Senningova operácia je paliatívna operácia nekorigovanej transpozície veľkých ciev, ktorá je bez chirurgickej liečby nezlučiteľná s dlhodobým prežívaním. Nevýhodou riešenia je pravá komora v pozícii systémovej komory a početné supraventrikulárne poruchy rytmu, najmä dysfunkcia sinoatriálneho uzla. Záťažovým testom sme vyšetrili 12 chlapcov a 5 dievčat s transpozíciou veľkých ciev po operácii podľa Senninga. Pacienti boli vo veku 7 až 16 rokov, priemerne 11,8 roka, bez subjektívnych ťažkostí. Použili sme metódu bicyklovej ergometrie so záťažovým protokolom podľa Jamesa. Vyhodnocovali sme toleranciu vyšetrovaných na záťaž určením indexu funkčnej aeróbnej kapacity a počas vyšetrenia sme zaznamenávali poruchy srdcového rytmu. U 9 pacientov sme zistili zníženú toleranciu na záťaž a žiadny vyšetrený pacient nedosiahol počas maximálnej záťaže normu maximálnej srdcovej frekvencie. Obraz dysfunkcie sinoatriálneho uzla malo na začiatku testu 10 pacientov. Iba u jedného pacienta sa pri maximálnej záťaži neobnovil sínusový rytmus. U tohto pacienta sa aj dalšími vyšetreniami potvrdila závažná porucha sinoatriálneho uzla a má implantovaný kardiostimulátor. Záťažové vyšetrenie je vhodný test na zhodnotenie závažnosti porúch rytmu a na preverenie fyzickej zdatnosti pacientov po Senningovej operácii.
Senning operation is a paliative correction of D-transposition of the great arteries, which is without surgical intervention universally fatal. Disadvantage of this correction is the right ventricle in the position of systemic one, and many supraventricular rhythm disorders, mainly sinus node dysfunction. In our group of patients after Senning correction who underwent excercise testing, there were 12 boys and 5 girls in the age of 7-16 years, mean 11,8 years. All the patients had no subjective problems. Authors used the method of bicycle ergometry and evaluated excercise tolerance by determinating index of functional aerobic impairment (FAI). They also recorded all the rhythm disorders during the testing. In 9 patients they determinated low excercise tolerance and none of the patients did reach normal maximal heart rate at the peak excercise. Sinus node dysfunction occured in 10 patients at the beginning of the testing. Only in one patient normal sinus rhythm did not occure during the whole testing. This patient required another testing and serious sinus nodal dysfunction was stated. Therefore he needed pacemaker implantation. Exercise testing is a suitable method for evaluating rhythm disorders and excercise tolerance in patients after Senning correction.
