V tomto článku je podán chronologický souhrn vývoje endoskopické enukleace prostaty, od holmiové enukleace, přes bipolární a thuliovou enukleaci až po enukleaci prostaty zeleným laserem.
In this paper the evolution of endoscopic prostate enucleation is reviewed in chronological order based on energy technology starting first with holmium laser, then bipolar electrocautery, following by thulium laser and finally greenlight laser enucleation of the prostate.

OBJECTIVES: To compare brain responses to peroneal electrical transcutaneous neuromodulation (peroneal eTNM®) and transcutaneous tibial nerve stimulation (TTNS), two methods for treating overactive bladder (OAB), using functional magnetic resonance imaging (fMRI). The present study was not designed to compare their clinical efficacy. MATERIALS AND METHODS: This study included 32 healthy adult female volunteers (average age 38.3 years (range 22-73)). Brain MRI using 3 T scanner was performed during three 8-min blocks of alternating sequences. During each 8-min block, the protocol alternated between sham stimulation (30 s) and rest (30 s) for 8 repeats; then peroneal eTNM® stimulation (30 s) and rest (30 s) for 8 repeats; then, TTNS stimulation (30 s) and rest (30 s) for 8 repeats. Statistical analysis was performed at the individual level with a threshold of p = 0.05, family-wise error (FWE)-corrected. The resulting individual statistical maps were analyzed in group statistics using a one-sample t-test, p = 0.05 threshold, false discovery rate (FDR)-corrected. RESULTS: During peroneal eTNM®, TTNS, and sham stimulations, we recorded activation in the brainstem, bilateral posterior insula, bilateral precentral gyrus, bilateral postcentral gyrus, left transverse temporal gyrus, and right supramarginal gyrus. During both peroneal eTNM® and TTNS stimulations, but not sham stimulations, we recorded activation in the left cerebellum, right transverse temporal gyrus, right middle frontal gyrus, and right inferior frontal gyrus. Exclusively during peroneal eTNM® stimulation, we observed activation in the right cerebellum, right thalamus, bilateral basal ganglia, bilateral cingulate gyrus, right anterior insula, right central operculum, bilateral supplementary motor cortex, bilateral superior temporal gyrus, and left inferior frontal gyrus. CONCLUSIONS: Peroneal eTNM®, but not TTNS, induces the activation of brain structures that were previously implicated in neural control of the of bladder filling and play an important role in the ability to cope with urgency. The therapeutic effect of peroneal eTNM® could be exerted, at least in part, at the supraspinal level of neural control.

Maligní paratestikulární nádory jsou velmi raritní a v některých případech se obtížně rozeznávají od jiných lézi v oblasti třísla. Liposarkom je diagnostikován histopatologickým vyšetřením. Níže popisujeme kazuistiku 76letého muže s diagnózou liposarkomu semenného provazce.
Malignant paratesticular tumours are very rare, however it is really difficult to distinguish from the other lesions in the area of the groin. Liposarcoma is diagnosed by histopatological examination. We present a case of the 76 years old man with the diagnosis liposarcoma of the spermatic cord.
Hrbáček J, Hanáček V, Tláskal V, Saláková M, Čermák P, Tachezy R, Zachoval R. Detekce mikrobiomu horních močových cest u pacientů s urolitiázou: studie proveditelnosti. Cíl: Zjistit, zda existuje mikrobiální osídlení horních močových cest (HMC) a pokud ano, tak liší-li se od mikrobiomu dolních močových cest (DMC). Materiál a metoda: Soubor tvoří pacienti podstupující endoskopický operační zákrok v anestezii pro litiázu HMC. Moč z močového měchýře byla odebrána aseptickou katetrizací, z HMC cystoskopicky zavedeným ureterálním katétrem. Po izolaci DNA byla bakteriální 16S rDNA sekvenována na platformě Illumina MiSeq a získaná data zpracována příslušnými bioinformatickými nástroji. Část izolované DNA byla využita k detekci virových nukleových kyselin pomocí kvantitativní polymerázové řetězové reakce (qPCR). Párové vzorky vždy z HMC a DMC byly porovnány z hlediska struktury bakteriálního společenstva a přítomnosti vybraných DNA virů. Výsledky: Zařazeno bylo devět pacientů (8 mužů a 1 žena) průměrného věku 49,7 let, z nichž u osmi byla provedena sekvenace bakteriálních nukleových kyselin a u sedmi qPCR na přítomnost sedmi běžných DNA virů. Bohatost a diverzita mikrobiomu (alfa diverzita) HMC a DMC nebyla statisticky významně odlišná. Struktura mikrobiálních komunit z HMC a DMC (beta diverzita) vykázala větší podobnost v rámci jednoho pacienta, než byla podobnost všech vzorků HMC, resp. všech vzorků DMC. Virová DNA byla detekována jak v HMC, tak v DMC. Závěr: Výsledky naší pilotní studie poukazují na přítomnost bakteriálního a virového osídlení horních močových cest u pacientů s urolitiázou.
Hrbáček J, Hanáček V, Tláskal V, Saláková M, Čermák P, Tachezy R, Zachoval R. Profiling upper urinary tract microbiota: a feasibility study in patients with urinary stone disease. Aim: To investigate the putative existence of upper urinary tract microbiota and if present, to compare the microbial communities from the kidney with those in the urinary bladder. Patients and Methods: Patients were undergoing endoscopic procedures under anesthesia for upper urinary tract stones. Bladder urine was obtained by aseptic catheterisation, renal urine samples were collected via ureteric catheter inserted into the renal pelvis under cystoscopic and radiographic guidance. Pairwise comparison of urine samples from the same individual were made with regard to their bacteriome and virome. Results: A total of nine patients (8 males, 1 female) provided both samples for analysis. Next-generation sequencing of the V4 hypervariable region of the 16S rDNA using primers 515F and 806R was performed on eight of them and quantitative polymerase chain reaction for the presence of viral nucleic acids of seven common DNA viruses was performed on seven of them. There were no significant differences in alpha diversity measures (number of OTUs, iChao1, ACE, Shannon and Simpson indices) of samples from the kidney and bladder from the same individual. Likewise, microbial communities from the upper and lower urinary tract within the same individual were more similar to each other than samples within the respective group (e.g. all kidney samples). Viral nucleic acids were detected in the upper as well as lower urinary tract. Conclusion: Upper urinary tracts seem to be inhabited by microbial communities whose composition resembles that of the lower urinary tract.

Stejskal J, Adamcová V, Zachoval R. Hodnocení bolestivosti transperineální biopsie prostaty v lokální anestezii. Cíl: Cílem práce je zhodnotit bolestivost transperineální biopsie prostaty (TPBxP) v lokální anestezii (LA) pomocí subjektivního hodnocení pacientů podstupujících výkon. Metody: Soubor obsahuje 55 pacientů, kteří podstoupili TPBxP v LA. Ve 26 případech se jednalo o první biopsii a ve 29 případech o opakovanou biopsii. V průměru bylo odebíráno 15 vzorků na pacienta (12-18). LA byla provedena pomocí 50 ml 1% mezokainu, premedikace perorálními či intravenózními analgetiky nebyla v našem souboru použita. Pacienti hodnotili bolestivost výkonu na škále od 1 (žádná bolest) do 10 (maximální představitelná bolest), separovaně pro aplikaci LA a odběr transperineálních bioptických vzorků. Získaná data byla korelována s věkem pacientů, velikostí prostaty, počtem odebíraných vzorků a hodnotou PSA. Výsledky: Průměrný věk pacientů byl 66,85 let (54-78). Průměrné PSA bylo 10,11 ng/ml (0,66-53,6). Průměrná velikost prostaty byla 46 ml (17-126). Průměrná bolestivost aplikace LA byla 2,04 (1-7, medián 2) a průměrná bolestivost vlastní biopsie byla 4,05 (1-7, medián 4). Z hodnocených korelací jsou významné pouze vyšší pozitivní korelace bolestivosti aplikace LA a bolestivosti odběru biopsií. Dále potom negativní korelace mezi věkem pacienta a jím udávanou bolestivostí. Závěr: TPBxP v LA je pacienty dobře tolerována. Vlastní biopsie je dle pacientů bolestivější než aplikace LA. Starší pacienti snáší výkon v LA lépe. Počet odebraných vzorků, velikost prostaty a hodnota PSA nemají na bolestivost výkonu vliv.
Stejskal J, Adamcová V, Zachoval R. Evaluation of pain of transperineal prostate biopsy under local anaesthesia. Aim: The goal of the analysis is to evaluate patient reported pain during transperineal prostate biopsy (TPBxP) under local anaesthesia (LA). Methods: We evaluated 55 patients who had a TPBxP under LA comprising 26 first and 29 repeated biopsies. On average 15 cores were obtained in each patient (12-18). The local anaesthesia was administered using 50cc of mesocaine applied to the perineal skin and prostate apex. No intravenous or oral analgesics were used. The patients reported the painfulness of both local anaesthesia and prostate biopsy using a scale of 1 (no pain) to 10 (highest pain imaginable). The data were then correlated with the patients' age, prostate size, PSA level and the number of taken biopsy cores. Results: The mean patient age was 66.85 years (54-78), the mean PSA was 10.11ng/ml (0.66 - 53.6), and the average prostate size was 46cc (17-126). The mean patient reported pain was 2,04 (1-7, median 2) for the application of the LA and 4.05 (1-7, median 4) for the biopsy. There was a positive correlation between the application of LA and the biopsy itself. Furthermore, there was a negative correlation between patient age and the reported painfulness of TPBxP. Conclusion: TPBxP is reasonably well tolerated by patients. The biopsy itself is more painful than the application of LA. Older patients tend to tolerate the procedure better. The PSA level, prostate size and the number of biopsy cores have no effect on the patient reported pain.
