OBJECTIVE: Little is known about the association between subjectively experienced levels of diabetes distress (DD) and personality traits (PTs), even when levels of DD appear stable over time. This study aimed to use the Alternative Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) Model for Personality Disorders (AMPD) to associate specific maladaptive PTs with experienced DD and to describe differences in the constellation of PTs between people with type 1 diabetes (PWT1D) and type 2 diabetes (PWT2D). METHODS: A total of 358 participants with diabetes mellitus (DM) (56.2% female, mean age 42.33 years, standard deviation (SD) = 14.33) were evaluated using the Diabetes Distress Scale (DDS) and the shortened 160-item version of the Personality Inventory for DSM-5 (PID-5). Psychometric properties of the DDS were evaluated first, then the association between DDS and PID-5 scores, and the differences between groups based on diabetes type and DD level, were analyzed. RESULTS: Strong associations were found between the PID-5 Negative Affectivity (NEF) domain and the emotional burden (β = 0.852, pHolm < 0.001) and regimen distress (β = 0.435, pHolm = 0.006) DDS subscale scores. PWT1D had a higher level of personality pathology than PWT2D, as did participants with elevated levels of DD across most domains and facets of PID-5. CONCLUSIONS: Our findings suggest that attention should be paid to the level of NEF among people with diabetes in relation to their emotional burden and perception of regimen distress. We recommend a distinction between people based on their diabetes type. Implications for clinical practice and interventions for DD perceived through the lens of the dimensional DSM-5 PT model are discussed.

Background: Comparative efficacy of rehabilitation interventions in persons with acute ischemic stroke (PwS) is limited. This randomized trial assessed the immediate and lasting effects of five interventions on clinical and mobility outcomes in 75 PwS. Methods: Five days after stroke, 75 PwS were randomized into five groups: physical therapy (CON, standard care, once daily); walking with a soft robotic exoskeleton (ROB, once daily); agility exergaming once (EXE1, once daily) or twice daily (EXE2, twice daily); and combined EXE1+ROB in two daily sessions. Interventions were performed 5 days per week for 3 weeks. Outcomes were assessed at baseline, post-intervention, and after 5 weeks of detraining. Results: Modified Rankin Scale (primary outcome) and Barthel Index showed no changes. EXE1, EXE2, ROB, and EXE1+ROB outperformed standard care (CON) in five secondary outcomes (Berg balance scale, 10m walking speed, 6-min walk test with/without robot, standing balance), with effects sustained after 5 weeks. Dose effects (EXE1 vs. EXE2) were minimal, while EXE1+ROB showed additive effects in 6-min walk tests. Conclusions: These novel comparative data expand evidence-based options for therapists to design individualized rehabilitation plans for PwS. Further confirmation is needed.

BACKGROUND: The study aimed to assess the acute effects of overcoming isometric split squats on subsequent single-leg drop jump (DJ) performance and the contralateral effect in volleyball players, considering training status differences. METHODS: Ten male elite (ELI) and ten amateur (AMA) volleyball players took part in two experimental sessions in which they performed conditioning activity (CA) consisted of 3 sets of overcoming isometric 3-second split squats, differing in the limb used: dominant (DL) or non-dominant (ND-L). Single-leg DJ was performed before and at the 4th and 8th minute post-CA for both limbs. The best post-CA attempt in terms of jump height (JH) was analyzed. RESULTS: Results revealed a significant increase in jump height (JH) in the non-dominant limb (ND-L) from 13.7 ± 2.6 cm to 15.5 ± 2.7 cm after CA performed by the ND-L (p = 0.001; Hedge's g = 0.65). Similarly, the reactive strength index (RSI) of the ND-L improved from 0.40 ± 0.06 to 0.45 ± 0.08 (p = 0.008; g = 0.58). Additionally, contact time in the ND-L increased significantly from 342 ± 36 ms to 375 ± 42 ms after CA performed by the dominant limb (p = 0.001; g = 0.66). In the elite (ELI) group, JH significantly increased from 16.4 ± 2.4 cm to 18.3 ± 3.3 cm (p < 0.001; g = 0.79), while RSI in the dominant limb (DL) improved from 0.47 ± 0.06 to 0.53 ± 0.07 (p = 0.011; g = 0.68). CONCLUSIONS: Results suggest that the examined CA effectively induces the post-activation performance enhancement in DJ among elite volleyball players, with a predominantly local impact on the limb executing the CA. INTRODUCTION: Human and animal skin is colonized by a complex microbial population. An imbalance of these microorganisms is often associated with dermatological diseases. METHODS: The aim of this work was to describe the skin bacterial microbiota composition of healthy dogs and dogs with inflammatory skin lesions. Genomic DNA was sequenced using primers that target the V4 region of the bacterial 16S rRNA gene. Superficial skin swabs were collected from eight body areas of six healthy dogs (n = 48) and directly from inflammatory altered canine skin (n = 16). RESULTS: The skin of healthy dogs was predominantly colonized by phylum Bacillota (34.4 ± 27.2%), followed by Actinomycetota (32.2 ± 20.3%), Pseudomonadota (16.4 ± 12.2%), and Bacteroidota (8.7 ± 11.6%). At the level of genera, Streptococcus spp. (19.4 ± 26.1%) was the most abundant genus across all samples collected from healthy skin, followed by Curtobacterium (5.4 ± 12.1%), Bacteroides (5.2 ± 11.1%) and Corynebacterium_1 (4.3 ± 13.2%). More specifically, Streptococcus spp. was the most abundant on the chin (49.0 ± 35.5%), nose (37.9 ± 32.1%), perianal region (21.1 ± 28.2%), abdomen (11.0 ± 12.8%), dorsal back (12.4 ± 10.3%) and interdigital area (5.5 ± 2.2%). Curtobacterium spp. was predominant on inner pinna (17.8 ± 24.8%) and axilla (6.7 ± 10.8%). Alpha diversity analysis (Shannon index) showed maximum on interdigital area but minimum on a chin (p-value: 0.0416). Beta diversity analysis showed clustering across samples from the individual skin sites but also across samples collected from individual dogs. Staphylococcus spp. was the most abundant genus in 12/16 samples collected from inflammatory skin. In addition, a lower bacterial diversity was observed in samples from skin lesions compared to samples from healthy canine skin. DISCUSSION: The results confirm the fact that the microbiome of healthy skin is very diverse. Compared to other studies, streptococci predominated on healthy canine skin. Shannon index showed only minor differences in diversity between different parts of canine skin. Results of beta-diversity showed the fact that the main force driving the skin microbiota composition is the individual, followed by the skin site. On the area of skin lesions, dysbiosis was observed with a significant predominance of staphylococci.

BACKGROUND: Germline genetic susceptibilities of rare cancers of the esophagus, stomach, small intestine, testis, (nonmedullary) thyroid gland and bone with high familial risks are not well known. Here, we use familial risk data from the Swedish Family-Cancer Database which contains records of cancers in Swedish families obtained over a century. We compare familial risks for offspring diagnosed with any of these cancers when their parent had or had not that cancer. We review the global literature of the reported constitutional variants that may explain part of the familial risk. MAIN BODY: Familial risks for esophageal and stomach cancers are about 2.0 and apart from early-onset stomach cancer few high-risk variants are known. Genetic studies may be hampered by dominant environmental risk factors for these cancers. Small intestinal carcinoids have a very high familial risk (28 between siblings) but no high-risk genes have been identified to explain this. Low-risk polygenic variants have been identified. Small intestinal adenocarcinoma is a manifestation in Lynch syndrome. Testicular and thyroid cancers are characterized by high familial risk (about 5) which may be explained largely by a polygenic background, although thyroid cancer is a component in a number of rare cancer syndromes. Several predisposing genes have been identified for bone cancer (familial risk 7). CONCLUSIONS: The discussed cancers are rare and they present with a relatively high familial risk, in spite of lacking identified high-penetrant constitutional variants. It is possible that the polygenic component, already recognized for testis cancer, is stronger than previously expected. Thus polygenic models with rare high/moderate- and low-risk variants could fit the familial risk and shape the germline genetic landscape of these cancers. Polygenic background may have clinical implications.

Space and time are fundamental attributes of the external world. Deciphering the brain mechanisms involved in processing the surrounding environment is one of the main challenges in neuroscience. This is particularly defiant when situations change rapidly over time because of the intertwining of spatial and temporal information. However, understanding the cognitive processes that allow coping with dynamic environments is critical, as the nervous system evolved in them due to the pressure for survival. Recent experiments have revealed a new cognitive mechanism called time compaction. According to it, a dynamic situation is represented internally by a static map of the future interactions between the perceived elements (including the subject itself). The salience of predicted interactions (e.g. collisions) over other spatiotemporal and dynamic attributes during the processing of time-changing situations has been shown in humans, rats, and bats. Motivated by this ubiquity, we study an artificial neural network to explore its minimal conditions necessary to represent a dynamic stimulus through the future interactions present in it. We show that, under general and simple conditions, the neural activity linked to the predicted interactions emerges to encode the perceived dynamic stimulus. Our results show that this encoding improves learning, memorization and decision making when dealing with stimuli with impending interactions compared to no-interaction stimuli. These findings are in agreement with theoretical and experimental results that have supported time compaction as a novel and ubiquitous cognitive process.

OBJECTIVES: Illicit drug use presents a significant challenge to global health and public safety, requiring innovative and effective monitoring strategies. This study aimed to evaluate the current landscape of wastewater-based epidemiology (WBE) for monitoring illicit drugs in Europe, focusing on collaboration, current practices, and barriers, while identifying opportunities for improvement. STUDY DESIGN: Cross-sectional survey-based study. METHODS: Coordinated by the Sewage Analysis CORe Group Europe (SCORE) and the European Union Drugs Agency (EUDA), two surveys were conducted in 2023 targeting researchers and stakeholders using WBE for illicit drugs. Data were analysed to identify trends, gaps, and opportunities for improving WBE implementation. RESULTS: The findings indicate a robust research infrastructure and diverse analytical methods among European institutions. Two-thirds of the participating countries reported using WBE data to inform policy. However, challenges persist, particularly in securing funding and coordination, as well as generating national estimates from multiple locations and addressing specific local policy needs. CONCLUSIONS: WBE has proven to be a valuable tool for monitoring illicit drug trends and informing drug policies. To unlock its full potential, sustained funding, methodological standardization, and enhanced cooperation are essential. This study provides critical insights into the European WBE landscape, offering a roadmap for strengthening the integration of actionable WBE data into public health and policy frameworks.

Background: In the absence of uniform European regulations, there have been many differences in the training of perfusionists across Europe. Furthermore, there has been no uniform or single European accreditation of the profession. One of the objectives of The European Board of Cardiovascular Perfusion (EBCP) is to standardise and monitor training of perfusionists across Europe whilst offering support in accordance with national regulations. This goal is particularly imminent as there have been numerous newly founded National perfusion societies, particularly from Eastern European countries, which are now established members of EBCP.Purpose: In this article, we provide an updated overview or 'snapshot' of current European perfusion training programs that were accessible in 2022. Nationally acquired data refers to 2022 unless stated otherwise. The last overview of Perfusion education in Europe was reported over 15 years ago including 20 countries.Research Design: For this report thirty-two national EBCP delegates plus representatives from Austria were contacted at the beginning of 2023 to complete a pro forma questionnaire about their national perfusion training programmes. The data has been summarized in this article and five additional derived parameters were calculated.Results: We received responses from 31 countries, providing specific national training characteristics which are summarized, listed and benchmarked by country in this article.Conclusion: There have been several national and supranational initiatives towards the recognition of perfusion as a profession in Europe, however so far without success for the majority of countries. For this reason, it remains essential for EBCP, as the only European professional perfusionist body, to define education standards and competencies for perfusionists and to monitor training by accreditation of dedicated perfusion schools across Europe.
BACKGROUND: Observational data on composite scores often comes with missing component information. When a complete-case (CC) analysis of composite scores is unbiased, preferable approaches of dealing with missing component information should also be unbiased and provide a more precise estimate. We assessed the performance of several methods compared to CC analysis in estimating the means of common composite scores used in axial spondyloarthritis research. METHODS: Individual mean imputation (IMI), the modified formula method (MF), overall mean imputation (OMI), and multiple imputation of missing component values (MI) were assessed either analytically or by means of simulations from available data collected across Europe. Their performance in estimating the means of the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), the Bath Ankylosing Spondylitis Functional Index (BASFI), and the Ankylosing Spondylitis Disease Activity Score based on C-reactive protein (ASDAS-CRP) in cases where component information was set missing completely at random was compared to the CC approach based on bias, variance, and coverage. RESULTS: Like the MF method, IMI uses a modified formula for observations with missing components resulting in modified composite scores. In the case of an unbiased CC approach, these two methods yielded representative samples of the distribution arising from a mixture of the original and modified composite scores, which, however, could not be considered the same as the distribution of the original score. The IMI and MF method are, thus, intrinsically biased. OMI provided an unbiased mean but displayed a complex dependence structure among observations that, if not accounted for, resulted in severe coverage issues. MI improved precision compared to CC and gave unbiased means and proper coverage as long as the extent of missingness was not too large. CONCLUSIONS: MI of missing component values was the only method found successful in retaining CC's unbiasedness and in providing increased precision for estimating the means of BASDAI, BASFI, and ASDAS-CRP. However, since MI is susceptible to incorrect implementation and its performance may become questionable with increasing missingness, we consider the implementation of an error-free CC approach a valid and valuable option.
