PURPOSE: Bile duct injury (BDI) remains the most serious complication following cholecystectomy. However, the actual incidence of BDI in the Czech Republic remains unknown. Hence, we aimed to identify the incidence of major BDI requiring operative reconstruction after elective cholecystectomy in our region despite the prevailing modern 4 K Ultra HD laparoscopy and Critical View of Safety (CVS) standards implemented in daily surgical practice among the Czech population. METHODS: In the absence of a specific registry for BDI, we analysed data from The Czech National Patient Register of Reimbursed Healthcare Services, where all procedures are mandatorily recorded. We investigated 76,345 patients who were enrolled for at least a year and underwent elective cholecystectomy during the period from 2018-2021. In this cohort, we examined the incidence of major BDI following the reconstruction of the biliary tract and other complications. RESULTS: A total of 76,345 elective cholecystectomies were performed during the study period, and 186 major BDIs were registered (0.24%). Most elective cholecystectomies were performed laparoscopically (84.7%), with the remaining open (15.3%). The incidence of BDI was higher in the open surgery group (150 BDI/11700 cases/1.28%) than in laparoscopic cholecystectomy (36 BDI/64645 cases/0.06%). Furthermore, the total hospital stays with BDI after reconstruction was 13.6 days. However, the majority of laparoscopic elective cholecystectomies (57,914, 89.6%) were safe and standard procedures with no complications. CONCLUSION: Our study corroborates the findings of previous nationwide studies. Therefore, though laparoscopic cholecystectomy is reliable, the risks of BDI cannot be eliminated.

Kolorektální trauma se v soudobých válečných konfliktech vyskytuje v 5–10 %. Nejčastější příčinou je střelné nebo střepinové poranění, kontuzně-lacerační mechanismus je ve válečné zóně ojedinělý. I přes moderní léčebné postupy je ale zatíženo vysokou mírou morbidity, zejména pokud není včas diagnostikováno a léčeno. Management chirurgické léčby upřesňují jednoduchá skórovací schémata – colon injury scale, rectal injury scale a Flintův skórovací systém. Resekční výkony tračníku s primární nebo odloženou anastomózou nemají vyšší riziko komplikovaného hojení a v dnešní době jsou upřednostňovány před konstrukcí terminálních stomií. Ty jsou indikovány při závažné oběhové nestabilitě při hemoragicko-traumatickém nebo septickém šoku s pokročilou difuzní peritonitidou. K traumatu intraperitoneálního segmentu rekta se přistupuje stejně jako k tračníku. Poranění extraperitoneálního rekta bez devastace měkkých tkání může být ošetřeno transanální suturou nebo i bez ní. Naopak devastující poranění rekta spolu s měkkými tkáněmi pánve se mají primárně zajistit derivační stomií a odloženou rekonstrukcí. Presakrální drenáž nebo laváž rekta již nejsou doporučovány.
The rate of colorectal trauma is 5–10 % in modern war conflicts. The most common causes include gunshots or shrapnel injuries; the contusion-laceration mechanism occurs in sporadic cases in the war zone. Despite modern surgical procedures, however, it is associated with a high rate of morbidity, especially if it is not diagnosed and treated in time. Surgical management is specified by simple scoring schemes – the colon injury scale, rectal injury scale and the Flint grading system. Colonic resection with primary or delayed anastomosis is not associated with a higher risk of complicated healing and is nowadays preferred over the construction of terminal stomas. These are indicated only for cases with severe hemodynamic instability in traumatic-hemorrhagic or septic shock with severe diffuse peritonitis. Trauma to the intraperitoneal segment of the rectum is treated in the same way as trauma to the colon. An extraperitoneal rectal injury without soft tissue devastation can be treated with or without a transanal suture. On the contrary, devastating injuries to the rectum including the pelvic soft tissues should be primarily controlled with a stoma with delayed reconstruction. Presacral drainage or rectal stump lavage are no longer recommended.

V článku představujeme 3 kazuistiky s penetrujícím poraněním břicha z naší praxe. Pro hemodynamickou nestabilitu či mechanismus úrazu byla vždy provedena urgentní laparotomie. Penetrující poranění břicha jsou spojena s vysokým rizikem život ohrožujících nitrobřišních poranění, vyžadují urgentní revizi a často jsou doprovázena pooperačními infekcemi peritoneální dutiny. V posledních letech se rozšiřuje tendence k miniinvazivním přístupům či dokonce k neoperační léčbě. Tento trend je patrný zejména ve Spojených státech amerických, kde se lékaři setkávají s vyšším počtem penetrujících poranění oproti převládajícím tupým poraněním břicha v Evropě. Autoři popisují nezbytnost dodržet všechny doporučené postupy v přednemocniční i nemocniční fázi ošetření těchto pacientů a srovnávají je s recentní literaturou.
We present 3 case-reports with penetrating abdominal injury from our practice in this article. An urgent laparotomy was performed in all cases because of haemodynamic instability or the mechanism of injury. Penetrative abdominal traumas are associated with a high risk of life-threatening intra-abdominal injuries, require urgent revision and are often accompanied by postoperative infections of the peritoneal cavity. In recent years, there has been a growing tendency towards mini-invasive approaches or even non-operative treatment. This trend is particularly evident in the United States of America, where doctors experience a higher number of penetrating injuries compared to the prevalent blunt force trauma in Europe. The authors describe the need to follow all recommended procedures in the pre-hospital and hospital phases of treatment of these patients and compare them with recent literature.

Úvod: Traumata břicha představují pro chirurga častou problematiku. V diferenciální diagnostice hrají nejvýznamnější roli poranění parenchymatózních orgánů. Naproti tomu poranění dutých orgánů trávicí trubice jsou u těchto pacientů méně častá. Jejich diagnostika je při izolovaném typu poranění obtížná a je nutné komplexní zhodnocení všech dostupných vyšetření. Může tak dojít k pozdnímu stanovení diagnózy, potažmo indikaci chirurgické revize dutiny břišní, což může mít negativní vliv na morbiditu a mortalitu těchto pacientů. Metody: Proběhl retrospektivní sběr dat pomocí informačního systému WinMedicalc 2000. Do studie jsme zařadili pacienty Chirurgické kliniky Lékařské fakulty v Plzni hospitalizované v letech 2008−2017, operované pro poranění střeva. Hodnotili jsme zejména vyšetřovací postup, rozsah poranění a časové faktory. Výsledky: Náš soubor tvořilo celkem 41 pacientů, 30 mužů a 11 žen. Průměrný věk nemocných byl 41 let, 4 pacienti byli dětští. 23 pacientů bylo polytraumatizovaných, 9 pacientů mělo sdružená poranění, u 9 pacientů se jednalo o izolované poranění střeva. Celkem 6 pacientů zemřelo. Kličky tenkého střeva byly poraněny u 17 pacientů, tlusté střevo u 14 a oba úseky trávicí trubice u 10 pacientů. Poranění střeva bylo diagnostikováno 17krát na podkladě CT vyšetření (celkem provedeno 31krát), u 23 pacientů bylo zjištěno až peroperačně. Průměrná doba hospitalizace byla 20,5 dne, přičemž vyšší byla u kombinace poranění tenkého a tlustého střeva (25,6 dne). U dvou pacientů byl zpočátku zvolen konzervativní postup, revize dutiny břišní byla indikována na základě progrese klinického nálezu. Závěr: Zhodnotili jsme spektrum pacientů, jejich poranění, diagnostiky a léčby na našem pracovišti. Z výsledků plyne, že zobrazovací vyšetření jsou jako samostatná diagnostická metoda velmi nespolehlivá, nutné je komplexní zhodnocení všech dostupných informací a zejména fyzikální vyšetření se zaměřením na nález na břiše.
Introduction: Patients with abdominal trauma appear frequently. The most vulnerable organs in these patients are the liver and the spleen. Injuries of the small and large intestines are relatively less frequent. The diagnostic process of these injuries is complex and requires an analysis of all results. Therefore it is highly probable that the final diagnosis can be delayed and so can be delayed an indication of surgical exploration of the abdominal cavity, which can have serious consequences on the morbidity and mortality of these patients with intestinal trauma. Methods: We collected our data using the WinMedicalc 2000 software. We searched for patients hospitalised in years 2008−2017 in the Department of Surgery, Faculty of Medicine in Pilsen subjected to surgical revision of the abdominal cavity for intestinal trauma. Results: Our set comprised 41 patients, including 30 men and 11 women. The mean age of the patients was 41 years, 4 of the patients were children. Twenty-three of the patients suffered from polytrauma, while 9 of the patients had a relatively isolated injury of either the small intestine or the colon. Six of the patients died. The small intestine was injured in 17 patients, the colon was injured in 14 patients and both were injured in 10 patients. The intestinal injury was diagnosed in 17 cases based on CT imaging (performed 31 times in total); 23 cases were diagnosed in the peroperative period. Conclusions: We assessed a set of patients with an intestinal injury in terms of specific diagnosis, severity of trauma, diagnostic process and treatment. The results are similar to the results of studies in large sets of patients. Even though imaging methods can help to reach the right diagnosis, they are insufficient as a sole diagnostic method, and physical examination plays a major role.
The presented case report describes primarily an unrecognized liver injury after the resuscitation with cardiac massage due to myocardial infarction. After myocardial revascularization, the hemodynamic instability and anaemia in the patient persisted. The performed CT scan confirmed a liver injury, namely the rupture of liver parenchyma with hemoperitoneum, which had to be treated surgically. It represents a complication of cardiac massage that occurs, though less frequently. Key words: resuscitation, liver trauma.

The authors refer to the case of an 18-year-old man with severe mental retardation and severe cerebral palsy with motor anxiety and tendency to self-harm who has stomped his abdomen at the edge of the bed. Both ultrasonic and CT findings are suspected to be tortuous and infarcted wandering spleen.

Pri liečbe chirurgických diagnóz v detskom veku sa stala miniinvazívna chirurgia štandardnou liečbou. V literatúre je len málo štúdií, ktoré by popisovali využitie laparoskopie pri tupej traume brucha u detí. V článku predkladáme naše skúsenosti s laparoskopiou pri tupej traume brucha u detí. Popisujeme dve kazuistiky pacientov s perforáciou čreva po tupej traume brucha riešenej laparoskopicky. U oboch pacientov bola perforácia čreva identifikovaná laparoskopicky, oboch sme aj laparoskopicky vyriešili. Prvého pacienta sme ošetrili laparoskopicky pokračujúcou sutúrou perforácie čreva, druhého pacienta sme riešili laparoskopicky asistovane. Obaja pacienti sa zhojili bez ťažkostí. Miniinvazívna chirurgia pri suspektnej perforácii hornej časti gastrointestinálneho traktu je možnou metódou voľby. Domnievame sa, že laparoskopia môže poskytnúť dôležité informácie pre diagnostiku a liečbu poranení vnútrobrušných orgánov. U detí môže spresniť diagnostiku a v indikovaných prípadoch môže znížiť množstvo komplikácií laparotomickej explorácie dutiny brušnej. Miniinvazívna chirurgia pri tupej traume brucha je možná u hemodynamicky stabilného pacienta.V rukách skúseného ,,laparoskopického“ chirurga môže znížiť pooperačnú bolesť pacienta, zlepšiť následný kozmetický efekt, skrátiť dobu hospitalizácie, ako aj umožniť skorší návrat dieťaťa do školských a športových aktivít. Nemožno opomenúť, že u hemodynamicky nestabilného pacienta ostáva ,,zlatým“ štandardom exploratívna laparotómia.
Minimally invasive techniques have now become standard for the treatment of many surgical conditions in children. There are a few studies that describe the utility of laparoscopy in BAT in children. In this article, we describe the complete laparoscopic surgical treatment of two patients after a single blunt abdominal trauma, both with bowel perforation. In both cases, the perforation was identified and closed, one laparoscopically with an ongoing suture, the second jejune perforation was closed by laparoscopic-assisted techniques. Both patients had an uneventful postoperative recovery. Therapeutic laparoscopic treatment of patients with upper gastrointestinal perforation is feasible. We hypothesize, that diagnostic laparoscopy provides important information for the treatment of children with abdominal trauma and is accompanied by improved diagnostic accuracy, reduction of nontherapeutic laparotomy rates, and a reduction of morbidity. Minimally invasive surgery in children after BAT is suitable for hemodynamic stable patients, could improve pain scores, cosmetic effect, shorter hospital stays, shorter operative times and shorter return to school/activities. However, at any point in the patient’s care, in case the unstable hemodynamic is encountered, exploratory laparotomy is the procedure of choice.
