BACKGROUND: Preservation of mobility independence is a primary goal in older adults with physical frailty and sarcopenia (PF&S). Interventions based on the combination of physical activity (PA) and nutritional counselling have been indicated as strategies for the management of this condition, although their effectiveness is not confirmed in all investigations. A possible explanation for this uncertain scenario relies in the impact of the adherence to PA interventions. Hence, the present study investigated the impact of the adherence to PA sessions on the incidence of mobility disability in older adults with PF&S. METHODS: This is a secondary analysis of an evaluator blinded, randomised controlled trial, developed in 16 clinical sites across 11 European countries, from January 2016 to 31 October 2019. Participants were community-dwelling older adults (70+ years) with PF&S enrolled in the SPRINTT trial (NCT02582138). PF&S was operationalised as having a total score from 3 to 9 on the short physical performance battery (SPPB), low appendicular lean mass and ability to complete the 400-m walk test in < 15 min. Data from participants allocated to a multicomponent intervention (PA with technological support plus nutritional counselling) and a healthy ageing lifestyle education programme (control group) were analysed. Adherence to PA was assessed based on the number of weekly sessions attended. According to recommendations of the American College of Sports Medicine, adherence was categorised as below recommendations (< 2 sessions/week, BR), meeting recommendations (2-3 sessions/week, MR), and above recommendations (> 3 sessions/week, AR). The primary outcome was incident mobility disability, operationalised as incident inability to complete the 400-m walk test in < 15 min during up to 36 months of follow-up. RESULTS: Data of 1444 participants (mean age 79.3 years, 72.6% women) were analysed. In those with SPPB scores of 3-7, MR and AR groups had lower risk of mobility disability compared with controls [MR HR (95% CI): 0.57 (0.41-0.78), p = 0.001; AR HR (95% CI): 0.33 (0.23-0.46), p < 0.001] and BR groups [MR: HR (95% CI): 0.48 (0.34-0.69), p < 0.001; AR: HR (95% CI): 0.27 (0.18-0.38), p < 0.001] in a dose-dependent manner. In those with SPPB scores of 8 or 9, the BR group had a higher risk of mobility disability than controls. MR and AR groups had a lower risk of mobility disability than the BR group. CONCLUSIONS: In older adults with PF&S, adherence to PA recommendations is associated with lower incidence of mobility disability. This benefit depends on the degree of adherence as well as baseline physical performance. TRIAL REGISTRATION: ClinicalTrials.gov NCT02582138.

Promoting healthy aging through physical activity (PA) is crucial as the global population grows older. Traditional interventions often fail to engage older adults, underlining the need for personalized, timely approaches. Smartphone-delivered PA interventions can offer personalized support during opportune moments for behavioral change. The current study examined whether the receptivity of inactive older adults influences compliance with mHealth walking suggestions after inactivity, and explored their experiences with it. Thirty healthy older adults (mean age 73.9 years) participated in the study and answered event-based EMA questionnaires via HealthReact after each 30-minute inactivity period. Emotions, physical complaints, intention, self-efficacy, perceived walking, and environmental permissiveness were assessed. Walking suggestions followed each EMA, and semi-structured interviews were conducted post-study. Multilevel logistic regressions in R were applied, and qualitative data were thematically analyzed using NVivo. Results show that higher intention, self-efficacy, and environmental permissiveness positively correlated with compliance, while higher perceived walking negatively correlated. Participants generally found the suggestions motivating and well-timed, but some reported increased alertness and pressure. Consequently, tailoring interventions to individual needs and targeting receptive moments can enhance compliance and promote healthier aging through increased PA. Future mobile interventions should consider self-efficacy, intention, prior activity, and environmental conditions to improve effectiveness.

Obezita a diabetes mellitus 2. typu (DM2T) sú významnými rizikovými faktormi rozvoja kognitívnej dysfunkcie a neurodegeneratívnych ochorení. Ich spoločný patofyziologický základ zahŕňa inzulínovú rezistenciu, chronický subklinický systémový zápal a neurozápal, poruchy mikrobiómu, hormonálnu dysreguláciu a štrukturálne zmeny mozgu. Tieto faktory vedú k zhoršeniu pamäte, exekutívnych funkcií a k akcelerácii neurodegenerácie. Pozitívne účinky úpravy životného štýlu – vrátane zníženia telesnej hmotnosti, zvýšenia fyzickej aktivity a úpravy výživy a stravovacích návykov – sa prejavujú zlepšením inzulínovej senzitivity v mozgu, zvýšením neurotrofických faktorov, redukciou systémového zápalu a neurozápalu a zlepšením metabolizmu. Kombinácia behaviorálnych a farmakologických intervencií môže spomaliť kognitívny pokles a znížiť riziko demencie u populácie s obezitou a poruchou metabolizmu glukózy.
Obesity and type 2 diabetes (T2D) are important risk factors for the development of cognitive dysfunction and neurodegenerative diseases. Their common pathophysiological substrate includes insulin resistance, chronic subclinical systemic inflammation, neuroinflammation, shifts in the intestinal microbiome composition, hormonal dysregulation, and structural changes of the brain. These factors lead to impaired memory, executive functions, and accelerated neurodegeneration. The positive effects of lifestyle modifications — including weight loss, increased physical activity, and improved dietary composition — are manifested by improved insulin sensitivity in the brain, increased neurotrophic factors, reduced systemic inflammation and neuroinflammation, and improved metabolism. A combination of behavioral and pharmacological interventions may slow cognitive decline and reduce the risk of dementia in patients with obesity, prediabetes and T2D.

Improving the quality of the most basic health behavior among youth may contribute to short-term body composition management with long-term implications for noncommunicable disease regression. This investigation aimed to assess the impact of primary school physical activity (PA), dietary, or dual approach interventions on pupils' body weight (BW) and body mass index (BMI). A systematic review and meta-analysis was completed following a study protocol and a trial registration (PROSPERO: CRD4202347770) with the PRISMA approach. Publications in English or German were included with school-based randomized controlled trials on diet and/or PA. Pupils of primary schools (aged 5-10) with no major nutritional deficiency or unstable health condition were included. The Boolean search strategy revealed a total of 9479 articles, qualifying 39 studies with 20 462 pupils (including 10 211 girls and 10 251 boys) for quantitative synthesis. The interventions were mostly PA (n = 31), several were dietary (n = 6), and some were dual approach (n = 5). Random effects meta-analyses revealed PA intervention (n = 20) to have an effect size of +0.07 kg (95% CI: -0.01 to 0.15) and -0.12 kg/m2 (95% CI: -0.23 to -0.01). Low statistical heterogeneity was found for BW (I2 = 0%; P = 1.000) and BMI (I2 = 0%; P = .9688), respectively. The findings indicate a scarcity of top-quality scientific research performed on healthy diet for body weight management in primary schools. PA intervention for elementary school pupils provides support for a healthier body composition profile amidst the current world health crisis.

The aim of this study was to compare the effects of 3 sets of isometric conditioning activity (ICA), each with an equal total duration (9 s per set) but with different distributions contractions, on force production during ICA and subsequent countermovement jump (CMJ) performance. Fifteen resistance-trained males participated in this study (age: 22.1 ± 2.4 years; body mass: 85.1 ± 9.7 kg; height: 181.3 ± 6.5 cm; relative one-repetition maximum (1RM) in back squat: 1.59 ± 0.32 kg/kg of body mass). Participants completed 3 conditions, each consisting of ICA in the half-back squat with a pushing isometric muscle action (PIMA) position at a 120-degree knee angle against an immovable barbell, differing in repetition distribution: 3 sets of 9 repetitions lasting 1 s each (SUST-1), 3 sets of 3 repetitions lasting 3 s each (SUST-3), 3 sets of single repetitions lasting 9 s (SUST-9), and a control condition (CTRL) without ICA. A 1-minute rest was allowed between sets. Approximately 3 min pre-ICA and at 15 s, 3-, 6-, 9-, and 12-minutes post-ICA, the CMJ performance was assessed. Moreover, peak force (PF) production, and force generated at 100 (Force100) and 200 ms (Force200) during each ICA were evaluated. A two-way repeated measures ANOVA indicated a main effect of time on CMJ height (F = 2.674; p = 0.029; ηp2=0.171) but did not show significant differences between conditions (F = 0.934; p = 0.434; ηp2=0.067) or interactions (F = 0.826; p = 0.648; ηp2=0.060). Post-hoc comparisons indicated a significant decrease in CMJ height at the 9th minute compared to the 3rd minute (35.7 ± 5.6 cm vs. 36.8 ± 5.5 cm post-CA; d = 0.161; p = 0.048). In addition, no significant interactions or main effects were found for CMJ contraction time, PF and Force100 during ICA. However, a main effect of condition was demonstrated on Force200 (F = 19.181; p < 0.001; ηp2= 0.013). Post-hoc comparisons revealed higher Force200 values in SUST-1 (mean difference [MD] = 549 ± 137 N; d = 1.049; p < 0.001) and SUST-3 (MD = 348 ± 112 N; d = 0.665; p = 0.002) compared to the SUST-9 condition. None of the ICAs used in the present study had any effect on the CMJ performance. However, the significantly higher Force200 values noted in the SUST-1, and SUST-3 conditions compared to the SUST-9 condition suggest that the specific distributions of isometric contraction influence the force generated during their execution.

Investigating the consistency in countermovement jump performance after high-velocity and high-force PAPE: A multiday analysis. J Strength Cond Res 39(7): e843-e853, 2025-Postactivation performance enhancement (PAPE) is widely acknowledged in both practical application and research. However, studies have primarily focused on single experimental PAPE sessions, neglecting the repeatability of protocols with the same individuals over time. The purpose of this study was to determine whether PAPE, involving high-velocity and high-force conditioning activities, could be observed at consistent time points across multiple sessions. Sixteen elite speed skaters (9 men: 23.1 ± 2.6 years and 7 women: 24.2 ± 4.7 years) participated in 6 sessions for each independent study. For all sessions, a standardized warm-up was performed, followed by 3 baseline bodyweight countermovement jumps (CMJ), and a potentiation protocol of 5 repeated band-assisted countermovement jumps at approximately 20% bodyweight reduction, or a single 5-second maximal effort isometric squat. Subsequently, subjects rested for 3, 5, and 7 minutes before performing a single bodyweight CMJ at each time point. Multiday analyses of variance showed mixed results for both protocols, with most effect sizes ranging from trivial to small. Linear mixed models indicated that at the individual level, subjects did not consistently jump higher at any specific post-PAPE rest time. The variance between baseline and post-PAPE jump height at the rest time that produced the greatest result was neither statistically significant nor statistically different ( p > 0.05), compared with other rest times. Postactivation performance enhancement was identifiable across sessions but variability in jump performance and inconsistent effects made it difficult to observe repeatable responses within individuals over time, thus questioning the idea of "individualizing" PAPE in practice.

BACKGROUND: Long-term exposure to air pollution is associated with a higher incidence of various non-infectious diseases. However, not only air pollution, but also other risk factors, such as lifestyle, can play a role in the occurrence of these diseases or premature deaths from them. The study aimed to compare the lifestyle of residents of two differently air polluted regions and to determine how lifestyle is affected by socioeconomic variables. METHODS: In the framework of the project Healthy Aging in Industrial Environments, two cohorts of persons from an industrial area and a control area were established. The cohorts consisted of individuals aged 35 to 65 years. Lifestyle factors included diet, BMI, alcohol and cigarette consumption, duration of sleep, physical activity, and time spent doing hobbies. Influencing factors included region, sex, age, education, family status, and economic situation. Fully adjusted binary and ordinal logistic regression models were used for evaluation, and the output was the odds ratio (OR) with 95% confidence intervals (CI). RESULTS: The effect of more air polluted industrial region was related to higher BMI (OR = 1.23; 95% CI: 1.08-1.4) and physical activity (OR = 1.31; 95% CI: 1.13-1.51) and surprisingly to lower smoking level (OR = 0.84; 95% CI: 0.74-0.99). CONCLUSION: The results of our study are useful in targeting public health strategies and intervention programs to specific populations, and the results will be share with public awareness groups that focus on prevention and the physiological aspects of physical activity.

The majority of motor tasks in sports are executed unilaterally, however research on the impact of unilateral conditioning activities (CAs) on both unilateral and bilateral sports tasks remains limited. Therefore, the aim of this study was to evaluate the effects of isometric and plyometric unilateral CAs on unilateral and bilateral jumping performance. The study involved fifteen resistance-trained males who participated in three experimental sessions: 3 sets of 3 s of maximum isometric single-leg quarter-squats or 3 sets of 5 single-leg tuck jumps as CAs, along with a control condition without CA. Measurements of single-leg jump (SLJ) and countermovement jump (CMJ) were taken 5 min before, and at approximately the 3th, 6th and 9th minute after the CA. The analysis did not show any statistically significant interactions nor a main effect of condition or time (p > 0.05) for CMJ height and relative peak power. However, a main effect of time (p = 0.02) to increase non-dominant SLJ height from baseline to best post-CA time-point was found (+ 0.8 ± 2.5 cm; Cohen's d = 0.22). Neither isometric nor plyometric CAs significantly affected CMJ and SLJ performance. The observed increase in jump height for the non-dominant leg is likely due to motor learning rather than the effects of the applied CAs.

BACKGROUND: Chronic low back pain (CLBP) is one of the most common musculoskeletal problems worldwide. Even though regular exercise is recommended as the primary conservative approach in treating this condition, significant part of patients lead sedentary lifestyle. Motivation to exercise is one of the variables that effects the adherence of exercise-based treatments. This study aimed to characterize the motives for exercise, as posited by self-determination theory, in persons with CLBP, and to identify subgroups (clusters) of motivational profiles in combination with socioeconomic and clinical characteristics using k-means cluster analysis. METHODS: Data were collected between September 2022 and September 2023. A total of 103 adults with CLBP completed the paper-pencil Exercise Self-Regulation Questionnaire (SRQ-E) and provided self-reported measures on anthropometric and socio-economic characteristics. Inclusion criteria were age (≥ 18 years) and non-specific CLBP (lasting longer than 12 weeks). Exclusion criteria included specific lumbar spine pathology (e.g., fracture, cancer), worsening neurological symptoms, recent injection therapy (within 3 months), and current alcohol or drug misuse. RESULTS: Three distinct motivational clusters were identified among the 103 participants: two clusters were characterized by predominantly autonomous motivation (moderately motivated cluster: 31.1%; highly motivated cluster: 54.4%), while one cluster (controlled convinced cluster: 14.6%) showed a higher level of controlled motivation. Associations were observed between the controlled cluster and factors such as higher disability scores, longer duration of pain, greater number of completed physiotherapy sessions, and elevated BMI. Notably, the controlled motivation cluster was linked with poorer clinical outcomes. CONCLUSIONS: This study provides insights into the exercise motivation of patients with CLBP, revealing that while most patients were primarily autonomously motivated, a notable subgroup exhibited lower, controlled motivation. The presence of controlled motivation was associated with worse functioning, longer pain duration, and increased utilization of physiotherapy services. Although these findings suggest a link between motivational profiles and clinical outcomes, the cross-sectional design limits causal inferences. Východiská: Na význam stresu pri nádorových chorobách poukazovali lekári už od čias Galéna. Avšak až v posledných dvoch desaťročiach umožnil kombinovaný onkologický a neurovedný výskum exaktne preskúmať tento vzťah a popísať dráhy a mechanizmy, ktoré sprostredkúvajú stimulačný vplyv stresu na nádorový proces. Tento nepriaznivý účinok stresu je sprostredkovaný hlavne mediátormi sympatikoadrenálneho systému, noradrenalínom a adrenalínom. Tieto katecholamíny aktiváciou adrenergických receptorov v nádorovom mikro- a makroprostredí stimulujú proliferáciu nádorových buniek a neoangiogenézu a inhibujú protinádorovú imunitu a znižujú účinnosť štandardnej protinádorovej liečby. Zistilo sa tiež, že intervencie obmedzujúce pôsobenie stresu na organizmus nie len že zlepšujú kvalitu života onkologických pacientov, ale môžu zlepšovať aj ich prežívanie. Vzhľadom na komplexnosť vplyvu stresu na organizmus sa experimentálne a klinické štúdie v prevažnej väčšine zameriavajú na skúmanie účinku jednej intervencie obmedzujúcej stimulačný vplyv sympatikoadrenálneho systému na nádorový proces. Cieľ: Zámerom tohto názorového článku je poukázať na možnosť synergického účinku kombinácie viacerých intervencií obmedzujúcich aktiváciu sympatikoadrenálneho systému a na základe dostupných údajov navrhnúť takú kombináciu týchto intervencií, ktorá je aplikovateľná v podpornej liečbe onkologických pacientov už aj v súčasnosti. Záver: Protokol Synergy, ktorý zahŕňa nefarmakologické intervencie zamerané na obmedzenie pôsobenia stresu, má potenciál zlepšiť kvalitu života a u určitých pacientov aj prognózu ich choroby. Zavedenie tohto protokolu do bežnej klinickej praxe síce bude vyžadovať vyriešenie personálnych a finančných aspektov, ktoré s jeho implikáciou súvisia, výsledkom ale môže byť významné zvýšenie úrovne starostlivosti o onkologických pacientov.
Background: The importance of stress in cancer has been noted by physicians since the time of Galen. However, it is only in the last two decades that combined oncological and neuroscientific research has allowed to explore this relationship in an exact way and to describe the pathways and mechanisms that mediate the stimulatory effect of stress on cancer. This adverse effect of stress is mediated mainly by the mediators of the sympathoadrenal system, norepinephrine and epinephrine, which, by activating adrenergic receptors in the tumor micro- and macro-environment, stimulate tumor cell proliferation and neoangiogenesis and inhibit antitumor immunity, reducing the efficacy of standard anticancer therapies. It has also been found that interventions reducing the effects of stress on the body not only improve the quality of life of cancer patients but may also improve their survival. Given the complexity of the impact of stress on the organism, experimental and clinical studies have overwhelmingly focused on investigating the effect of a single intervention reducing the stimulatory influence of the sympathoadrenal system on the cancer process. Purpose: The aim of this opinion article is to highlight the possibility of a synergistic effect of a combination of several interventions limiting the activation of the sympathoadrenal system and, based on the available data, to propose a combination of these interventions that is applicable in the supportive treatment of cancer patients even nowadays. Conclusion: The Protocol Synergy, which includes non-pharmacological interventions aimed at reducing the effects of stress on the cancer patient, has the potential to improve the quality and, in certain patients, the prognosis of their disease. Although the introduction of this protocol into routine clinical practice will require addressing the personnel and financial aspects associated with its implementation, it has the potential to significantly improve the level of care for cancer patients.
