Hodgkinův lymfom (HL) je ve většině případů vyléčen první linií terapie. Relapsy se vyskytují u 10–15 % pacientů v časných stadiích s nepříznivými prognostickými faktory a u 20 % pacientů s pokročilým HL. Při prvním relapsu je nejdůležitějším prognostickým faktorem odpověď lymfomu na záchrannou léčbu (chemosenzitivita). V léčbě se nejčastěji používají režimy na bázi cisplatiny nebo karboplatiny. Autologní transplantace periferních kmenových buněk (ASCT) dosahuje lepších výsledků ve srovnání s konvenční chemoterapií a je standardním léčebným postupem. Tandemová ASCT je určena pro relabující/refrakterní pacienty s vysokým rizikem selhání druhé linie léčby. Přesto asi u 50 % pacientů léčba ASCT selhává. Standardní strategie léčby relapsu po ASCT není jednoznačně určena. Experimentálně se zkoušejí nové léky. Za experimentální postup se považuje i druhá ASCT. Alogenní transplantace s myeloablativním režimem u opakovaných relapsů HL má vysokou potransplantační úmrtnost. Alogenní transplantace užívající režimy s redukovanou intenzitou snížily potransplantační úmrtnost, ale ani tato léčba nezabrání progresi a/nebo relapsu u poloviny až 2/3 pacientů.
Hodgkin lymphoma (HL) is usually cured by first-line therapy: in patients in initial stages with unfavorable features relapses occur in 10-15 % of cases and in up to 20 % of patients in advanced stages of HL. The most important prognostic factor at first relapse is chemosensitivity to salvage therapy. No standard salvage regimen is currently recommended, but cisplatinum or carboplatinum based regimens are most widely used. Autologous stem cell transplantation (ASCT) is superior over conventional chemotherapy and remains the standard of treatment in relapsed/refractory Hodgkin lymphoma patients. Tandem transplantation is indicated in poor-risk patients at relapse. ASCT failures occur in approximately 50% of these patients during the follow-up. Experimental approaches after ASCT failure include new agents. The second ASCT is another experimental method. The role of myeloablative allogeneic SCT in multiple relapses of HL is still unclear due to its high treatment-related mortality. Allogeneic transplantation with reduced intensity regimens reduced transplant-related mortality, but even this therapy did not prevent relapses/progressions in about a half or two-thirds of patients.

Standardem léčby u mladších pacientů s relabovaným nebo refrakterním Hodgkinovým lymfomem po 1. linii léčby je nadále vysokodávkovaná chemoterapie s autologní transplantací kmenových buněk. Z prognostických faktorů v relapsu je nejvýznamnější klinické stadium IV, krátká doba do relapsu po 1. linii léčby, špatný stav tělesné kondice, velká nádorová masa a nedosažení alespoň parciální remise po záchranné chemoterapii. Dosud není definován standard záchranné chemoterapie před autologní transplantací. Nejčastěji používané režimy jsou DHAP (dexametazon, cytarabin, cisplatina) a ICE (ifosfamid, karboplatina, etoposid). Standardním přípravným režimem před autologní transplantací je BEAM (BCNU – karmustin, etoposid, cytarabin, melfalan). U vysoce rizikových pacientů se zkouší podání tandemové autologní transplantace. V konsolidaci po autologní transplantaci u pacientů s vysokým rizikem relapsu je doporučeno podání brentuximab vedotinu. V relapsu po autologní transplantaci je standardem podání brentuximab vedotinu a je vhodné zvážit následnou alogenní transplantaci kmenových buněk u mladých pacientů. Brentuximab vedotin v kombinaci s bendamustinem, nivolumab a pembrolizumab a jejich kombinace s dalšími léky se nadále zkoumají v rámci studií u pacientů s relabovaným nebo refrakterním Hodgkinovým lymfomem.
High-dose chemotherapy with autologous stem cell transplantation remains the current standard of treatment for young patients with Hodgkin lymphoma in first relapse or in those who are refractory to first-line treatment. The most important prognostic factors in relapses are clinical stage IV, poor performance status, bulky mass, and less than partial remission after salvage chemotherapy. Standard salvage chemotherapy in relapse before autologous transplantation has not been defined; however, DHAP and ICE are most frequently used in this setting. A standard conditioning regimen before autologous transplantation is BEAM. Tandem autologous transplantation has been investigated in high-risk patients. Brentuximab vedotin is recommended as a consolidation treatment in patients with a high risk of relapse after autologous transplantation. Brentuximab vedotin is the standard of treatment for relapse after autologous transplantation, and subsequent allogeneic stem cell transplantation should be considered in young patients. Bretuximab vedotin in combination with bendamustine, nivolumab, and pembrolizumab, and combinations thereof with other drugs, were investigated in clinical trials in relapsed or refractory patients with Hodgkin lymphoma.

Přestože v terapii lymfomů dosahujeme v posledních letech pokroku, stále zůstává nezanedbatelné procento nemocných s refrakterním nebo relabujícím onemocněním. Záchranná intenzivní chemoterapie s podporou autologní transplantace je léčebný standard u řady těchto pacientů. V intenzivním výzkumu jsou také alogenní transplantace krvetvorných buněk, především po přípravě redukované intenzity. Předkládaný přehledný článek shrnuje současné poznatky a doporučení k záchranné léčbě u pacientů s nejčastěji se vyskytujícími lymfomy.
Despite achieving promising treatment results in patients with lymphoma, there is still a significant proportion of patients who relapse or have refractory disease. Salvage therapy followed by high dose treatment with autologous stem‑cell transplantation is the standard of care in many of them. The role allogeneic stem‑cell transplantation, especially after reduced intensity conditioning, is under extensive investigation. This review article presents current knowledge and recommendation in the salvage treatment of relapsed/refractory lymphomas. A LC-MS/MS method was developed and validated for the determination of cyclosporine A (CsA) and its three phase 1 metabolites AM1, AM9, and AM4N in whole blood and lymphocytes isolated on the Histopaque gradient. 200 microL of whole blood was precipitated with 10 mol/L zinc sulfate in acetonitrile/methanol (40:60, v/v) and lymphocytes isolated from 1.5 mL blood were extracted with acetonitrile/methanol (40:60, v/v). The analytes and internal standard cyclosporine D were separated on RP column BEH C18, 2.1 x 50 mm, 1.7 microm using gradient LC-MS/MS analysis in positive electrospray mode. Time of analysis was 5 min. Linearity in blood was 5-2000 microg/L for CsA, AM1, and AM9; 2-500 microg/L for AM4N; and 2-500 microg/L for all substances in lymphocytes. Coefficient of variations was 1.8-9.8% and recovery was 92.0-110.0%. The method was used in early and chronic renal transplant patients for therapeutic drug monitoring of CsA to compare either its share in lymphocytes as target organ or binding to one lymphocyte. The same parameters were calculated for all metabolites tested.
Mnohopočetný myelóm je charakterizovaný vysokým stupňom rezistencie voči konvenčnej chemoterapii, čo vedie k nízkemu počtu dosiahnutých kompletných remisií a limitovanému prežívaniu. V súčasnosti sa za štandardný terapeutický postup považuje aplikácia vysokodávkovanej chemoterapie nasledovaná autológnou transplantáciou krvotvorných buniek. Uvedenou liečbou možno dosiahnuť vyšší počet kompletných remisií (25 − 50 %) ako konvenčnou terapiou. Cieľom zlepšenia výsledkov liečby mnohopočetného myelómu je skvalitnenie transplantačných postupov (tandemová transplantácia, „double“ transplantácia), ako aj inkorporácia nových liekov (thalidomid, lenalidomid, bortezomib), tak do indukčnej, ako aj udržiavacej terapie.
Multiple myeloma is characterized by a high degree of resistance to conventional therapy, which results in a low complete remission rate and in limited survival. The fact that high-dose therapy (HDT) can overcome tumor resistance resulting in a complete remission rate ranging from 25 to 50 %, has prompted the use of HDT followed by autologous stem cell rescue as part of the front-line therapy during the last 15 years. The important issue concerning autologous transplantation is how to improve its efficacy. One way would be a tandem approach (double autologous or autologous followed by dose- reduced intensity allogeneis), the other way would consist of the incorporation of novel agents with new mechanisms of action such as thalidomide, lenalidomide and bortezomib.

Graft-versus-host disease (GvHD) contributes significantly to morbidity and mortality after allogeneic hematopoetic stem cell transplantation. Diagnosis is currently based on clinical features and tissue biopsies. GvHD is generally diagnosed at advancedstages and management of GvHD is then labour-intensive or impossible. Aim of this project is to characterize the genome and proteome alterations linked with GvHD using current diagnostic, prognostic and treatment procedures.
Reakce štěpu proti hostiteli (GvHD) je závažnou komplikací po alogenní transplantaci krvetvorných buněk. V současné klinické praxi je GvHD diagnostikována pouze na základě klinického obrazu, resp. histologického a biochemického vyšetření, v mnoha případech až v době, kdy je GvHD v pokročilém stádiu a její zvládnutí je komplikované či nemožné. Cílem tohoto projektu je charakterizovat změny genomu a proteomu související s GvHD pomocí nejnovějších genomických (cDNA čipy) a proteomických (hmotnostní spektrometrie, proteinové čipy) technik. Tyto poznatky by měly umožnit navržení nových diagnostických, prognostických a léčebných postupů.

Integral membrane proteins are under-represented in standard proteomic analyses, mostly because of their low expression and absence of trypsin-cleavage sites in their hydrophobic transmembrane segments. Novel and effective strategies for membrane proteomic analysis aim at soluble N-glycosylated segments of integral membrane proteins (CSC, SPEG, N-glyco-FASP) or selectively target the hydrophobic transmembrane alpha-helical segments employing chemical peptide cleavage by CNBr (hpTC). We combined a solid phase enrichment of glycopeptides (SPEG) with a transmembrane segment-oriented hpTC method and a standard "detergent and trypsin" approach into a three-pronged "Pitchfork" strategy to maximize the membrane proteome coverage in human lymphoma cells. This strategy enabled the identification of >1200 integral membrane proteins from all cellular compartments, including 105 CD antigens, 24 G protein-coupled receptors, and 141 solute carrier transporters. The advantage of the combination lies in the complementarity of the methods. SPEG and hpTC target different sets of membrane proteins. HpTC provided identifications of proteins and peptides with significantly higher hydrophobicity compared to SPEG and detergent-trypsin approaches. Among all identified proteins, we observed 32 so-called "missing proteins". The Pitchfork strategy presented here is universally applicable and enables deep and fast description of membrane proteomes in only 3 LC-MS/MS runs per replicate. SIGNIFICANCE: Integral membrane proteins (IMPs) are encoded by roughly a quarter of human coding genes. Their functions and their specific localization makes IMPs highly attractive drug targets. In fact, roughly half of the currently approved drugs in medicine target IMPs. Our knowledge of membrane proteomes is, however, limited. We present a new strategy for the membrane proteome analysis that combines three complementary methods targeting different features of IMPs. Using the combined strategy, we identified over 1200 IMPs in human lymphoma tissue from all sub-cellular compartments in only 3 LC-MS/MS runs per replicate. The three-pronged "Pitchfork" strategy is universally applicable, and offers a fast way toward a reasonably concise description of membrane proteomes in multiple samples.

Východisko. Hodkinův lymfom patří mezi kurabilní onemocnění s vysokým procentem úspěšnosti. U nemocných, u nichž dojde k selhání léčby, je možné použít záchrannou terapii s vysokodávkovanou terapií (HDT) s autologní transplantací krvetvorných buněk (ASCT). Metody a výsledky. Na I. interní klinice VFN bylo tímto způsobem léčeno od roku 1994 do roku 2005 84 nemocných s věkovým mediánem 30,5 roku, u nichž bylo provedeno celkem 105 HDT s ASCT. Nemocní byli indikováni z důvodu nedosažení kompletní remise po prvé linii terapie (n 16) nebo pro relaps a progresi v následném průběhu choroby (n 68). Po záchranné léčbě bylo v době transplantace onemocnění hodnoceno jako chemosenzitivní u 65 nemocných a jako chemorezistentní u 19. Nejčastějším režimem použitým v léčbě byla chemoterapie BEAM v 82 případech, 22 nemocných bylo léčeno v programu tandemové transplantace. Jako zdroj krvetvorných buněk byla použita kostní dřeň u 4 ASCT, samotné periferní progenitorové buňky u 85 ASCT a kombinace kostní dřeně a periferních progenitorových buněk u 16 ASCT. Ze 77 hodnotitelných nemocných byl stav po transplantaci hodnocen jako kompletní remise u 39 nemocných (50,6 %), parciální remise u 31 nemocných (40,3 %) a jako stabilní onemocnění nebo progrese u 7 nemocných (9,1 %). Peritransplantační mortalita byla u nemocných transplantovaných pomocí periferních krvetvorných buněk 3,9 %. Při mediánu sledování 5,3 roku byla pravděpodobnost pětiletého přežití bez významné události (EFS) 43,1 % a celkového přežití (OS) 53,2 %. Ve skupině chemosenzitivních nemocných byla pravděpodobnost EFS 48,6 % a OS 62,9 %. Stav při transplantaci a dosažený výsledek měly signifikantní prognostický význam. Z 39 pozorovaných úmrtí jich bylo 26 zapříčiněno progresí HL, u 5 nemocných byl zjištěn sekundární nádor. Závěry. Vysokodávkovaná terapie s autologní transplantací krvetvorných buněk u HL je metodou umožňující dlouhodobé přežití bez další progrese onemocnění téměř u poloviny indikovaných nemocných. Nejlepší výsledky jsou dosaženy u nemocných s chemosenzitivním onemocněním.
Backround. Majority of patients with Hodgkin's Lymphoma (HL) can be cured by first line therapy. The high dose therapy (HDT) with autologous stem cell transplantation (ASCT) is the option which can be used in the situation when the conventional therapy failed. Methods and Results. Beginning 1994 till 2005 84 pts with HL who did not respond the conventional chemotherapy underwent 105 HDT procedures with ASCT. The median age at the time of HDT was 30.5 years. The reason for salvage therapy followed by HDT with ASCT was the failure to achieve 1st complete remission - CR (n 16) or the subsequent relapse or progression (n 68). The disease status at the time of HDT after conventional salvage chemotherapy was assessed as chemosensitive in 65 pts (77.4 %) and chemoresistant in 19 pts (22.6 %). The most frequent HDT regimen used was BEAM (82 HDT), 22 pts entered into the tandem HDT program. Bone marrow only was used as the source of progenitor cells in 4 ASCT, peripheral blood progenitor cells (PBPC) only were used in 85 ASCT and the combination of both in 16 ASCT. The disease status after the HDT with ASCT was assessed (77 pts were qualifiable) as CR in 39 pts (50.6 %), PR in 31 (40.3%) and as stable disease or progression in 7 pts (9.1 %). Treatment related mortality in HDT with PBPC was 3.9 %. The median follow up is 5.3 years. The five year probability of event free survival (EFS) is 43.1 % and overall survival 53.2 %. The EFS and OS probability respectively for the chemosensitive patients was 48.6 % and 62.9 % respectively. The status at HDT and the results after it have prognostic significance. There were observed 39 deaths and 26 of them were caused by disease progression. Secondary tumor was observed in 5 pts and in all of them it caused the death. Conclusions. The HDT with ASCT allows the long term survival without disease progression in about a half of the patients with reasonable toxicity.

Cíl: Cílem práce bylo zhodnotit toleranci a vnímání léčby vysokodávkovanou chemoterapií s autologní transplantací periferních krvetvorných buněk (PBSC) a tandemovou transplantací u pacientů s mnohočetným myelomem v období hospitalizace od přijetí až do propuštění a krátce po propuštění do domácího ošetřování. Metodika: Data byla sbírána formou dvou dotazníků vytvořených pro toto průzkumné šetření na pracovišti Kliniky hematoonkologie Fakultní nemocnice Ostrava. První soubor tvořila skupina pacientů, kteří podstoupili vysokodávkovanou chemoterapii s autologní transplantací PBSC. Druhou skupinu tvořili pacienti, kteří podstoupili také tandemovou transplantaci. Kritériem výběru byla léčba první linie mnohočetného myelomu, zakončená vysokodávkovanou chemoterapií s autologní transplantací PBSC, provedena ve Fakultní nemocnici Ostrava. Výsledky: Šetřením bylo zjištěno, že dobrý psychický stav má pozitivní vliv na průběh a prožívání procesu transplantace krvetvornými buňkami. Pouze 13 % pacientů se vyjádřilo, že měli psychické potíže jako strach a úzkost z toho, co přijde, což taky ovlivnilo jejich ochotu podstoupit proces znovu, případně tento postup doporučit. Mezi největší potíže označili v pořadí nechutenství, zažívací potíže, psychické strádání a horečku. Závěr: Plná informovanost pacienta o celém procesu a včasná preventivní opatření vedou ke zmírnění nežádoucích účinků vysokodávkované chemoterapie a zlepšení celkové psychické pohody. Při indikaci procesu autologní transplantace krvetvorných buněk je nezbytné zhodnotit nejen fyzický stav, ale také psychickou stránku pacienta, která může výrazně ovlivnit vnímání celého procesu.
Goal: The main goal of the thesis was to evaluate tolerance and perception of treatment by high-dose chemotherapy in combination with peripheral blood stem cell autologous transplantation and tandem transplantation in patiens with multiple myeloma in the period from hospital admission to release and the next days at home. Methodology: The data was collected by two questionaires created specifically for this research on Department of Haematooncology, University Hospital Ostrava. The first group contains patients who were treated by high-dose chemotherapy in combination with peripheral blood stem cell autologous transplantation. The second group contains these patients who were treated by tandem transplantation. Inclusion criteria included patients with multiple myeloma treated by the first line of chemotherapy followed by autologous transplantation of PBSC in University Hospital Ostrava. Results: Good psychological condition has positive effect for the process and experiencing of hematopoietic stem cell transplantation. Only 13 % of patients said they had a fear and anxiety about what was coming, which also affected their willingness to repeat the process or recommend this procedure. The biggest problems of patients contained loss of appetite, digestive problems, psychological distress and fever. Conclusion: Full patient awareness of the entire process and timely preventive measures go along with alleviation the adverse effect of high dose chemotherapy and improve overall mental well- -being. It is necessary to evaluate not only the physical condition but also the mental aspect of the patient, which can significantly affect the perception of the process.
