Electronic Health Record (EHR) systems are now being developed in many places. More advanced systems provide also reminder facilities, usually based on if-then rules. In this paper we propose a method how to build the reminder facility directly upon the guideline interchange format (GLIF) model of medical guidelines. The method compares data items on the input of EHR system with medical guidelines GLIF model and is able to reveal if the input data item, that represents patient diagnosis or proposed patient treatment, contradicts with medical guidelines or not. The reminder facility can be part of EHR system itself or it can be realized by a stand-alone reminder system (SRS). The possible architecture of stand-alone reminder system is described in this paper and the advantages of stand-alone solution are discussed. The part of the EHR system could be also a browser that would present graphical GLIF model in easy to understand manner on the user screen. This browser can be data driven and focus attention of user to the relevant part of medical guidelines GLIF model.

Cíle: Matematické modelování v medicíne (fyziologii) se objevuje ve výzkumu, klinické praxi i výuce. Pro implementaci modelu existuje rada vhodných jazyku a modelovacích nástroju. Pro lepší názornost pri používání modelu pri výuce vznikají grafické simulátory. Je treba, aby spolu mohli tvurci modelu lépe spolupracovat. Proto by melo být možné propojit modely napsané v ruzných jazycích. Výukové simulátory musí být pro studenty i vyucující dobre dostupné. Nutnost instalace aplikace muže být prekážkou. Je potreba zlepšit možnosti sdílení modelu napsaných v jazyce Modelica. Metody: Standardizované rozhraní FMI umožnuje propojení modelu napsaných v rozdílných jazycích. Modely využívané ve výuce jsou vytváreny na platforme .NET, která umožnuje jejich spouštení v prohlížeci. Vzniká editor modelu v jazyce Modelica spustitelný v prohlížeci. Bude propojený s webovým úložištem modelu. Výsledky: Byl implementován simulacní runtime, solver a simulacní centrum v jazycích .NET. Jsou využívány ve webových výukových simulátorech. Budou využity také ve spojení se vznikajícím webovým editorem modelu.
Background: Mathematical modelling in medicine (physiology) takes place in research, clinical practice and learning. There are several suitable modelling languages and tools for physiological model implementation. Graphical simulators are created in order to make models used in teaching more illustrative. Objectives: It is important to make model developers' cooperation easier. For this reason it should be possible to interconnect models written in different languages. Elearning simulators must be well available for students and teachers. Necessity to install the application can be an obstruction. Methods for sharing models written in Modelica language should be enhanced. Methods: Standardised interface FMI allows interconnection of models written in different languages. E-learning models are developed on .NET platform. It enables them to run in web browser. The editor for models written in Modelica is being developed. It will run in browser and will be interconnected with web based model repository. Results: Simulation runtime, solver and simulation centre was implemented in .NET languages. They are used in web e-learning applications. They will be also used together with web-based model repository, that is being developed now. Conclusions: New tools will simplify model creation and interconnection. They will also improve collaboration of model developers.

Well defined biomacromolecular patterns such as binding sites, catalytic sites, specific protein or nucleic acid sequences, etc. precisely modulate many important biological phenomena. We introduce PatternQuery, a web-based application designed for detection and fast extraction of such patterns. The application uses a unique query language with Python-like syntax to define the patterns that will be extracted from datasets provided by the user, or from the entire Protein Data Bank (PDB). Moreover, the database-wide search can be restricted using a variety of criteria, such as PDB ID, resolution, and organism of origin, to provide only relevant data. The extraction generally takes a few seconds for several hundreds of entries, up to approximately one hour for the whole PDB. The detected patterns are made available for download to enable further processing, as well as presented in a clear tabular and graphical form directly in the browser. The unique design of the language and the provided service could pave the way towards novel PDB-wide analyses, which were either difficult or unfeasible in the past. INTRODUCTION: Monitoring for various health conditions (e.g., breast cancer, hypertension) has become common practice. However, there is still no established tool for regular monitoring of cognition. In this pilot longitudinal study, we examined the utility and feasibility of internet-based cognitive self-monitoring using data from the first 12 months of this ongoing study. METHOD: Cognitively healthy community-dwelling older adults (Montreal Cognitive Assessment ≥ 26) were enrolled on a rolling basis and were trained in self-administration of the internet-based version of the CogState Brief Battery. The battery uses playing cards and includes Detection, Identification, One Back, and One Card Learning subtasks. RESULTS: Of the 118 participants enrolled, 26 dropped out, mostly around first in-home session. Common reasons for participant attrition were internet browser problems, health problems, and computer problems. Common reasons for delayed session completion were being busy, being out of town, and health problems. Participants needed about one reminder phone call per four completed sessions or one reminder email per five completed sessions. Performance across the monthly sessions showed slight (but significant) improvement on three of the four tasks. Change in performance was unaffected by individual characteristics with the exception of previous computer use, with less frequent users showing greater improvement on One Card Learning. We also found low intraindividual variability in monthly test scores beyond the first self-administered testing session. CONCLUSIONS: Internet-based self-monitoring offers a potentially feasible and effective method of continuous cognitive monitoring among older adults.

R-loops are common non-B nucleic acid structures formed by a three-stranded nucleic acid composed of an RNA-DNA hybrid and a displaced single-stranded DNA (ssDNA) loop. Because the aberrant R-loop formation leads to increased mutagenesis, hyper-recombination, rearrangements, and transcription-replication collisions, it is regarded as important in human diseases. Therefore, its prevalence and distribution in genomes are studied intensively. However, in silico tools for R-loop prediction are limited, and therefore, we have developed the R-loop tracker tool, which was implemented as a part of the DNA Analyser web server. This new tool is focused upon (1) prediction of R-loops in genomic DNA without length and sequence limitations; (2) integration of R-loop tracker results with other tools for nucleic acids analyses, including Genome Browser; (3) internal cross-evaluation of in silico results with experimental data, where available; (4) easy export and correlation analyses with other genome features and markers; and (5) enhanced visualization outputs. Our new R-loop tracker tool is freely accessible on the web pages of DNA Analyser tools, and its implementation on the web-based server allows effective analyses not only for DNA segments but also for full chromosomes and genomes.

After the first cases of influenza A(H1N1)v in Mexico and the USA became public, Fresenius Medical Care established a case-based reporting of cases for all of its dialysis clinics located in Europe, Middle East, Africa and Latin America. This paper aims to describe mortality rates of patients on dialysis and to assess the risk profiles. METHODS: The survey was developed in Lotus Notes with a secure browser-based form. The form was open to 602 Fresenius Medical Care clinics located in Europe, Middle East, Africa and Latin America. RESULTS: As of 3 September 2009, 306 cases have been reported by 85 clinics located in Argentina, Chile, Brazil, UK and Spain. The mean age was 52.7 +/- 17.7 years. The majority of cases (70.6%) were from 20- to 44-year-old and 45- to 64-year-old subgroups. Moreover, 35.3% had no associated comorbidity, 20.3% had two and 4.6% three comorbidities, with heart disease being the most frequent. Fever was the most common symptom, present in 94.4% of the cases, followed by cough (78.8%) and muscle and joint pain (69.3%). Eighty-seven percent were treated with antiviral agents, the majority with oseltamivir. One hundred and three patients (34%) were admitted to hospital because of influenza. Pneumonia was reported for 69 cases, out of which 52 patients belonged to a high-risk group. Mortality rate of all the patients (confirmed, probable and suspected cases) was around 5%. CONCLUSION: End-stage renal disease patients should be included in first ranks of the priority list for the influenza A (H1N1)v vaccine, as already advocated by some healthcare authorities.

National Medical Library (NML) of the Czech Republic operates MEDVIK Portal - a web application for access to several bibliographic databases produced by NML and cooperating institutions. The main databases are Medvik Catalogue describing library collections with Union catalogue of Czech medical libraries holdings, and Bibliographia medica Czechoslovaka - Czech national medical bibliography. The total amount of bibliographic records approximates 700 thousands. Technical design of MEDVIK portal with multiple underlying databases was not sufficient for fast and reliable retrieval which has led to further development. An aggregated database specially optimized for searching tasks has been created. The database is updated automatically from production databases and allows efficient access to all bibliographic data from one access point using full-text search approach. This design provides usual Google-like search experience, but there is the need to add more functionality to achieve better precision and recall. This need comes from obvious limits of full-text search and also from our users feedback and recent NML's survey of health workers information behaviour and needs. NML translates Medical Subject Headings (MeSH) into Czech language using MTMS. Majority of our bibliographic records are indexed with MeSH descriptors with qualifiers and NML Subjects terms which can be used for concept-based and context-sensitive tools. We have focused on MeSH semantics and NML Subjects to develop following features: search term suggestion and query builder, detailed filters, multiple clustering of search results, tag clouds for query representation and browsing support, related documents display, and the Subject browser - aggregating MeSH, NML Subjects and Supplementary Concepts Records. The techniques used to implement these features will be presented together with detailed new user interface explanation.
Introduction: Investigation of molecular mechanisms of human disorders, especially rare diseases, require exploration of various knowledge repositories for building precise hypotheses and complex data interpretation. Recently, increasingly more resources offer diagrammatic representation of such mechanisms, including disease-dedicated schematics in pathway databases and disease maps. However, collection of knowledge across them is challenging, especially for research projects with limited manpower. Methods: In this article we present an automated workflow for construction of maps of molecular mechanisms for rare diseases. The workflow requires a standardized definition of a disease using Orphanet or HPO identifiers to collect relevant genes and variants, and to assemble a functional, visual repository of related mechanisms, including data overlays. The diagrams composing the final map are unified to a common systems biology format from CellDesigner SBML, GPML and SBML+layout+render. The constructed resource contains disease-relevant genes and variants as data overlays for immediate visual exploration, including embedded genetic variant browser and protein structure viewer. Results: We demonstrate the functionality of our workflow on two examples of rare diseases: Kawasaki disease and retinitis pigmentosa. Two maps are constructed based on their corresponding identifiers. Moreover, for the retinitis pigmentosa use-case, we include a list of differentially expressed genes to demonstrate how to tailor the workflow using omics datasets. Discussion: In summary, our work allows for an ad-hoc construction of molecular diagrams combined from different sources, preserving their layout and graphical style, but integrating them into a single resource. This allows to reduce time consuming tasks of prototyping of a molecular disease map, enabling visual exploration, hypothesis building, data visualization and further refinement.
