IgA nefropatie (IgAN) je autoimunitní onemocnění s mezangiální depozicí imunitních komplexů obsahujících polymerní IgA1 (pIgA1) se sníženým podílem galaktózy v glykanech vázaných O-glykosydickou vazbou k pantové oblasti IgA1 (Gd-pIgA1). Imunitní komplexy vznikají zřejmě v cirkulaci specifickou vazbou afinitně maturované autoprotilátky izotypu IgG nebo IgA s cukernou komponentou Gd-pIgA1. Klíčovou složkou epitopu rozlišovaného autoprotilátkou je N-acetylgalaktosamin (GalNAc), bezprostředně připojený k aminokyselinám threoninu 228 a/nebo serinu 230 pantové oblasti IgA1. GalNAc na jedné nebo obou z uvedených aminokyselin není u IgAN pacientů modifikován připojením galaktózy (Gal), na rozdíl od zdravých jedinců. Pro zdravé jedince je charakteristická struktura S/T-GalNAc-Gal. Proto u IgAN hovoříme o galaktózové deficienci. Sérové koncentrace Gd-IgA1 jsou tak slibnými biomarkery. Frakce výše uvedených cirkulujících imunokomplexů o molekulové hmotnosti 800 – 900 kDa stimuluje mezangiální buňky, což se v bioptickém hodnocení projeví mezangiální proliferací, segmentální glomerulosklerózou a zprostředkovaně i endokapilární hypercelularitou a tubulární atrofií s intersticiální fibrózou. Afinitní maturace autoprotilátek IgG nebo IgA nabízí otázku jejich vzniku. Předpokládá se, že je spojen s iniciální aktivací určitých klonů naivních B lymfocytů molekulami obsahujícími terminální GalNAc buď virového, bakteriálního či endogenního původu. Recentní poznatky o patogenezi IgA nefropatie nabízejí směry vývoje budoucí kauzální terapie spočívající v zásahu do tvorby Gd-pIgA1, do tvorby specificky reagujících autoprotilátek, do tvorby imunitních komplexů a zásahu ovlivňujícího aktivaci mezangiálních buněk. Který z těchto postupů najde uplatnění v klinické praxi, snad ukáže již blízká budoucnost
IgA nephropathy (IgAN) is an autoimmune disease characterized by mesangial deposition of immune complexes containing polymeric IgA1 (pIgA1) with abnormally low galactose content in oligosaccharides linked by O-glycosidic bond to the hinge region of pIgA1 molecule (Gd-pIgA1). Immune complexes are formed probably in the circulation after recognition of Gd-pIgA1 by affinity-maturated autoantibody of IgG or IgA isotype specifically recognizing a glycan moiety of Gd-pIgA1. Key component of the recognized epitope is N-acetylgalactosamine (GalNAc), linked to amino acids threonine 228 and/or serine 230 of the IgA1 hinge region which is not modified in IgAN patients by attachment of galactose (Gal), in contrast to normal subjects in which typical structure is S/T-GalNAc- Gal. Serum levels of Gd-IgA1 represent promising biomarker. The fraction of above mentioned circulating immune complexes corresponding to the molecular weight 800 – 900 kDa stimulate mesangial cells which is detectable using light microscopy as a the mesangial hypercellularity, segmental glomerulosclerosis and endocapillary hypercellularity, and tubular atrophy or interstitial fibrosis. Affinity maturation of above-described autoantibody leads to the question about their origin, which has been associated with various antigens containing terminal GalNAc of either viral, bacterial or endogenous origin. Recent knowledge about the pathogenesis of IgAN offer new directions for development of causal therapy of the future based on specific affection of Gd-pIgA1 formation, autoantibody production, circulating immune complexes formation or activation of mesangial cells. Which of these approaches will found application in clinical praxis remains to be answered hopefully in close future.

IgA nefropatie (IgAN) je nejčastější primární glomerulonefritida. Ve složité patogenezi tohoto onemocnění má podstatnou roli tvorba specifických protilátek proti abnormálně glykosylovaným formám imunoglobulinu A1. Průběh IgAN je vysoce variabilní, renální funkce se progresivně zhoršují u 40 % nemocných a asi polovina z nich vyvine v průběhu 20 let chronické renální selhání. V předloženém článku jsou přehlednou formou stručně shrnuty současné názory na léčbu IgAN s ohledem na klinické, laboratorní a histologické projevy onemocnění. Základem léčby IgAN jsou inhibitory angiotenzin konvertujícího enzymu nebo blokátory receptoru pro angiotenzin II. U pacientů s přetrvávající proteinurií > 1 g/24 h a s histologickými známkami aktivity onemocnění v biopsii hodnocené podle MEST skóre jsou indikovány glukokortikoidy, které mohou z dlouhodobého hlediska působit renoprotektivně, signifikantně zpomalují pokles glomerulární filtrace (GF) a rozvoj chronické renální insuficience i u nemocných s GF < 50 ml/min (0,83 ml/s). U nemocných se závažnými formami onemocnění volíme kombinovanou terapii glukokortikoidy a cyklofosfamidem. Současná doporučení ohledně dalších imunosupresivních léčiv u IgAN, jako je azathioprin, mykofenolát mofetil, cyklosporin A, takrolimus, případně rituximab, nejsou jednotná. Z nových možností léčby jsou naděje vkládány do léčby budesonidem, což je glukokortikoid působící lokálně ve střevě v místě Peyerových plaků, který redukuje tvorbu aberantně glykosylovaných forem IgA1. Podle recentně publikované studie NEFIGAN budesonid snižuje u pacientů s IgAN proteinurii a stabilizuje GF.
IgA nephropathy (IgAN) is the most common primary glomerulonephritis. In the complex pathogenesis of this disease plays a significant role, the production of specific antibodies against the abnormally glycosylated forms of immunoglobulin A1. Course of IgAN is highly variable, kidney function progressively worsens in 40 % of patients, and about half of them develop chronic renal failure over 20 years. The present article briefly summarizes current views on IgAN treatment regarding clinical, laboratory and histological manifestations of the disease. Essential for the treatment of IgAN are angiotensin converting enzyme inhibitors or angiotensin II receptor blockers. Patients with persistent proteinuria > 1g / 24h and histological signs of active disease in biopsy, evaluated per the MEST score, are indicated for glucocorticoids treatment that can protect kidney in the long term, significantly slow down the decrease in glomerular filtration rate (GFR) and the development of end-stage renal disease even in patients with GFR < 50ml/min (0,83ml/s). For patients with severe forms of the disease, we choose combined regiment with glucocorticoids and cyclophosphamide. Current recommendations for use of other immunosuppressive drugs in IgAN such as azathioprine, mycophenolate mofetil, cyclosporine A, tacrolimus or rituximab are inconsistent. Regarding the new treatment options, much hope is being put on budesonide, a glucocorticoid acting locally on the intestinal Peyer‘s plaques, reducing the production of aberrantly glycosylated forms of IgA1. In the NEFIGAN trial budesonide reduced proteinuria and stabilized GFR in patients with IgAN.

IgA nefropatie (IgAN) je nejčastější glomerulonefritida. Na její etiologii se podílí zvýšená tvorba polymerního imunoglobulinu A1 s abnormálním složením některých cukerných řetězců, reakce těchto abnormálních forem IgA1 se specifickými autoprotilátkami za vzniku cirkulujících imunitních komplexů a depozice těchto komplexů do mezangia ledvin s následnou zánětlivou aktivací mezangiálních buněk vedoucí až v 50 % případů k terminálnímu selhání ledvin. Součástí patogeneze je souhra genetické predispozice a vlivy prostředí, zejména mikrobiální povahy. Současná terapie není dostatečně efektivní a není zaměřena na příčinu onemocnění, a proto se hledají účinnější a specifičtější cesty terapie, které by zasahovaly jednotlivé etapy patogenetického procesu vývoje IgAN. S hromaděním poznatků přibývají nové otázky ohledně přesných mechanizmů rozvoje patologického procesu, z nichž ty hlavní jsou diskutovány v textu.
IgA nephropathy (IgAN) is the most common type of glomerulonephritis. Its etiology involves an increased production of polymeric immunoglobulin A1 with an abnormal composition of some carbohydrate chains. The reaction of these abnormal forms of IgA1 with specific autoantibodies while circulating immune complexes arise and settle in the renal mesangium with subsequent inflammatory activation of mesangial cells which in up to 50% of cases results in end-stage kidney failure. Pathogenesis involves an interplay of genetic predisposition and environmental effects, mainly of microbial nature. Current therapy is not sufficiently effective and lacks the focus on the cause of the disease, therefore more efficient and specific ways of therapy are being sought to target the individual stages of the pathogenetic process of IgAN development. With the accumulation of knowledge, new questions arise, concerning detailed mechanisms of the pathological processes, as discussed in the text.

BACKGROUND: Galactose-deficient O-glycans in the hinge region (HR) of immunoglobulin A1 (IgA1) play a key role in the pathogenesis of IgA nephropathy (IgAN). O-Glycans of circulatory IgA1 consist of N-acetylgalactosamine (GalNAc) with a β1,3-linked galactose; both sugars may be sialylated. In patients with IgAN, α2,6-sialylated GalNAc is a frequent form of the galactose-deficient O-glycans. Prior analyses of IgA1-producing cells had indicated that α2,6-sialyltransferase II (ST6GalNAc-II) is likely responsible for sialylation of GalNAc of galactose-deficient IgA1, but direct evidence is missing. METHODS: We produced a secreted variant of recombinant human ST6GalNAc-II and an IgA1 fragment comprised of Cα1-HR-Cα2. This IgA1 fragment and a synthetic HR peptide with enzymatically attached GalNAc residues served as acceptors. ST6GalNAc-II activity was assessed in vitro and the attachment of sialic acid to these acceptors was detected by lectin blot and mass spectrometry. RESULTS: ST6GalNAc-II was active with both acceptors. High-resolution mass spectrometry analysis revealed that up to three sialic acid residues were added to the GalNAc residues of the HR glycopeptide. CONCLUSIONS: Our data provide direct evidence that ST6GalNAc-II can sialylate GalNAc of galactose-deficient IgA1. As serum levels of galactose-deficient IgA1 with sialylated glycoforms are increased in IgAN patients, our data explain the corresponding part of the biosynthetic pathway.

IgA nefropatie je v současné době nejvíce studovanou glomerulonefritidou. Je definována průkazem převažujících nebo s jinými imunoglobuliny „kodominujících“ depozit imunoglobulinu A1 v mezangiu glomerulů. Depozita IgA1 nejpravděpodobněji pocházejí z cirkulujících imunitních komplexů, může ale jít i o agregaci IgA1 pozměněné struktury či o vazbu na receptory pro IgA1 na mezangiálních buňkách. Příčinou tvorby imunitních komplexů odpovědných za IgA nefropatii je nedostavěný O-glykosidický řetězec imunoglobulinu A1, na jehož konci v důsledku anomálií glykosyltransferáz chybí galaktóza. Na tento cukerný antigen se váží přirozeně se vyskytující protilátky třídy IgG nebo IgA1. V klinickém obrazu rozeznáváme časné stadium obvykle se vyznačující erytrocyturií a proměnlivé stadium pozdní, dané buď klinickou remisí, nebo perzistencí erytrocyturie či u jedné třetiny pacientů narůstající proteinurií, zvýšeným krevním tlakem a klesající funkcí ledvin. Prognózu IgA nefropatie nedovedeme v časném stadiu odhadnout ani z klinické prezentace ani z morfologického nálezu ani z množství odchylně glykosylovaného IgA1 v cirkulaci. Spolehlivým ukazatelem závažné prognózy je teprve pozdní stadium se stoupající proteinurií a hypertenzí a v biopsii s atrofií tubulů a intersticiální sklerózou. Hlavním trendem v léčbě IgA nefropatie je důraz na podávání inhibitorů ACE/sartanů, které sami podáváme již při mikroalbuminurii. Jestliže při této léčbě proteinurie nepoklesne pod 1 g/24 h, je lékem volby prednison. Nové poznatky o patogeneze choroby nabízejí možnosti zásahu na úrovni tvorby nefritogenních imunitních komplexů blokádou dnes již známé antigenní determinanty na imunoglobulinu A1 a/nebo na základě kompetitivní inhibice přirozeně se vyskytujících příslušných protilátek.
IgA nephropathy is currently the most frequently investigated glomerulonephritis. The disease is defined by the presence of dominant or co-dominant deposits of IgA1 in the glomerular mesangium. Circulating immune complexes are most likely the source of the deposited IgA1. However, it is also possible that the aggregates of structurally altered IgA1 or enhanced binding to IgA receptors expressed on mesangial cells lead to deposition. The cause of the formation of immune complexes responsible for IgA nephropathy lies in the incomplete O-linked oligosaccharide side chains, which, due to the deficiency of corresponding glycosyltransferases, lack terminal galactose residues leading to the exposure of N-acetylgalactosamine. Naturally occurring antibodies of the IgG or IgA1 isotype bind to this sugar antigen. In the clinical course, we differentiate between the early stage usually characterized by hematuria, and a variable late stage characterized either by a clinical remission, by persistence of hematuria, or by increasing proteinuria and blood pressure and decreasing renal function in one third of the patients. In the early stage, it is difficult to predict the prognosis of IgA nephropathy, either on the basis of clinical presentation and morphological findings, or according to the level of galactose-deficient IgA1 in the circulation. The reliable criteria of serious prognosis emerge only in the later stages of the disease and include proteinuria, hypertension, and histologically apparent tubular atrophy and interstitial sclerosis. The dominant trend in the treatment of IgA nephropathy is the emphasis on administration of ACE inhibitors/sartans, which are introduced into the treatment at the time of microalbuminuria. If proteinuria does not decrease below 1 g/24 h, treatment with prednisone is justifiable. New findings concerning the molecular/cellular mechanism involved in the pathogenesis of IgA nephropathy suggest the possible therapeutical interference with the generation of nephritogenic immune complexes by a selective blocking of the IgA1 molecules with altered glycan structures using monovalent reagents.

Glycosylation abnormalities have been observed in autoimmune diseases and cancer. Here, we compare mechanisms of aberrant O-glycosylation, i.e., formation of Tn and sialyl-Tn structures, on MUC1 in breast cancer, and on IgA1 in an autoimmune disease, IgA nephropathy. The pathways of aberrant O-glycosylation, although different for MUC1 and IgA1, include dysregulation in glycosyltransferase expression, stability, and/or intracellular localization. Moreover, these aberrant glycoproteins are recognized by antibodies, although with different consequences. In breast cancer, elevated levels of antibodies recognizing aberrant MUC1 are associated with better outcome, whereas in IgA nephropathy, the antibodies recognizing aberrant IgA1 are part of the pathogenetic process.

The human UDP-N-acetyl-α-d-galactosamine:polypeptide N-acetylgalactosaminyl-transferase 2 (GalNAc-T2) is one of the key enzymes that initiate synthesis of hinge-region O-linked glycans of human immunoglobulin A1 (IgA1). We designed secreted soluble form of human GalNAc-T2 as a fusion protein containing mouse immunoglobulin light chain kappa secretory signal and expressed it using baculovirus and mammalian expression vectors. The recombinant protein was secreted by insect cells Sf9 and human HEK 293T cells in the culture medium. The protein was purified from the media using affinity Ni-NTA chromatography followed by stabilization of purified protein in 50mM Tris-HCl buffer at pH 7.4. Although the purity of recombinant GalNAc-T2 was comparable in both expression systems, the yield was higher in Sf9 insect expression system (2.5mg of GalNAc-T2 protein per 1L culture medium). The purified soluble recombinant GalNAc-T2 had an estimated molecular mass of 65.8kDa and its amino-acid sequence was confirmed by mass-spectrometric analysis. The enzymatic activity of Sf9-produced recombinant GalNAc-T2 was determined by the quantification of enzyme-mediated attachment of GalNAc to synthetic IgA1 hinge-region peptide as the acceptor and UDP-GalNAc as the donor. In conclusion, murine immunoglobulin kappa secretory signal was used for production of secreted enzymatically active GalNAc-T2 in insect baculovirus expression system.
