The use of structural autografts from the iliac crest for reconstruction of bony defects in the hand and foot was described by Wilson and Lance in 1965. However, very few series of this technique are published and long-term results are unknown. We present a single-institution series of 23 patients with a mean follow-up of 92 months. We also describe a novel modification of this technique using the anterior superior iliac spine for reconstruction of the adjacent joint. Failure rate was 13% at mean of 17.3 months. All patients had a firm grip in the hand and walked without support. Bony fusion was achieved in all cases regardless of the type of fixation used. The use of ASIS for joint reconstruction was successful, particularly in low-demand joints. We encountered 1 case of instability and progressive stiffness in weight-bearing joints. This is a reliable method of reconstructing bony defects in the hand or foot following tumor resections.

Tigecyklin je glycylcyklinové antibiotikum. Spektrum účinku zahrnuje multirezistentní bakterie, včetně MRSA (meticilin rezistentní Staphylococcus aureus) a enterobakterií tvořících širokospektrou beta-laktamázu (ESBL, extended spectrum beta-lactamase). Je indikováno u komplikovaných infekcí kůže a měkkých tkání s vyloučením infekcí diabetické nohy a u komplikovaných nitrobřišních infekcích. U pacientů v intenzivní péči jsou zkušenosti s jeho použitím omezené. V článku uvádíme naše zkušenosti s použitím tohoto antibiotika u dvou pacientů hospitalizovaných na jednotce intenzivní péče (JIP) pro infekci způsobenou multirezistentními kmeny Stophylococcus oureus, Enterococcus faecium a Escherichia coli.

The authors describe the concomitant presence of plantar fibromatosis and Dupuytren's disease in a 33-year-old man. A lesion located under the sesamoid bones of the first metatarsophalangeal joint on the right showed an aggressive tendency (rapid growth, pain, impossibility to put weight on the medial side of the foot). Another lesion located proximal to the first one was smaller and painless. Neither plain radiography nor computed tomography showed any structural changes of the skeleton. Contrast-enhancement on magnetic resonance imaging revealed two lesions on the sole of the foot. Since a concurrence of plantar fibromatosis and malignant tumour could not be ruled out, an excision of both lesions and the adjacent plantar aponeurosis was made for biopsy examination. The operative procedure was carried out from two incisions. The intra-operative findings included proliferative growth and bleeding in the lesion located under the first metatarsophalangeal joint, and delimited growth without noticeable bleeding in the other lesion. Based on histological examination, the diagnosis of plantar fibromatosis was made for both lesions. The differential diagnosis and therapy of plantar fibromatosis is discussed.

Seniorský věk vyžaduje specifický přístup a diagnostickou rozvahu při indikaci operačního výkonu jak u akutních tak i u plánovaných výkonů. Naše kazuistika upozorňuje na neodůvodněné oddalování akutních operačních výkonu u seniorů až do doby provedení výkonu z vitální indikace, což pro tyto nemocné znamená výrazné zhoršení kvality života v období „čekání na výkon“, vede ke zvýšenému riziku vzniku trvalé poruchy mobility a soběstačnosti a zvýšenému riziku dlouhodobé pooperační úmrtnosti. V závěru doporučujeme některá vyšetření, která je potřebné provést k odpovědné rozvaze při indikaci chirurgických výkonu u seniorů.
Elderly demands specific approach and diagnostic decision in surgery indication both of acute and scheduled procedures. Our case history points out to the unreasoned delay of acute surgical procedures in senior patients until the surgery is performed from “vital indication” which results in the worsened quality of life of patients in the “waiting for procedure” period, the increased risk of permanent mobility impairment and the self-sufficiency decrease and the increased risk of long-term post surgical mortality. In conclusion, we recommend certain necessary examinations to be performed before responsible discretion to surgery indication in elderly patients.

Autoři ve své práci uvádějí možnosti operační léčby deformit prstů nohy u pacientů s revmatoidní artritidou. Noha je u pacientů s revmatoidní artritidou postižena velice často, až v 95 %. Nejčastěji dochází k valgózní deformitě palce a k poklesu příčné a podélné klenby nožní. Dochází k luxaci v metatarzofalangeálních kloubech a hlavičky metatarzů prominují v plosce nohy. Tyto deformity velice ztěžují chůzi nemocného. Nejčastěji jsou používány resekční operace, kdy jsou resekovány hlavičky metatarzů a na palci je resekována báze základního článku prstu. V článku je kladen důraz na včasnost operační léčby a na spolupráci revmatologa, protetického oddělení a ortopeda.
This paper reviews the possibilities in surgical treatment of toe deformities in patients with rheumatoid arthritis (RA). Foot is frequently affected in patients with RA – up to in 95 %. The most frequent deformities are the hallux valgus and flat-foot. Typical is the luxation of metatarsophalangeal heads, which protrude in the sole. This causes difficulties in walking. The resection is the most frequently performed procedure in which the metatarsal heads of leader toes are removed as well as the basal part of big toe. Early indication for operationandcollaboration between rheumatologist, prothetic department and orthopedic surgeon is stressed.
