Donation after circulatory death (DCD) has become an accepted practice in many countries and remains a focus of intense interest in the transplant community. The present study is aimed at providing a description of the current situation of DCD in European countries. Specific questionnaires were developed to compile information on DCD practices, activities and post-transplant outcomes. Thirty-five countries completed the survey. DCD is practiced in 18 countries: eight have both controlled DCD (cDCD) and uncontrolled DCD (uDCD) programs, 4 only cDCD and 6 only uDCD. All these countries have legally binding and/or nonbinding texts to regulate the practice of DCD. The no-touch period ranges from 5 to 30 min. There are variations in ante and post mortem interventions used for the practice of cDCD. During 2008-2016, the highest DCD activity was described in the United Kingdom, Spain, Russia, the Netherlands, Belgium and France. Data on post-transplant outcomes of patients who receive DCD donor kidneys show better results with grafts obtained from cDCD versus uDCD donors. In conclusion, DCD is becoming increasingly accepted and performed in Europe, importantly contributing to the number of organs available and providing acceptable post-transplantation outcomes.

Intenzivní medicína je úzce spojena s transplantační medicínou jak v péči o dárce orgánů, tak o jejich příjemce. Narůstající nepoměr mezi nabídkou a zvyšující se poptávkou orgánů pro transplantaci vede k hledání nových způsobů zvýšení dostupnosti orgánů. Odběr a transplantace párových a regenerujících orgánů od žijících dárců se jeví jako jeden z nejvýhodnějších postupů nejen z hlediska medicínského, ale i ekonomického. V práci jsou diskutovány legislativní, etické a ekonomické otázky prodeje a finanční náhrady dárcům orgánů.
Intensive care medicine has tight connections to both sides of transplantation medicine - organ donors are usually recruited from ICU on the one hand, while on the other hand organ recipients are usually admitted to ICU after organ transplantation. The increasing gap between organ supply and the number of patients on the waiting lists has led to searching for new alternatives and ways to obtain more available organs. Transplanting one of paired organs (or part of single organ) from living donors seems to be one of the best options in terms of medical outcome and cost. The legal, ethical and economic issues of the „organ market“ are discussed in the presented paper.

The heart transplant program at the Institute for Clinical and Experimental Medicine in Prague was established on January 31, 1984. Through November 2012, 881 orthotopic cardiac transplantations have been performed, with an annual rate of about 40 procedures. Current legislation concerning solid organ transplantations in the Czech Republic is described. Like other centers, we have noticed an increasing age of donors, and, reflecting the shortage of grafts, we have expanded our selection criteria for heart transplantation. The advent of a mechanical circulatory support program at our center in April 2003 has given us another valuable tool in the management of chronic heart failure patients. Currently, around half of our patients are transplanted from mechanical support. Nonischemic etiology of heart failure is a leading cause of transplantation at our center, followed by ischemic cardiomyopathy, valvular heart lesions, and adult congenital heart diseases. Our current immunosupression protocol, including induction therapy, is outlined in detail and survival rates, as well as most common complications and our treatment strategies, are also discussed.
