Glioblastoma stem-like cells (GSCs) are critical for the aggressiveness and progression of glioblastoma (GBM) and contribute to its resistance to adjuvant treatment. MicroRNAs (miRNAs) are small, non-coding RNAs controlling gene expression at the post-transcriptional level, which are known to be important regulators of the stem-like features. Moreover, miRNAs have been previously proved to be promising diagnostic biomarkers in several cancers including GBM. Using global expression analysis of miRNAs in 10 paired in-vitro as well as in-vivo characterized primary GSC and non-stem glioblastoma cultures, we identified a miRNA signature associated with the stem-like phenotype in GBM. 51 most deregulated miRNAs classified the cell cultures into GSC and non-stem cell clusters and identified a subgroup of GSC cultures with more pronounced stem-cell characteristics. The importance of the identified miRNA signature was further supported by demonstrating that a Risk Score based on the expression of seven miRNAs overexpressed in GSC predicted overall survival in GBM patients in the TCGA dataset independently of the IDH1 status. In summary, we identified miRNAs differentially expressed in GSCs and described their association with GBM patient survival. We propose that these miRNAs participate on GSC features and could represent helpful prognostic markers and potential therapeutic targets in GBM.

Proteases are directly involved in cancer pathogenesis. Expression of fibroblast activation protein (FAP) is upregulated in stromal fibroblasts in more than 90% of epithelial cancers and is associated with tumor progression. FAP expression is minimal or absent in most normal adult tissues, suggesting its promise as a target for the diagnosis or treatment of various cancers. Here, we report preparation of a polymer conjugate (an iBody) containing a FAP-specific inhibitor as the targeting ligand. The iBody inhibits both human and mouse FAP with low nanomolar inhibition constants but does not inhibit close FAP homologues dipeptidyl peptidase IV, dipeptidyl peptidase 9, and prolyl oligopeptidase. We demonstrate the applicability of this iBody for the isolation of FAP from cell lysates and blood serum as well as for its detection by ELISA, Western blot, flow cytometry, and confocal microscopy. Our results show the iBody is a useful tool for FAP targeting in vitro and potentially also for specific anticancer drug delivery.

Představujeme výsledky nezávislého, firemně nepodporovaného sledování diabetiků 2. typu léčených inhibitorem dipeptidylpeptidázy 4 sitagliptinem. Do studie bylo zařazeno 30 mužů a 24 žen, u nichž byl dříve diagnostikován diabetes mellitus 2. typu (DM2T) a byli léčeni metforminem, případně kombinací metforminu a derivátu sulfonylurey nebo glinidu. Sitagliptin v dávce 100 mg 1krát denně byl přidán ke stávající medikaci u 38 pacientů s DM2T zavzatých do studie. Menší kontrolní skupina (n = 16) pacientů s DM2T byla sledována ve stejných časových intervalech k posouzení dynamiky sledovaných parametrů při absenci podávání sitagliptinu. 43 pacientů bylo sledováno po dobu 1 roku (28 sitaglipitin/15 kontrolní skupina), 11 pacientů pouze po dobu 1 měsíce (10 sitagliptin/1 kontrolní skupina). Sledovány byly zejména hladiny glykemie, glykovaného hemoglobinu (HbA1c), cholesterolemie, triacylglycerolemie, hladina sérové amylázy a C-reaktivního proteinu. Provedena byla ultrasonografie jater. U pacientů užívajících sitagliptin došlo k mírnému, ale statisticky významnému poklesu základních parametrů metabolické kompenzace diabetu, především lačné glykemie, ale i hladiny glykovaného hemoglobinu. V kontrolní skupině, léčené dosavadní léčbou bez přidání sitagliptinu, jsme nenalezli ani významný pokles glykemie nalačno, ani pokles hladiny glykovaného hemoglobinu. Nebyl pozorován pokles cholesterolemie, ani triacylglycerolemie. U 1/3 pacientů léčených sitagliptinem se zřetelně snížil stupeň jaterní steatózy při sonografickém hodnocení. Tento nález nebyl pozorován v kontrolní skupině. Na rozdíl od kontrolní skupiny došlo k mírnému vzestupu hladin amyláz v intervenované skupině, přičemž u naprosté většiny pacientů zůstala sérová amyláza v hranicích normy. U 2 nemocných jsme nalezli po roce sledování vzestup sérové amylázy nad hranici normy. Přestože výsledky hladin amyláz u všech ostatních nemocných zůstaly v hranicích normy, mezi počátkem sledování a koncem sledování jsme v sitagliptinové skupině nalezli statisticky významný vzestup. Tento nález nebyl ve skupině kontrolní. Nezávažné infekce dýchacích a močových cest se vyskytly podobně často jak v sitagliptinové, tak v kontrolní skupině.
We present the results of an independent, drug company-unsupported follow-up of patients with type 2 diabetes mellitus (T2DM) treated with the dipeptidyl peptidase 4 inhibitor sitagliptin. 29 patients (16 men, 13 women) used sitagliptin 100 mg daily for one year as an add-on to their chronic antidiabetic therapy. 16 type diabetic patients formed a control group – they used their chronic antidiabetic therapy without sitagliptin. 10 additional patients (6 men and 4 women) were enrolled in the study and treated with sitagliptin for one month. Body weight, BMI, glycaemia, glycated hemoglobin (HbA1c), cholesterolemia, triacylglycerolemia and serum amylases were determined and abdominal ultrasonography was performed. Because significant changes in immunological tests had been found especially after one month of treatment, 10 additional patients (6 men and 4 women) were enrolled in the study and treated with sitagliptin for one month. Sitagliptin treatment led to a significant body weight loss of 1 kg per year. In the control group, no significant change was observed. Similar results were noticed in HbA1c level and fasting glycaemia – mild but statisticaly significant reduction in the sitagliptin group both after one month and one year (not in HbA1c), no difference in the control group. There was no change in cholesterolemia, or in triacylglycerolemia. In 33% of patients in the sitagliptin group, the level of liver steatosis decreased by ultrasonographic evaluation. This was not found in any of the patients case in the control group. The serum amylase levels increased slightly over the upper limit in two sitagliptin treated patients. In the other sitagliptin treated patients serum amylase remained within the laboratory limits, but slight, statistically significant elevation of serum amylases was observed in the intervened group. This result was not found in the control group. There were not differences in the frequency between occurence of mild respiratory infections in the sitagliptin and control group. Marginally significant decrease was observed in the intervened group.

AIM: To assess intraindividually the effects of DPP-IV inhibition on the subpopulations of immune cells in type 2 diabetes mellitus (DM2) patients during the course of treatment with sitagliptin. METHODS: In this open label non-randomized observational study with a control group DM2 patients were examined before the initiation of the DPP-IV inhibitor administration (sitagliptin 100mg once daily) and then after 4weeks and 12months. Inhibition of the blood plasma DPP-IV enzymatic activity was determined by a chromogenic assay, the immunophenotyping of the blood cell subpopulations was performed using flow cytometry and blood plasma cytokine concentrations were quantified using an array-based multiplex ELISA. All parameters were evaluated in relation to the entry values in individual patients. RESULTS: The blood plasma DPP-IV enzymatic activity was effectively inhibited during the sitagliptin treatment. A significant decrease of the proportion of Treg cells (to 86±31% (median±SD) of entry values, p=0.001) and an increase of Th1 cells (to 120±103% (median±SD) of entry values, p=0.004) were observed after 4weeks but not after one year of the sitagliptin treatment. No changes were observed in the ratio of CD4(+)/CD8(+) cells, in the quantity of NK and Th2 cells and blood plasma cytokine levels. CONCLUSIONS: Sitagliptin treatment may cause temporary changes of the proportion of lymphocyte subpopulations in patients with DM2. The consequent deregulation of the immune system should be considered as a possible cause of the eventual side effects of long term DPP-IV inhibition.

Malignant gliomas represent tumors with dismal prognosis. Previous work of the applicant demonstrated a grade dependent increase of dipeptidyl peptidase (DPP)-IV enzymatic activity in human astrocytic tumors. This enzymatic activity is a common attribute of several multifunctional molecules belonging to the “DPP-IV activity and/or structure homologues” (DASH). These include among others fibroblast activation protein (FAP), whose pathogenetic role is presumed in several human malignancies. In order to determine the expression, localization and molecular forms present in the human astrocytic tumors, bioptic material obtained from patients undergoing therapeutic tumor resection will be used, and the data will be correlated with the clinical and histopathological parameters. The study on the pathogenetic role of FAP and its enzymatic activity will utilize an orthotopic xenotransplantation model and glioma cell lines transfected with wild type and mutated, enzymatically inactive FAP.
Maligní gliomy představují nádorová onemocnění se špatnou prognózou. Předchozí práce navrhovatele prokázala závislost dipeptidylpeptidase (DPP)-IV podobné aktivity na stupni malignity astrocytárních tumorů. Tato aktivita je společným atributem skupiny „DPP -IV aktivitou a/nebo strukturou homologních“ (DASH) molekul. Mezi tyto molekuly patří mj. fibroblastový aktivační protein (FAP), jehož patogenetický význam je předpokládán u řady humánních malignit. K určení exprese, tkáňové lokalizace a molekulových forem FAP přítomných v lidských astrocytárních tumorech bude použit bioptický materiál odebraný od pacientů podstupujících terapeutickou resekci nádoru, data budou korelována s klinickými a histopatologickými parametry. Ke studiu patogenetických mechanismů účasti FAP a jeho enzymové aktivity na gliomagenezi bude využito ortotopického xenotransplantačního modelu a gliomových linií transfekovaných enzymově aktivní a mutovanou, enzymově neaktivní formou FAP.

Dipeptidyl peptidase 4 (DPP-4, identical to CD26) is multifunctional enzyme, participating on a number of immunoregulatory processes. In patients with the diabetes mellitus type 2 treated by DPP-4 inhibitors, changes DPP-4 activity and concentration, concentration of eosinophyllic cationic protein and selected cytokines in blood plasma will be followed. In parallel, proportion of CD26 expressing cells within the CD4 and CD8 cells, together with the relevant enzymatic activity, Treg (CD4+CD25+CD127-/FoxP3+) and CD86+ monocytic cells and alternatively NK cells (presence and activation) and intracellular cytokines in CD4+ IFNgamma/ IL4/ IL17 (Th1/Th2/Th17) supplemented with IL2 and eventually TGFbeta and IL10 will be investigated. The results might help to assess causality of DPP-4 inhibitors' induced immunosupression observed in animals. The results also should help to better define indications of the treatment, namely respective to the autoimmune and oncological comorbidities.
Dipeptidylpeptidáza 4 (DPP-4, identická s CD26) je multifunkční enzym, významný pro řadu imunoregulačních pochodů. U pacientů s diabetem mellitem 2. typu léčených inhibitory DPP-4 budou sledovány důsledky inhibice tohoto enzymu na aktivitu a koncentraci DPP-4, koncentraci eosinofilního katonického proteinu a vybraných cytokinů v krevní plasmě. Dále budou zjištěny poměry populací a exprese CD26 na CD4 a CD8 lymfocytech a jejich DPP-4 aktivita, stanovení Treg (CD4+CD25+CD127-/FoxP3+) a CD86+ monocytů, případně NK (zastoupení a aktivace) a kvantifikace intracelulárních cytokinů v CD4+ IFNgamma/ IL4/ IL17 (Th1/Th2/Th17) doplněné o IL2 a event. TGFbeta a IL10. Vyšetření budou provedena před a po 4 týdnech a 12 měsících léčby DPP-4 inhibitory. Výsledky umožní posoudit vliv inhibice DPP-4 na navození imunosuprese pozorované u experimentálních zvířat a posoudit její mechanismus. To může přispět k racionalizaci indikace s přihlédnutím k případným komorbiditám, zejména autoimunitním a neoplastickým.
