OBJECTIVE: This study aimed to evaluate the accuracy and effectiveness of different strategies for the diagnosis of acute myocardial infarction (AMI) in the elderly in real-life clinical practice. METHODS: Patients older than 70 years presenting to the emergency department with chest pain were included. The performance of six decision aid rules (T-MACS, HEART, EDACS, TIMI, GRACE, and ADAPT) and solo troponin T strategy for diagnosing AMI was evaluated by calculating sensitivity, specificity, odds ratios, negative and positive predictive values. RESULTS: A total of 250 patients, with a mean age of 78.5 years, were enrolled. Forty-eight patients (19.2 %) had an acute myocardial infarction in a 30 day follow-up period. The sensitivity for ruling-out AMI was 100 % for T-MACS, HEART, and ADAPT; 97.9 % for EDACS, 93.8 % for TIMI, and 81.3 % for GRACE and solo TnT strategy. For ruling-in AMI, the specificity was 97.5 % for T-MACS, 95 % for TIMI, 83.2 % for HEART, 81.7 % for GRACE, and 46 % for ADAPT. CONCLUSION: T-MACS decision aid had the best performance for rule-out and rule-in diagnostics of AMI. Risk stratification of patients with suspected acute coronary syndrome based on decision aid rules can be used in real-life practice, even in the population of the elderly (Tab. 6, Fig. 1, Ref. 17).

Počet vyšetření pro bolesti na hrudi v rozvinutých zemích neustále narůstá. Jejich diferenciální diagnostika je poměrně široká a určit přesnou příčinu obtíží z jednoho vyšetření často není možné ani při použití moderních vyšetřovacích metod. Největší výzvou tak zůstává optimální výběr vyšetřovacích modalit a prvků do logické a účelné strategie. Rozhodovací protokoly byly vytvořeny za účelem efektivní diagnostiky a stratifikace pacientů na urgentním příjmu. Jejich použití zmiňují evropské i americké doporučené postupy. Cílem tohoto článku je představit současný přehled dostupných protokolů pro rychlé rozhodování na urgentním přijmu, vysvětlit jejich přínos, limitace a nevýhody.
The number of examinations for acute chest pain in developed countries is rising. The differential diagnosis for chest pain is wide and, even with the use of modern technologies, it is often impossible to determine the exact cause. The choice of optimal diagnostic modalities for a logical and meaningful strategy remains challenging. Decision aid rules have been created for a quick and effective management of patient with chest pain in acute care. They are mentioned in European and American guidelines. This article aims to present an overview of available models for the decision making in the emergency department and explain their benefits, limitations and disadvantages.

Cílem tohoto sdělení je popsat praktický přístup k managementu pacientů se ztluštěním stěn levé komory srdeční (LKS). Příčinou ztluštění stěn LKS může být hypertrofie myokardu při arteriální hypertenzi, chlopenních vadách, sportovním srdci nebo při hypertrofické kardiomyopatii. Svým fenotypem napodobují hypertrofii LKS také infiltrativní a střádavá onemocnění myokardu, i když z morfologického hlediska se o pravou hypertrofii myokardu nejedná. Došlo k významným pokrokům v diferenciální diagnostice této skupiny onemocnění, k dispozici jsou nové možnosti specifické léčby, především hypertrofické obstrukční kardiomyopatie a transthyretinové amyloidózy myokardu.
The aim of this paper is to describe apractical approach to the management of patients with thickening of the walls of the left ventricle (LV). Thickening of LV walls may be caused by myocardial hypertrophy in arterial hypertension, valve defects, athletic heart, or hypertrophic cardiomyopathy. Due to their phenotype, LV hypertrophy is mimicked by infiltrative and storage diseases of the myocardium, although from amorphological point of view it is not true myocardial hypertrophy. There have been significant advances in the differential diagnosis of this group of diseases, new specific treatment options are available, especially for treatment of hypertrophic obstructive cardiomyopathy and transthyretin amyloid cardiomyopathy.

OBJECTIVE: Promptly establishing maintenance therapy could reduce morbidity and mortality in patients with bipolar disorder. Using a machine learning approach, we sought to evaluate whether lithium responsiveness (LR) is predictable using clinical markers. METHOD: Our data are the largest existing sample of direct interview-based clinical data from lithium-treated patients (n = 1266, 34.7% responders), collected across seven sites, internationally. We trained a random forest model to classify LR-as defined by the previously validated Alda scale-against 180 clinical predictors. RESULTS: Under appropriate cross-validation procedures, LR was predictable in the pooled sample with an area under the receiver operating characteristic curve of 0.80 (95% CI 0.78-0.82) and a Cohen kappa of 0.46 (0.4-0.51). The model demonstrated a particularly low false-positive rate (specificity 0.91 [0.88-0.92]). Features related to clinical course and the absence of rapid cycling appeared consistently informative. CONCLUSION: Clinical data can inform out-of-sample LR prediction to a potentially clinically relevant degree. Despite the relevance of clinical course and the absence of rapid cycling, there was substantial between-site heterogeneity with respect to feature importance. Future work must focus on improving classification of true positives, better characterizing between- and within-site heterogeneity, and further testing such models on new external datasets.

AIMS: Long-term persistence of perfusion defect after pulmonaryembolism (PE) may lead to the development of chronic thromboembolic pulmonary hypertension. Identification of patients at risk of such a complication using a scoring system would be beneficial in clinical practice. Here, we aimed to derive a score for predicting persistence of perfusion defects after PE. METHODS: 83 patients after PE were re-examined 6, 12 and 24 months after the PE episode. Data collected at the time of PE and perfusion status during follow-ups were used for modelling perfusion defects persistence using the Cox proportional hazards model and validated using bootstrap method. RESULTS: A simple scoring system utilizing two variables (hemoglobin levels and age at the time of PE) was developed. Patients with hemoglobin levels over 140 g/L who were older than 65 years were at the highest risk of perfusion defects; in patients with the same hemoglobin levels and age <65 years, the risk was reduced by 79%, and by 89% in patients with hemoglobin <140 g/L. CONCLUSION: The proposed scoring system may be useful in clinical practice for identifying patients with high risk of persisting perfusion defects, flagging them for closer follow up, thus improving the effectiveness of long-term treatment of patients after PE.
