Horizontal gene transfer (HGT) is a key driver in the evolution of bacterial genomes. The acquisition of genes mediated by HGT may enable bacteria to adapt to ever-changing environmental conditions. Long-term application of antibiotics in intensive agriculture is associated with the dissemination of antibiotic resistance genes among bacteria with the consequences causing public health concern. Commensal farm-animal-associated gut microbiota are considered the reservoir of the resistance genes. Therefore, in this study, we identified known and not-yet characterized mobilized genes originating from chicken and porcine fecal samples using our innovative pipeline followed by network analysis to provide appropriate visualization to support proper interpretation.

INTRODUCTION: Existing advance care planning (ACP) definitional frameworks apply to individuals with decision-making capacity. We aimed to conceptualize ACP for dementia in terms of its definition and issues that deserve particular attention. METHODS: Delphi study with phases: (A) adaptation of a generic ACP framework by a task force of the European Association for Palliative Care (EAPC); (B) four online surveys by 107 experts from 33 countries, September 2021 to June 2022; (C) approval by the EAPC board. RESULTS: ACP in dementia was defined as a communication process adapted to the person's capacity, which includes, and is continued with, family if available. We identified pragmatic boundaries regarding participation and time (i.e., current or end-of-life care). Three interrelated issues that deserve particular attention were capacity, family, and engagement and communication. DISCUSSION: A communication and relationship-centered definitional framework of ACP in dementia evolved through international consensus supporting inclusiveness of persons with dementia and their family. HIGHLIGHTS: This article offers a consensus definitional framework of advance care planning in dementia. The definition covers all stages of capacity and includes family caregivers. Particularly important are (1) capacity, (2) family, (3) engagement, and communication. Fluctuating capacity was visualized in relation to roles and engaging stakeholders.

PURPOSE: The Core Outcome Measures Index (COMI) is a short, multidimensional instrument translated into several languages that covers five domains recommended in the assessment of outcome in patients with low-back and neck pain. The purpose of this study was to cross-culturally adapt the COMI from English to Czech language and to test the face and construct validity and reproducibility of its results in patients with low-back and neck pain. METHODS: Participants (n = 125) were included from primary and secondary care. The participants reported moderate pain and disability levels. All participants filled in the COMI forms before and after surgery. Descriptive statistics, Wilcoxon paired test, Crombach's alpha, principal component analysis and information entropy calculation were used. RESULTS: The instrument was successfully forward and back-translated. It can be seen that the questionnaire applied as part of our intervention study produces answers with a sufficient degree of variability and with a satisfactory degree of representation of extreme values. It can be also seen that the questionnaire can diagnose an objectively occurring change associated with the surgeon within the intervention procedure. Our other findings support the idea of a possible reduction in the number of questions that measure the same latent variable. Our investigations also showed that it is possible to reduce the range of the point scale of the perception of pain to 5 degrees of intensity and thus unify the range with the other questions. CONCLUSION: The Czech COMI shows acceptable properties and is thus suitable to use as a short instrument for measuring important domains in patients with low-back and neck pain.

We investigated neural correlates of Emotion Recognition Accuracy (ERA) using the Assessment of Contextualized Emotions (ACE). ACE infuses context by presenting emotion expressions in a naturalistic group setting and distinguishes between accurately perceiving intended emotions (signal), and bias due to perceiving additional, secondary emotions (noise). This social perception process is argued to induce perspective taking in addition to pattern matching in ERA. Thirty participants were presented with an fMRI-compatible adaptation of the ACE consisting of blocks of neutral and emotional faces in single and group-embedded settings. Participants rated the central character's expressions categorically or using scalar scales in consequent fMRI scans. Distinct brain activations were associated with the perception of emotional vs. neutral faces in the four conditions. Moreover, accuracy and bias scores from the original ACE task performed on another day were associated with brain activation during the scalar (vs. categorical) condition for emotional (vs. neutral) faces embedded in group. These findings suggest distinct cognitive mechanisms linked to each type of emotional rating and highlight the importance of considering cognitive bias in the assessment of social emotion perception.

OBJECTIVES: This study aimed to explore the relationship between plasma proteome and the clinical features of Major Depressive Disorder (MDD) during treatment of acute episode. METHODS: In this longitudinal observational study, 26 patients hospitalized for moderate to severe MDD were analyzed. The study utilized Liquid Chromatography with Tandem Mass Spectrometry (LC-MS/MS) alongside clinical metrics, including symptomatology derived from the Montgomery-Åsberg Depression Rating Scale (MADRS). Plasma protein analysis was conducted at the onset of acute depression and 6 weeks into treatment. Analytical methods comprised of Linear Models for Microarray Data (LIMMA), Weighted Correlation Network Analysis (WGCNA), Generalized Linear Models, Random Forests, and The Database for Annotation, Visualization and Integrated Discovery (DAVID). RESULTS: Five distinct plasma protein modules were identified, correlating with specific biological processes, and uniquely associated with symptom presentation, the disorder's trajectory, and treatment response. A module rich in proteins related to adaptive immunity was correlated with the manifestation of somatic syndrome, treatment response, and inversely associated with achieving remission. A module associated with cell adhesion was linked to affective symptoms and avolition, and played a role in the initial episodes and treatment response. Another module, characterized by proteins involved in blood coagulation and lipid transport, exhibited negative correlations with a variety of MDD symptoms and was predominantly associated with the manifestation of psychotic symptoms. CONCLUSION: This research points to a complex interplay between the plasma proteome and MDD's clinical presentation, suggesting that somatic, affective, and psychotic symptoms may represent distinct endophenotypic manifestations of MDD. These insights hold potential for advancing targeted therapeutic strategies and diagnostic tools. LIMITATIONS: The study's limited sample size and its naturalistic design, encompassing diverse treatment modalities, present methodological constraints. Furthermore, the analysis focused on peripheral blood proteins, with potential implications for interpretability.

OBJECTIVES: To create a Czech version of the Labor Coping Scale (LCS) evaluation tool intended for midwives when caring for a woman with labor pain, to evaluate its psychometric properties and to find out the opinion on it in clinical practice. MATERIALS AND METHODS: Are use a combination of methods. By using repeated back translation, the Czech version of the LCS tool was created. It included an assessment of the tool's content validity index and a questionnaire survey determining the midwives' approach to labor pain (N = 419), supplemented by a focus group method (N = 16 midwives). RESULTS: A new evaluation tool for the management of labor pain was created - the Pain Coping Scale, which assesses the management of labor pain, not intensity, like previous evaluation scales. It is a tool that maps 5 areas on a scale of 0-10. CONCLUSION: As part of her work, a midwife should be able to adequately assess a woman's management of labor pain, using a suitable tool, to reveal the factors that influence the development and experience of labor pain, and then choose an appropriate strategy in the care of a woman with labor pain. A new LCS labor pain assessment tool could help with this in the Czech environment.

INTRODUCTION: Leber hereditary optic neuropathy (LHON) is the most frequent mitochondrial disease causing dyschromatopsia and progressive central visual loss that is subacute in progression and painless. Several studies have been published assessing QoL in patients with LHON, but no estimate of the economic burden has been reported to date. This study aims to quantify direct non-medical and indirect costs (productivity loss) incurred by LHON patients and their informal caregivers in Czechia and Slovakia, as well as to assess their quality of life. METHODS: The study was performed in 27 adults and children with LHON. To determine the socioeconomic burden of LHON, separate questionnaires for adults, children, and their parents were developed, including demographic and socioeconomic data. The following data were collected: age, education, family size, severity of LHON, non-medical direct and indirect costs of LHON. RESULTS: The mean age of adult respondents was 36.1 years (SD 13.1; n = 21). The total cost of absenteeism was EUR 1003 per person/year in adult employees, and EUR 2711 per person/year in children's parents. The productivity loss as a consequence of LHON due to combined relative absenteeism and relative presenteeism was estimated at EUR 9840 per an adult patient/year, and EUR 6298 per a parent/year, respectively. The mean cost of informal care was estimated at EUR 4502 (SD 4772; n = 6) per person/year. The mean VFQ-25 score for adult patients with LHON was 43.47 (SD 15.86). CONCLUSION: The results of this study clearly show that patients with LHON and their families face an extensive socioeconomic burden related to this rare disease. Early, timely and appropriate access to diagnosis, treatment, and reimbursement decisions, but also to psychological counselling and services may help the patients and their relatives adapt and cope with the challenging aspects of vision loss and life with the disease.

PRCIS: Deep sclerectomy (DS) with fibrin adhesive can constitute a safe alternative to the classic procedure using sutures, providing nonallergenic, nontoxic, and secure adhesion with no sign of aqueous humor outflow obstruction postoperatively. OBJECTIVE: To evaluate short and medium-term postoperative results of DS with a fibrin sealant. PATIENTS AND METHODS: This prospective, noncomparative, interventional case series involves 12 eyes of 12 patients with uncontrolled open angle glaucoma who underwent DS with Esnoper (Clip or V2000) implant between February 2021 and March 2022. A novel method of wound closure (sclera, Tenon fascia, and conjunctiva) employing fibrin glue was used instead of classic sutures. Surgical outcomes assessed include: intraocular pressure and glaucoma therapy reduction, best-corrected visual acuity changes, and number of complications registered peri and postoperatively. All measurements were performed preoperatively, as well as at 1 day, at 1 and 2 weeks, and at 1, 2, 3, 6, 9, and 12 months after surgery. RESULTS: The mean intraocular pressure decreased from 24.0 ± 9.1 mm Hg to 13.8 ± 6.3 mm Hg at 1 year postoperatively ( P < 0.001). Kaplan-Meier survival analysis revealed complete and qualified success rates of 83.3% and 91.7%. The mean glaucoma therapy decreased from 3.2 ± 1.1 to 0.8 ± 1.3 drugs 12 months after surgery ( P < 0.001). Nd:YAG goniopunture was performed in 2 eyes at 1 and 12 months postoperatively. No significant best-corrected visual acuity changes were registered. Perioperatively, we noted a trabeculo-descemet microperforation in 1 eye, transient hypotony in 5 eyes, and mild hyphema in 2 eyes. CONCLUSIONS: Fibrin adhesive provided an effective closure in sutureless DS in the patients included in our study. This modification of classical DS may simplify the surgical technique, ensure secure wound adaptation, optimize healing, and lower the risk of inflammation and fibrosis postoperatively.

Úvod: Addenbrookský kognitivní test (The Addenbrooke‘s Cognitive Examination; ACE-III) je komplexní nástroj hodnotící pět kognitivních oblastí (pozornost, paměť, slovní produkce, jazyk, zrakově-prostorové schopnosti). Je citlivý na mírnou kognitivní poruchu i časná stádia demence a nahrazuje dřívější verzi (ACE-R). Cíl: Kulturní adaptace a validace české verze (ACE-III-CZ). Soubor a metodika: Do studie byli zařazeni zdraví jedinci a pacienti v počátečním stadiu demence ve věku 60 a více let z neurologických a geriatrických ambulancí čtyř fakultních nemocnic. Ověřovali jsme reliabilitu (Cronbachův koeficient alfa, McDonaldův koeficient omega), souběžnou validitu ACE-III-CZ a Mini-Mental State Examination (MMSE) (Paersonův korelační koeficient) a pomocí analýzy křivky ROC hodnoty AUC, senzitivitu a specifitu testu. Výsledky: Soubor tvořilo 250 respondentů (122 s diagnózou demence [průměrný věk 76,5 ± 7,1 let; 67,2 % žen] a 128 kognitivně zdravých [průměrný věk 71,1 ± 5,2 let; 77,3 % žen]). Vnitřní konzistence ACE-III-CZ byla velmi dobrá (Cronbachova alfa = 0,906; McDonaldovo omega = 0,937). Věk a stupeň vzdělání významně korelovaly s celkovým skórem a všemi doménovými skóry ACE-III-CZ (p < 0,001). Vysoké AUC (0,996; 95% CI 0,990–1,000) potvrdilo dobrou schopnost testu rozlišit osoby s demencí a bez demence. Optimální hraniční hodnota byla 89,5 bodu se senzitivitou 98 % a specificitou 100 %. Závěr: Výsledky naznačují vysokou diagnostickou schopnost ACE-III-CZ a její využitelnost v české klinické praxi.
Introduction: The Addenbrooke‘s Cognitive Examination (ACE-III) is a comprehensive tool assessing five cognitive domains (attention, memory, verbal production, language, and visuospatial ability). It is sensitive to both mild cognitive impairment and early stages of dementia and replaces an earlier version (ACE-R). Aim: Cultural adaptation and validation of the Czech version (ACE-III-CZ). Materials and methods: Healthy individuals and patients with early-stage dementia aged 60 years and older from neurological and geriatric outpatient clinics of four university hospitals were included in the study. We assessed reliability (Cronbach‘s alpha coefficient, McDonald‘s omega coefficient), concurrent validity of ACE-III-CZ and Mini-Mental State Examination (MMSE) (Pearson’s correlation coefficient), AUC values, sensitivity and specificity of the test using an ROC curve analysis. Results: The sample consisted of 250 subjects (122 with dementia [mean age 76.5 ± 7.1 years; 67.2% females] and 128 cognitively healthy [mean age 71.1 ± 5.2 years; 77.3% females]). The reliability of the ACE-III-CZ was very good (Cronbach‘s alpha = 0.906; McDonald‘s omega = 0.937). Age and level of education had significant correlation with the total score and all ACE-III-CZ domain scores (P < 0.001). A high AUC (0.996; 95% CI 0.990–1.000) confirmed a good ability of the test to differentiate between those with and without dementia. The optimal cut-off point was 89.5 with a sensitivity of 98% and a specificity of 100%. Conclusion: Results suggest a high diagnostic ability of ACE-III-CZ and its applicability in Czech clinical practice.
