PURPOSE OF THE STUDY The purpose of this study is to remind of the specific features of diagnosis and therapy of dorsal dislocations of the glenohumeral joint to prevent frequent neglect or misdiagnosis during primary treatment. Also, we strived to present our experience and outcomes of posterior dislocation treatment by McLaughlin procedure and its modified version. MATERIAL AND METHODS We retrospectively evaluated 7 patients who had been treated at our department in the period 2015-2019 by McLaughlin procedure or its modification. The group consisted of 5 men and 2 women aged 33-79 years. In five patients, the dislocation occurred during an epileptic seizure, in two patients during an accident. The patients were treated by McLaughlin procedure or the Neer modification of McLaughlin procedure. The deltoideopectoral surgical approach was always used. Based on the preoperative planning, we decided, or intraoperatively changed our decision, as to whether mere transposition of the subscapularis is satisfactory or whether transposition of the lesser tuberosity to the defect, a bone graft and arthroplasty are necessary. Postoperatively, the operated limb was fixed for 6 weeks in neutral position, which was followed by rehabilitation. RESULTS We evaluated subjective and objective outcomes of the surgery, the radiological finding and in 2 patients also a control CT scan. The function of the shoulder was assessed using the Constant shoulder score and the OSIS scoring system. The resulting score: the mean OSIS was 55.7 points (92.8%) and the mean Constant score was 86.2 points (86.2%), which we regard as a very good result. All patients consider the surgery successful and are satisfied with the outcome. No patient reported recurrent dislocation. DISCUSSION When treating the posterior dislocation of the shoulder, vital is the shoulder reducibility and also the presence or the size of reverse Hill-Sachs defect. The transfer of the subscapularis tendon can be performed as an open procedure or arthroscopically. Inveterate irreducible posterior dislocation of the shoulder is quite a rare diagnosis, which is why even at our department the McLauglin procedure is performed in the order of units per year only. CONCLUSIONS The surgical solution of inveterate posterior dislocation of the shoulder is hardly ever uncomplicated. The reduction alone is often insufficient and some other way of shoulder stabilisation is necessary, often times addressing the humeral head defect. In our to date experience, the McLaughlin procedure or the Neer s modification is an elegant and safe method to treat posterior dislocation with a humeral head defect. The functional outcomes are very good and enable the patients to reengage in everyday activities. Thanks to this procedure shoulder joint arthroplasty can be avoided in younger patients. Nonetheless, in defects that are greater than 50% of the head the replacement is necessary. Due to frequently associated rotator cuff injuries, the most often procedure indicated by us is the reverse total shoulder replacement. Key words: posterior dislocation of the shoulder, reverse Hill-Sachs defect, McLaughlin procedure, Neer s modification.

PURPOSE OF THE STUDY This study aims to evaluate the results of patients treated by proximal row carpectomy at a follow-up of at least 5 years after the surgery. MATERIAL AND METHODS A total of 25 patients were treated by proximal row carpectomy for degenerative changes of the wrist as a consequence of the previous trauma or avascular necrosis of the lunate bone, of whom 21 patients underwent a follow-up examination at least 5 years postoperatively. Proximal row carpectomy was indicated in 15 patients for SLAC wrist, in 4 patients for avascular necrosis of the lunate bone, in 1 patient for SNAC wrist, and in 1 patient for inveterate dislocation of the wrist. The follow-up clinical and radiological examinations were performed at least 5 years after the surgery. The range of wrist motion, grip strength, presence of pain at rest or pain under loads, total clinical score according to the Green and O'Brien scoring system, patient satisfaction with the outcome of surgery were assessed. By fluoroscopy the range of wrist motion, degenerative changes of the radiocapitate joint, and translation of the capitate bone with respect to the distal radius were evaluated. RESULTS Postoperative improvements in the range of motion and grip strength were confirmed. Also, the pain relief at rest and under loads was achieved. Five years after the surgery, most of the patients (85.6%) reported an overall improvement. The total clinical score according to the Green and O'Brien scoring system improved from 35.8 preoperatively to 63.1 postoperatively. DISCUSSION The advantage of this procedure is a low percentage of complications, relative technical simplicity, maintenance of functional motion of the wrist, satisfactory grip strength, and pain relief. There are no complications related to the implant, no risk of a non-union. CONCLUSIONS As shown by our results obtained 5 years after the surgery as well as the published data, in the indicated cases the proximal row carpectomy is an appropriate surgical technique to treat the degenerative changes of the wrist. In most of the patients, favourable functional results and pain relief can be expected. Key words:proximal row carpectomy, SLAC wrist, SNAC wrist, avascular necrosis of the lunate bone, dislocation of the wrist.

Cíl. Cílem studie bylo zjistit kvalitu života pacientů před a po implantaci totální endoprotézy kolenního nebo kyčelního kloubu pomocí vybraných kódů Mezinárodní klasifikace funkčních schopností, disability a zdraví. Pacienti a metodika. Z faktorů prostředí byly největšími facilitátory rodina a přátelé a bariérami stavební uspořádání okolí.
Objective. The objective of the study was to determine the life quality of patients before and after total knee or hip replacement using selected codes from the International Classification of Functioning, Disability and Health. Patients and methodology. One hundred patients with knee osteoarthritis and 100 respondents with hip osteoarthritis were interviewed in two phases: before operation and after it (in 3 and 6 months) using the tool the International Classification of Functioning, Disability and Health. Results. There was chosen set of 19 codes based on respondents‘ answers that described the most frequently mentioned problems of the respondents with osteoarthritis of the weight-bearing joints. Physical health and pain codes indicated lowest quality of life before the surgery. These codes also showed the greatest improvement after surgery. Family and friends were the largest facilitators from the environmental factors, accessibility deficiencies were the largest barriers. Conclusions. Selected classification codes indicate the objective improvement of health status and life quality of patients after total endoprosthesis. They can be understood as a set of codes recommended for patients with osteoarthritis in the Czech Republic based on the conducted correlations with other questionnaires evaluating the quality of life of these patients and compared with foreign studies.

Materiál a metodika: V letech 2004–2012 bylo diagnostikováno celkem 5 pacientů s prokázaným glomus tumorem v subunguální oblasti distálních článků prstů ruky. Šlo o 3 ženy a 2 muže průměrného věku 32,2 roků (26–47 roků).
Introduction: Glomus tumor is a rare and benign vascular tumor. Although symptoms specific for this tumor are quite clear, there is still a delay between the onset of symptoms, diagnosis and subsequent surgical therapy. The authors monitor the time from the onset of symptoms to the diagnosis and management of the problems. Material and Methods: Between 2004–2012, a total of 5 patients were diagnosed with subungual glomus tumor in the area of the distal phalanges of the hand. It involved 3 women and 2 men with the mean age of 32.2 years (26–47 years). During the first examination, we monitored the duration of symptoms, number and specialty of the doctors who examined the patient, and what examinations were performed. When the cold test was positive, MRI was performed and the patients were indicated for surgical revision. Tissue samples in all patients were histologically examined. Patients were followed for 2 years. Results: It was found that the patients had clinical symptoms for an average of 2.4 years. In our group, the patients were examined by an average of 5.4 physicians (3–9 physicians). On examination before surgery, three patients reported changes in the nail bed and two patients reported no change. When following the patients 2 years after the surgery, relapse occurred in one patient and it was treated with reoperation. During regular follow-up 2 years after the surgery, 4 patients were without nail deformity. In one patient, there was resulting nail deformity. Relapse occurred in only one case. Discussion: Because the glomus tumor is a rare lesion, occurring most frequently in the nail bed, early diagnosis is still a problem. Even in literature, we encounter a similar time frame from the onset of symptoms until the final diagnosis of 1.9 to 8 years. Conclusion: Although clinical signs and problems concerning the glomus tumor are very obvious, there still remains a long time for diagnosis. It would certainly be most beneficial for patients with persistent symptoms not to be referred to different specialists, but directly to a department that specializes in hand surgery.

Úvod: Poranění volární, flexorové ploténky proximálního interfalangeálního kloubu jsou často přehlédnutá poranění, která vedou k rozvoji deformity prstu typu swan neck, a pokud nejsou řešená, k rozvoji těžkých artrotických změn. Materiál a metodika: Autoři představují soubor 12 pacientů odoperovaných v letech 2006– 2012. Extenzi v PIP kloubu mělo 9 pacientů omezenou do 10° a 2 pacienti do 20°.
Introduction: Injury to the volar, flexor plate proximal interphalangeal joint are often overlooked, leading to the development of deformity “swan neck” type deformities in the fingers, and if they are not addressed to the development of severe osteoarthritic changes. Material and Methods: The authors present a group of 12 patients who underwent surgery between 2006 and 2012. Their average age was 29.6. The time period between the injury and the surgery was 6 months to 14 years. The surgery was performed under local. The volar ZIG-ZAG incision on the PIP joint of the distal part of the plate was fixed two mA absorbable sutures to the base central section, while fixing the PIP joint 10 ° - 15 ° flexion. In one case, when the bone at the base was of poor quality, we fixed the FP to the bone using Mitek anchors. After the surgery, a dorsal plaster splint was applied, which allows the patient to actively exercise flexion in all joints of the fingers, right from the 2nd day after the surgery, according to the the patient individual condition. The fixation was removed after 6 weeks and the patients were started on a course of rehabilitation, while preventing hyperextension of the PIP joint. Patients were evaluated 6 months after surgery, evaluation the range of motion and their subjective experience. Whether the patient was able to fully return to sports or normal activities was also a part of the review. Results: The swan neck deformity of the fingers on their hands was corrected in all patients. 11 patients subjectively rated the result as excellent. One patient, where the condition was complicated by the complex regional pain syndrome (CRPS), operation was not satisfied with the results of the surgery. Extension of the PIP joint was limited to 10° in 9 patients and to 20° in 2 patients. In two of these patients the interval between the injury and the surgery was longer 10 years and the flexor release plate had to be loosened more, yet these patients rated the result as excellent. In one patient, where the condition was complicated by CRPS, the extension of the PIP joint is limited to 30°. Of the 12 patients, 11 have returned to playing sports. Discussion: Injuries to the flexor plate limit function of the hand, and because of that, reconstruction is a method that may lead to correction of its function. Our results clearly show that even surgeries performed a long time after the initial injury may have good results if the patient did not develop arthritic joint damage in the meanwhile.

Soubor pacientů a metoda: Autoři představují soubor pacientů (31 zápěstí), u nichž byla provedena selektivní denervace karpu excizí nervus interosseus anterior a posterior. Soubor byl předoperačně zhodnocen pomocí Distability of the Arm, Shoulder and Hand (DASH) skóre a stejný soubor pacientů byl zhodnocen dva roky po operaci.
Introduction: Arthritis of the wrist is a frequent consequence of inadequately treated wrist injuries. A range of methods has been published for pain reduction but these frequently limit, or completely eliminate, movement. Selective denervation of the wrist is method that can be used to reduce wrist pain. Patients and methods: The authors present a set of 31 wrists selectively denervatied by resection of nervus interosseus anterior and posterior. The file was preoperatively evaluated by Distability of the Arm, Shoulder and Hand (DASH) score and the same group of patients was evaluated two years after the surgery. Patient satisfaction was evaluated on a scale of 1–5. Results: Preoperative mean on the DASH score was 43.52 points and postoperative 24 points. Therefore, patients improved by a mean of 19.52 points. On the 1–5 scale, nine (29%) patients reported 1 (very satisfied), 12 (39%) reported 2 (satisfied), six (19%) reported 3 (neither satisfied nor dissatisfied), four (13%) marked 4 (unhappy) and none used 5 (very dissatisfied). Discussion: Selective denervation of the wrist is a method that can reduce pain with no effect on motion in a large proportion of patients. Its advantages include its technical simplicity and an opportunity to still use other treatments if selective denervation of the wrist does not improve the condition. The authors declare they have no potential conflicts of interest concerning drugs, products, or services used in the study. The Editorial Board declares that the manuscript met the ICMJE “uniform requirements” for biomedical papers.

In recent years, wrist injuries are increasingly addressed both conservatively and surgically. Awareness about proper rehabilitation is sadly very limited, even when we should be keeping in mind that rehabilitation contributes at least 50% towards achieving a good result, i.e. restoring the function lost or impaired as a result of the trauma. Proper care for the limb in the postoperative period is of great importance, and if it is not performed consistently and diligently, it can have serious consequences. The therapy should be commenced at a time when the hand is in a cast to minimise swelling and pain. The aim of this paper is to present the basic principles for rehabilitation after injuries to the wrist and the hand.

Introduction: There are many surgical procedures which can be used to resolve recurrent dislocation of the shoulder; some of them are open surgeries, though at present arthroscopic techniques are used increasingly. The aim of this work is to evaluate the long-term results of Bankart repairs conducted in the Orthopaedic Department of the Pardubice Regional Hospital. The authors evaluate not only recurrent dislocation, but also pain, motor limitation, and the development of osteoarthritis. Results are compared with other types of stabilizing operations. Method: 22 patients were followed after the Bankart repair for recurrent anterior dislocation of the shoulder. The procedure was conducted by the standard method described by Bankart in 1939 without the lateralization of the musculus subscapularis attachment. Patients were examined after a mean of 13.4 years after the surgery; the examination included a clinical examination and an X-ray. Clinical results were evaluated according to the Rowe score; osteoarthritic changes on X-rays were evaluated according to Hawkins. Results: All patients reported an improved status and satisfaction after surgery. All of the patients, moreover, were able to carry out their regular daily activities, including recreational sport. 64% of the patients were able to resume the sports they had pursued before the accident. When evaluating the Rowe score, the authors identified excellent results in 54% of patients, good results in 41% of patients, unsatisfactory results in 5% of patients, and no bad result was present. One patient experienced a redislocation 6 years after the operation. External rotation was limited most frequently after the surgery. Development of osteoarthritis of the glenohumeral joint was found in 41% of the patients. Discussion: The authors noticed a significantly lower limitation of motion in comparison with other types of shoulder stabilizing surgeries. The incidence of the development of osteoarthritis of the glenohumeral joint is similar compared to the Max Lange procedure, and significantly lower when compared to the Putti-Platt procedure. The results of Bankart repairs in the authors´ sample are comparable to the long-term results of the same operation by foreign authors. There is certainly a change of the biomechanics of the stabilized shoulder, leading to a motor reduction and an earlier development of osteoarthritis. The authors, however, did not find a correlation between reduced mobility and the development of osteoarthritis in their study, as often published in foreign literature. The occurrence of redislocation in the authors´ population was similar to other types of open surgeries. Conclusion: Recently, arthroscopic methods of shoulder joint stabilization have been replacing open stabilizations. Nevertheless, with a view to the good long-term results of the Bankart repair and even higher prevalence of redislocation after arthroscopic procedures, the authors believe that Bankart repairs could still represent an appropriate method, especially in patients with a bone defect in the humeral head, where arthroscopic methods fail more often.

The correct diagnosis of the ulnar sided wrist pain requires profound knowledge of the wrist anatomy, kinetics of the wrist and all pathological conditions that may cause such pain. In most cases the case history, clinical examination, x-ray examination and wrist arthroscopy are sufficient to establish the correct diagnosis. Yet it is difficult to identify the problem as clinical symptoms are similar and often overlap. This article provides a brief overview of the most frequent pathologies and mechanisms of injury, basic diagnostic procedure and treatment.
