Úvod: Věkově předčasný kolorektální karcinom je definován jako karcinom diagnostikovaný před 50. rokem věku. Jeho incidence v posledních letech celosvětově vzrůstá. Cílem naší práce bylo zhodnotit výskyt věkově předčasného kolorektálního karcinomu a pokročilých neoplazií v reálné praxi endoskopického pracoviště. Metodika: Retrospektivně jsme hodnotili výskyt kolorektálního karcinomu a pokročilých neoplazií v populaci < 50 let věku ve srovnání s populací ≥ 50 let. Hodnotili jsme všechna koloskopická vyšetření provedená v Nemocnici ve Frýdku-Místku od ledna 2012 do konce června 2021. Výsledky: Ve sledovaném období bylo provedeno celkem 18 257 koloskopických vyšetření, 14 728 v populaci ≥ 50 let, 3 529 v populaci < 50 let. Za sledované období bylo diagnostikováno 29 pacientů s časným kolorektálním karcinomem (5,21 % z celkových 557) a 106 pacientů s časnou pokročilou neoplazií (4,47 % z celkových 2 371). Incidence časného kolorektálního karcinomu a pokročilých neoplazií se nelišila mezi pohlavími na rozdíl od incidence v populaci ≥ 50 let. V naší studii nebyla prokázána statisticky významná asociace mezi rodinnou anamnézou kolorektálního karcinomu a výskytem časného kolorektálního karcinomu (p = 0,316) ani časných pokročilých neoplazií (p = 0,323). Závěr: Věkově předčasný kolorektální karcinom představoval 5,21 % všech kolorektálních karcinomů diagnostikovaných za sledované období. Incidence kolorektálního karcinomu a pokročilých neoplazií v populaci < 50 let byla 0,82 % a 3,00 %, ve srovnání s 3,59 % a 15,38 % v populaci ≥ 50 let. Na rozdíl od starších pacientů není ve výskytu věkově předčasného kolorektálního karcinomu významný rozdíl mezi pohlavími.
Background: Early onset colorectal cancer is defined as cancer diagnosed before the age of 50 years. Its incidence has been increasing worldwide in recent years. The aim of our study was to assess the real-life incidence of early onset colorectal cancer, advanced neoplasias (colorectal cancer and/or advanced adenoma) and all neoplastic lesions in total in a single non-university endoscopic center. Methods: We retrospectively evaluated the incidence of all neoplastic lesions, advanced neoplasias and colorectal cancer in population under 50 years of age compared to population aged 50 years and older. We collected the data from all colonoscopic examinations performed in non-university Hospital Frydek-Mistek from January 2012 to the end of June 2021. Results: During the observed period, 18,257 colonoscopic examinations were performed in total, 14,728 in population aged 50 years and older and 3,529 in population under 50 years of age. In our study, there were 29 patients with early onset colorectal cancer (5.21% of 557 total) and 106 patients with early onset advanced neoplasias (4.47% of 2,371 total). The incidence of colorectal cancer, advanced neoplasias and all neoplasias was higher among men in older study group, but the incidence of early onset colorectal cancer and advanced neoplasias in younger group did not significantly differ between the sexes, only the incidence of all neoplastic lesions was also higher among men. Positive family history of colorectal cancer was not significantly associated with risk of early onset colorectal cancer (p = 0.316) or early onset advanced neoplasia (p = 0.323) in our study. Conclusions: Early onset colorectal cancer represented 5.21% of all colorectal cancers diagnosed during the study period. The incidence of colorectal cancer and advanced neoplasias in population under 50 years of age was 0.82% and 3.00%, compared with 3.59% and 15.38% in older population. In contrast to older population, the incidence of early onset colorectal cancer and advanced neoplasias did not differ between the sexes.

BACKGROUND AND AIMS: Colonoscopy with polypectomy are associated with a reduction in the incidence of colorectal cancer (CRC), as well as mortality, secondary to CRC. Because of the variation in physicians' performance and the risk of interval CRC after a colonoscopy, several quality indicators have been established. ADR (adenoma detection rate) is a generally accepted quality indicator. But it is also a target of possible gaming and achieving an adequate number of colonoscopies only from screening may be a problem for some practices. The aim of this study was to compare ADR for colonoscopies done for various indications and to look for correlations between the ADR of screening and all examinations. METHODS: We retrospectively assessed the quality indicators of all colonoscopies performed in a nonuniversity hospital, Frydek-Mistek, from January 2013 to December 2017. We calculated the ADR for all colonoscopies in patients over 50 years of age (subdivided into screening, surveillance, diagnostic) and separately only for screening colonoscopies. Correlations were made using the Pearson's correlation coeficient. RESULTS: The sample was composed of 6925 patients over 50 years of age (3620 men, 3305 women, mean age 66.2 years). The ADRs for screening and surveillance were higher than for diagnostic colonoscopies for all of the endoscopists, and the ADRs for all colonoscopies were lower than for screening, but sufficiently over 25%. There was a positive correlation between the ADR of screening and all colonoscopies (r=0.906, P<0.005). CONCLUSIONS: The calculation of ADR for all colonoscopies was possible in our endoscopic department, and there was a positive correlation with ADR for screening colonoscopies. ADR for all colonoscopies is a good tool for calculating real ADR from large sample sizes without gaming. TRIAL REGISTRATION: ClinicalTrials.gov (NCT03730441).
Souhrn: Východiska a cíl: Adenoma detection rate (ADR) je všeobecně uznávaným ukazatelem kvality koloskopie. Mean adenoma per colonoscopy (MAP), tedy počet všech nalezených adenomů při koloskopii, zohledňuje kvalitu vyšetření celého tlustého střeva a měl by nejlépe odrážet kvalitní koloskopii. Cílem naší práce bylo zjistit, zda ADR či polyp detection rate (PDR) refl ektují kvalitu koloskopie lépe ve srovnání s MAP. Metodika: Retrospektivně jsme hodnotili parametry kvality u všech koloskopií provedených v Beskydském Gastrocentru Nemocnice ve Frýdku- Místku od ledna 2013 do prosince 2017. Sledovali jsme ADR, PDR i MAP u všech koloskopií jedinců nad 50 let věku (screeningové, dispenzární, dia gnostické) a zvlášť pro screeningová vyšetření. Do souboru nebyly zahrnuty sigmoideoskopie a vyšetření z indikace idiopatických střevních zánětů, plánované terapeutické výkony a řešení komplikací. K výpočtu vztahu mezi MAP, ADR a PDR byl použit Pearsonův korelační koefi cient, p < 0,05 bylo považováno za signifi kantní. Výsledky: Soubor k výpočtu ADR, PDR a MAP tvořilo 6 925 jedinců nad 50 let vč. (3 620 mužů, 3 305 žen, průměrného věku 66,2 let). Mezi hodnotami MAP a ADR pro screening i pro všechny koloskopie byly nalezeny statisticky významné pozitivní korelace (p < 0,005; p = 0,009), mezi hodnotami MAP a PDR pro screening i všechny koloskopie nebyly významné korelace (p = 0,055; p = 0,176). Závěr: V našem souboru byly nalezeny statisticky významné korelace mezi MAP a ADR jednotlivých endoskopistů pro screeningové i všechny koloskopie, mezi MAP a PDR však nebyly významné korelace. Pokud budeme považovat MAP za nejobjektivnější parametr kvality, tak kvalitu screeningové koloskopie ukazuje lépe ADR než PDR.
Background and aim: The adenoma detection rate (ADR) is generally accepted to be a quality indicator. The mean number of adenomas per colonoscopy (MAP) is considered to be the most objective quality indicator. This study aimed to assess if ADR or the polyp detection rate (PDR) better reflects the quality of colonoscopy with regard to MAP. Methods: Quality indicators of all colonoscopies performed at Hospital Frydek-Mistek from January 2013 to December 2017 were retrospectively assessed. ADR, PDR, and MAP were calculated for all colonoscopies performed in patients older than 50 years, excluding those that were performed for therapeutic purposes, examination of inflammatory bowel disease patients, and management of complications and sigmoidoscopies (screening, surveillance, and diagnostic), as well as specifically for screening colonoscopies. Correlations between MAP and ADR/PDR were assessed by calculating Pearson’s correlation coefficients p < 0.05 was considered significant. Results: A total of 6,925 patients (3,620 men and 3,305 women) with a mean age of 66.2 years were included. There was a positive correlation between MAP and ADR in all colonoscopies (p < 0.005) and screening colonoscopies (p = 0.009), but not between MAP and PDR (p = 0.055 and p = 0.176). Conclusion: There was a significant positive correlation between MAP and ADR, but not between MAP and PDR, in all and screening colonoscopies. Considering that MAP is the most objective quality indicator, the quality of colonoscopy is better reflected by ADR than by PDR.

Úvod: Mezi méně obvyklé indikace akutní koloskopie patří řešení některých případů střevní obstrukce, případně pseudoobstrukce. Cílem práce bylo zhodnotit četnost, indikace a výsledky kolonické dekomprese. Metodika: Retrospektivně jsme hodnotili všechny akutní koloskopie prováděné pro střevní obstrukci či pseudoobstrukci se zavedením desuflačního setu a derotace volvulu provedené v období srpen 2011 až prosinec 2016. Sledovali jsme věk, pohlaví a komorbidity pacienta, etiologii akutního stavu, indikace k dekompresi, do jaké střevní etáže byl zaveden endoskop při akutní koloskopii a jaký byl efekt dekomprese a další osud pacienta. Výsledky: Za sledované období bylo provedeno celkem 23 kolonických dekompresí s použitím desuflačního setu u 21 pacientů. Soubor tvoří 11 mužů a 10 žen průměrného věku 68 let, medián 73 let. Indikacemi k dekompresi byl Ogilvieho syndrom u 14 pacientů, volvulus sigmatu u 2 pacientů, toxické megakolon při klostridiové enterokolitidě u 1 pacienta, paralytický ileus při ischemické kolitidě v rámci syndromu multiorgánové dysfunkce u 1 pacienta, ileus při benigní stenóze anastomózy rekta u 1 pacienta a mechanický ileus z adhezí původně považovaný za Ogilvieho syndrom u 2 pacientů. Vzhledem k tomu, že dekomprese byla nutná ve většině případu u letitých polymorbidních pacientů, byla 30denní mortalita 28,6 %. Závěr: Nejčastější indikací k dekompresi kolon v našem souboru byl Ogilvieho syndrom. Vzhledem k tomu, že šlo většinou o polymorbidní letité pacienty, byla mortalita do 30 dnů po výkonu vysoká. Endoskopicky lze úspěšně řešit i volvulus sigmatu. Efekt dekomprese je obecně vyšší se zavedením desuflačního setu.
Introduction: Endoscopic management of some types of intestinal obstruction or pseudo-obstruction is an unusual indication for acute colonoscopy. The aim of this study was to assess the frequency of, indications for, and results of colonic decompression. Methods: All colonoscopies for intestinal obstruction or pseudo-obstruction with insertion of a decompression tube and endoscopic reduction of the sigmoid volvulus carried out from August, 2011, to December, 2016 at our institution, were retrospectively assessed. For each patient, age, sex, comorbidities, etiology of ileus and indication for decompression, part of the colon reached during acute colonoscopy, effect of decompression, and follow-up were recorded. Results: During the study period, 23 colonic decompressions were performed in 21 patients (11 men and 10 women) with an average age of 68 years (median, 73 years). The indication for decompression was Ogilvie’s syndrome in 14 patients, sigmoid volvulus in two patients, pseudomembranous colitis with toxic megacolon in one patient, paralytic ileus caused by ischemic colitis in one patient with multiple organ dysfunction syndrome, ileus due to benign anastomotic stricture in the rectum in one patient, and mechanical ileus that was initially considered to be Ogilvie’s syndrome in two patients. Because most of the patients were elderly with comorbidities, the 30-day mortality rate was 28.6%. Conclusions: The most frequent indication for colonic decompression was Ogilvie’s syndrome. The 30-day mortality rate was high because most of the patients were elderly with comorbidities. Sigmoid volvulus can be successfully managed endoscopically, and decompression is more effective with the insertion of a decompression tube.

Prezentujeme případ nemocného, u kterého bylo nutné pro těžkou ataku kolitidy refraktorní na medikamentózní léčbu provést akutní kolektomii. Před provedením rekonstrukční operace byla na základě nového nálezu atypických vředů v pahýlu rekta revidována diagnóza směrem k indeterminované kolitidě s rysy Crohnovy choroby a bylo rozhodnuto o jiném typu rekonstrukční operace. U nemocného byla preferována ileorektální anastomóza před ileopouch‑anální anastomózou.
A case of a young man treated by acute colectomy for severe colitis refractory to pharmacologic therapy is presented. Based on the presence of new atypical ulcers in his rectal stump, the disease was reclassified from ulcerative colitis to Crohn's disease before subsequent reconstructive surgery. Ileorectal anastomosis was prefered to ileal pouch anal anastomosis. Doporučovaný věk pro zahájení kolonoskopického screeningu je pro populaci s průměrným rizikem 50–55 let, není ale stanoven žádný horní věkový limit. Přestože prevalence neoplastických lézí s věkem stoupá, očekávaná délka života a s ní i výtěžnost screeningových vyšetření se snižuje. Cílem naší studie bylo porovnat výsledky screeningových kolonoskopií v populaci seniorů starších 70 let s mladší populací. Metodika: Srovnávali jsme data ze screeningových kolonoskopií provedených od ledna 2012 do listopadu 2014 u seniorů starších 70 let věku včetně s mladšími jedinci, ke statistickému hodnocení byl použit chí-kvadrát test. Výsledky: Ve sledovaném období bylo provedeno celkem 1 240 screeningových kolonoskopií, z toho 239 u pacientů nad 70 let věku a 1 001 ve skupině pod 70 let. Polyp detection rate i adenoma detection rate byly v obou skupinách srovnatelné. Ve skupině seniorů nad 70 let bylo významně více pokročilých neoplazií a karcinomů, intubace céka pak byla významně nižší. V obou skupinách nebyly zaznamenány komplikace. Závěry: Screeningové kolonoskopie v populaci seniorů nad 70 let byly v naší studii bezpečné. U této věkové skupiny byla zjištěna vyšší detekce pokročilých neoplazií, ovšem nižší podíl úplných vyšetření.
The recommended age for a first colonoscopy screening in an average risk population is 50–55 years, but there is no upper limit. Although the prevalence of neoplastic lesions increases with age, the life expectancy decreases, which also reduces the efficiency of screening. The aim of our study was to evaluate the outcomes of screening colonoscopies in a population of over 70 years of age and compare them with those obtained in younger patients. Methods: The outcomes of colonoscopy examinations in a population aged 70 years and over vs. the outcomes obtained in patients under 70 years were compared using the chi-square test. Results: In total, 1,240 screening colonoscopies were performed, 239 in the group aged 70 years and over, and 1,001 in the group under 70 years. The polyp detection rate and adenoma detection rate were comparable in both groups. There were significantly more advanced neoplasms and carcinomas in the older group and the caecal intubation rate was lower. There were no complications in either group. Conclusions: Screening colonoscopy in a population of over 70 years of age in our study was safe, with a higher detection of advanced neoplasms, but with a lower completion rate.
