Koncepcia odboru lekárenstvo definuje lekárenstvo ako základný odbor farmácie, ktorého hlavnou úlohou je poskytovanie lekárenskej starostlivosti ako neoddeliteľnej súčasti poskytovania zdravotnej starostlivosti. Predstavuje súbor odborných činností farmaceuta orientovaných na zabezpečovanie humánnych a veterinárnych liekov a zdravotníckych pomôcok a na optimalizáciu účinnej, bezpečnej a kvalitnej farmakoterapie. Prakticky je lekárenstvo aplikovanou disciplínou, keďže využíva poznatky iných farmaceutických, medicínskych, psychologických, sociálnych a ekonomických vied. Pre svoju interdisciplinárnosť je potrebné rozširovať teóriu lekárenstva tak, aby odzrkadľovala všetky aspekty svojej pôsobnosti. Ako jedna z možností je definovať lekáreň ako samostatnú funkčnú jednotku, ktorá pracuje na základe platných právnych noriem tak, aby na jednej strane zabezpečovala poskytovanie zdravotnej (lekárenskej) starostlivosti a na druhej strane udržiavala svoju rentabilitu. Aby boli kvalita starostlivosti o pacienta a ekonomické hľadisko v rovnováhe je potrebné definovať termín lekárenská logistika aj v podmienkach slovenskej (českej) farmácie ako včasné uspokojenie požiadaviek pacienta (klienta) lekárne, pričom určený liek alebo zdravotnícka pomôcka a príslušné informácie musia byť v správny čas v požadovanom množstve, požadovanej kvalite na správnom mieste.
The conception of the field of pharmaceutical service defines pharmaceutical service as the basic part of pharmacy, the principal task of which is to provide pharmaceutical care as an inseparable part of providing health care. It represents a set of professional activities of the pharmacist oriented to securing human and veterinary pharmaceutical products and health care products and to optimising effective, safe and quality pharmacotherapy. Technically, pharmaceutical service is an applied discipline, as it makes use of knowledge gained in other pharmaceutical, medical, psychological, social, and economic sciences. Because of its interdisciplinary character it is necessary to extend the theory of pharmaceutical service in such a way so that it may reflect all aspects of its sphere of activity. One of the possibilities is to define the pharmacy premises as an independent functional unit which operates on the basis of valid legal standards in such a way that on the one hand it secures the provision of health (pharmaceutical) care, and on the other hand it maintains its costeffectiveness. To keep the quality of care of the patient and the economic aspect in balance, it is necessary to define the term pharmaceutical logistics also under the conditions of Slovak (Czech) pharmacy as early satisfaction of the requirements of the patient (client) in the pharmacy premises, which means that the appropriate pharmaceutical product or health care product and the appropriate information must be at the right time in the required amount and required quality in the right place.

BACKGROUND AND PURPOSE: Diagnostic transcranial Doppler ultrasound (TCD) is commonly used in patients with acute stroke before or during treatment with intravenous thrombolysis (IVT). We aimed to assess how much TCD delays IVT initiation and whether TCD influences outcomes. METHODS: We analyzed data from the SITS-ISTR (Safe Implementation of Thrombolysis in Stroke-International Stroke Thrombolysis Register) collected from December 2002 to December 2011. Outcomes were door-to-needle time, symptomatic intracerebral hemorrhage, functional outcome per the modified Rankin Scale, and mortality at 3 months. RESULTS: In hospitals performing any TCD pre-IVT, 1701 of 11 265 patients (15%) had TCD before IVT initiation. Door-to-needle time was higher in patients with pre-IVT TCD (74 versus 60 minutes; P<0.001). At hospitals performing any TCD during IVT infusion, of 9044 patients with IVT, 747 were examined with TCD during IVT. No treatment delay was seen with TCD during IVT. After multivariate adjustment, TCD during IVT was independently associated with modestly increased excellent functional outcome (modified Rankin Scale, 0-1; adjusted odds ratio, 1.28; 95% confidence interval, 1.06-1.55; P=0.012) and lower mortality (adjusted odds ratio, 0.73; 95% confidence interval, 0.55-0.95; P=0.022). CONCLUSIONS: We recommend that TCD, if performed, should be done during IVT infusion, to avoid treatment delay. The association of hyperacute TCD with beneficial outcomes suggests potential impact on patient management, which warrants further study.

The importance of environmental sustainability is becoming more and more obvious, so the rationale behind long-term usage of solely non-renewable energy sources appeared questionable. This study aims to identify, using Kaplan-Meier survival analysis and logistic regressions, the main determinants that affect the duration of Russian non-renewable energy exports to different regions of the world. Data were retrieved from the databanks of the World Development Indicators (WDI), World Integrated Trade Solution (WITS), and the French Centre for Prospective studies and International Information (CEPII). The obtained results point to the fact that approximately 52% of energy products survive beyond their first year of trading, nearly 38% survive beyond the second year, and almost 18% survive to the twelfth year. The survival of Russian non-renewable energy exports differs depending on the region, and the affecting factors are of different importance. The duration of Russian non-renewable energy exports is significantly linked to Russia's GDP, Total export, and Initial export values. A decline in Russia's GDP by 1% is associated with an increasing probability of a spell ending by 2.9% on average, in turn growing Total export and Initial export values positively linked with the duration of non-renewable energy exports from Russia. These findings may have practical relevance for strategic actions aimed at approaching both energy security and environmental sustainability.
