Od března 1993 do června 2001 bylo provedeno 80 vyšetření u 78 pacientů (u jednoho pacienta šlo o selektivní test). Montrealský protokol byl adaptován pro české poměry. Práce se zaměřuje na zkoušku řeči a na paměťovou zkoušku. Jazyková dominance byla určena po úspěšných bilaterálních injekcích u 86 % pacientů. Z 66 osob u osmi (12,1 %) byla zjištěna atypická lateralizace řeči. Objevení se artikulované řeči bylo statisticky vysoce významně (p<0,001) dříve po aplikaci do pravé hemisféry, než po aplikaci do levé. Při aplikaci do levé hemisféry byl podstatně větší rozptyl doby objevení se řeči. Paměťové výkony byly významně ovlivněny lateralizací leze a stranou, která byla částečně inaktivována. Jako anestetizující látka byl použit u 40 testů amobarbital sodný, u 36 testů methohexital, ve 3 případech thiopental, jednou byl pokus ukončený ještě před aplikací barbituranu. Po amobarbitalu začínala hemiplegie významně později než po methohexitalu při inaktivaci levé hemisféry, rozdíl nebyl významný při aplikaci do pravé hemisféry. První artikulovaná řeč byla po vzniku hemiplegie významně dříve při amobarbitalu, jak při inaktivaci levé, tak pravé hemisféry. Procento správného vybavení paměťových položek bylo u amobarbitalu významně nižší než u methohexitalu, avšak pouze při inaktivaci levé hemisféry. Jsou diskutovány otázky s převodem metody, technické překážky zdárného průběhu, otázky interpretace získaných skórů a varianty v provádění vyšetření. Zatím není spolehlivá neinvazivni alternativa této metody, v řadě indikaci se dokonce rozšiřují možnosti využití a zjemňují techniky provádění. V současnosti je test považovaný za standardní vyšetření ve specializovaných centrech pro epilepsii k posouzení pravděpodobnosti rozvoje pooperačních poruch řeči nebo paměti.
Between March 1993 and June 200180 examinations were made in 78 patients (in one patient a selective test). The Montreal protocol was adapted for local conditions. The investigation is focused on a language and memory test. Language dominance was assessed after successful bilateral injections in 80 % patients. Of 66 subjects in eight (12.1 %) an atypical lateralization of speech was recorded. Development of articulated speech was highly significantly earlier (p<0.001) after administration into the right hemisphere than after administration into the left one. After administration into the left hemisphere there was a much greater scatter of time before speech developed. Memory was significantly influenced by lateralization of the lesion and the side which was partly inactivated. As anaesthetic in 40 tests sodium amobarbital was used, in 36 tests methohexital, in 3 instances thiopental, in one instance the experiment was finished before administration of the barbiturate. After amobarbital hemiplegia set in significantly later than after methohexital during inactivation of the left hemisphere, the difference was not significant when administered into the right hemisphere. The first articidated speech after the development of hemiplegia was significantly sooner after amobarbital. This applied to inactivation of the left as well as the right hemisphere. The percentage of correct recovery of memory items was after amobarbital significantly lower than in methohexital but only for inactivation of the left hemisphere. The authors discuss methodological questions, technical impediments of a successful development, problems of interpretation of assembled scores and variants as regards implementation of the examination. So far no reliable non-invasive alternative of this method exists, in some indications the possibilities of its application are even extended and the techniques of its implementation are refined. At present the test is considered a standard examination in specialized centres for epilepsy for evaluation of the probable development of postoperative speech or memory disorders.

OBJECTIVES: To evaluate the effect of smear layer deproteinization using hypochlorous acid (HOCl) with/without metal chlorides (SrCl2 and ZnCl2) on the microtensile bond strength (μTBS) of two simplified adhesives to dentin. METHODS: Human dentin surfaces with a standardized smear layer were pretreated using a 105 ppm HOCl solution with/without SrCl2 (0.05 M, 0.1 M, 0.2 M, 0.4 M) or ZnCl2 (0.05 M, 0.1 M, 0.2 M) for 5 s, 15 s, or 30 s. After the deproteinizing solution was washed out with water for 5 s, 15 s, or 30 s, pretreated surfaces were bonded with one-step self-etch adhesive Bond Force II or universal adhesive Clearfil Universal Bond Quick, and μTBS was measured after 24 h. Additionally, the deproteinizing effects of HOCl solutions with/without the metal chlorides were compared by measuring changes in the amide:phosphate ratio using attenuated total reflection Fourier transform infrared spectroscopy. Statistical analysis was performed using multifactor ANOVA, Tukey's post hoc tests and t-tests (p < 0.05). RESULTS: Pretreatment with pure HOCl for 15 s and 30 s significantly decreased the amide:phosphate ratio (p < 0.05), indicating effective deproteinization, but the μTBS of both adhesives increased significantly only if HOCl was washed out for 30 s (p < 0.05). Increasing the concentrations of metal chlorides enabled shortening of the wash-out time down to 5 s while maintaining the improved μTBS (p < 0.05). The deproteinizing effect of HOCl was not significantly altered by the addition of metal chlorides (p > 0.05). SIGNIFICANCE: The effectiveness of smear layer deproteinization using HOCl can be improved by the addition of metal chlorides, as their increasing concentration allowed to shorten the wash-out time from 30 s down to 5 s.

This study investigated the influence of application/wash-out times of sodium hypochlorite (NaOCl) and hypochlorous acid (HOCl) on dentin bond strength of one-step self-etch adhesives (1-SEAs). Human coronal dentin discs with a standardized smear layer were pretreated with 6% NaOCl or 100 ppm HOCl for 5 s, 15 s, or 30 s, and washed out with water for 5 s, 15 s or 30 s with or without the application of Clearfil DC Activator (CDA). No pretreatment was used as a control. The discs were bonded with a 1-SEA (Bond Force II or Clearfil Universal Bond Quick) and microtensile bond strength (μTBS) was measured after 24 h. Pretreatment with NaOCl for 15 s and 30 s significantly decreased μTBS (p<0.05), irrespective of wash-out time. The application of CDA recovered μTBS but did not outperform the control group. Conversely, pretreatment with HOCl for 15 s and 30 s followed by 30 s wash-out time significantly increased μTBS of 1-SEAs (p<0.05), regardless of CDA application.
OBJECTIVES: To evaluate the sealing performance of self-etch adhesives (SEA) after smear layer deproteinization with chemo-mechanical caries removal agents (CRA) to enamel and dentin cavity walls in comparison with 6% NaOCl smear layer deproteinizing. METHODS: Eighty extracted bovine incisors with tapered cylindrical cavities (2.5 mm upper diameter, 1.5 mm lower diameter, 2 mm depth) at the cemento-enamel junction were randomly divided into four pre-treatment groups; no treatment (control), Papacarie (papain enzyme-based CRA), Carisolv (NaOCl-based CRA), and 6% NaOCl. After pretreatment, G-Bond Plus (GBP, one-step SEA) or Clearfil SE Bond 2 (CSE, two-step SEA) was applied to the pre-treated cavity, and then filled with a flowable resin composite (Clearfil Majesty ES Flow). Gap formations at the enamel and dentin lateral walls, and on the dentin cavity floor were investigated using swept-source optical coherence tomography (SS-OCT) after 24-h water storage at 37 °C. Deproteinizing effects of the tested agents on smear layer-covered enamel and dentin were quantified by changes in the amide:phosphate ratio using ATR-FTIR analysis. RESULTS: Papacarie significantly reduced gap formation with both self-etch adhesives at all locations, compared to the control group (p < 0.05). In contrast, increased gap formation was observed with Carisolv and NaOCl. In most groups, CSE exhibited significantly lower gap formation than GBP (p < 0.05). ATR-FTIR revealed a significant decrease in the amide:phosphate ratio for all the deproteinizing agents (p < 0.05). CONCLUSIONS: Chemo-mechanical CRAs could effectively remove the organic phase of enamel and dentin smear layer. The papain enzyme-based gel Papacarie could improve the sealing performance of the self-etch adhesives to both enamel and dentin.

Práce podává přehled oblastí, které by mělo neuropsychologickč vyšetření pokrývat (lateralita, osobnost a psychosociální charakteristiky, kognitivní výkonnost s důrazem na posouzení paměti a speciální neuropsychologické charakteristiky, především ve vztahu k lokalizaci a typu leze). Je podána informace o základních užívaných metodách s důrazem na metody autorem převedené do češtiny (HalsteadReitan Neuropsychological Test Battery - HRNB, Neuropsychological Battery for Epilepsy-NBE, Washington Psychosocial Seizure Inventory - WPSI, Personal Inventory (Bear-Fedio), Epilepsy Surgery Inventory (ESI-55), paměťové vyšetření při Wada-testu). Jsou diskutovány problémy kolem organizace a načasování vyšetření. Je doporučeno co nejpodrobnější vyšetřování (především metodami umožňujícími mezinárodní srovnání) a jeho opakování při katamnestickém sledování po operaci.
The author gives an account of the areas which should be covered by the neuropsychological examination (laterality, personality and psychosocial characteristics, cognitive abilities with emphasis on evaluation of memory and special neuropsychological characteristics, in particular in relation to the site and type of lesion). The author provides information on the basic methods used with emphasis on methods translated by the author into Czech (Halstead-Reitan Neuropsychological Test Battery HRNB, Neuropsychological Battery for Epilepsy - NBE, Washington Psychosocial Seizure Inventory WPSI, Personal Inventory (Bear-Fedio), Epilepsy Surgery Inventory (ESI-55), examination of memory in Wada test). The author discusses problems of organization and timing of examinations. He recommends detailed examinations (in particular using methods comparable on an international scale) and their repetition in catamnestic investigations after surgery.

Several functional MR imaging studies evaluating the lateralisation of linguistic functions in patients who underwent Wada testing have been reported. There is extensive variance in the Laterality index (LI) calculation across the studies, and the optimal calculation method remains unclear. We attempted to calculate the LI in different ways in the same subjects, in order to find the LI calculation method with the highest correlation to the Wada test. Fifteen patients (10 females, 5 males) suffering from medically intractable temporal lobe epilepsy (TLE) (12 left, 3 right) were admitted for the study. The patients underwent a standardized bilateral intracarotid short-acting barbiturate test. Language testing included spontaneous speech, oral comprehension, reading, object and picture naming, and repetition. All the tasks were scored separately in order to increase the possibility of correlation between Wada and LI. A silent phonemic verbal fluency task (VFT) was used as a language paradigm for functional measurement. Regions of interest (ROIs), with a known association with language function (Broca's area, the lateral prefrontal cortex, etc.), were defined. First, the LIs were calculated from the ROIs using a previously reported method (simple suprathreshold count). Next, we used several new methods of LI calculation (t-weighting of voxels, methods independent of the choice of the statistical threshold, etc.) The most significant correlation with Wada was proven in the LIs that were evaluated from Broca's area (up to R = 0.94, P = 1 x 10(-7)). However, the new LI calculation methods used in the present study did not produce a statistically significant benefit in comparison to previously reported methods.

Funkčné mapovanie mozgu charakterizuje oblasti CNS, ktoré sú zahrnuté do vizuálnych, senzitívnych, motorických, sluchových, pamäťových, rečových funkcií a pozornosti. V minulosti bolo možné určiť lokalizáciu jednotlivých funkcií mozgu pomocou invazívnych vyšetrovacích metód, napr. pomocou priamej elektrickej kortikálnej stimulácie alebo Wada testu. V súčasnosti je tendencia využívať skôr neinvazívne metódy (fMRI, TMS, PET, TCD, MEG), z ktorých každá umožňuje vyšetriť mozog z rôzneho pohľadu a na základe iného princípu. Cieľom dnešného skúmania je nájsť metódu zabezpečujúcu ideálne priestorové a časové rozlíšenie, čo by mohlo výrazne napomôcť v chápaní funkčnej organizácie mozgu.
Functional cerebral mapping characterizes areas of CNS, which are involved in visual, sensory, motor, auditory, memory, speech functions and attention. In past it was possible to determine localization of cerebral function only invasively by direct electrical cortical stimulation or Wada test. Recently there are attempts to use non-invasive methods (fMRI, TMS, PET, TCD, MEG) to investigate the brain from different point of view and principle. The aim of recent studies is to find an available method with ideal spatial and temporal resolution which could help us in better understanding of functional brain organization.
