Východiská a metodika: Koordinačné stredisko skríningu kolorektálneho karcinómu (KrCa) na Slovensku zbiera už 7. rok kvantitatívne a kvalitatívne údaje o skríningu tohto karcinómu pomocou elektronickej komunikácie a online štatistického spracovania kolonoskopických formulárov z certifikovaných kolonoskopických pracovísk. Výsledky: V priebehu 6 rokov bolo vyhodnotených 37 912 preventívnych kolonoskopií. Priemerný vek zúčastnených asymptomatických jedincov na skríningu KrCa bol 62,8 roka. Polypy boli odňaté spolu 13 977 osobám. Počet osôb s histologizovanými polypmi činil 13 235. Aspoň jeden adenóm malo 11 159 jedincov, 1 783 osobám bola zistená v adenóme ťažká dysplázia. Ďalším 1 109 jedincom bol kolonoskopicky odňatý polyp, ktorý spĺňal histologické kritéria adenokarcinómu. Za 6 rokov sledovania stúpol záchyt adenómov v skríningovanej populácii z 28 na 31 % a výskyt karcinómu klesol z 3,47 na 2,45 %. Záver: Výsledky skríningu sú zobrazením reprezentatívnej vzorky priemerne rizikovej populácie vo veku nad 50 rokov z hľadiska vzniku KrCa. Aim and Method: A coordination centre for colorectal cancer screening in Slovakia collects 7-year quantitative and qualitative data from electronic communications and online statistically-processed colonoscopy forms at certified workplaces. Results: Over the course of the last 6 years, 37,912 preventive colonoscopies were evaluated. The polyps were taken out in 13,977 individuals. The number of people with polyps analysed by histology was 13,235. At least one adenoma was observed in 11,159 individuals. An adenoma with severe dysplasia was observed in 1,783 individuals. Polyps with histological criteria of adenocarcinoma were removed from 1,109 subjects. The occurrence of adenomas has increased from 28 to 31% and the occurrence of colorectal cancer has decreased from 3.47 to 2.45%. Conclusion: The results provide information on the average risk of individuals over 50 years old of developing colorectal carcinoma.

Domáca parenterálna výživa je náhradná cesta a forma výživy u pacientov s ireverzibilným zlyhaním intestinálnych funkcií, u ktorých enterálna výživa nie je možná, resp. dlhodobo nedostačujúca s prehlbujúcim sa energetickým deficitom. Zahájením parenterálnej výživy v domácom prostredí sa zlepšuje kvalita života pacientov a tým aj priebeh základného ochorenia s optimalizáciou klinických prejavov malnutrície. K najčastejším indikáciám zahájenia domácej parenterálnej výživy patria rozsiahle redukcie absorbčnej plochy pri intestinálnych resekciách (pri chirurgických komplikáciách M. Crohn, pri ischemickej nekróze tenkého čreva v dôsledku trombózy vo VMS, resp. embólii alebo AS uzávere pri AMS), ale tiež aj funkčné zlyhanie čreva rôznej etiológie a nádorové ochorenia s dostatočnou utilizáciou základných živín. Home parenteral nutrition is an alternative pathway in patients with irreversible failure of intestinal functions, in whom enteral nutrition is either not possible or is insufficient in the long term, with growing energetic deficiency. The initiation of home parenteral nutrition improves patients' quality of life and also the course of the disease, leading to an optimisation of clinical symptoms of malnutrition. The most common indications for the initiation of home parenteral nutrition include an extensive reduction of the absorption area after intestinal resection (in patients with surgical complications of Crohn's disease, ischemic necrosis of the small intestine due to VMS thrombosis, embolism or closure of the AMS), functional failure of the gut from various causes and cases of cancer with sufficient utilisation of essential nutrients. Súvislosti medzi diabetes mellitus (DM) a karcinómom pankreasu zatiaľ nie sú jednoznačne objasnené. V našej práci sme chceli overiť, či pacienti s karcinómom pankreasu majú štatisticky významne nižšiu hladinu C-peptidu v priebehu orálneho glukózového tolerančného testu (oGTT) oproti kontrolnej skupine, bez ohľadu na súčasnú diagnózu diabetes mellitus, keďže takýchto štúdií bolo zatiaľ publikovaných málo. Metódy: V našej práci sme analyzovali hodnoty C-peptidu v 120 min oGTT u 29 pacientov s karcinómom pankreasu a 23 pacientov s chronickou pankreatitídou – kontrolný súbor. Výsledky: Hodnoty C-peptidu v 120 min oGTT u pacientov s karcinómom pankreasu neboli nižšie ako u kontrolného súboru, rozdiel nebol štatisticky významný (p = 0,257). Pomocou oGTT sme u 15 (51 %) pacientov s karcinómom pankreasu a u deviatich (39 %) pacientov v kontrolnej skupine zistili DM. Nezistili sme štatisticky významný rozdiel v hladinách C-peptidu u pacientov s DM a bez neho u pacientov s karcinómom pankreasu (p = 0,383) ani u pacientov s chronickou pankreatitídou (p = 0,877). Záver: Nepotvrdili sme nižšie hodnoty C-peptidu počas oGTT u pacientov s karcinómom pankreasu v porovnaní s kontrolným súborom. Pre posúdenie významu súvislostí diabetes mellitus a karcinómu pankreasu v diagnostike karcinómu sú potrebné ďalšie štúdie s väčším počtom pacientov. The mechanism responsible for diabetes mellitus (DM) in patients with pancreatic cancer remains controversial. The aim of our study is to verify that pancreatic cancer patients had lower C-peptide levels during an oral glucose tolerance test (oGTT) than the control subjects, regardless of the presence of diabetes mellitus, since until now only a few similar studies have been published. Methods: In our study we analysed C-peptide response at 120 minutes during the oGTT in 29 patients with pancreatic cancer and 23 patients with chronic pancreatitis – control subjects. Results: C-peptide levels, observed at 120 minutes during the oGTT in patients with pancreatic cancer, were not lower than those of control subjects, and the difference was not statistically significant (p = 0.257). By means of the oGTT, we detected diabetes mellitus in 15 (51%) patients in the pancreatic cancer group and in nine (39%) control subjects. There were no statistically significant differences in C-peptide levels in patients with and without DM, or in the pancreatic cancer group (p = 0.383) or in control subjects with chronic pancreatitis (p = 0.877). Conclusions: We did not confirm that a characteristic feature of pancreatic cancer patients is decreased C-peptide production during the oGTT in comparison with the control subjects. Further studies are needed in a large number of patients to assess the importance of impaired glucose tolerance or diabetes mellitus in pancreatic cancer detection. V odbornej literatúre bolo dosiaľ opísaných viac ako 25 chorôb, syndrómov a iných aberácií, ktoré sa spájajú s črevnou mikroflórou. Špecifické vzájomné vzťahy medzi niektorými baktériami a zdravím človeka sa týkajú napr. druhov Bifidobacterium spp., Faecalibacterium prausnitzii a Akkermansia muciniphila. Antibiotická terapia je dôležitá v boji proti patogénnym mikroorganizmom pri udržiavaní zdravia hostiteľa. Avšak antibiotiká neútočia iba na patogénne baktérie v čreve, ale majú tiež ničivý účinok na ekológiu komenzálnych druhov. Táto okolnosť prispieva aj k problémom s antibiotickou rezistenciou vrátane druhotného prenosu rezistentných génov. Vyriešenie otázok týkajúcich sa postantibiotickej dysbiózy bude kľúčom k vývoju terapeutických stratégií, ako je cielená terapia a probiotiká, predchádzanie narušeniu mikrobiómu a podpora obnovenia užitočných druhov.
Nowadays more than 25 diseases, syndromes, or other aberrations have been associated with the intestinal microbiota. Specific correlations have been made with potentially health-associated bacteria, such as some Bifidobacterium spp., Faecalibacterium prausnitzii and Akkermansia muciniphila. Antibiotic therapies are important in combating disease-causing microorganisms and maintaining host health. It is important to consider that antibiotics not only target pathogenic bacteria in the gut, but also can have damaging effects on the ecology of commensal species. This can reduce intrinsic colonization resistance and contribute to problems with antibiotic resistance, including lateral transfer of resistance genes. Solving of problems and questions regarding postantibiotic dysbiosis will be crucial for development of strategies, such as target therapeutics and probiotics, to prevent perturbations in the gut microbiota, the restoration of beneficial species and improvements in host health.
