RATIONALE & OBJECTIVE: Research on pediatric kidney replacement therapy (KRT) has primarily focused on Europe and North America. In this study, we describe the mortality risk of children treated with maintenance peritoneal dialysis (MPD) in different parts of the world and characterize the associated demographic and macroeconomic factors. STUDY DESIGN: Prospective cohort study. SETTING & PARTICIPANTS: Patients younger than 19 years at inclusion into the International Pediatric Peritoneal Dialysis Network registry, who initiated MPD between 1996 and 2017. EXPOSURE: Region as primary exposure (Asia, Western Europe, Eastern Europe, Latin America, North America, and Oceania). Other demographic, clinical, and macroeconomic (4 income groups based on gross national income) factors also were studied. OUTCOME: All-cause MPD mortality. ANALYTICAL APPROACH: Patients were observed for 3 years, and the mortality rates in different regions and income groups were calculated. Cause-specific hazards models with random effects were fit to calculate the proportional change in variance for factors that could explain variation in mortality rates. RESULTS: A total of 2,956 patients with a median age of 7.8 years at the start of KRT were included. After 3 years, the overall probability of death was 5%, ranging from 2% in North America to 9% in Eastern Europe. Mortality rates were higher in low-income countries than in high-income countries. Income category explained 50.1% of the variance in mortality risk between regions. Other explanatory factors included peritoneal dialysis modality at start (22.5%) and body mass index (11.1%). LIMITATIONS: The interpretation of interregional survival differences as found in this study may be hampered by selection bias. CONCLUSIONS: This study shows that the overall 3-year patient survival on pediatric MPD is high, and that country income is associated with patient survival.

V letech 2009–2018 bylo na klinice nefrologie léčeno peritoneální dialýzou celkem 123 pacientů. Z tohoto 41 pacientů zahájilo dialyzační léčení akutně, bez předchozí ambulantní přípravy (pacienti z ulice). Transplantaci podstoupilo 64 pacientů, na hemodialýzu bylo převedeno pro neadekvátnost metody nebo neřešitelnou hyperhydrataci 13 pacientů, z nichž 11 bylo po několika měsících úspěšně transplantováno. Zemřelo 21 pacientů léčených peritoneální dialýzou a dva pacienti po převedení na hemodialýzu. Ve skupině transplantovaných pacientů bylo méně diabetiků, pacienti byli mladší a neměli další komorbidity. Hodnoty albuminu v průběhu léčby se nesnížily, stejně tak byly stálé hodnoty hemoglobinu a CRP na rozdíl od skupiny pacientů, kteří ve sledovaném období zemřeli. Incidence peritonitidy byla 1/42 měsíců léčby. Peritoneální dialyzační katétr pro infekci byl odstraněn u 9 pacientů. Pacienti s infekcí byli starší, bylo zde více diabetiků a na konci léčby měli tito pacienti nízké hodnoty albuminu a vyšší CRP ve srovnání s pacienty bez infekce. Pacienti z ulice se nelišili od pacientů přicházejících z nefrologické ambulance. Peritoneální dialýza je vhodnou metodou léčby pro pacienty indikované k transplantaci ledvin a je metodou první volby i pro pacienty bez předchozího nefrologického sledování. Urgentní peritoneální dialýza zkracuje dobu, kdy je pacient hemodialyzován cestou centrálního žilního katétru, a tím snižuje riziko infekčních komplikací. Pacienti s diabetem, polymorbidní a starší pacienti mají vyšší riziko infekčních komplikací a volba metody by měla být individualizovaná.
There were total of 123 patients treated with peritoneal dialysis at our department between the years 2009 and 2018. Out of these, 41 patients started dialysis without being previously followed by nephrologist. 64 patients underwent renal transplantation. 13 patients were transferred to haemodialysis due to hyperhydratation or inadequacy of dialysis, 11 of them underwent successful renal transplantation in the following months. Out of the patients on peritoneal dialysis, 21 patients died and two more died after being transferred to haemodialysis. The group of transplanted patients contained more patients of younger age, less patients with diabetes and had less patients with comorbidities. Levels of albumin, haemoglobin and CRP remained stable, except in the patients who died during the followed period. Incidence of peritonitis was 1/42 months of treatment. Peritoneal dialysis catheter had to be removed due to infectious complication in 9 patients. Patients with infection were older, more often with diabetes and they also had lower levels of albumin and higher levels of CRP at the end of the treatment when compared to patients without infectious complications. There was no difference between the patients who were previously followed at outpatient nephrology clinic and those who were not. Peritoneal dialysis is a suitable method of treatment for patients indicated to renal transplantation and it is a method of choice also for the patients not previously followed by nephrologist. Urgent peritoneal dialysis shortens the period of haemodialysis treatment via central venous catheter, thus reducing the risk of infectious complications. Older patients, patients with diabetes and patients with comorbidities are at higher risk of infectious complications and the choice of type of dialysis should be based on individual decision in these cases.
