Endotel má mimo ochranné i velmi komplexní parakrinní, endokrinní a autokrinní funkce. Z hlediska cévního tonu lze funkce endotelu chápat jako rovnováhu mezi produkcí vazodilatačních a vazokonstrikčních substancí. Mezi nejdůležitější vazodilatační látky patří oxid dusnatý (NO), PGI2, EDHF (endotel-deriving hyperpolarising factor), adenozin a další, mezi nejvýznamnější vazokonstrikční pak endotelin 1 a angiotenzin 2. Cílem léčebného ovlivnění sekreční funkce endotelu by měla být maximalizace produkce vazodilatancií a minimalizace produkce vazokonstrikčních látek. Zlepšení endotelové funkce lze nejúčinněji dosáhnout dodržováním zdravého životního stylu (změna diety na středomořský typ, dostatečná fyzická aktivita, redukce hmotnosti a zanechání kouření) a v indikovaných případech zahájením léčby ACE inhibitory a statiny. Řada studií prokázala, že všechna tato opatření zlepšila prognózu nemocných a velký díl tohoto efektu má na svědomí právě ovlivnění endotelové dysfunkce.
Endothelium has both a protective and a very complex para-, endo- and autocrine functions. Vascular tone is determined as a balance between the production of vasodilating and vasoconstrictive substances. Among the most important vasodilators are nitric oxide (NO), PGI2, endothelial-derived hyperpolarizing factor (EDHF), adenosine and others, among the most important vasoconstrictors, endothelin-1 and angiotensin-2. Therapeutic goal of improving endothelial secretory function should be maximizing production of vasodilators and minimizing production of vasoconstrictors. Improvement of endothelial function can be achieved most effectively by following a healthy lifestyle (changing the diet to a Mediterranean type, sufficient physical activity, weight reduction and smoking cessation) and by initiating the treatment with ACE inhibitors and statins, when indicated. Large number of trials have shown that all of these interventions improve the prognosis of patients and a part of this effect may be mediated by improving endothelial dysfunction.

(i) Purpose. The fluid challenge (FC) is a well-established test of preload reserve. Only limited data exist in regard to the FC efficacy based on infusion time. Slow administration may be associated with lack of effect based on fluid redistribution and external conditions changes. On the contrary, fast administration may lead to brisk fluid overload and damage to the endothelium and endothelial glycocalyx (EG). The aim of this trial was to compare the FC infusion time on its hemodynamic effects and EG. (ii) Methods. Prospective randomized single-center trial of fast (5-10 minutes) versus slow (20-30 minutes) administration of 500ml balanced crystalloid FC in spinal surgery (cohort OR) and septic shock (cohort SEP) patients. Hemodynamic response was assessed using standard monitoring and blood flow measurements; damage to EG was assessed using the perfused boundary region (PBR) via intravital microscopy monitoring in the sublingual region within relevant time points ranging up to 120 minutes. (iii) Results. Overall, 66 FCs in 50 surgical and 16 septic patients were assessed. Fluid administration was associated with increase of PBR in general (1.9 (1.8-2.1) vs. 2.0 (1.8-2.2); p= 0.008). These changes were transient in OR cohort whereas they were long-lasting in septic fluid responders. The rate of fluid responsiveness after fast versus slow administration was comparable in global population (15 (47%) vs. 17 (50%); p=0.801) as well as in both cohorts. (iv) Conclusions. Fluid challenge administration was associated with increased PBR (and presumable EG volume changes) which normalized within the next 60 minutes in surgical patients but remained impeded in septic fluid responders. The fluid responsiveness rate after fast and slow FC was comparable, but fast administration tended to induce higher, though transient, response in blood pressure.

Maintenance of norepinephrine (NE)-induced contraction is dependent on Ca2+ influx through L-type voltage-dependent Ca2+ channels (VDCC), which is opposed by nitric oxide. Adrenergic receptors are coupled with different G proteins, including inhibitory G proteins (Gi) that can be inactivated by pertussis toxin (PTX). Our study was aimed to investigate the effects of endothelium removal, PTX pretreatment and acute VDCC blockade by nifedipine on the contractions of femoral arteries stimulated by norepinephrine. We used 12-week-old male WKY, half of the rats being injected with PTX (10 µg/kg i.v., 48 h before the experiment), which considerably reduced their blood pressure (BP). Contractions of isolated arteries were measured using Mulvany-Halpern myograph. NE dose-response curves determined in femoral arteries from PTX-treated WKY rats were shifted to the right compared to those from control WKY. On the contrary, removal of endothelium augmented NE dose-response curves shifting them to the left. Acute VDCC blockade by nifedipine (10-7 M) abolished all differences in NE dose-response curves which were dependent on the presence of either intact endothelium or functional Gi proteins because all NE dose-response curves were identical to the curve seen in vessels with intact endothelium from PTX-treated animals. We can conclude that BP reduction after PTX injection is accompanied by the attenuation of NE-induced contraction of femoral arteries irrespective of endothelium presence. Moreover, our data indicate that both vasodilator action of endothelium and Gi-dependent vasoconstrictor effect of norepinephrine operate via the control of Ca2+ influx through VDCC.

Východisko. V pilotní studii byla použita nová metoda neinvazivního měření endoteliální dysfunkce pomocí analýzy rychlostních křivek, získaných dopplerometrickým měřením gradientů a. brachialis. Metody a výsledky. Po přechodné mechanické okluzi předloktí byl hodnocen index decelerace toku krve v brachiální arterii mezi 1. a 2. minutou postokluzní periody – tzv. decelerační index . Do studie bylo zařazeno18 probandů s dosud neléčenou hypercholesterolémií. Po 6 měsíční léčbě fluvastatinem se významně změnil charakter rychlostní křivky, decelerační index se zvýšil z původních cca 3 % na více než 7 % (p<0,05), což bylo v asociaci nejen s předpokládaným snížením hladin LDL cholesterolu, ale také se signifikatním poklesem biochemických markerů endoteliální dysfunkce (trombomodulin a von-Willebrand faktor). Závěry. Výsledek je v souladu s dosud publikovanými studiemi, prokazujícími zlepšení endoteliální funkce při léčbě statiny pomocí jiných sonografických metod.
Background. A new method of noninvasive assessment of endothelial dysfunction was used in this pilot study. Study has been set on a. brachialis dopplerometry velocity curves analysis. Index of blood stream deceleration between the first and second minute of the postocclusion periode was measured on a. brachialis after transient mechanical occlusion of the forearm – so called Deceleration Index. Methods and Results. Eighteen patients with so far untreated hypercholesterolemia were involved in our study. Profile of velocity curves became significantly different after six months of fluvastatin therapy, Deceleration Index increased from 3 % to more than 7 % (p<0.05). That correlated with LDL-CH levels decrease, but also with the significant decrease of biochemical markers of endothelial dysfunction (trombomodulin, von Willebrand factor). Conclusions. The result is corresponding with previously published studies that have proved endothelial function improvement after statin therapy using another ultrasonographic methods.
