Nádory krčního úseku jícnu jsou vzácným onemocněním, které se standardně léčí samostatnou (definitivní) chemoradioterapií bez chirurgického výkonu. Chemoradioterapie je provázena několika kontroverzemi, z nichž hlavní je nejistota v účinné dávce a přínosu dávkové eskalace. Běžná aplikovaná normofrakcionovaná dávka je přes 60 Gy. Příznivá dávková distribuce predikuje benefit protonové radioterapie (PRT). V PTC Praha lze výsledky PRT hodnotit u 29 nemocných, kteří byli ozařováni do c. d. 70 GyE technikou „pencil beam scanning“ IMPT s konkomitantní chemoterapií carboplatina + paclitaxel. U 18, resp. 7 nemocných byla dosažena kompletní, resp. parciální regrese, response rate je 86,3 %. Medián sledování je 14 měsíců (2,6–66,0). Medián přežívání je 13,7 měs., 2leté přežívání 45 %. Medián doby do relapsu/progrese je 28,7 měs., 2leté přežívání bez relapsu je 52 %. V době analýzy přežívá 13 nemocných, 12 z nich v kompletní regresi onemocnění. Akutní toxicita nepřesahuje stupeň 2 kromě kožní (st. 3 u 22,6 %). Chronická toxicita nepřesahuje st. 2 kromě 2 případů tracheoezofageální píštěle v místě primárního postižení u onemocnění stadia T4. PRT v mírně eskalované dávce s konkomitantní chemoterapií dosáhla vysokou protinádorovou účinnost, naproti tomu parametry přežívání jsou zřejmě vlivem velkého podílu primárně lokoregionálně pokročilých onemocnění méně příznivé. Přesto určitý podíl nemocných dlouhodobě přežívá s kompletní regresí onemocnění. Obligátní výskyt lymfopenie a řada recentních poznatků v protinádorové imunologii vyžaduje rozvahu nad dosavadním konceptem určování objemu a frakcionace.
Cancer of cervical esophagus belongs to rare diseases. Its standard treatment includes definitive chemoradiotherapy without subsequent surgery. There are several controversies concerning chemoradiotherapy, the most substantial of them being uncertain effective dose and uncertain benefit of dose escalation. The doses exceeding 60 Gy in 30 fractions are frequently administered. A benefit of proton radiotherapy (PRT) is predicted by favorable dose distribution. The results of PRT administered in PTC Prague have been assessed in 29 patients who have been irradiated up to 70 Gy (equivalent) in 35 fractions using pencil beam scanning technique with concomitant weekly chemotherapy carboplatinum plus paclitaxel. A complete and partial regression has been achieved in 18 and 7 patients respectively, response rate 86,3%. The median follow up time is 14 months (2,6-66,0). Median survival time is 13,7 months, 2-year survival 45%, median time to relapse/progression 28,7 months and 2-year relapse free survival 52%. 13 patients survive at the time of analysis, 12 in complete regression. The acute toxicity does not exceed the grade 2, except radiation dermatitis (22,6% grade 3). The late toxicity does not exceed grade 2, except tracheoesophageal fistulation in 2 patients with primary T4 lesion. The PRT in moderately escalated dose with concomitant chemotherapy achieved a high antitumor efficacy opposed to less favorable survival results, presumably originating of substantial number of locoregionally advanced diseases. Still a durable complete regression long time survival is being observed in a certain share of patients. A lymphopenia is obligatory and in a scope of recent observations in antitumor immunology it desires a deeper contemplation on target volume and fractionation concept.

BACKGROUND: Metastatic esophageal carcinoma (EC) has a poor prognosis and only limited treatment options. While immune checkpoint inhibitors (ICIs) have improved the treatment of a broad spectrum of cancers, patients with EC mostly fail to respond to this treatment. For that reason, it is crucial to understand the immune phenotype of each cancer patient and moreover, to understand how different therapies modulate the cancer microenvironment and sensitize the tumors to the treatment with ICIs. SUMMARY: We have conducted a systematic review of the literature to evaluate the potential of ICI therapy in combination with chemotherapy, radiotherapy, and/or biologic therapy in EC patients. In our review, we have discussed the effects of diverse treatment approaches on the tumor microenvironment of EC. In addition, we have reviewed the current phase II and III clinical trials in EC patients to provide a rationale for immunotherapy application in combination settings with chemotherapy, radiotherapy, and/or biologic therapy. KEY MESSAGES: A great effort is already underway in clinical trials evaluating the combinatorial administration of ICIs and other treatment modalities in metastatic EC patients. PD-L1 expression status was shown to be higher in the squamous cell carcinoma (SCC) as compared to adenocarcinoma. Thus, ICIs plus chemotherapy are being discussed as a particularly feasible option for patients with SCC. Radiation was shown to induce the expression of immune checkpoint molecules and to promote the priming and activation of cytotoxic T cells which provides a rationale for ICI administration in a combination with radiotherapy. The combination of ICIs with biologic therapy was shown to be safe; however, the impact on the clinical outcomes of EC patients varied among studies.

Základem prvoliniové léčby pacientů s pokročilým HER2- -negativním adenokarcinomem žaludku, gastroezofageální junkce a jícnu a nemocných se spinocelulárním karcinomem jícnu byla donedávna chemoterapie, jejíž výsledky byly neuspokojivé. Nové možnosti léčby těchto pacientů otevřely studie CheckMate 649 a CheckMate 648, ve kterých bylo pozorováno signifikantní prodloužení celkového přežití při použití nivolumabu v kombinaci s chemoterapií (studie CheckMate 649 – pacienti s adenokarcinomem žaludku, gastroezofageální junkce a jícnu), resp. při použití nivolumabu v kombinaci s chemoterapií nebo ipilimumabem (studie CheckMate 648 – pacienti se spinocelulárním karcinomem jícnu), přičemž přidání imunoterapie nevedlo k výraznějšímu zhoršení toxicity. Na základě těchto studií je nivolumab (v kombinaci s chemoterapií, resp. v kombinaci s chemoterapií nebo ipilimumabem) v těchto indikacích schválen a i hrazen, za předpokladu splnění definovaných kritérií.
Until recently, chemotherapy was the mainstay of first- -line treatment for patients with advanced HER2-negative adenocarcinoma of the stomach, gastroesophageal junction and esophagus, and patients with squamous cell carcinoma of the esophagus, with disappointing results. New treatment options for these patients were opened by the CheckMate 649 and CheckMate 648 trials, in which a significant prolongation of overall survival was observed when nivolumab was used in combination with chemotherapy (CheckMate 649 trial, patients with adenocarcinoma of the stomach, gastroesophageal junction and oesophagus) and in combination with chemotherapy or ipilimumab, respectively (CheckMate 648 trial, patients with oesophageal squamous cell carcinoma). The addition of immunotherapy did not lead to a significant worsening of toxicity. Based on these studies, nivolumab (in combination with chemotherapy or in combination with chemotherapy or ipilimumab) is covered in these indications, provided that defined criteria are met.

The new era of immunotherapy is successfully implemented in the treatment of metastatic/locally advanced esophagogastric adenocarcinoma (EGAC), as it has been investigated in combinations with/without chemotherapy in human epidermal growth factor receptor 2 (Her2)-positive and Her2-negative tumors. Recent approvals of immune checkpoint inhibitors (ICI) enrich the therapeutic landscape in nearly every therapeutic line. Based on CHECKMATE-649, the combination of nivolumab and chemotherapy in first-line therapy of programmed cell death protein 1 (PD-L1)-positive patients with advanced gastroesophageal junction cancer (GEJC), esophageal cancer (EC), and gastric cancer (GC) was approved in Europe for PD-L1 combined positivity score (CPS) ≥ 5 patients and independently from PD-L1 score in the USA and Asia. Based on KEYNOTE-590, patients with advanced GEJC and EC qualify for the combination of pembrolizumab plus chemotherapy in Europe (CPS ≥ 10) and the USA. For Her2-positive patients, trastuzumab with first-line chemotherapy plus pembrolizumab has beneficial response rates and resulted in approval in the USA (KEYNOTE-811). In third-line therapy, superior overall survival (OS) was achieved by the administration of nivolumab (approval in Japan, ATTRACTION-02), and pembrolizumab shows a positive effect on the duration of response (KEYNOTE-059). Questions of resistance to immunotherapy or the role of gender in response to ICI need to be clarified. This review provides an overview of the current approvals of ICI in advanced EGAC and reflects results of relevant phase II/III trials with focus on possible biomarkers, including PD-L1 CPS and microsatellite-instability (MSI) status.

Od uvedení anti-CTLA4 protilátky ipilimumab v roce 2011 v paliativní léčbě maligního melanomu, s do té doby nevídanými výsledky, se imunoterapie pevně etablovala v léčbě řady zhoubných nádorů. Od srovnávání účinnosti imunoterapie se standardem léčby ve druhé a první linii paliativní léčby se vzhledem k povzbudivým výsledkům začíná imunoterapie inhibitory kontrolních bodů (anti-CTLA-4, anti-PD1, anti-PD-L1) prosazovat v adjuvantní indikaci či v rámci tumor-agnostického přístupu, nádory horního gastrointestinálního traktu (GIT) nevyjímaje. Přehledový článek nabízí shrnutí proběhlých klinických studií s použitím imunoterapie checkpoint inhibitory v léčbě nádorů horního GIT.
Since the introduction of the anti-CTLA4 antibody ipilimumab in 2011 in the palliative treatment of malignant melanoma with previously unprecedented results, immunotherapy has become firmly established in the treatment of a number of cancers. Given the encouraging results of immunotherapy in the first-line and the second-line setting compared to standard of care, checkpoint inhibitors (anti-CTLA-4, anti-PD1, anti-PD-L1) begin to permeate into the adjuvant setting or within the tumor-agnostic approach, including upper gastrointestinal (GI) tumors. This review offers a summary of clinical trials investigating immunotherapy with checkpoint inhibitors in the treatment of upper GI tumors.
Neoadjuvantní chemoradioterapie následovaná chirurgickou resekcí představuje zlatý standard léčby nádorů jícnu a gastroezofageální junkce. Postavení neoadjuvantní chemoradioterapie je definováno několika studiemi a její indikace je poměrně jasná, kdežto vlastní indikace chirurgického výkonu, ačkoli v České republice obvyklá, je mnohem méně jednoznačná. Benefit adjuvantní chemoterapie byl prokázán v několika retrospektivních hodnoceních a v současné době máme i data prokazující benefit imunoterapie ve formě checkpoint inhibitorů po komplexní léčbě nádorů jícnu a gastroezofageální junkce u pacientů, kteří nedosáhli kompletní remise po neoadjuvantní chemoradioterapii, a to na základě klinické studie CheckMate 577. Nivolumab získal registraci v této indikaci, nicméně úhrada z veřejného zdravotního pojištění v tuto chvíli chybí.
Neoadjuvant chemoradiotherapy followed by surgical resection represents a gold standard of the treatment of the esophageal and gastroesophageal cancer. The position of the neoadjuvant chemoradiotherapy in the treatment algorithm is clear however the indication of the esophagectomy is quite vague. Several retrospective studies have confirmed the benefit of the adjuvant chemotherapy and at this moment there are studies which showed the benefit of adjuvant immunotherapy as check-point inhibitors in patients with esophageal and gastroesophageal cancer which didn't achieve complete remission after neoadjuvant chemoradiotherapy based on the CheckMate 577 study. Nivolumab gained the registration in this indication in Czech Republic however the healthcare reimbursement is still missing.
