The interaction on level of absorption, plasma-protein binding of renal excretion is less frequent. Pharmacodynamic interaction of antiepileptics with other drugs is seldom synergistic with a possibility of declination of dose. The increase of adverse drug reactions is more common. Purely pharmacodynamic interaction was described with oxcarbazepin, perampanel and pregabalin. The new antiepileptics have lower interaction potential - most of them are excreted either via kidney or extrahepatal (e. g. gabapentin, lacosamide, levetiracetam, topiramate, vigabatrin). Enzymatic induction is either not present or minimal. The inductive effect is necessary to be taken into account in carbamazepine, phenytoin and phenobarbital. Inhibitive effect is held in valproic acid and felbamate. The interactive potential among newer antiepileptics is the highest in lamotrigine, oxcarbazepine and rufinamide. Drug interactions were not found in lacosamide, pregabalin, stiripentol and vigabatrin.

K enzymatické indukci pak nedochází vůbec nebo zcela minimálně. Ze starších látek lze počítat s projevem indukčního efektu u karbamazepinu, fenytoinu a fenobarbitalu. Inhibiční efekt se uplatňuje u valproátu a felbamátu. Z nových látek je interakční potenciál nejvyšší u lamotriginu, oxcarbazepinu a rufinamidu. Lékové interakce nebyly popsány u lacosamidu, pregabalinu, stiripentolu a vigabatrinu.

Účel studie: Hodnocení vlivu kyseliny valproové (VPA) na hladinu karbamazepinu (CBZ) a karbamazepin-10,11-epoxidu (CBZ-E) při dlouhodobé léčbě přípravky s pomalým uvolňováním. Metoda: U 262 vzorků odebraných před podáním od 175 dětských pacientů (<15 let) byla hodnocena celková denní dávka (PDD), dávka na kg (DBW), koncentrace CBZ, CBZ-E, poměry CBZ koncentrace/PDD, CBZ-E koncentrace/PDD, CBZ-E/CBZ a rozložení koncentrací CBZ ve vztahu k terapeutickému rozmezí 4-9mg. Podrobná analýza byla provedena u 11 pacientů, kteří byli vlastní kontrolou. Výsledky (uvedeny jako průměr ± směrodatná odchylka): Ačkoliv pacienti léčení CBZ + VPA užívali vyšší dávky než pacienti na monoterapii: PDD 557 ± 204 vs. 400 ± 187mg, p <0.001; DBW 20.2 ± 10.5 vs. 13.5 ± 5.8mg/kg, p <0.001, nebyl zaznamenán rozdíl v průměrné koncentraci CBZ (5.1 ± 1.4 vs. 5.3 ± 1.9mg/l), CBZ-E (1.2 ± 0.8 vs. 1.1 ± 0.9mg/l), poměru CBZ-E/CBZ (0.24 ± 0.12 vs. 0.22 ± 0.19) ani v rozložení koncentrací podle terapeutického rozmezí. V terapeutickém optimu bylo 86% vs. 77% koncentrací. Koncentrace nad horní hranicí terapeutického rozmezí se nevyskytovaly. U CBZ + VPA byly nižší poměry koncentrace CBZ/PDD: 10.3 × 10-3 ± 4.2 × 10-3, vs. 14.9 × 10-3 ± 6.4 × 10-3 mg × L-1/mg, p <0.001; CBZ/DBW: 0.31 ± 0.17, vs. 0.44 ± 0.19 mg × l-1/mg × kg-1, p < 0.001. Obdobně u 11 pacientů, kteří byli vlastní kontrolou: poměr koncentrace CBZ/PDD 10.9 × 10-3 ± 3.7 × 10-3 vs. 15.0 × 10-3 ± 5.7 × 10-3, p < 0.01; CBZ/DBW 0.35 ± 0.10 vs. 0.40 ± 0.14, p <0.05. Zdá se, že VPA má indukční efekt na metabolismus CBZ. Na dávce závislé nežádoucí účinky (ataxie, bolest hlavy) byly popsány u 4 pacientů na monoterapii. Koncentrace CBZ a CBZ-E se nelišila od pacientů bez NÚL, byl však vyšší poměr CBZ-E/CBZ 6.8 × 10-3 ± 1.6 × 10-3 vs. 3.1 × 10-3 ± 3.6 × 10-3, a CBZ-E/PDD 0.32 ± 0.06 vs. 0.21 ± 0.20, p < 0.05. Závěr: Významná interakce mezi CBZ a VPA byla úspěšně eliminována úpravou dávkování pomocí TDM.
The effect of valproic acid (VPA) on plasma level of carbamazepine (CBZ) and carbamazepine-10,11-epoxide (CBZ-E) in long-term treatment was evaluated. Method: A total of 262 samples were obtained predose from 175 pediatric patients (<15 years). Daily dose (PDD), dose related to bodyweight (DBW), plasma levels (PL) of CBZ, CBZ-E, CBZ level/dose ratio, CBZ-E level/dose ratio and CBZ-E/CBZ ratio, the distribution of PL according to the therapeutic range 4-9mg/L, and seizure frequency were compared. A detailed analysis was done in 11 individuals who served as their own control. Results (given as mean ± SD): Patients on a CBZ + VPA regimen were treated with higher daily doses: PDD 557 ± 204 vs. 400 ± 187mg, respectively, p <0.001; DBW 20.2 ± 10.5 vs. 13.5 ± 5.8mg/kg, respectively, p <0.001. The mean CBZ PL and the distribution of PL according to the therapeutic range were not different: 5.1 ± 1.4 vs. 5.3 ± 1.9mg/L, respectively. There was 86% of therapeutic PL CBZ + VPA and 77% in monotherapy. No toxic level was found. CBZ-E PL remained unchanged in CBZ + VPA (1.2 ± 0.8 vs. 1.1 ± 0.9mg/L, respectively) as did the CBZ-E/CBZ ratio (0.24 ± 0.12 vs. 0.22 ± 0.19, respectively). The CBZ level/dose was decreased in CBZ + VPA: 10.3× 10-3 ± 4.2×10-3 vs. 14.9×10-3 ± 6.4×10-3mg×L-1/mg, respectively, p<0.001; so was the CBZ level/DBW: 0.31 ± 0.17 vs. 0.44 ± 0.19mg×L-1/mg×kg-1, p <0.001. These findings suggest an inductive effect of VPA on CBZ metabolism. Similar outcomes were found in 11 individuals who served as their own control: CBZ level/dose 10.9×10-3 ± 3.7×10-3 vs. 15.0×10-3 ± 5.7×10-3, respectively, p <0.01; CBZ level/DBW 0.35 ± 0.10 vs. 0.40 ± 0.14, respectively, p <0.05. Dose-dependent adverse drug reactions (ataxia, headache) were reported in four individuals on CBZ monotherapy. No difference in the mean PL of CBZ or CBZ-E was found, while the CBZ-E/CBZ ratio and CBZ-E level/dose ratio were increased: 6.8×10-3 ± 1.6×10-3 vs. 3.1×10-3 ± 3.6×10-3, and 0.32 ± 0.06 vs. 0.21 ± 0.20, p <0.05). No difference in seizure frequency was found. Conclusion: The CBZ-VPA interaction is considerable, but it is sufficiently eliminated due to TDM dose adjustment. Carbamazepine is an antiepileptic drug with a narrow therapeutic index, which requires an efficient method for blood level monitoring. Finger-prick dried blood spot (DBS) collection is an alternative microsampling technique, which is less invasive than conventional venipuncture. Paper-based molecularly imprinted-interpenetrating polymer networks (MI-IPN) were developed as blood collection devices, which allowed for selective on-spot microextraction of carbamazepine from DBS. A hybrid of homogeneous polystyrene and silica gel polymer was synthesized and coated on a Whatman® Grade 1 filter paper. Proteins and other interferences in the blood samples were eliminated by using the MI-IPN collection devices, and the resulting DBS extracts were suitable for direct injection into the capillary electrophoretic instrument. The lower limit of quantitation of 4 μg/mL in capillary blood was achieved by the sweeping-micellar electrokinetic chromatography method using a KCl-containing matrix, which was sufficient for the therapeutic drug monitoring purposes. Method accuracies were in the range of 88.4 ± 4.5% to 94.5 ± 2.7% with RSD values ≤ 5.1%. The developed paper-based MI-IPN provided superior extraction efficiencies (92.2 ± 2.5%) in comparison with commercially available DBS collection cards, i.e., Whatman® 903 protein saver card (59.8 ± 2.8%) and GenCollect™ 2.0 card (47.2 ± 1.4%). The paper-based MI-IPN devices for DBS collection and on-spot extraction were characterized by simple fabrication, low costs, disposability, and reduction in sample preparation steps, and their further developments might open new perspectives in clinical applications, such as in therapeutic drug monitoring. Lakosamid (LCM) v monoterapii a v přídatné léčbě fokální epilepsie je v České republice dostupný osm let. Zkušenosti s jeho užíváním v graviditě a během laktace jsou minimální. Přibývá žen s fokální epilepsií, u nichž léčba LCM vede ke kontrole záchvatů. Pokud antiepileptika vhodná k léčbě fokálních záchvatů s dokumentovanou nízkou teratogenitou (lamotrigin, levetiracetam) selhala, nebo je žena netolerovala, je otázkou, jakou další antiepileptickou léčbu lze doporučit. Pokud se LCM u ženy s fokální epilepsií ve fertilním věku osvědčil při kontrole záchvatů, lze v jeho podávání pokračovat i v případě plánování gravidity? Teratogenní riziko není známo. Je nutné upravovat dávku LCM během těhotenství? Data o eventuálních změnách plazmatické koncentrace LCM v průběhu gravidity nejsou dostupná. Pokud žena užívající LCM porodí, může kojit? Potenciální riziko pro kojené novorozence/děti není známo. Studie na zvířatech prokázaly, že se LCM do mateřského mléka vylučuje, což lze předpokládat i u člověka vzhledem k malé vazbě na plazmatické bílkoviny. V článku jsou prezentována data z období těhotenství a laktace u pacientky při monoterapii LCM.
Treatment with lacosamide (LCM) in monotherapy and adjunctive therapy of focal epilepsy is available in the Czech Republic for 8 years. Experience with its use during pregnancy and lactation is minimal. Lacosamide is used and efficient in women with epilepsy (WWE). If other antiepileptics suitable for the treatment of focal seizures with documented low teratogenicity (lamotrigine, levetiracetam) have failed or a woman has not tolerated them, the question is what other antiepileptic treatment can be recommended? If LCM in a woman with focal epilepsy at fertile age is proven to control seizures, can it be continued for the duration of pregnancy planning? A teratogenic risk is not known. Is it necessary to modify the LCM dose during pregnancy? The data on possible changes in the plasma concentration of LCM during pregnancy are not available. If a woman taking LCM gives birth, can she breastfeed? A potential risk to breastfed infants/children is not known. Animal studies have shown that LCM is excreted in breast milk which can be expected in humans as well due to low binding with plasma proteins. The article presents data from the period of pregnancy and lactation in a patient on LCM monotherapy.

BACKGROUND: Lamotrigine has become the most frequently prescribed drug in the treatment of pregnant women with epilepsy. Although some relevant studies have found a wide milk/maternal serum as well as infant/maternal serum concentration ratio, different infant ages at the time of sampling and small number of patients preclude comparison. The aim of this study was to provide a consistent evaluation. METHODS: Data of 43 nursing women treated by lamotrigine were evaluated retrospectively. The authors followed the transport of lamotrigine during the first postnatal month from mothers to breastfed infants through maternal milk between the years 2002 and 2017. RESULTS: Lamotrigine concentrations varied from 1.1 to 14.9 mg/L in the maternal serum, from <0.66 to 9.1 mg/L in the milk and between <0.66 and 6.9 mg/L in the infant serum. The milk/maternal serum concentration ratio ranged from <0.18 to 0.74 and the infant/maternal serum concentration ratio measured between <0.15 and 0.74. Highly significant correlations were found between milk and maternal serum levels and between infant serum levels and milk, maternal serum levels, lamotrigine daily dose, and also maternal dose related to the body weight. CONCLUSIONS: The authors confirmed the wide range of the milk/maternal serum concentration ratio and the infant/maternal serum concentration ratio. Although the degree of lamotrigine exposure to the breastfed infants was smaller than during gestation, 16% of the infant serum levels measured were within the therapeutic range used for the general epileptic population. Lamotrigine concentration monitoring in breastfed infant, in our opinion, is the most relevant aspect for the analysis of actual lamotrigine exposure in infants, especially in those with clinical symptoms.

Valproic acid (VPA) is currently one of the four most often prescribed antiepileptic drugs (AEDs) in pregnancy. However, only a small number of studies have measured suckling infant serum levels of the drug. We studied the transport of VPA from breastfeeding mothers to the mature milk and breastfed infants and the influence of comedication with enzyme-inducing AEDs. The data of 30 nursing women treated by VPA were analyzed retrospectively. Mature milk, maternal, and infant serum levels were collected between the 6th and 32nd postnatal day and measured by gas chromatography during the years 1996-2017. Valproic acid levels varied from 5.4 to 69.0 mg/L (mean: 39.0 ± 16.1 mg/L) in the maternal serum, from <1.0 to 16.7 mg/L (mean: 1.6 ± 3.9 mg/L) in the milk, and from <1.0 to 17.5 mg/L (mean: 4.2 ± 4.3 mg/L) in the infant serum. The milk/maternal serum level ratio ranged from <0.03 to 0.25 (mean: 0.03 ± 0.06) and the infant/maternal serum level ratio from <0.03 to 0.61 (mean: 0.11 ± 0.13). Sixty-seven percent of milk and 33% of infant VPA concentrations were below the limit of quantification. No correlations were observed between maternal serum and milk levels or between maternal and infant serum levels. In conclusion, none of the milk or infant serum VPA levels reached the lower limit of the reference range used for the general population with epilepsy, so the degree of VPA exposure in breastfed infants is less than during gestation. Nevertheless, if signs of potential adverse reactions manifest, infant serum concentrations should be measured.
