Endoskopické výkony při obstrukcích žlučových cest plně převzaly post metody první volby léčby i s jejich různými komplikacemi. Uvádíme kazuistiku muže došetřovaného pro obstrukční ikterus s následně zjištěným pneumoskrotem po endoskopickém výkonu (ERCP). V diskusi se zamýšlíme nad klinickou diagnostikou, klinickými známkami lézí duodena a možnostmi terapeutického postupu.
Therapeutic endoscopy is mostly primary method in treatment of biliary tree obstruction with its various complications. A case review describing an oncological pacient examined for obstructive icterus, with recognised pneumoscrotum after endoscopic examination (ERCP). In discussion we try to point out the need of knowledge of clinical examination, clinical signs of duodenal lesions and optimal treatment measures.

A two-year-old girl with two weeks of abdominal pain, vomiting, and food refusal, ten months after percutaneous endoscopic gastrostomy insertion because of inadequate peroral intake, was admitted to a tertiary centre hospital. On admission, the extracorporeal part of the gastrostomy was much shortened. X-ray examination revealed migration of the end of the gastrostomy tube with a left-shifted course of the tube through the duodenum. Gastroscopy and subsequently laparotomy were performed. A longitudinal pressure necrosis was identified under the tube, with two perforations in the duodenojejunal region. Ten centimeters of that duodenojejunal region were resected, and end-to-end anastomosis was made. The migration of the gastrostomy was probably caused by insufficient care by the parents. Pathophysiologically, the tube caused the pressure necrosis in the duodenojejunal area; this was supported by histology. This is a hitherto undescribed complication of a percutaneous endoscopic gastrostomy, showing that migration of the gastrostomy to the deeper part of the small bowel can lead to pressure necrosis, a potentially life-threatening condition in children which cannot be treated without invasive procedures.

Komplikace po diagnostické gastroskopii jsou vzácné a řídké. Raritou jsou zejména ty, které si způsobí pacient. Autorky prostřednictvím článku informují o perforaci vzniklé při diagnostické duodenoskopii s následnou biopsií v souvislosti s nespolupracujícím klientem.
Complications of diagnostic gastroscopy are rare especially the ones that the patient causes himself. The authors inform about a case of uncooperative patient who sustained a perforation during diagnostic duodenoscopy with biopsy.

Gastroduodenální vředová choroba je charakterizována vředovým postižením žaludku či dvanáctníku, přičemž častěji dochází k výskytu vředových lézí v oblasti duodena. Mezi závažné komplikace tohoto onemocnění patří krvácení, perforace vředu či penetrace do okolního orgánu. Naše kazuistika prezentuje případ staršího muže, u něhož došlo k penetraci žaludečního vředu do slinivky břišní. Ta nebyla diagnostikována a pacient se do péče našeho pracoviště dostal pro recidivující hematemézu bez prokázaného zdroje krvácení. V průběhu diagnostického procesu bylo vysloveno podezření na přítomnost intramurálně uloženého tumoru žaludku. Na naší klinice byla úspěšně provedena excize suspektní léze s následným histologickým potvrzením diagnózy penetrujícího vředu. Pooperační průběh byl komplikován rozvojem pankreatické píštěle, avšak konzervativním postupem bylo dosaženo úplného zotavení pacienta.
Gastroduodenal ulcer disease is characterized by ulceration of the stomach or duodenum, with ulcer lesions occurring more frequently in the duodenum. Serious complications of this disease include bleeding, perforation of the ulcer and penetration into surrounding organ. Our report presents the case of an older man who had a gastric ulcer penetrating into the pancreas. This was not diagnosed at the time of the penetration. The patient came to our care for recurrent hematemesis without a proven source of bleeding. During the diagnostic process, the presence of an intramural gastric tumor was suspected. An excision of the suspected lesion was successfully performed at our clinic, followed by histological confirmation of perforated ulcer. The postoperative course was complicated by the development of a pancreatic fistula. However, conservative treatment resulted in complete recovery of the patient.
