INTRODUCTION: Timed performance tests were introduced to overcome the disadvantages of subjective evaluation of bradykinesia in Parkinson's disease (PD). We aimed to verify their discriminative properties and compare them with the motion capture analysis of finger tapping. METHODS: We included 22 PD patients (10 M, 12 F), mean age 64 (range 48-82) yrs, Hoehn & Yahr stage 2 (1-2.5) and 22 (10 M, 12 F) normal controls, mean age 66 (41-82) yrs. The key tapping subtest of the Halstead-Reitan battery, the Purdue Pegboard test, and the Bradykinesia-Akinesia Incoordination (BRAIN) test were performed according to the test manuals. The finger tapping subtest of the UPDRS-III, item 23 was recorded using a contactless 3D motion capture system Optitrack-V120. Average frequency (AvgFrq), maximum opening velocity (MaxOpV) and amplitude decrement (AmpDec) were computed and simultaneous video recordings of finger tapping were rated by two experts. RESULTS: The AmpDec and MaxOpV motion capture measures best differentiated between PD patients and controls (AUC = 0.87 and 0.81). Of the instrumental tests, only the Purdue Pegboard attained significance in differentiating PD patients from controls (AUC = 0.80). In PD patients, MaxOpV correlated with the finger tapping ratings and BRAIN test, and AvgFrq correlated with the BRAIN and Halstead-Reitan test scores. Moreover, correlations were found between the Purdue Pegboard and finger tapping ratings. CONCLUSIONS: Contactless 3D motion capture of finger tapping allowed an independent analysis of individual components of bradykinesia, demonstrating the amplitude decrement and maximum opening velocity as the most powerful discriminators between PD patients and controls.

Pohyb je základní lidská potřeba, o jejímž nedostatku nemá orga nismus aktuální, relevantní informaci. Důsledkem stále se snižujícího objemu realizovaných pohybových aktivit je sedavý životní styl. Hypokinéza, definovaná jako nedostatek pohybu bez objektivních příčin, je důsledkem nedostatečné nabídky vhodných pohybových aktivit hlavně v dětském věku. To se odráží jak v ne dostatečné zdatnosti, tak v nízké pohybové gramotnosti mladé i dospělé populace. Důsledkem je celá řada zd ravotních komplikací, nadváhou a obezitou počínaje a hypertenzí, diabetes mellitus 2. typu a dalšími chro nickými onemocněními neinfekčního typu konče na straně jedné a nízká dovednostní úroveň na straně druhé, což vše ve svém důsledku zásadně ovlivňuje aktuální pohybový režim současné populace. V praxi to znamená s nížení pracovní výkonnosti, zvýšení rizika pracovních selhání, zhoršení regenerace po pracovním zatížení a hlavně zhoršení kvality života. Omezený výběr použitelných pohybových aktivit, v důsledku nedostatečné připravenosti, pohybový deficit ještě více prohlubuje. Základem nápravy – režimových opatření je zvýšení objemu pravidelně realizovaných nenáročných, laciných pohybových aktivit realizovatelných v „domácích“ podmínkách a patřičná edukace
The movement is a basic human need, the lack of which has body any actual, relevant info rmation. As a result of the ever- decreasing amount of ph ysical activity is implemented sedentary lifestyle. Hypokinesis, defined as lack of movement without objective reasons, is due to the insufficient supply of appropriate physical activities especially in children. This is reflected in the lack of fitnes s and low physical literacy in young and adult population. The result is a variety of health complications, starting with overweight and obesity and hypertension, diabetes mellitus 2. type, and other chronic non- infectious diseases type ending on the one hand and the low skill level on the other hand, all of which have the effect of significantly a ffects the current motion mode of the current population. In practice, this means reduced work performance, increased risk of job failure, impaired regeneration after work load and especially the qualit y of life. Limited choice of applicable physical activities due to lack of preparation, motion deficit deepens. The aim of our work is to assess the influence of tiredness on changes in respiratory parameters during monotonous hypokinetic loading. In the current phase of the work, we are looking for the answer to the question whether these changes can predict upcoming tiredness or indicate only. On the pilot measurement, three males and two females aged 25-35 were participated. To capture the transition states of consciousness, a measurement was proposed in two modes: active vs. tired individual. For recording of breathing, two chest straps and a spiroergometer were used. Symptoms of tiredness were monitored by ECG and EEG. The course of the experiment was recorded by a video camera. Currently, we process the obtained data and from actual results, we can confirm that the upcomming tiredness can be with certainty found in the respiratory curves. The upcoming tiredness assessed merely by subjective analysis of videos was apparently associated with changes in breathing pattern. Quite a clear answer, if changes of the breathing pattern has predictive or indicative character, we will obtain from further comparison the respiratory curves with results of analyses of EEG and ECG signals, which are currently under processing.

This study aims to investigate the structure of the motor symptoms of Parkinson's disease (PD), as measured by the Motor Section of the Unified Parkinson's Disease Rating Scale (UPDRS). The dimensionality of the Motor Section of the UPDRS was studied using structural equation modeling. The UPDRS measures were obtained from 405 patients with PD [237 men (39 "off", 170 "on", 28 unknown) and 168 women (21 "off", 140 "on", 7 unknown)]. The ordinal character of UPDRS scores and sample size substantiated the use of robust diagonally weighted least squares model estimation. It was shown that the Motor Section of the UPDRS incorporates five main latent symptom factors (rigidity, tremor, bradykinesia of the extremities, axial/gait bradykinesia, speech/hypomimia) plus two additional factors for laterality, which account for asymmetry of tremor, rigidity and bradykinesia of the extremities. Tremor seems to be an independent symptom factor of PD. Other latent variables are substantially correlated. (c) 2008 Movement Disorder Society.

The dimensionality and reliability of the Motor Section of the Unified Parkinson Disease Rating Scale (UPDRS III) was studied with non-parametric Mokken scale analysis. UPDRS measures were obtained on 147 patients with PD (96 men, 51 women, mean age 61, range 35-80 yrs). Mokken scale analysis revealed a four-dimensional structure of the UPDRS III. Left-sided bradykinesia and rigidity appeared to co-occur with axial signs, gait disturbance, and speech/hypomimia, whereas right-sided bradykinesia and rigidity formed a second scale. Two further small scales were found consisting of right- and left-sided tremor. Results from the scale analysis reveal that all four subscales are strong. The reliability of the two tremor scales is low because they only contain three and four items, respectively.
