During standard molecular diagnostic procedure, two Czech families with APC (Adenomatous polyposis coli gene) mosaicism have been detected. A woman with attenuated familial adenomatous polyposis (AFAP, OMIM #175100) was recently inspected by next generation sequencing. Standard bioinformatics pipeline, restricted to variants with at least 20% of reads (for germline variants) would miss mutation p.G1412X (NM_000038.5) present in 17% of reads. This novel variant was not present in any of her two children. Another woman with a clinical manifestation of attenuated FAP was tested 16 years ago without conclusive APC mutation found when denaturing gradient gel electrophoresis (DGGE), protein truncation test (PTT), multiplex ligation probe amplification (MLPA) and direct Sanger sequencing were applied. Recent inspection of her son showed clear mutation p.Q1062X (NM_000038.5, NP_000029.2) leading to premature stop codon. This finding led to re-evaluation of this protein position in his mother and detection of mosaicism (11% of allele, 22% of heterozygous cells in blood), which was primarily overlooked. Mutations in both patients were confirmed by allele-specific real time PCR (AS qPCR). In both index patients it was possible to detect and quantify the mosaic allele in biological samples of polyps, adjacent colonic mucosa and buccal swabs. In cases of sporadic appearance of FAP, besides blood we plan to preferably inspect also other samples, where mosaic fraction might be under detection limit of bioinformatics pipelines (<3%). For our future routine NGS sequencing analysis we will apply our in-house somatic variant detection pipeline to minimize the false negative calls when genes with high level of de-novo mutations are analyzed.

Gitelmanův syndrom (GS) je autosomálně recesivní nemocnění, které se vyznačuje hypokalemickou metabolickou alkalózou s přítomností hypokalciurie a hypomagnezemie. Patří mezi nejčastější vrozené „salt-wasting“ tubulopatie, u nichž je primární porucha funkce Na+-Cl- kotransportéru (NCCT) v distálním stočeném kanálku a hypokalemie je sekundární. Hypomagnezemie je způsobena poruchou zpětné reabsorpce hořčíku TRPM6 kanálem, který je lokalizován v těsné blízkosti NCCT. Klinicky trpí pacienti zejména únavou a hypotenzí způsobenou ztrátami soli a vody, dále křečemi a tetanií. U některých nemocných lze prokázat chondrokalcinózu, která vede k vleklým bolestem a opakovaným aseptickým zánětům v kloubech. Průběh onemocnění nicméně bývá benigní a zřídka vede ke strukturálním změnám ledvin či k poruše renální funkce. V letech 2004–2006 jsme v ČR v rámci grantového projektu zahájili vyšetřování nemocných se suspekcí na GS dle klinických a laboratorních nálezů a v následujících letech se tato metodika zavedla do běžné laboratorní praxe. Do roku 2011 jsme identifikovali v české populaci celkem 7 různých kauzálních mutací v genu SLC12A3 (z toho 4 nové), který je zodpovědný za vznik tohoto onemocnění. U většiny nemocných, u nichž svědčily klinické nálezy pro možnost přítomnosti GS, mutace nalezena byla, a to v heterozygotní formě; 4 jedinci pak byli homozygoti. Převaha nalezených mutací byly missense mutace a nejčastějším typem v české populaci byla záměna 1315 G>A v genu SLC12A3, která způsobuje poruchu glykozylace NCCT transportéru. Dále byla nalezena celá řada jednonukleotidových polymorfizmů, které se mohou podílet na klinické manifestaci onemocnění.
The Gitelman syndrome (GS) is an autosomal recessive disorder characterized by hypokalemic metabolic alkalosis and presence of hypocalciuria and hypomagnesemia. It is one of the most common congenital “salt-wasting” tubulopathies, where the impairment of function of the Na+-Cl- cotransporter (NCCT) in the distal convoluted tubule is primary and hypokalemia secondary. Hypomagnesemia is caused by the impairment of magnesium reabsorption through TRPM6 channel which is located just by NCCT. Clinically, patients suffer from fatigue and hypotension due to loss of salt and water and also have cramps and tetany. In some patients chondrocalcinosis can be identified which leads to protracted pain and repeated aseptic inflammations in the joints. The course of the disease, though, is typically benign, and it rarely leads to structural changes in the kidneys or renal impairment. In the period of 2004–2006 we commenced examination of patients with suspected GS based on clinical and laboratory findings within a grant project in the Czech Republic, and in the following years this methodology was introduced to the common laboratory practice. By the year 2011 we had identified 7 different causal mutations in the gene SLC12A3 (4 of them new) among the Czech population, which is responsible for the origin of this disease. The majority of patients, whose clinical findings indicated the presence of GS, had the mutation actually detected, specifically in heterozygous form; 4 individuals were then homozygous. Most of the identified mutations were missense mutations and the most common type found among the Czech population was the change 1315 G>A within the geneSLC12A3, which causes impairment of glycosylation of the NCCT transporter. Further a great number of single-nucleotide polymorphisms were found that may be involved in clinical manifestation of the disease.

Cíl: Cílem této studie bylo ověřit použitelnost poměru expresí genů NPHS2 a SYNPO při diagnostice fokální segmentální glomerulosklerózy (FSGS) a minimálních změn glomerulů (MCD) u českých pacientů a zároveň identifikovat nové markery, které by mohly vést k jednoznačnému rozlišení mezi oběma diagnózami. Typ studie: Pilotní studie Materiál a metody: Do studie bylo zařazeno 24 vzorků od pacientů s FSGS a MCD. Exprese genů byla stanovena metodou relativní kvantitativní real-time PCR provedené na kartách TaqMan ® Array Micro Fluidic Cards s následnou normalizací dat k referenčnímu genu GAPDH . Výsledky a závěry: Nebyly prokázány žádné statisticky významné rozdíly v expresi studovaných genů mezi pacienty s FSGS a MCD. Zároveň se nepodařilo potvrdit použitelnost poměru expresí genů NPHS2 a SYNPO pro rozlišení FSGS a MCD u českých pacientů.
Objective: The aim of this study was the verification of the applicability of NPHS2 / SYNPO ratio for diagnosis of focal seg - mental glomerulosclerosis (FSGS) and minimal change disease (MCD) in Czech patients and identification of new markers for distinguishing between MCD and FSGS. Material and methods: Our study was performed on 24 samples from patients with FSGS and MCD. Gene expressions were determined using relative quantitative real-time PCR and TaqMan ® Array Micro Fluidic Cards and normalized to the reference gene GAPDH . Results and conclusion: Our results did not find any significant difference in the podocin/synaptopodin ratio and in any other studied genes between FSGS and MCD.
BACKGROUND: Autosomal recessive polycystic kidney disease (ARPKD) is an early-onset form of polycystic kidney disease that often leads to devastating outcomes for patients. ARPKD is caused by mutations in the PKHD1 gene, an extensive gene that encodes for the ciliary protein fibrocystin/polyductin. Next-generation sequencing is presently the best option for molecular diagnosis of ARPKD. Our aim was to set up the first study of ARPKD patients from the Czech Republic, to determine the composition of their mutations and genotype-phenotype correlations, along with establishment of next-generation sequencing of the PKHD1 gene that could be used for the diagnosis of ARPKD patients. METHODS: Mutational analysis of the PKHD1 gene was performed in 24 families using the amplicon-based next-generation sequencing (NGS) technique. In patients without 2 causal mutations identified by NGS, subsequent MLPA analysis of the PKHD1 gene was carried out. RESULTS: Two underlying mutations were detected in 54% of families (n = 13), one mutation in 13% of families (n = 3), and in 33% of families (n = 8) no mutation could be detected. Overall, seventeen different mutations (5 novel) were detected, including deletion of one exon. The detection rate in our study reached 60% in the entire cohort of patients; but 90% in the group of patients who fulfilled all clinical criteria of ARPKD, and 42% in the group of patients with unknown kidney pathology. The most frequent mutation was T36M, accounting for nearly 21% of all identified mutations. CONCLUSIONS: Next-generation sequencing of the PKHD1 gene is a very useful method of molecular diagnosis in patients with a full clinical picture of ARPKD, and it has a high detection rate. Furthermore, its relatively low costs and rapidity allow the molecular genetic analysis of patients without the full clinical criteria of ARPKD, who might also have mutations in the PKHD1 gene.
