Cíl studie: Stanovit podíl pacientů se zhoršením kognitivních funkcí při předanestetickém vyšetření u seniorů ≥ 60 let podstupujících plánované operace v cévní chirurgii a prozkoumat, zda se kognitivní funkce liší u jednotlivých kategorií cévních operací. Druhotným cílem bylo ověřit, zda je takové vyšetření kognitivních funkcí proveditelné a pro pacienty přijatelné. Typ studie: Prospektivní observační studie. Typ pracoviště: Oddělení cévní chirurgie fakultní nemocnice. Materiál a metoda: Po souhlasu etické komise byli do studie mezi 1. 7. 2017 – 30. 6. 2018 zařazováni po sobě jdoucí senioři ≥ 60 let indikovaní k plánovaným cévně chirurgickým výkonům. Vyloučeni byli pacienti s diagnózou demence či mentální retardace, se závažnými poruchami smyslů, neschopností použít dominantní ruku či s jazykovou bariérou. Ke zjištění kognitivní funkce byl použit krátký screeningový Clock-in-the-Box (CIB) test. Výsledným parametrem byla četnost možné kognitivní dysfunkce podle CIB skóre ≤ 5 bodů (rozmezí 0–8). Významnost rozdílů mezi kategoriemi cévních výkonů byla hodnocena χ2 testem, P ≤ 0,05. Výsledky: Zařazeno bylo 122 pacientů, analyzováno 100, 63 mužů, průměrný věk 70,7 ± 7 let, vzdělání nejvýše střední 93, ASA nejčastěji 3. Možná kognitivní dysfunkce u jedné třetiny (34 pacientů), rozdíly mezi různými kategoriemi cévních výkonů (aorta, tepny dolních končetin, karotické endarterektomie, varixy) byly nevýznamné. Test je při předanestetickém vyšetření proveditelný, pacienty nezatěžuje, považují ho za užitečný a podstoupili by ho v případě potřeby znovu. Závěr: Třetina pacientů podstupujících plánované výkony cévní chirurgie vykazuje podle testu kreslení hodin (Clockin- the-Box test) preexistující zhoršení kognitivních funkcí. Test je dostatečně krátký, takže je při předanestetickém vyšetření proveditelný a pro pacienty přijatelný.
Objective: To determine the proportion of patients with possible cognitive dysfunction in a population of seniors ≥ 60 years undergoing elective vascular surgical procedures at the pre-anaesthetic visit, and to examine if there is a difference in cognitive functions among individual categories of vascular surgical procedures. The secondary aim was to find if such cognitive testing is feasible and acceptable for patients. Design: Prospective, observational trial. Setting: Department of vascular surgery at a teaching hospital. Materials and methods: Following ethical board approval, consecutive patients ≥ 60 years of age planned for elective vascular surgical procedures were approached between July 1st 2017 and June 30th 2018. Exclusion criteria: Documented dementia or mental retardation, serious disorders of senses, inability to use the dominant hand, language barrier. To determine the cognitive function, a short screening Clock-in-the-Box (CIB) test was used. Outcome parameter: The proportion of patients with possible cognitive dysfunction was defined by CIB score ≤ 5 (range 0-8). The significance of differences among individual categories of surgical procedures was evaluated with χ2-test, P ≤ 0.05. Results: 122 patients were approached, of whom 100 were analysed, 63 of them male, mean age 70.7 ± 7 years, education mostly lower than completed secondary, ASA 3. Possible cognitive dysfunction was identified in 34 patients, the differences between the surgical categories (aorta, arterial reconstruction on the lower extremities, carotid endarterectomies, varicose veins) were not significant. CIB test during the pre-anaesthetic visit was feasible and not a burden for patients who considered it useful and would agree to it in future, if necessary.

Pro screening demence u symptomatických osob s poruchou paměti a dalšími kognitivními příznaky je nezbytné provést podrobné objektivní zhodnocení psychických, především kognitivních a afektivních funkcí. Klinické vyšetření doplníme standardizovanými škálami. V úvodní části sdělení je podán přehled nejčastěji užívaných screeningových posuzovacích škál pro demence a mírné kognitivní poruchy a doporučený diagnostický algoritmus u nemocných s kognitivní poruchou. V druhé části sdělení je podán popis Sedmiminutového screeningového testu (7MST) a přehled výsledků validizační studie provedené u 136 geriatrických nemocných. Byla potvrzena validita testu pro screening Alzheimerovy demence (AD) se senzitivitou 95% a specificitou 93%. Pro svou jednoduchost, vysokou diskriminační schopnost a reprodukovatelnost se tento test může stát vhodným doplněním klinické praxe v diferenciální diagnostice kognitivních poruch vyššího věku.
In dementia screening of persons by memory disturbing suffering, in conection with other cognitive syndromes, an objective assesment of psychic and before all of cognitive and affective functions is important to provide. The clinical outfinding we must fulfill by standartized scales. The introduction of this article showes a brief outline of the most often used screening assesment scales for dementia diagnose and in the case of light cognitive disorders and the recomended diagnostic algorhytm in patients by cognitive disorder suffering. The second part describes the Seven-minutes screening testing (7MST) and a summary of results of the evaluation study in 136 geriatric patients provided. The validity of this testing in the case of Alzheimer disease were confirmed by sensitivity of 95 per cent and by specifity of 93 per cent. In the way of its simplicity, hight discrimination ability and repeatability could this testing be a siutable complement in the clinical practice to establish the differential diagnostics in cognitive disorders in higher age.
Rozpoznáni osob, u kterých v budoucnu může dojit k rozvoji demence, i když v přítomnosti jsou ještě asymptomatické, pomáhá připravit taková opatření, která mohou nemoc zpomalovat nebo vedou k prevenci progrese Dnemocněni u všech, kteří jsou ohroženi. Velice praktickejšou v takových případech neuropsychologické testy, aeré měří kognitivní funkce. Pomocí MMSE-testu, který je používán ke screeningovým účelům, se podaří zjistit závažnější kognitivní poruchy a v mnoha případech také rozvinuté demence. Tento test není dostatečně cidivý pro diagnózu predemencí a časných stadií demencí. Montrealský kognitivní test (Montreal Cognitive Assessment - MoCA), je jednoduchým testem použitelným v případech, kdy si pacienti stěžují na poruchy paměti, ale při použití MMSE nalézáme ještě normální skóre.
Identifying people who are likely to progress to dementias whilst they are still relatively asymptomatic will help to ensure that interventions to slow or prevent progression can be targeted on most in need. Neuropsychological assessment is the most practical approach as it measures directly cognitive function. Brief screening tests, such as MMSE, will detect more severe cognitive impairment and most cases of estabhshed dementia, but are insensitive to early pre-dementias and dementia. The Montreal Cognitive Assessment - MoCA, is a short user-friendly screening for those with memory complaints who otherwise are normal on MMSE.

Při určení hloubky kognitivní poruchy bez ohledu na její etiologii jsou platná diagnostická kritéria popisující syndrom mírné kognitivní poruchy (z anglického mild cognitive impairment MCI) a syndrom demence. V praktické diagnostice stále nacházejí svou roli screeningové metody. Jejich pomocí dokážeme s určitou pravděpodobnostní odhadnout kognitivní stav pacienta. V tomto článku bychom rádi představili zásady screeningového klinického vyšetření kognice a užitečnost vybraných nejrozšířenějších screeningových metod v České republice. Věnujeme se zásadám rozhovoru při kognitivním vyšetření a problematice subjektivních stížností na kognici. Za nejdostupnější a nejpoužívanější krátké screeningové metody v ČR považujeme Mini-Mental State Examination a Montrealský kognitivní test, pozornost věnujeme klinickému využití stále hojně v praxi zastoupeného Testu hodin a dalších krátkých kognitivních zkoušek.

When determining the degree of cognitive impairment regardless of its aetiology, the diagnostic criteria describing the syndrome of mild cognitive impairment and the dementia syndrome are applicable. Screening methods still play a role in practical diagnosis. They can be used to assess the patient's cognitive status with a certain degree of probability. This article aims at presenting the principles of a clinical screening test for cognition and the utility of selected most widely spread screening methods in the Czech Republic. It deals with the principles of cognitive testing interview and the issue of subjective cognitive complaints. The Mini-Mental State Examination and Montreal Cognitive Assessment are considered to be the most commonly used and available screening methods in the Czech Republic. Attention is also paid to the clinical use of the still widely used Clock Drawing Test and other short cognitive tests.

Kognitivní dysfunkce patří k základním příznakům deprese. Má nepochybně význam pro fungování v denním životě, negativně ovlivňuje kvalitu života, návrat do práce a práceschopnost. Nálezy ukazují na mírné postižení více kognitivních domén. Některé komponenty kognitivní dysfunkce mohou souviset se závažností symptomů deprese. Pacienti s depresivní poruchou vykazují obecně nižší celkovou kognitivní výkonnost, což předpokládá globální difuzní postižení kognice. Dle jiné hypotézy je depresivní porucha spojena s výraznější poruchou specifických kognitivních domén, nejvíce exekutivních funkcí a pamětí. Kognitivní dysfunkce se vyskytuje v akutní depresi a může přetrvávat i v remisi. Redukce příznaků u deprese není následována kognitivním zlepšením odpovídajícího stupně. Kognitivní porucha se horší s každou další epizodou a porucha v nesymptomatické fázi souvisí s řadou předchozích epizod. Kognitivní deficit je u schizofrenní, bipolární a depresivní poruchy kvalitativně podobný, avšak rozdíly jsou v závažnosti. Nejvýraznější postižení je v sestupném pořadí u schizofrenní, bipolární a depresivní poruchy. Nejčastěji užívané screeningové nástroje pro měření kognitivní dysfunkce u ostatních psychických poruch, hlavně demence, nejsou dostačující pro stanovení mírné kognitivní poruchy u deprese. Pacientovo subjektivní vnímání kognitivní dysfunkce nesouvisí vždy s výkonem v objektivních neuropsychologických testech. Dnes již byly vyvinuty kognitivní baterie specifické pro afektivní poruchy. Slibný se jeví THINC-it, screeningový nástroj zahrnující objektivní i subjektivní měření kognitivních funkcí. Je již volně dostupný a aplikovatelný pomocí tabletu nebo počítače.
Cognitive dysfunction is a core component of depressive disorder. Cognitive impairments impact on patient functioning and negatively influence quality of life, return to work and ability to perform adequately at work. The findings show a mild impairment of more cognitive domains. Some of the cognitive deficits may be associated with the severity of psychopathological symptoms of depression. Patients with depressive disorder show a lower global cognitive performance, which means a global diffusion impairment of cognition. According to another hypothesis the depressive disorder is associated with a more pronounced impairment of specific cognitive domains, mostly with impairment of executive and memory functions. Cognitive dysfunction occurs in acute depression and can persist into remission. The alleviation of affective symptoms is often not paralleled by an equal improvement of cognition. With every episode there is a decrease in cognitive function and that inter-episode cognitive function is related to the number of previous episodes. Cognitive deficits are qualitatively similar in schizophrenia and bipolar and unipolar depression, however the severity of impairment differs between these disorders, with the most severe deficits seen in patients with schizophrenia, followed by bipolar disorder, and the least severe deficits noted in individuals with depressive disorder. The mostly used screening tools for measuring cognitive dysfunction in other psychiatric disorders, especially dementia, are not sufficiently sensitive to detect cognitive dysfunction in depression. Further, patient’s subjective experience of cognitive dysfunction is not always related to performance on objective neuropsychological measures. Nowadays, cognitive batteries more specific to affective disorders have been developed. A promising screening tool is the THINC-it, which includes subjective and objective measures of cognitive function. The THINC-it is yet free downloadable and easily used by tablets and laptops.

Kognitivní funkce a jejich deficit u schizofrenie a bipolární poruchy jsou v posledních letech v centru pozornosri výzkumníků a kliniků, protože je prokázán jejich negativní dopad na funkční schopnosti pacientů. V naší pilotní studii jsme použili originální skríninkový nástroj. Stručný kognitivní test (SCOT), který jsme vyvinuli adaptací původního švédského testu. Cílem studie bylo ověřit základní psychometrické vlastnosti testu SCOT, porovnat výkon pacientů se schizofrenií a bipolární poruchou se zdravými kontrolami a korelovat SCOT skór s kognitivní subškálou PANSS (PANSS-kog) a Škálou celkového fungování (Global Assessment of Funcrioning; GAP). Do Studie bylo zařazeno celkem 30 pacientů a 25 zdravých kontrol. Výsledky prokázaly, že ve srovnání s kontrolami byl skór pacientů v testu SCOT významně nižší (p < 0,001). SCOT skór ve skupině pacientů dále významně koreloval se subškálou PANSS-kog (p = 0,009) i se škálou GAF (p = 0,017). SCOT prokázal rovněž dobrou reliabilitu (split-half reliabilita činila 0,780). Při použití skóru 14 pro hranici detekce kognitivního postižení dosáhla senzitivita, specificita, pozitivní a negativní prediktivní hodnota nejlepších parametrů. V naší pilotní studii jsme prokázali, že SCOT lze snadno administrovat v klinické praxi (testování i vyhodnocení netrvá déle než 10 minut), test vykazuje konstrukční validitu (testování paměti, exekutivních funkcí a abstrakce), konvergentní validitu (korelace s kognitivním faktorem PANSS-kog) a ekologickou validitu (korelace se škálou celkového fungování GAF), s dobrou reliabilitou. K definitivnímu zhodnocení přínosu testu SCOT je zapotřebí reprezentativní soubor pacientů se schizofrenií a bipolární afektivní poruchou, kteří budou rovněž testováni standardním neuropsychologickým vyšetřením k potvrzení přítomnosti kognitivního deficitu.
Cognitive functions and their impairment in schizophrenia and bipolar disorder have become recently the focus of attention of re searchers and clinicians. Cognitive deficit has a negative impact on patients functioning. In our pilot study, we used original screening tool, Short Cognitive Test (SCOT), developed and adapted from an earlier Swedish test. Study aims were to examine basic psychom etric properties of SCOT, to compare results of schizophrenia and bipolar patients with those obtained from healthy controls, and to correlate SCOT scores with the PANSS cognitive subscale (PANSS-cog) and Global Assessment of Functioning (GAF). Study sample con- sisted of 30 patients and 25 control subjects. The results showed that patients had significantly lower SCOT scores as compared to controls (p < 0.001). Furthermore, in the patients group SCOT score correlated significantly with the PANSS-cog (p = 0.009) and GA F (p = 0.017). SCOT also yielded good reliability (split-half reliability 0.780). Using cut-off score of 14 to delineate cognitive imp airment, sensi- tivity, specificity, positive and negative prediction values resulted in the most optimal parameters. Our pilot study confirmed that SCOT can be easily administered in the clinical setting (testing and evaluation takes up to 10 minutes), the test showed constructiv e validity (testing of memory, executive functions, and abstraction), convergent validity (correlation with the PANSS-cog), and ecological validity (correlation with GAF), with a good reliability. In order to definitely assess contribution of SCOT, a representative sample of schizophrenia and bipolar patients who will be tested also with standard neuropsychological measures to verify cognitive deficit, is needed.
Intracelulární parazit Toxoplasma gondii (TG) se řadí mezi potenciální infekční agens, která mohou hrát roli v etiologii a patogenezi některých schizofrenních onemocnění. Kognitivní dysfunkci vázanou na infekci TG lze pokládat za faktor vulnerability pro vznik některých podtypů schizofrenie. Hypotézu o souvislosti mezi TG infekcí a schizofrenií podporují především tyto nálezy: a) statisticky významně zvýšené titry protilátek proti TG U pacientů s první epizodou schizofrenie, b) pozitivní korelace mezi průkazem TG protilátek a skóry psychopatologie u prvních epizod schizofrenie, c) zvýšené kognitivní odchylky u schizofrenních nemocných se sérologickým průkazem TG infekce oproti pacientům bez známek TG infekce, d) kognitivní a osobnostní změny u neschizofrenních osob infikovaných TG a behaviorální změny na zvířecích modelech. Cílem CNS projektu je vyšetřit TG infikovanou a neinfikovanou podskupinu schizofrenních pacientů denního stacionáře s ohledem na: 1) odlišnou závažnost jejich symptomů a kognitivního výkonu, 2) odchylný vývoj jejich kognitivního profilu v průběhu jednoho roku, 3) jejich odlišnou schopnost účastnit se a těžit ze strukturovaného stacionárního programu pro onemocnění schizofrenního spektra. Klinický význam projektu CNS spočívá v replikaci předchozích studií postnatální latentní toxoplazmózy jako faktoru modifikujícího schizofrenní onemocnění, který by vyústil v aktivnější preventivní opatření, přiměřenou kognitivní rehabilitaci u schizofrenních pacientů infikovaných TG a začlenění protilátek proti TG do vyšetřovacích protokolů u populace vysoce ohrožené rozvojem schizofrenie.
Intracellular parasite Toxoplasma gondii (TG) is considered to be an infectious agent, which plays a role in the aetiology and pathogenesis of some cases of shizophrenia. TG infection-related cognitive dysfunction is considered as an element of vulnerability for the development of some schizophrenia subtypes. The hypothesis that links TG infection and schizophrenia is supported above all by these findings: a) a statisti- cally significant increase of antibody titres to TG in first-episode schizophrenia patients, b) a positive correlation of the a ntibodies to TG and psychopathology scores in first-episode schizophrenia, c) an increased level of cognitive abnormalities in schizophrenia patien ts with serologic evidence of TG infection compared to the patients without signs of TG infection, and d) cognitive and personality changes in TG infected non- -schizophrenic subjects and behavioural changes in animal models. The objectives of the CNS project are to investigate TG infec ted and non- -infected subgroups of outpatients with schizophrenia spectrum disorders — participants of the structured day-care programme — with respect to 1) the differences in their symptom severity and cognitive performance, 2) the development of a different cognitive profile in the course of one year follow-up, and 3) a different capacity to participate in and benefit from a structured day-care programme for schizoph renia spectrum disorders. Clinical implications of the CNS project could be a replication of previous studies of postnatal latent toxoplasmosi s as a disease- -modifying factor in schizophrenia, resulting in more assertive preventive measures, more appropriate cognitive rehabilitation of TG infected schizophrenia patients and more inclusion of TG antibodies in screening batteries of high-risk schizophrenia population.
