INTRODUCTION: The target of this study was to compare thromboelastography coagulation parameters in pregnant and non-pregnant women. If appropriate, we would propose recommendations for new reference ranges for pregnant women in their third trimester. MATERIALS AND METHODS: Prospective observational study, comparing, by using thromboelastography, the blood samples of 60 healthy women in third trimester of pregnancy (group GRAV) to the samples of the control group of 43 healthy non-pregnant fertile women (group NON-GRAV). Selective percentiles were used to determine new reference limits. RESULTS: Mean values and standard deviations (SD) in both groups were as follows (GRAV vs NON-GRAV): time r 4.7 min (SD 1.7) vs. 7.8 (SD 2.8); time k1.5 min (SD 0.5) vs. 2,7 (SD 0,9); alpha angle 69.6° (SD 5.5) vs. 54.4 (SD 11.5); maximum amplitude 71.3mm (SD 4.5) vs. 63.1 (SD 5.4); coagulation index 2.7 (SD 1.8) vs. -1.9 (SD 3.0); LY60 1.1% (SD 1.8) vs. 4.8 (SD 3.6). Due to statistically significant differences between both groups, we established, based on our results, these new thromboelastography reference limits for pregnant women: time r 2-8 min ("common" range 4-8 min), time k 1-3 min ("common" range 1-4 min), alpha angle 60-77° ("common" range 47-74°), maximum amplitude 64-76 mm ("common" range 55-73 mm), LY60 0-3% ("common" range 0-15%), coagulation index 0-5 ("common" range (-3) - (+3)). CONCLUSIONS: It may not be suitable to use the same reference ranges for pregnant women as for the general population. Therefore, we suggest new reference limits for thromboelastography in pregnant women.

BACKGROUND: Normative reference values for echocardiographic chamber quantification are of great importance; however, this can be challenging. Our aim was to derive these values including degrees of abnormality from a random Central European population sample with a homogeneous subset of healthy subjects. METHODS: We analysed echocardiograms obtained in a randomly selected population sample during the Czech post-MONICA survey in 2007/2008. Overall, 1850 out of 2273 persons of the whole sample of three districts had adequate echocardiograms (81.4%). A healthy subgroup defined by the absence of known cardiovascular disease was used to define normal reference range limits (n = 575, median age 42 years [IQR 34-52], 57% females). The whole population sample with predefined percentile cut-offs was used to define degrees of abnormality. RESULTS: Left ventricular (LV) size tended to decrease with age, while LV mass increased with age in both males and females and in both the healthy and general populations. LV dimensions were larger in males, except for body surface area-indexed LV diameter. M-mode derived LV measurements were larger and LV mass higher compared to 2D measurements. Right ventricle basal dimension was larger in males. CONCLUSIONS: Our study provides reference ranges for echocardiographic measurements obtained in a healthy subgroup derived from an epidemiological study of a Central European population. Where feasible, degrees of abnormality are provided based on the whole population sample including patients with disease. Our data show that age, gender and measurement method significantly affect cardiac dimensions and function and should be always taken into account.

BACKGROUND: The Enhanced Liver Fibrosis score has been recognized as a non-invasive test for liver fibrosis. However, reference intervals, biological variation and analytical performance have not been studied in detail so far. The aim was to acquire data that are essential for correct interpretation. METHODS: A total of 40 apparently healthy volunteers were evaluated for reference ranges of serum concentration of hyaluronic acid, aminoterminal propeptide of type III collagen, and tissue inhibitor of metalloproteases-1, and calculated ELF score. A subgroup of 20 subjects was evaluated repeatedly for 7 weeks. For all variables, reference intervals, within-subject and between-subject biological variations, reference change values, and the indexes of individuality were assessed. Analytical performance (intermediate precision) and interlaboratory comparison were also evaluated. RESULTS: The reference ranges were 5.1-62.7 μg/L for HA, 3.56-12.6 μg/L for PIIINP, 143.6-265.3 μg/L for TIMP-1, and 7.14-9.55 for the ELF score. The within-subject variations were 32.7, 10.6, 4.2, and 3.2% for HA, PIIINP, TIMP-1, and ELF score, respectively. Similarly, the between-subject variations were 59.0, 13.3, 12.8, and 5.2%. For the ELF score, RCV was 10.1% and II was 0.62. The intermediate precisions were <5%, <6%, and <10% for HA, PIIINP, and TIMP-1, respectively. CONCLUSION: The reference range of the ELF score overlap with the area defined as moderate fibrosis by the manufacturer. High biological variation of HA was diminished by the natural logarithm in the calculation of the ELF score. The use of the ELF score has suitable analytical and acceptable biological performance characteristics for clinical practice. However, the transfer of results evaluated in healthy persons to the populations with chronic liver diseases deserves caution.
