Léčba gastrointestinálního traktu (GIT) může být za určitých podmínek významně ovlivněna ledvinami a případně jejich postižením. Jedním z možných scénářů této interakce je souběžné postižení ledvin a orgánů GIT v rámci jednoho onemocnění, další možností je postižení ledvin zhoršující prognózu GIT onemocnění, a konečně adekvátní volba a úprava léčiv vylučovaných renální cestou je nezbytná pro prevenci nefrotoxického účinku. Postižení ledvin může být nežádoucím účinkem léčby GIT a limitovat její další pokračování. V této souvislosti byla recentně věnována pozornost následujícím klinickým situacím: vznik akutního poškození ledvin při léčbě inhibitory protonové pumpy, renální komplikace při léčbě zánětlivého onemocnění střev a vznik akutní fosfátové nefropatie při užití projímadel obsahujících fosfáty. Včasné rozpoznání mechanizmů vedoucích k postižení ledvin může zamezit rozvoji ireverzibilnímu poškození ledvin a umožnit účinnou léčbu GIT.
The treatment of gastrointestinal tract (GIT) diseases may, under specific conditions, be significantly influenced by the kidneys or by kidney disorders. One of the potential scenarios of such interaction is the concurrent involvement of the kidneys and the GIT organs within one disorder, another option being the negative impact of impaired renal function on the prognosis of the GIT disease and, finally, the need for an adequate choice and dose adjustment of renally eliminated medication to avoid nephrotoxicity. Renal impairment may occur as an adverse effect of the treatment of the GIT condition and may limit further therapy. In this context we have recently focused on the following clinical situations: the development of acute kidney injury during treatment with proton pump inhibitors, renal complications of inflammatory bowel disease management and the development of acute phosphate nephropathy due to the use phosphate containing laxatives. An early identification of the mechanisms leading to renal injury can prevent the development of irreversible renal lesions and facilitate an efficient treatment of the GIT.

K současnému postižení jater a ledvin dochází poměrně často a toto duální postižení může mít různou podobu. Nové poznatky naznačují, že při postižení jater a ledvin se mohou uplatňovat ně kte ré společné patogenetické mechanizmy a spíše než o izolovaném postižení obou orgánů lze hovořit o systémovém procesu vedoucímu k významnému zhoršení celkové prognózy. U cirhózy jater je akutní poškození ledvin spojeno s významným vzestupem mortality a je důležitým faktorem pro posouzení transplantability jater. Na případech akutního poškození ledvin (AKI – acute kidney injury) se značným podílem, nikoli však výlučně, uplatňuje hepatorenální syndrom, jehož podstatou jsou hemodynamické změny v ledvinách a průvodní systémové a zánětlivé změny v ledvinách a dalších orgánech; pozornost je však v současnosti věnována také dalším typům AKI, které jsou ve zvýšené míře rozpoznávány. U chronického onemocnění jater je intenzivně studován vztah mezi nealkoholovou tukovou chorobou jater (NAFLD – non-alcoholic fatty liver disease) a chronickým onemocněním ledvin (CKD – chronic kidney disease). NAFLD je v současnosti často řazeno mezi jaterní projevy metabolického syndromu a asociováno s dalšími systémovými onemocněními, jako jsou kardiovaskulární onemocnění, diabetes mellitus a v neposlední řadě s CKD. Pro úspěšné řešení klinických problémů na ose játra-ledviny je nezbytná úzká spolupráce specialistů z obou orgánových oborů.
The liver and kidneys are commonly injured concomitantly. Such dual impairment can manifest in diff erent forms. Recent knowledge suggests that common mechanisms are involved in the pathogenesis of renal and hepatic injury and that it is sensible to emphasize the systemic nature and ominous prognosis of dual organ injury, rather than to consider these two types of injury in isolation. In cases of liver cirrhosis, acute kidney injury (AKI) is associated with a signifi cant increase in mortality and is an important factor when assessing eligibility for liver transplantation. Hepatorenal syndrome is primarily a purely hemodynamic phenomenon accompanied by infl ammatory and systemic changes in the kidneys and other organs. This syndrome often underlies AKI in cases of liver cirrhosis; however, other types of AKI are gaining growing attention and being increasingly recognized. In cases of chronic liver disease, the relationship between non-alcoholic fatty liver disease (NAFLD) and chronic kidney disease (CKD) has been studied. NAFLD is currently considered the hepatic component of metabolic syndrome and is negatively associated with cardiovascular disease, diabetes mellitus, and CKD. Close cooperative of renal and hepatic specialists is required for successful management of clinical problems in the liver-kidney axis.

The aim of the study was to characterize by molecular profiling two glomerular diseases: IgA nephropathy (IgAN) and focal segmental glomerulosclerosis (FSGS) and to identify potential molecular markers of IgAN and FSGS progression. The expressions of 90 immune-related genes were compared in biopsies of patients with IgAN (n=33), FSGS (n=17) and in controls (n=11) using RT-qPCR. To identify markers of disease progression, gene expression was compared between progressors and non-progressors in 1 year follow-up. The results were verified on validation cohort of patients with IgAN (n=8) and in controls (n=6) using laser-capture microdissection, that enables to analyze gene expression separately for glomeruli and interstitium. In comparison to controls, patients with both IgAN and FSGS, had lower expression of BAX (apoptotic molecule BCL2-associated protein) and HMOX-1 (heme oxygenase 1) and higher expression of SELP (selectin P). Furthermore, in IgAN higher expression of PTPRC (protein-tyrosine phosphatase, receptor-type C) and in FSGS higher expression of BCL2L1 (regulator of apoptosis BCL2-like 1) and IL18 compared to control was observed. Validation of differentially expressed genes between IgAN and controls on another cohort using laser-capture microdissection confirmed higher expression of PTPRC in glomeruli of patients with IgAN. The risk of progression in IgAN was associated with higher expression EDN1 (endothelin 1) (AUC=0.77) and FASLG (Fas ligand) (AUC=0.82) and lower expression of VEGF (vascular endothelial growth factor) (AUC=0.8) and in FSGS with lower expression of CCL19 (chemokine (C-C motif) ligand 19) (AUC=0.86). Higher expression of EDN1 and FASLG along with lower expression of VEGF in IgAN and lower expression of CCL19 in FSGS at the time of biopsy can help to identify patients at risk of future disease progression.

Dědičná onemocnění ledvin jsou významnou a specifickou skupinou ledvinných onemocnění, vyznačující se často tendencí k progresi do stadia chronického selhání a systémovým a multiorgánovým charakterem onemocnění. Byl odhalen genový podklad většiny dědičných nefropatií, byl učiněn významný pokrok při výzkumu jejich etiologie a patogeneze a recentně byly zavedeny inovativní postupy jejich léčby. Postižení gastrointestinálního traktu (GIT) u dědičných chorob ledvin může mít různou podobu, rozsah a význam. Ačkoli jen v menšině případů tvoří postižení GIT nedílnou součást klinického obrazu dědičného onemocnění ledvin jako např. cystické postižení jater u polycystické choroby ledvin autozomálně dominantního typu, mohou být i méně časté či výrazné projevy postižení GIT prvním či závažným extrarenálním projevem dědičné nefropatie, které vyžaduje včasné stanovení diagnózy a léčbu.
Hereditary kidney diseases are an important and specific group of kidney diseases having often systemic and multiorgan characteristics and a tendency to progress to chronic renal failure. The genetic background of the majority of hereditary nephropathies has been reported, an important step has been made to uncover their etiology and pathogenesis, and recently new innovative therapies have been proposed. Gastrointestinal tract (GIT) involvement in hereditary kidney diseases presents in different forms, extents, and intensities. Though only a minor part of hereditary kidney disease, GIT involvement is an integral component of the clinical picture; for example, liver cysts are an important component of autosomal dominant polycystic kidney disease. Even mild symptoms of GIT involvement can be an initial and potentially serious extrarenal manifestation of hereditary nephropathy, which deserves early diagnosis and initiation of treatment.
