OBJECTIVE: The substantial material and legislative investments in establishing and maintaining cytological screening in the Czech Republic represent barriers to a direct transition to primary HPV screening. Therefore, the LIBUSE project was implemented to test the efficacy of phasing in HPV DNA testing as a co-test to cytology in routine screening of women >30 years of age. METHODS: Women aged 30 to 60 years who underwent regular annual Pap smears were co-tested for HPV DNA with selective 16/18 genotyping at 3-year intervals. All HPV 16/18-positive cases and/or cases with a severe abnormality in cytology were sent for colposcopy; HPV non-16/18-positive cases and LSILs were graded using p16/Ki67 dual-stain cytology, and positive cases were sent for colposcopy. RESULTS: Overall, 2409 patients were included. After the first combined screening (year 'zero') visit, 7.4% of women were HPV-positive and 2.0% were HPV16/18-positive; only 8 women had severe Pap smear abnormalities. Triage by dual staining was positive in 21.9% of cases (28/128). Biopsy confirmed 34 high-grade precancer lesions. At the second combined visit (year 'three'), the frequency of HPV infection (5.3% vs. 7.4%) frequency of HPV16/18 (1.1% vs. 2.0%), referrals for colposcopy (35 vs. 83), and biopsy verified high-grade lesions (5 vs. 34) were significantly lower (all P ≤ 0.001). CONCLUSION: The addition of HPV DNA testing with selective genotyping of HPV16/18 to existing cytology screening significantly increased the safety of the program. The gradual introduction of HPV testing was well received by healthcare professionals and patients, and can facilitate transformation of the cytology-based screening. OBJECTIVE: The main aim was to determine the overall vaccine effectiveness (VE) against recurrent cervical intraepithelial neoplasia grade 2 or worse (CIN2+) including specific VE associated with timing of human papillomavirus (HPV) vaccination using data from published studies. DESIGN: Meta-analysis and meta-regression. DATA SOURCES: A computerised literature search was undertaken using the MEDLINE, EMBASE, International Pharmaceutical Abstracts, Derwent Drug File, ProQuest Science and Technology, Cochrane and MedRxiv databases. To be eligible, the studies, with no language restrictions, had to be published between 1 January 2001 and 25 May 2023. REVIEW METHODS: Included were studies with an unvaccinated reference group that assessed CIN2+ recurrence irrespective of the HPV genotype in women undergoing conisation provided. The present study was carried out in compliance with the Preferred Reporting Items for Systematic Review and Meta-Analyses and Meta-analysis Of Observational Studies in Epidemiology guidelines. The risk of study bias was assessed using the Newcastle-Ottawa Quality Assessment Scale. The Grading of Recommendations Assessment, Development, and Evaluation guidelines were used to assess the strength of evidence for the primary outcome. Data synthesis was conducted using meta-analysis and meta-regression. RESULTS: Out of a total of 14 322 publications, 20 studies with a total of 21 estimates were included. The overall VE against recurrent CIN2+ irrespective of the HPV genotype achieved 69.5% (95% CI: 54.7% to 79.5%). While the HPV vaccine valency, follow-up duration, type of study including its risk of bias had no effect on VE, the highest VE of 78.1% (95% CI: 68.7% to 84.7%) was reported for women receiving their first dose not earlier than the day of excision. This outcome was supported by additional analyses and a VE prediction interval ranging from 67.1% to 85.4%. CONCLUSIONS: The outcome of this meta-analysis and meta-regression convincingly showed the beneficial effect of post-excisional HPV vaccination against CIN2+ recurrence. Studies published to date have been unable to determine whether or not vaccination, completed or initiated before conisation, would be associated with more favourable results. Cervical mucus (CM) is a viscous fluid that is produced by the cervical glands and functions as a uterine cervix plug. Its viscosity decreases during ovulation, providing a window for non-invasive sampling. This study focuses on proteomic characterization of CM to evaluate its potential as a non-invasively acquired source of biomarkers and in understanding of molecular (patho)physiology of the female genital tract. The first objective of this work was to optimize experimental workflow for CM processing and the second was to assess differences in the proteomic composition of CM during natural ovulatory cycles obtained from intrauterine insemination (IUI) cycles and in vitro fertilization (IVF) cycles with controlled ovarian hyperstimulation. Proteomic analysis of CM samples revealed 4370 proteins involved in processes including neutrophil degranulation, cellular stress responses, and hemostasis. Differential expression analysis revealed 199 proteins enriched in IUI samples and 422 enriched in IVF. The proteins enriched in IUI were involved in phosphatidic acid synthesis, responses to external stimulus, and neutrophil degranulation, while those enriched in IVF samples were linked to neutrophil degranulation, formation of a cornified envelope and hemostasis. Subsequent analyses clarified the protein composition of the CM and how it is altered by hormonal stimulation of the uterus.

Cíl: V České republice lze ženě uměle ukončit těhotenství farmakologickou metodou (UUT-F) v I. trimestru do 49. dne sekundární amenorey. Cílem studie je analýza významu stanovení lidského choriového gonadotropinu (hCG) v séru/moči a ultrazvukového (UZ) vyšetření při diagnostice těhotenství a hodnocení výsledku UUT-F. Metodika: V letech 2017 a 2018 bylo na našem pracovišti provedeno UUT-F celkem u 109 žen sekvenčním podáním mifepristonu (600 mg perorálně) a misoprostolu (400 mcg perorálně). Stanovení hCG v séru/moči a UZ vyšetření byly provedeny při diagnostice těhotenství i hodnocení výsledku UUT-F. Výsledky: Při diagnostice těhotenství byla pozitivní a středně silná korelace mezi hCG v séru a průměrem gestačního váčku v dutině děložní (GS – gestational sac) (r = 0,711; p < 0,0001) i velikostí zárodku v dutině děložní (CRL – crown-rump length) (r = 0,605; p < 0,0001). Délka těhotenství byla 42–49 dní amenorey (průměr 45,6; medián 45), ženy byly ve věku 16–44 let (průměr 29,4; medián 29). Při kontrolním vyšetření po UUT-F byla u 13,8 % žen (15/109) hodnota hCG v séru > 1 000 IU/l a u 17,4 % (20/109) byl pozitivní low sensitivity urine pregnancy test (LSUP test). UZ vyšetření diagnostikovalo „Pokračující těhotenství“ u pěti žen a zamlklé těhotenství u jedné ženy (hCG v séru bylo vždy > 1 000 IU/l a LSUP test byl vždy pozitivní). U 5,5 % žen (6/109) byla provedena následná chirurgická intervence vč. „Pokračujícího těhotenství“ (n = 5); u zamlklého těhotenství (n = 1) byl podán další misoprostol a chirurgickou intervenci nebylo nutné provést. Závěr: Při diagnostice těhotenství je pozitivní a středně silná korelace mezi hCG v séru a CRL. Při kontrolním vyšetření po UUT-F umožní negativní LSUP test spolehlivě vyloučit pokračující těhotenství. V případě pozitivního LSUP testu by mělo být provedeno ultrazvukové vyšetření, nicméně chirurgická intervence by neměla být indikována pouze na základě rozšíření dutiny děložní.
Objective: In the Czech Republic, it is possible, to carry out Medical Termination of Pregnancy (MToP) in the 1st trimester up until the 49th day of secondary amenorrhea. The aim of the study is to analyse the significance of serum/urine human chorionic gonadotropin (hCG) assessment and ultrasound (US) examination in pregnancy diagnosis and MToP follow-up. Methods: In 2017–2018, MToP was carried out in a total of 109 women by administering a combination of mifepristone (600 mg orally) and misoprostol (400 mcg orally). Serum/urine (LSUP – low sensitivity urine pregnancy test) hCG assessment and US examination were performed at pregnancy diagnosis and MToP follow-up. Results: At pregnancy diagnosis, there was a positive and medium strong correlation between serum hCG and size of the gestational sac – GS (R = 0.711; P < 0.0001) and crown-rump length of the embryo – CRL (R = 0.605; P < 0.0001). Gestational age was 42–49 days (average 45.6, median 45 days), the women were 16–44 years of age (average 29.4, median 29 years). In MToP follow-up, serum hCG level was > 1,000 IU/L in 13.8% of women (15/109) and a positive LSUP test in 17.4% (20/109). US examination diagnosed ongoing pregnancy in five women and missed abortion in one woman (serum hCG was always > 1,000 IU/L and LSUP test was always positive). In 5.5% of women (6/109), a subsequent surgical intervention was carried out including those with ongoing pregnancy (N = 5); missed abortion (N = 1) was treated by additional misoprostol, where surgical intervention was not necessary. Conclusion: At pregnancy diagnosis, there is a positive and medium strong correlation between serum hCG and CRL. In MToP follow-up, a negative LSUP test enables reliable exclusion of ongoing pregnancy and missed abortion. In case of a positive LSUP test, US examination should be performed; however, surgical intervention should not be indicated solely on the basis of uterine cavity dilatation.

Cíl: Cílem studie byla analýza výsledků provádění screeningu vrozených vad (VV) a geneticky podmíněných onemocnění (GPO) plodu v Centru fetální medicíny (CFM) Porodnicko-gynekologické kliniky (PGK) Fakultní nemocnice Olomouc (FNOL). Soubor a metodika: Prospektivní kohortová studie. V období od 1. 1. 2020 do 31. 12. 2021 bylo provedeno celkem 14 160 zdravotních služeb u 4 916 těhotných žen. V rámci screeningu VV a GPO plodu byla u 501 těhotných žen zjištěna abnormalita vyžadující další klinický management, u 170 z nich byla diagnostikována VV plodu a ve 20 případech bylo při laboratorním genetickém vyšetření diagnostikováno i GPO. Všechny diagnostikované VV a GPO plodu byly odeslány/ hlášeny podle platné metodiky Národního zdravotního informačního systému (NZIS) do Národního registru reprodukčního zdraví (NRRZ) do Modulu VV. Výsledky: Zvýšené vypočtené individuální riziko geneticky podmíněného onemocnění plodu bylo diagnostikováno v I. trimestru těhotenství u 10,7 % plodů (319/ 2 968) a ve II. trimestru u 0,9 % plodů (27/ 2 948). Hodnota šíjového projasnění > 3,5 mm byla při ultrazvukovém vyšetření v I. trimestru těhotenství diagnostikována u 0,9 % plodů (26/ 2 968). V rámci screeningu VV a GPO plodu byla u 501 těhotných žen zjištěna abnormalita vyžadující další klinický management, u 72,1 % žen (361/ 501) bylo zvýšené riziko geneticky podmíněného onemocnění plodu a bylo indikováno diagnostické vyšetření z genetického materiálu plodu získaného invazivním odběrem choriových klků nebo plodové vody. Celkem 31,3 % z nich (113/ 361) invazivní výkon odmítlo a 2,5 % (9/ 361) se k plánovanému výkonu nedostavilo, invazivní výkon byl proveden u 66,2 % (239/ 361). Závěr: Porovnání výsledků provádění screeningu VV a GPO plodu v CFM PGK FNOL s jinými specializovanými pracovišti v ČR je v současné době obtížně proveditelné, ale informace z NRRZ by v budoucnu mohly objektivní a transparentní porovnání umožnit.
Objective: The aim of the study was to analyze the results of the screening for congenital defects (CD) and genetic diseases (GD) of the fetus in the Fetal Medicine Centre at the Department of Obstetrics and Gynecology, University Hospital in Olomouc. Materials and methods: Prospective cohort study. In the period from 1 January 2020 to 31 December 2021, a total of 14,460 health services were performed on 4,916 pregnant women. Within the screening of CD and GD of the fetus, 501 pregnant women were found to have an abnormality requiring further clinical management, 170 of them were diagnosed with a CD of the fetus and in 20 cases a GD of the fetus was diagnosed by a laboratory genetic examination. All diagnosed fetal CD and GD were sent/ reported according to the valid methodology of the National health information system (NHIS) to the National register of reproductive health (NRRH) to the CD Module. Results: An increased calculated individual risk of genetic fetal disease was diagnosed in the first trimester of pregnancy in 10.7% of fetuses (319/ 2,968), and in the second trimester in 0.9% of fetuses (27/ 2,948). Nuchal translucency (NT) > 3.5 mm was diagnosed in 0.9% of fetuses by ultrasound examination in the first trimester of pregnancy (26/ 2,968). In fetal CD and GD screening, 501 pregnant women were found to have an abnormality requiring further clinical management, 72.1% of women (361/ 501) had an increased risk of genetic fetal disease, and diagnostic examination of the fetal genetic material obtained by invasive procedure (chorionic villus sampling or amniocentesis) was indicated. A total of 31.3% of them (113/ 361) refused the invasive procedure and 2.5% (9/ 361) did not attend the planned procedure; the invasive procedure was performed in 66.2% (239/ 361). Conclusion: Comparing the results of CD and GD fetal screening in our medical facility with other specialized medical facilities in the Czech Republic is currently difficult to do, but information from the NRRH could allow objective and transparent comparisons in the future.

PURPOSE: The effect of SAM vaginal gel, a medical device containing adsorptive silicon dioxide and antioxidative sodium selenite and citric acid, on histologically-proven cervical intraepithelial neoplasia type 2 (CIN2) as well as p16 positive CIN1, and on the presence of the onco-marker p16 was investigated. METHODS: 216 women aged 25-60 years were randomized to either receive an intravaginal daily dose of SAM gel for three 28-day periods, or be followed-up without intervention. The primary endpoint was efficacy, defined as a combined histological and cytological regression. At baseline and after 3 months participants had: a guided biopsy including p16 immunohistochemical (IHC) staining, only if a lesion was visible at colposcopy; a cervical smear for cytology, high-risk human papillomavirus (hr-HPV) and a p16/Ki-67 test. At 6 months a further cytology and p16/Ki-67 test was performed. RESULTS: Regression of CIN lesions was observed in 78 out of 108 patients (72.2%) in the SAM gel arm and in 27 out of 108 patients (25.0%) in the control arm. Similarly, the change in the p16/Ki-67 cytological test status was significantly in favor of the treatment arm. The prevalence of hr-HPV decreased significantly (p < 0.001) in the treatment arm, from 87.0% to 39.8%, while it slightly increased in the control arm, from 78.7% to 83.3%. At 6 months the cytological regression in the treatment group and the highly significant effect on p16/Ki-67 was still present. CONCLUSION: SAM vaginal gel enhances the regression of cervical lesions and clears hr-HPV and p16/Ki-67 in smears significantly, thus offering an active non-destructive management to prevent cervical cancer.
