For the last decade the reforming of the national healthcare systems has covered the majority of the economic development in the developed countries and has turned into a process varying in intensity and scope. The health care system in Bulgaria started to be reformed because of the population's worsened health status, the shortcomings of the health care system as well as the need for the adoption of the universal health model as a prerequisite for equal participation in the family of European nations. The reforms and the new requirements confronted by the healthcare have necessitated the application of new ways of organizing healthcare activities. A possible approach includes integrated care aimed at the optimization of the processes, the improvement of the resources utilization and efficiency enhancement as well as quality improvement of the provided medical care.

Článek se zabývá analýzou produkce a nakládání se zdravotnickými odpady a odhaluje trendy, které poukazují na nastavení trhu s odpady v České republice. Legislativní změny budou tyto trendy ovlivňovat. Konkrétní změny z pohledu hygienické služby a provozu zdravotnických zařízení jsou prezentovány po prvním seznámení s prováděcí vyhláškou č. 273/2021 Sb., o podrobnostech nakládání s odpady. Dále jsou zde nastíněny širší souvislosti a další výzvy v oblasti nakládání s odpady ve zdravotnictví v návaznosti na udržitelnost a dlouhodobou transformaci na oběhové hospodářství.
Analysis of the production and management of healthcare waste reveals trends that point to an atypical set-up of the waste market in the Czech Republic. Legislative changes will influence these trends. Specific changes from the perspective of the Regional Public Health Authorities and the operation of healthcare facilities are presented following the first introduction of the implementing Decree No 273/2021. A broader context and further challenges in the field of health care waste management in relation to sustainability and a long-term transformation to a recycling economy are also outlined.

INTRODUCTION AND AIMS: Czech music therapists lacked the skills and knowledge to adapt their practice to virtual music therapy (VMT) at the beginning of the COVID-19 pandemic, when physical contact was restricted. The aims of this project were to implement the best evidence-based recommendations concerning VMT in the practice of Czech music therapists and increase the number of therapists using VMT. METHODS: The project used the JBI Evidence Implementation Framework to promote change in the practice of participants. A baseline audit of 26 practitioners was conducted using questionnaires and interviews. The audit criteria consisted of 8 best practice recommendations, followed by the implementation of targeted strategies and a follow-up audit. RESULTS: Of the 26 music therapists, 18 began practicing VMT following the rollout of the implementation strategies. Strategies included educational workshops, instructional materials, technological support, individual counseling, and interactive training sessions. There was an increase in compliance with all audit criteria. The most critical barriers were lack of knowledge about VMT and lack of technical skills; lack of resources; technological challenges; low confidence of music therapists to provide VMT; and reduced need for VMT after the lockdown restrictions were relaxed. CONCLUSIONS: The implementation project helped to overcome temporary pandemic restrictions, increase the quality of VMT practice, develop strategies for VMT training, and identify reasons for using VMT during the pandemic and beyond.

One Health je vedecky podložený holistický koncept, ktorý uznáva úzke prepojenie medzi zdravím ľudí, zvierat a životného prostredia a má mnohé prínosy. Pre úspešnú implementáciu vyžaduje multisektorový, multidisciplinárny prístup na lokálnej, národnej aj celosvetovej úrovni. Je spojený s mnohými výzvami, ako sú napr. prekonanie zdržanlivých prístupov jednotlivých vedných disciplín, nedostatočná harmonizácia terminológie, priorít a koncepcie naprieč disciplínami a sektormi, mnoho neistôt, ktoré je potrebné vyhodnotiť a vstupov pre aktivity, ktoré sa musia určiť. Ďalšími výzvami sú aj problematické hodnotenie pomerov prínosov a nákladov, keďže je len málo porovnateľných opatrení, nerovnováha medzi sektormi v schopnosti, kapacite a alokácii zdrojov, právne prekážky alebo iné štrukturálne prekážky spolupráce, holistické, komplexné riešenia, ktoré môžu byť ťažko predajné politicky a môžu vyžadovať budovanie nových systémov a kapacít (aj s nákladmi). Napriek viacerým výzvam, ktoré so sebou prináša, je riešením pre efektívne riadenie rizika nielen pre zdravie ľudí, ale aj zdravie zvierat a životného prostredia.
One Health is a science-based holistic concept that recognises the close link between human, animal and environmental health and has many benefits. For successful implementation, it requires a multi-sectoral, multidisciplinary approach at the local, national and global levels. It is associated with many challenges, such as overcoming reluctance from individual scientific disciplines, insufficient harmonisation of terminology, priorities and concepts across disciplines and sectors and many uncertainties to be assessed and inputs for activities to be identified. Other challenges include problematic assessment of cost-benefit ratios as there are few comparable measures, imbalances between sectors in capacity, resource and capacity allocation, legal or other structural barriers to cooperation and comprehensive holistic solutions that can be difficult to sell politically and may require creation of new systems and capacities (even with costs). Despite the many challenges it poses, it is a solution for effective risk management not only for human health, but also for animal health and the environment.
