenge, largely due to the minimal ability of articular hyaline cartilage to regenerate. The authors present the long-term clinical and MRI results of treating deep chondral and osteochondral defects of the ankle joint by the method of implantation of autologous chondrocytes in the form of a solid chondrograft. MATERIAL AND METHODS The method of solid chondrograft implantation in the ankle joint was used in our Department from the year 2003. Between 2003 and 2013, this method was used in 31 patients, 16 men and 15 women. Their average age at the time of implantation was 29 years (16 to 50 years). The follow-up period ranged from 16 to 145 months (average, 57 months). The clinical outcome was evaluated using the Mazur questionnaire. At follow-up all patients underwent regular MRI examinations and the results were assessed on the basis of Magnetic Resonance Observation of Cartilage Repair Tissue (MOCART) scores. Twelve patients required a second-look arthroscopy. RESULTS A comparison of the Mazur pre-operative scores with those obtained at 1, 2 and 5 years post-operatively showed marked improvement of ankle joint function. The average pre-operative value of 30.0 (based on responses of 31 patients) increased to the average of 89.7 (based on results of 11 patients). At 1 year post-operatively, the average MOCART score for a group of 18 patients was 78.3; at 5 post-operative years, the average value for nine patients examined was 77.0. Complete filling of defects at 1 year of follow-up was found in 88.1% and , at 5 years, it was recorded in 83.3% of the patients examined. DISCUSSION The articular hyaline cartilage is a highly differentiated tissue and its ability of repair is very limited. Therefore every damage to the articular surface should be regarded as a pre-arthritic condition// disease. Currently, there are several options of treating a damaged articular cartilage, but none of them makes its complete healing certain. A lot of studies concerned with longterm results of implanting autologous chondrocytes in the knee are available in the literature, but only few authors present long-term clinical and radiographic outcomes of ankle joint treatment similar to ours. CONCLUSIONS Based on our clinical and MRI results, the method of autologous chondrocyte implantation can be recommended since it has good long-term results, provides repair of articular cartilage and allows for patients' return to activities of daily living. Our study compares the histological and immunohistochemical cellular composition of two different chondrocyte-seeded biomaterials and the results of their transplantation. Our study cohort included 21 patients, comprising 19 men and two women with a mean age of 32 years, who were affected by single chondral lesions of the femoral condyles. These patients were enrolled in our study and treated with arthroscopic implantation of the tissue Hyalograft C and/or Brno culture. Brno culture bioengineered with a fibrin-based scaffold contains round cells showing features of differentiated chondrocytes expressing S-100 protein and α-smooth muscle actin. In contrast, in the case of Hyalograft C, the scaffold was made up of a fibrillar network composed of biomaterial fibres of the esters of hyaluronic acid and cells resembling fibroblasts and myofibroblasts and expressing only α-smooth muscle actin. The average size of the defects was 2.5 cm2. Patients were evaluated using the standardized guidelines of the International Knee Documentation Committee. During the comparison of bioptic samples obtained from both patient cohorts, we did not observe any important differences in the histological makeup of the newly formed cartilage. The histological analysis of these two groups of homogeneous patients shows that this bioengineered approach, under proper indications, may offer favourable and stable clinical results over time, in spite of the different matrix and cellular composition of the two transplants used.
