OBJECTIVES: We aimed to compare various methods for free light chain (fLC) quantitation in cerebrospinal fluid (CSF) and serum and to determine whether quantitative CSF measurements could reliably predict intrathecal fLC synthesis. In addition, we wished to determine the relationship between free kappa and free lambda light chain concentrations in CSF and serum in various disease groups. METHODS: We analysed 166 paired CSF and serum samples by at least one of the following methods: turbidimetry (Freelite™, SPAPLUS), nephelometry (N Latex FLC™, BN ProSpec), and two different (commercially available and in-house developed) sandwich ELISAs. The results were compared with oligoclonal fLC detected by affinity-mediated immunoblotting after isoelectric focusing. RESULTS: Although the correlations between quantitative methods were good, both proportional and systematic differences were discerned. However, no major differences were observed in the prediction of positive oligoclonal fLC test. Surprisingly, CSF free kappa/free lambda light chain ratios were lower than those in serum in about 75% of samples with negative oligoclonal fLC test. In about a half of patients with multiple sclerosis and clinically isolated syndrome, profoundly increased free kappa/free lambda light chain ratios were found in the CSF. CONCLUSIONS: Our results show that using appropriate method-specific cut-offs, different methods of CSF fLC quantitation can be used for the prediction of intrathecal fLC synthesis. The reason for unexpectedly low free kappa/free lambda light chain ratios in normal CSFs remains to be elucidated. Whereas CSF free kappa light chain concentration is increased in most patients with multiple sclerosis and clinically isolated syndrome, CSF free lambda light chain values show large interindividual variability in these patients and should be investigated further for possible immunopathological and prognostic significance.

Albuminurie (především v oblasti tzv. mikroalbuminurie) je považována za nejdůležitější indilcátor rozvoje diabetické nefropatie. Je také markerem zvýšeného rizika kardiovaskulárních onemocnění a mortality obecně. Termíny „mikroalbuminurie", „normoalbuminurie" a „makroalbuminurie" by se odbornou veřejností užívat neměly, měly by být nahrazeny souhrnným označením „močový albumin". Mohly by totiž navodit mylnou představu, že jsou do moči vylučovány nějaké menší či větší molekuly albuminu. V minulosti byla albuminurie nejčastěji vyšetřována ve vzorku moči sbírané za 24 hodin. Úskalí často nepřesného sběru moči vyústilo v zavedení poměru koncentrace albuminu/koncentrace kreatininu v nesbírané moči (albumin/creatinine ratio, ACR, v jednotkách mg/mmol nebo mg/g). V případě užití tohoto poměru je doporučováno vyšetření vzorku 1. ranní moči (který má nižší intraindividuální variabilitu než vzorek náhodný). Intaktní albumin se v moči vyskytuje ve dvou formách, imunoreaktivní a imunonereaktivní. Močový albumin je běžně stanovován imunochemickými metodami, např. imunoturbidimetricky, imunonefelometricky, RIA, ELISA. Tyto metody však detekují pouze imunoreaktivní formu albuminu. Vysokoúčinná kapalinová chromatografie (high-performance liquid chromatography, HPLC) pravděpodobně detekuje i albumin, který imunochemickým metodám unikl. Porovnání HPLC vs. imunochemicky stanovené albuminurie (většinou vyjádřené jako ACR) často ukazuje na pozitivní bias, která klesá se zvyšující se koncentrací albuminu. V některých studiích bylo dokonce prokázáno, že HPLC umožní identifikovat jedince s přítomnou albuminurií (a tím i ve vyšším riziku následných morbidit a mortality) o cca 2 až 4 roky dříve než imunochemické metody. Nedávno však bylo zjištěno, že při HPLC některé proteiny (např. transferin, a-1-kyselý glykoprotein, inhibitor proteináz a-1) koeluují s albuminem, tím by HPLC poskytovala výsledky falešně vyšší. Rozdíl mezi HPLC a imunochemickými metodami se tedy vysvětluje dvěma zcela odlišnými způsoby. 1. sníženou imunoreaktivitou močového albuminu, 2. nespecifičností HPLC metody pro vyšetření albuminurie. Tato kontroverzní zjištění se stávají východiskem i pro náš další výzkum.
Albuminuria (mainly in the field of so-called microalbuminuria) is regarded as the most important predictor of high risk for the development of diabetic nephropathy. In addition to predicting nephropathy, microalbuminuria is also a marker of increased risk of cardiovascular diseases and risk factor for all-cause mortality. The terms „microalbuminuria“, „normoalbuminuria“ and „macroalbuminuria“ should be discontinued, and replaced with the term „urine albumin“. The terms cause confusion, and uninitiated interpreters often wonder if there is a small, ordinary or large albumin molecule being excreted. The concentration of albumin excreted in 24 h was used in the past. The difficulty collecting 24 h urine samples has lead to the common recommendation to use an untimed urine collection and to report the ratio of the albumin concentration to the creatinine concentration. This albumin/creatinine ratio (ACR, in units of mg/mmol, mg/g) is widely viewed as an acceptable surrogate for albumin excretion rate. First morning void urine samples provide lower variability than random samples. Intact albumin in urine may exist in two forms, immunoreactive and immunounreactive. Previous estimates of albuminuria have only detected immunoreactive forms. Urinary albumin is traditionally measured using immunochemical methods such as immunoturbidimetry, immunonephelometry, radioimmunoassay, enzyme immunoassay. High-performance liquid chromatography (HPLC) probably detects additional albumin that is missed by immunochemical methods. Comparison of albumin/creatinine ratio by HPLC with immunochemical methods showed positive proportional bias, which decreased with increasing concentrations of albumin. Some studies have even demonstrated that the HPLC method can identify those with albuminuria approximately 2 to 4 years earlier than current methodology, and thus it identifies some people at increased risk of morbidity and mortality. Recently, several proteins (for example transferrin, ?-1-acid glycoprotein, ?-1-proteinase inhibitor) were found to be coeluted along with albumin during HPLC analysis. In this case HPLC could possibly provide falsely higher results. This implies that differences between HPLC and immunochemical methods are interpreted either as lowered immunoreactivity of urine albumin, or as nonspecificity of HPLC method for assessment urine albumin. These controversial findings are thus becoming the starting-point for our research.
We describe here a newly developed method for a contact-free optical pH measurement in yeast suspensions supplemented with glucose, and containing the pH sensitive triphenylmethane dye bromocresol green. It is suitable for performing the acidification power test (based on measuring the rate of pH drop of yeast suspension caused by active extrusion of acidity from cells after glucose addition) used for assessing yeast vitality in fermentation industries. Using this methodology we monitored the pH in yeast suspensions in the course of acidification in the pH range of 3.5-5.3. Optical pH measurement allows simultaneous testing of several samples, minimizes the sample volume, simplifies sample handling and reduces the hands-on time in sample processing.
