Významná časť pacientov s celiakiou nemá rozvinuté príznaky malabsorpčného syndrómu. Doteraz sa len málo štúdií zaoberalo body mass indexom (BMI) u pacientov s celiakiou a vplyvom bezlepkovej diéty naň. Jedinou známou účinnou liečbou celiakie je bezlepková diéta, preto sú na Slovensku od roku 2005 dostupné niektoré základné bezlepkové potraviny na recept v lekárňach s dotáciou štátu. Cieľ a metódy: Cieľom práce bolo zhodnotiť podiel diagnostikovaných pacientov s nadváhou a obezitou a zhodnotiť vplyv bezlepkovej diéty na BMI. Očakávaný nárast BMI u týchto pacientov by logicky mohol negatívne ovplyvniť ich morbiditu a mortalitu v budúcnosti. Vzhľadom na zavedenie čiastočného hradenia bezlepkových potravín zo zdravotného poistenia s možnosťou preskripcie sme sa pokúsili vyhodnotiť prínos tohto opatrenia. Štúdia bola vyhodnocovaná retrospektívne zo záznamov 153 pacientov s histologicky potvrdenou diagnózou celiakie. Doplnená bola jednoduchou otázkou ohľadom zhodnotenie vplyvu dostupnosti bezlepkových potravín na recept na dodržiavanie bezlepkovej diéty. Výsledky: Potvrdili sme, že neveľa pacientov (21,8 %) malo v úvode diagnózy BMI v pásme podvýživy, a naopak pomerne významná skupina (14,5 %) trpela nadváhou či obezitou. V sledovaní úpravy BMI sa jeho nárast potvrdil štatisticky významne iba u pacientov s úvodne nižším alebo normálnym BMI, nie však u pacientov s BMI v pásme nadváhy či obezity. Efekt preskripcie a čiastočného hradenia bezlepkových potravín na zlepšenie dodržiavania bezlepkovej diéty hodnotili pozitívne nielen pacienti dodržiavajúci úplnú bezlepkovú diétu (92,9 %), ale aj významné množstvo pacientov, ktorí bezlepkovú diétu nedodržiavajú kompletne (80 %). Porovnali sme dodržiavanie bezlepkovej diéty v skupine pacientov diagnostikovaných pred zavedením hradenia bezlepkových potravín na recept a pacientov diagnostikovaných v období odkedy je táto možnosť dostupná. Štatisticky významný rozdiel však medzi týmito dvomi skupinami (80,1 vs 88,1 %) v našom súbore nebol. Kľúčové slová: celiakia – body mass index – bezlepková diéta

A significant portion of patients with celiac disease do not suffer with developed symptoms of malabsorption syndrome. So far, only a few studies have focused on the body mass index (BMI) in patients with celiac disease and of the impact of a gluten-free diet. The only known effective treatment for celiac disease is a gluten-free diet, and therefore since 2005 certain basic prescription gluten-free products have been available in Slovak pharmacies and subsidised by the government. Aims and methods: The main focus was to assess the number of overweight or obese patients diagnosed with CD and to assess how a gluten-free diet affects BMI, because anticipated weight gain could negatively affect the future morbidity of overweight or obese patients. With a view to the implementation of a partial cover of prescription gluten-free foods by health insurance, we tried to assess the benefits of such a measure. The survey was made as a retrospective evaluation of 153 patients with histologically proven celiac disease. It was supplemented with a single question regarding the assessment of the impact of prescription gluten-free foods. Results: We confirmed that only a relatively small portion of patients (21.8%) had a BMI at the level of malnutrition and quite a significant portion (14.5%) suffered from being overweight or obese. A statistically significant increase in BMI was only confirmed in the case of patients with initially low or normal BMI, not in the case of overweight or obese patients. The effect of prescription and partial cover of gluten-free foods was positively assessed not only by the patients with a completely gluten-free diet (92.9%) but also by a significant number of patients not adhering to a totally gluten-free diet (80%). We compared adherence to a gluten-free diet in the group diagnosed before the implication of health insurance coverage of gluten-free foods, and the group of patients diagnosed while this option was available. However, there was no statistically significant difference between these two groups (80.1 vs 88.1%) of patients within our statistical sample. Cíle: Posoudit vhodnost Prostate Health Indexu (PHI) jako parametru vhodného ke zhodnocení agresivity karcinomu prostaty (PC) v porovnání s výsledkem preoperační biopsie a pooperační histologie prostaty. Metodika: Do studie bylo zařazeno 474 pacientů indikovaných k radikální prostatektomii, u kterých byly preoperačně zjištěny koncentrace prostatického specifického antigenu (PSA), free PSA (fPSA), [-2]proPSA a vypočteny hodnoty %freePSA a PHI. Gleasonovo skóre (GS) bylo stanoveno z preoperačního bioptického vyšetření a z pooperační histologie po radikální prostatektomii. Za pomoci hodnoty GS byly vytvořeny 2 podskupiny: GS6 a GS≥7. Toto rozdělení bylo provedeno dvakrát, nejprve na základě výsledků preoperační biopsie, podruhé dle závěru pooperační histologie. Následně bylo provedeno porovnání hodnot PSA, [-2]proPSA, %freePSA a PHI mezi skupinami s GS6 a GS≥7 jak pro rozdělení dle preoperační biopsie, tak pro rozdělení dle pooperační histologie. Výsledky: Při porovnání sledovaných parametrů mezi skupinami GS6 a GS≥7 vytvořených na podkladě výsledků preoperační biopsie byl zjištěn statisticky významný rozdíl v případě [-2]proPSA, PHI a PSA (p < 0,0001, p < 0,0001 a p = 0,0012), v případě %freePSA nebyl zjištěn statisticky významný rozdíl (p = 0,9306). Při porovnání sledovaných parametrů na základě definitivního GS byl mezi skupinami GS6 a GS≥7 zjištěn statisticky významný rozdíl ve všech sledovaných parametrech: PHI, [-2]proPSA, %freePSA a PSA (p < 0,0001, p < 0,0001, p < 0,0001 a p = 0,0012). Nejlepší hodnoty plochy pod křivkou (AUC) bylo dosaženo v případě PHI při rozdělení pacientů do skupin dle GS zjištěného na základě pooperační histologie (0,7649). Závěr: PHI je nejlepší ze sledovaných parametrů při kategorizaci pacientů na GS6 a GS≥7. Jeho implementace do zavedených algoritmů může být přínosem v diagnosticko-terapeutickém managementu pacientů s PC. PHI může pomoci při volbě mezi sledováním a radikální prostatektomií, případně zjednodušit rozhodování, zda zvolit šetrnější chirurgický zákrok nebo radikálnější postup.

Aim: To assess the Prostate Health Index (PHI) as a marker of tumor aggressiveness in comparison with prostate biopsy and postoperative histology. Methods: Our cohort consisted of 474 patients indicated for radical prostatectomy with preoperative measurements of total prostate-specific antigen, free prostate-specific antigen, [-2]proPSA, calculated %freePSA, and PHI. The Gleason score was determined during biopsy and after radical prostatectomy. Using the Gleason score, we divided the group of patients into the 2 subgroups: Gleason score 6 and Gleason score ≥7. This division was performed according to the biopsy Gleason score and according to the postoperative Gleason score. We compared total prostate specific antigen, [-2]proPSA, %freePSA, and PHI in the subgroups Gleason score 6 and Gleason score ≥7 after biopsy and the definitive score. Results: Of the calculated biopsy, Gleason score 6 and Gleason score ≥7 subgroups, [-2]proPSA, PHI and PSA (p < 0.0001, p < 0.0001 and p = 0.0012) were statistically significant. In case of %freePSA statistically significant differences were not present (p = 0.9306). Of the definitive Gleason score 6 and Gleason score ≥7 subgroups, PHI, [-2]proPSA, %freePSA, and PSA (p < 0.0001, p < 0.0001, p < 0.0001, and p = 0.0012) were statistically significant. The best area under the curve value (0.7649) was achieved by PHI when the subgroups were established according to the postoperative Gleason score. Conclusion: PHI is the best of tested markers for the categorization of Gleason score 6 and Gleason score ≥7 tumors. Implementation of PHI can be useful in the management of patients with prostate cancer. PHI can be a helpful marker for indication of active surveillance or radical prostatectomy, or simplify the decision of whether to perform nerve-sparing radical prostatectomy.
