CALME is a non-neoplastic enlargement of normal vulvar soft tissue in response to prepubertal hormonal stimulation resembling unilateral breast enlargement. The prognosis is good and biopsy or surgical treatment is not necessary. We present a 13-year-old pubertal girl who observed gradual enlargement of the right labia at the age of 11 years and two months. Unilateral labium majus hypertrophy without inflammatory and neoplastic features was diagnosed as pediatric asymmetric labia majora enlargement (CALME) after ultrasound and MR examination.
CALME je nenádorové zvětšení normální vulvální měkké tkáně v reakci na prepubertální hormonální stimulaci připomínající jednostranné zvětšení prsou. Prognóza je dobrá a biopsie nebo chirurgická léčba není nutná. Prezentujeme 13letou pubertální dívku, která pozorovala ve věku 11 let a dvou měsíců postupné zvětšení pravého stydkého pysku. Jednostranná hypertrofie labium majus bez zánětlivých a neoplastických rysů byla po ultrazvukovém a MR vyšetření diagnostikována jako dětské asymetrické zvětšení velkého stydkého pysku (CALME).

Akútne genitálne ulcerácie sú zriedkavo sa vyskytujúce kožné prejavy so širokou diferenciálnou diagnostikou. Etiopatogenéza ochorenia nie je úplne objasnená, hoci existujú určité príčinné súvislosti s daným ochorením. V texte je popísaný konkrétny prípad pacientky s genitálnymi vredmi, asociovaný s akútne prebiehajúcou Lymskou boreliózou.
Acute genital ulcers are rare skin manifestations with a wide differential diagnosis. The etiopathogenesis of the disease is not completely understood, although there are certain causal relationships with the condition. The text describes a specific case of a female patient with genital ulcers associated with acute Lyme disease.

Cíl: Epidemiologie a zhodnocení významu chirurgických okrajů v léčbě HSIL vulvy – analýza vlastního souboru. Soubor a metodika: Do studie byly prospektivně zařazovány ženy s nálezem histologicky potvrzené HPV asociované prekancerózy vulvy od 10/2016 do 1/2022. Celkem bylo zařazeno 65 žen. Po chirurgickém ošetření byly ženy podle chirurgického okraje rozděleny do skupin a sledovány v pravidelných intervalech. Výsledky: Sedmnáct žen (26 %) s diagnostikovanou HPV asociovanou prekancerózou vulvy bylo ve věku do 49 let, 48 žen (74 %) bylo ve věku nad 49 let. Recidivy HPV asociovaných prekanceróz byly v excizích s dosahem HSIL do okraje, resp. okrajem do 1 mm vč. a okrajem 1–3 mm diagnostikovány ve 12,3 %, resp. 1,5 % a 3,1 % případů. Riziko recidivy při dosahu léze do okraje je ve srovnání se zdravým okrajem 1–3 mm statisticky významné. Závěr: Při zachování minimálního zdravého okraje (1–3 mm) je riziko recidivy HPV asociovaných prekanceróz vulvy akceptovatelné při současném příznivém kosmetickém efektu a minimalizaci narušení psychosexuální funkcí žen. Nutností jsou dlouhodobé pravidelné kontroly.
Objective: Epidemiology and evaluation of the importance of surgical margins in the treatment of vulvar H-SIL – analysis of own data. Material and methods: The prospective study included women diagnosed with HPV-associated vulvar epithelial neoplasia from 10/2016 to 1/2022. A total of 65 women were included. After surgical treatment, the women were distributed to groups according to surgical margins and were followed-up at regular intervals. Results: Seventeen women (26%) diagnosed with HPV-associated vulvar intraepithelial neoplasia were under 49 years, whereas 48 women (74%) were older than 49 years. Recurrence rates of HPV-associated precancers were 12.3%, 1.5% and 3.1% in excisions with positive margins up to 1 mm peripheral margins and 1–3 mm peripheral margins, respectively. The risk of recurrence when the lesion reaches the margin is statistically significant, compared to a healthy margin of 1–3 mm. Conclusion: Keeping the minimal healthy margin (1–3 mm) seems to be an acceptable risk of recurrence of HPV-associated vulvar intraepithelial neoplasia with positive cosmetic effect and minimal risk of disturbing the psychosexual functions of women. Long-term regular follow-up is necessary.

Pigmentované léze vulvy představují širokou skupinu diagnostických jednotek, pro jejichž přesnou identifikaci je nezbytné histopatologické vyšetření. Článek předkládá přehled pigmentovaných lézí vulvy. Dále se podrobněji zabývá bazocelulárním karcinomem a prezentuje kazuistiku pigmentovaného bazocelulárního karcinomu u 85leté pacientky.
Pigmented vulvar lesions represent a wide range of diagnostic units with the essential role of precise histopathologic identification. The article reviews pigmented vulvar lesions. Furthermore, it deals with basal cell carcinoma in more detail and presents a case of an 85-year-old female with vulvar pigmented basal cell carcinoma.

Anatomical terminology is a cornerstone of medical terminology and since 1895 there has been a norm (nomenclature), regularly revised, extended, and updated. The latest official version issued by the Federative International Program on Anatomical Terminology was approved by the International Federation of Associations of Anatomists in 1998 as Terminologia Anatomica (TA). Second edition of the TA (TA2) has now been prepared for approval. Each change must be meticulously discussed and considered as it affects and influences not only anatomists but also all clinicians, physicians, scientists, teachers, and students in medical fields. There are several reasons for direct and deliberate changes, but also reasons for rejection of changes that could lead to disintegration and rejection of the nomenclature revision by the expert population. The latter reasons comprise changes of terms with originally different (inappropriate/incorrect/erroneous) meanings or with originally potentially vulgar meanings that are nowadays perceived as neutral. Some examples and explanations concerning the names of the principal structures of the female external genitalia (vulva, cunnus, pudendum, labia pudenda, rima pudendi, clitoris, and perineum) and some internal ones (vagina, uterus) in Latin and Greek are given. An overview is presented of the development and frequency of their usage, mainly in ancient times but also in medieval and later texts. It is suggested that the above-stated reasons provide no fundamental argument for changing the terminology, as all the Latin terms used in TA are nowadays considered neutral.

Aesthetic surgery of the female genitalia is on the rise. Many procedures are performed. Body dysmorphic disorder (BDD) is a psychiatric disorder and often goes unrecognized in plastic surgery. This review examines actual knowledge of plastic surgery in gynecology, knowledge of motivational factors of women, and BDD. We carried out a systematic electronic search. We used a combination of relevant keywords to construct the search strategy. Included studies were systematic reviews or primary studies published 1990-2017 and containing information about BDD. Reviewers independently assessed study eligibility, extracted data, and assessed quality, undertaking narrative synthesis. The research revealed a variety of plastic procedures available and why women consider undergoing labia reduction surgery.
