Neurochirugie je zásadní pro získání histologických vzorků, provádění správných chirurgických výkonů, pro indikace k výkonům a následné pooperační sledování. V posledních letech došlo jak ke zpřesnění indikací neurochirurgických intervencí, tak ke snížení komplikací. Zásadní je použití moderních zobrazovacích, navigačních a elektrofyziologických technik. Péče o pacienty s nádory je však multioborová, proto je nezbytná kooperace s dalšími odbornostmi.
Neurosurgery plays a crutial role in obtaining histological samples, in performance of adequate surgical procedures, in decision about surgical procedures and postoperative follow-up. Indications of neurosurgical procedures has been refined as well as the rate of complications has been reduced. Modern imaging, navigation and eletrophysiological techniques are of utmost importance. Care has to be multidisciplinar, thus cooperation between key care givers is vital.

BACKGROUND: Case reports have portrayed spinal cord atypical teratoid/rhabdoid tumor (spATRT) as an aggressive form of ATRT. We conducted a retrospective European survey to collect data on clinical characteristics, molecular biology, treatment, and outcome of children with intramedullary spATRT. METHODS: Scrutinizing a French national series and the European Rhabdoid Registry database, we identified 13 patients (median age 32 months; metastatic disease at diagnosis, n = 6). Systemic postoperative chemotherapy was administered to all patients; three received intrathecal therapy and six were irradiated (craniospinal, n = 3; local, n = 3). RESULTS: Median observation time was 8 (range, 1-93) months. Progression-free and overall survival rates at 1 and (2 years) were 35.2% ± 13.9% (26.4% ± 12.9%) and 38.5% ± 13.5% (23.1% ± 11.7%). Four patients (ATRT-SHH, n = 2; ATRT-MYC, n = 1; DNA methylation subgroup not available, n = 1) achieved complete remission (CR); two of them are alive in CR 69 and 72 months from diagnosis. One patient relapsed after CR and is alive with progressive disease (PD) and one died of the disease. Three patients (ATRT-MYC, n = 2; subgroup not available, n = 1) died after 7 to 22 months due to PD after having achieved a partial remission (n = 1) or stabilization (n = 2). Five patients (ATRT-MYC, n = 2; subgroup not available, n = 3) developed early PD and died. One patient (ATRT-MYC) died of intracerebral hemorrhage prior to response evaluation. CONCLUSIONS: Long-term survival is achievable in selected patients with spATRT using aggressive multimodality treatment. Larger case series and detailed molecular analyses are needed to understand differences between spATRT and their inracranial counterparts and the group of extradural malignant rhabdoid tumors.

Intradurálne extramedulárne nádory predstavujú pomerne malú skupinu spomedzi všetkých nádorov CNS. V tejto práci sú popísané ich najčastejšie typy, etiológia, makroskopická a histologická charakteristika, klinické prejavy, dia gnostika a liečba. Popisuje klinické výsledky pri jednotlivých operačných postupoch a ich vplyv na stabilitu chrbtice, radikalitu resekcie a výsledný neurologický stav pa cienta. Rozoberá dôležitosť predoperačnej prípravy, plánovania, polohy pa cienta počas operácie, perioperačnej medikamentóznej liečby a využitia možností prístrojového vybavenia. Hodnotí ich vplyv na pooperačný priebeh, možné komplikácie a celkovú prognózu pa cienta. Tieto aspekty práca hodnotí predovšetkým pri najčastejších typoch intradurálnych extramedulárnych nádorov, a to pri nádoroch nervových obalov a meningeóme. Stručne sú popísané aj menej časté nádory. V diskusii a závere práce sú zhrnuté jednoduché a stručné odporúčania týkajúce sa dia gnostiky a liečby intradurálnych extramedulárnych nádorov, ktoré sú zamerané na zlepšenie efektu liečby a minimalizovanie rizika vzniku komplikácií v súlade s dostupnými štúdiami.
Intradural extramedullary spinal cord tumors represent a relatively small group among all of the tumors of the CNS. This article describes the most frequent types of these tumors, their etiology, macroscopic and histologic characteristics, clinical symptoms, diagnosis and treatment. It describes the clinical outcome of several surgery techniques and their effect on the stability of the spine, the radicality of the resection and the consequent neurological condition of the patient. It analyses the importance of pre-operative preparation, planning, the position of the patient during surgery, perioperative medicamentous treatment and the use of instrumentation opportunities. It evaluates its impact on postoperative outcome, the eventual complications and the overall prognosis of the patient. These aspects are particularly pre-evaluated in the most frequent types of intradural extramedullary spinal cord tumors which are nerve sheath tumors and meningeoms. There is also a brief description devoted to less frequent tumors in this article. There are simple and concise recommendations on the diagnostics and therapy of these tumors in the discussion part and the closure, and these recommendations are aimed at the improvement of the treatment effect and minimizing the risk of complications in accordance with available studies.
Technický pokrok v radiační onkologii umožňuje stereotaktickou radiochirurgii a radioterapii využívat stále častěji. Vedle dedikovaných stereotaktických přístrojů, jako jsou GamaKnife nebo CyberKnife, lze v některých situacích dobře využít i moderní lineární urychlovače se stereotaktickou nadstavbou. V případech stereotaktického ozáření páteře a plic je však role kybernetického nože velmi často nezastupitelná.
The progress in radiation oncology allows stereotactic radiosurgery and radiotherapy to be used more and more frequently. Moreover, beside to dedicated stereotactic devices such as GamaKnife or CyberKnife, modern linear accelerators with a stereotactic superstructure can also be used in some indications. However, in case of spinal and lung stereotactic irradiation, the role of the CyberKnife is irreplaceable very often.

Radiotherapy with systemic corticosteroid therapy has been used to treat intramedullary spinal cord metastasis (ISCM), but recovery of function and long-term survival of these patients has been rarely observed. We report herein a small cell lung cancer (SCLC) patient with recurrent thoracic ISCM, who was successfully treated with radiotherapy and systemic corticosteroid therapy. A 70-year-old man, who was diagnosed as having SCLC seven months previously, developed thoracic ISCM. Soon after the detection of the lesion, the patient received radiotherapy with systemic corticosteroid therapy. Sensory disturbance in both extremities and neurogenic bladder and bowel dysfunction was recovered. The patient could walk after irradiation again. The patient received additional chemotherapy and survived 20 months after the diagnosis of ISCM recurrence. Prompt diagnosis and appropriate treatment for ISCM and effective chemotherapy for recurrent SCLC might be the favorable factors for such patients. Further studies will be required to define a favorable subset of patients most likely to benefit from a conventional approach.

Autor se zabývá v úvodu etymologií slova paliativní pro přesnější chápání jejího významu a vymezuje jeho použití v medicíně. Dále se soustřeďuje na výčet nejdůležitějších symptomů provázejících mozkové nádory, které ztrpčují život nemocných a na jejich paliativní ovlivnění. Zdůrazňuje nutnost multidisciplinárního přístupu, který by měl vést k co nejvíce snesitelné kvalitě života nemocných v jejich posledních měsících či letech života, což nespočívá pouze v odstranění symptomů nemoci. Podobný přístup popisuje i u maligních míšních nádorů včetně péče terminální.
First, the author deals with the etymology of the word palliative for a better understanding of its meaning and defines its use in medicine. Next, he presents a list of the major symptoms accompanying brain tumours that make the lives of patients harder and focuses on their palliative management. He emphasizes the need for a multidisciplinary approach which should result in a most bearable quality of life of patients in their last months or years of life, which is not only about relieving the symptoms of disease. A similar approach is described also for malignant spinal cord tumours including terminal care.

Primárne nádory miechy a chrbtice patria k zriedkavejším malignitám. Klasifikácie nádorov chrbtice a miechy sa rozlišujú podľa vzťahu k mieche a dura mater na intradurálne a extradurálne, pričom intradurálne nádory sa ďalej delia na intramedulárne a extramedulárne. Onkologický staging podľa Ennekinga je založený na klinických prejavoch, rádiologických znakoch a histopatologických nálezoch. Chirurgická klasifikácia podľa Weinsteina-Borianiho-Biaginiho (WBB) je postavená na anatomickom rozsahu a lokalizácii patologických lézii. Základnou liečebnou modalitou je chirurgická extirpácia. Adjuvantná rádio a chemoterapia je indikovaná v špecifických prípadoch. U metastazujúcich tumorov sú liečebnými modalitami: rádioterapia, chemoterapia, hormonoterapia, v indikovaných prípadoch chirurgická extirpácia.
Primary spinal and spinal cord tumours are uncommon malignancies. Based on their relation to the spinal cord and dura mater, spinal and spinal cord tumours are classified into intradural and extradural, with the former being further divided into intramedullary and extramedullary. The oncological staging according to Enneking is based on clinical manifestations, radiological features, and histopathological findings. The Weinstein-Boriani-Biagini (WBB) surgical classification takes into account the anatomical extent and localization of pathological lesions. The primary treatment modality is surgical extirpation. Adjuvant radiation and chemotherapy is indicated in specific cases. The treatment modalities for metastatic tumours include: radiation therapy, chemotherapy, hormonal therapy, or surgical extirpation where indicated.
Astrocytomas affect a significant portion of patients with intramedullary tumors. These infiltratively growing tumors are treated by a variety of methods -- biopsy and decompressive surgery, maximal safe resection, adjuvant oncological therapy. Also, numerous prognostic factors are reported in the literature. Better understanding of factors that influence prognosis may help in treatment planning with the goal of prolonging survival. We have thus undertaken an extensive literature review in order to define factors affecting prognosis. A total of 38 articles were studied. Only tumor grade was consistently reported as the major factor affecting prognosis. The influence of other clinical factors (age, gender, history length, functional status, tumor location or extent, syrinx or cyst presence) can be speculated upon, but cannot be assessed adequately from the available literature. For both low- and high-grade (HG) astrocytomas, maximal safe tumor resection should be the primary treatment objective but is often not feasible in contrast to other intramedullary and spinal neoplasms. Since the biological nature of spinal cord HG glioma is identical to that of the brain, the same treatment algorithm of maximal safe resection followed by concomitant radio- and chemotherapy would be sensible to implement.
