Maligní pleurální výpotek (MPV) se vyskytuje u značné části pacientů s nádorovým onemocněním, preferenčně u některých typů nádorů. Diagnostika MPV se opírá o zobrazovací vyšetření, u většiny pacientů je ale nutná i intervenční metoda (pleurální punkce, biopsie pleury, medicínská torakoskopie nebo video-asistovaná torakoskopie) s odběrem materiálu k cytologickému a/nebo histologickému vyšetření. Management MPV se odvíjí od rozsahu, rychlosti doplňování a míry symptomů pacienta, i od dalších okolností vztahujících se k aktuálnímu stavu pacienta a možnostem protinádorové léčby. U pacientů s rekurentním MPV je z léčebných opatření na prvním místě talková pleurodéza pohrudniční dutiny, na stejné úrovni doporučení se nachází zavedení tunelizovaného hrudníku drénu (IPC). U pacientů se současnou uvězněnou plící ("trapped lung") je metodou volby zavedení IPC. V poslední dekádě byl získán velký objem vědeckých poznatků, které lze aplikovat v rutinní klinické praxi. Cílem tohoto přehledového článku je představit aktuální trendy a doporučení v diagnostice a managementu MPV v souladu s aktuálním stavem poznání.
Malignant pleural effusion (MPE) occurs in a significant proportion of patients with malignancy, more frequently in certain types of tumors. Diagnosis of MPE is based on imaging, but in most patients, a pleural intervention is required (thoracentesis, pleural biopsy, medical thoracoscopy or video-assisted thoracoscopy) for cytology and/or histology sampling. Management of MPE depends on the fluid volume, patient's symptoms and reaccumulation rate. Further considerations include patient ́s general health condition, life expectancy and anticancer treatment options. In patients with recurrent MPE, talc pleurodesis or insertion of an indwelling pleural catheter (IPC) are recommended, with comparable efficacy and level of scientific evidence. In patients with MPE and "trapped lung", IPC placement is the treatment of choice. In the last decade, an extensive amount of scientific data regarding MPE management has been collected, resulting in improvements in the daily practice. The aim of this review is to present current trends and recommendations in the diagnosis and management of MPE in agreement with the current state of knowledge.

OBJECTIVE: This narrative review aims to describe and compare different interventional methods for the management of recurrent malignant pleural effusion and offers perspectives for the future era. BACKGROUND: Dyspnea as a result of the recurrent malignant pleural effusion is one of the main factors decreasing the quality of life in patients with oncologic diseases. To date, there is no strict guideline for the management of malignant pleural effusions. RESULTS: Several different techniques are available to prevent production of the effusion or to provide intermittent drainage, however, the principle of these methods remains palliative. The choice of treatment in any patient depends mainly on the preferences of the patient, speed of the fluid production, expandability of the lung, and predicted survival of the patients. The interventional methods of managing malignant pleural effusions are described in detail, including thoracocentesis, chemical pleurodesis, talc poudrage pleurodesis, slurry pleurodesis, thoracoscopic procedures, indwelling pleural catheters, implantable pleural ports, and pleuroperitoneal shunting. CONCLUSION: Pleurodesis and fully implantable devices such as pleural ports may become the most useful techniques in the future, mainly because of better comfort for the patients and no need for repeated pleural punctures (Ref. 55).

Maligní pleurální výpotek u pacientů s onkologickým onemocněním je známkou nepříznivé prognózy. U symptomatických pacientů máme několik paliativních možností řešení výpotku vedoucích ke zlepšení kvality života. Podle současného konsensu a doporučení není žádná metoda striktně preferována. Rozhodnutí o managementu u každého pacienta závisí na preferenci pacienta, rychlosti tvorby výpotku, expandibilitě plíce a na předpokládané době přežití.
Malignant pleural effusion in patients with oncological disease is a sign of an unfavorable prognosis. In symptomatic patients, we have several palliative options for dealing with the effusion leading to an improvement in quality of life. According to current consensus and guidelines, no method is strictly preferred. Management decisions for each patient depend on patient preference, rate of effusion formation, lung expandability, and expected survival time.
Maligní pleurální mezoteiiom patří bohužel stále mezi onemocnění se všeobecně špatnou prognózou. V tuto chvíli neexistují doporučení, která by jednoznačně definovala optimální léčebnou strategii. Z publikovaných dat se zdá, že multimodální přístup kombinující neoadjuvantní či adjuvantní chemoterapii na bázi platinového derivátu a pemetrexedu, chirurgickou resekci a eventuální peroperační laváž pleurální dutiny chemoterapeutikem přináší pacientům největší benefit. I když jde o přístupy, které jsou stále určeny pouze pro selektovanou skupinu pacientů v dobrém výkonnostním stavu, bez známek generalizace onemocnění a bez závažných komorbidit. Je třeba dalších klinických studií, které by jednoznačně definovaly optimální léčebný algoritmus pro maligní pleurální mezoteliom.
Malignant pleural mesothelioma represents a disease with poor prognosis. There is currently no accepted general standard of care however as available published data indicate the multimodality approach combining chemotherapy (neoadjuvant or adjuvant, based on cisplatin/carboplatin and pemetrexed), surgery and eventually perioperative pleural chemotherapy lavage seem to bring the most benefit. This approach is suitable for only limited group of patients in a good performance status, without any distant metastatic disease and without clinically significant comorbidities. Further research is therefore needed to define the optimal treatment algorithm for malignant pleural mesothelioma.
V tomto článku jsou shrnuty aktuální poznatky o terapii maligního pleurálního výpotku pomocí IPC (Indwelling Pleural Catheter- tunelizovaného hrudního drenu). Taktéž je zde uvedena kazuistika pacienta, který byl tímto systémem léčen.
The article summarizes current knowledge about the treatment of malignant pleural effusion using an indwelling pleural catheter. Also presented is a case study of a patient treated with this system.

Problematika pleurálních výpotků zasahuje do mnoha oblastí medicíny. Ročně postihne asi 0,5 % populace. V článku jsou sumarizovány patofyziologické podklady vzniku výpotků, jejich epidemiologie, popis jednotlivých typů výpotků a možnosti léčby. Větší pozornost je věnována diferenciální diagnostice a léčbě maligních pleurálních výpotků.
The problems of pleural effusions are connected with many areas of medicine. The problem affects approx. 0.5 % of the population every year. The paper summarizes pathophysiological data relating to the emergence of effusions, their epidemiology, description of particular types of effusions and possibilities of treatment. More attention is paid to the differential diagnosis and therapy for malign pleural effusions.
Maligní pleurální výpotky představují stále častější problém. Tunelizované pleurální katetry jsou již řadu let užívané v jejich paliaci a v současné době jsou ve světě stále oblíbenější metodou. V Česku zřejmě stále převládá indikace chirurgické indukce pleurodézy nad klinickými metodami. O zavedení tunelizovaného pleurálního katetru v Česku jsme nenalezli literární odkazy. Předkládáme dokumentaci zavedení tohoto katetru a rozbor přínosů a problémů spojených s touto metodou.
Malignant pleural effusions are an increasingly frequent problem. For many years, tunneled pleural catheters have been used for palliation of the effusions. Currently, this approach is rising in popularity in many countries. In the Czech Republic, however, surgical pleurodesis seems to prevail over clinical methods. In the literature, we have found no evidence of the use of tunneled pleural catheters in the Czech Republic. Presented are a guide for catheter placement and analysis of benefits and drawbacks associated with the method.

BACKGROUND: One option for the palliative treatment of recurrent malignant pleural effusion is powdered talc using thoracoscopy. This paper presents the results of selected systemic and local manifestations of the talc-induced inflammatory reaction using a videothoracoscope. METHOD: A total of 114 patients with repeated malignant pleural effusion were treated at the Cardiac Surgery Clinic in Hradec Kralove from January 2010 to December 2012. Those with a life expectancy more than ≥ 3 months were eligible for talcage surgery. The group was retrospectively divided according to treatment results into Group A (N1 = 98 - successful) and Group B (N2 = 16 - relapsing). The pleural effusion was quantified using ultrasound over 1 year at 3-month intervals. Systemic changes due to the inflammatory reaction (body temperature, serum leukocyte and CRP levels) were evaluated. Local indicators of inflammation included changes in the leukocyte cell population in the effusion and changes in the pleural CRP levels. The dynamics of local expression of membrane receptors TLR-2 and CD-64 on granulocyte and monocyte cell populations in the pleural effusion were also evaluated. RESULTS: The reaction after talcage, included a significant increase in axillary temperature and leukocyte count, 12 h after the procedure. The dynamics were different in the two groups. The dynamics of local inflammatory changes were an early increase in the pleural CRP levels in both groups. The time interval of local inflammatory development and duration was related to the treatment efficacy and showed a significant rise 2 h after talcage in Group A. In Group B the local inflammatory reaction was slower and the rise was only observed 24 h after talc application. A decrease in lymphocyte count and an increase in granulocyte count 2 h after talcage were found. After an initial drop in monocyte level, a rise occurred within 24 h after talcage. Changes in the expression of TLR-2 and CD-64 receptors in relation to their cell carriers were observed depending on time after talcage. CONCLUSION: The differences in the serum and pleural effusion CRP levels suggest that the surgical stress manifests itself locally in the pleural space with a lower intensity and time delay. The TLR-2 and CD-64 receptors exhibit different behaviour depending on the type of cell membrane where they are found. The inverse relation between the granulocyte increase and TLR-2 receptor decrease in the membrane immediately after talcage is a new finding. The dynamics of TLR-2 expression on the monocytes demonstrates a direct proportion between the increasing expression of the TLR-2 receptor and increasing percent fraction of the cell carrier.
