Deprese je velmi častou komorbiditou u neurologických onemocnění. Sdílí s nimi některé etiopatogenetické faktory a negativně ovlivňuje jejich průběh. Diagnostiku a léčbu provádí neurolog, v případě těžší a neúspěšně léčené deprese je žádoucí konzultace psychiatra. Článek se detailněji zabývá farmakoterapií u vybraných dlouhodobých neurologických onemocnění a soustředí se hlavně na nejnovější poznatky a specifika u jednotlivých onemocnění.
Depression belongs to frequent comorbidities in neurological diseases. There are some common etiopathogenetic factors and depression negatively influences the course of neurologic disorders. Diagnostics and treatment of comorbid depression is usually carried out by neurologist, however, in the case of severe and/ or unsuccessful treated depression psychiatric consultation is necessary. The paper deals in more details with pharmacotherapy for chosen long-term neurological disorders and is focused on the newest information and specifics in individual diseases.
PURPOSE: The aim of the study was to examine the neuropsychological performance and effort in patients with a confirmed PNES diagnosis. The second aim of the study was to investigate the relationship between validity indicators from the cognitive battery with validity and clinical scales from a personality scale. METHOD: Patients with PNES (N = 250; F:M 186:64; mean age 38.32 (13.23)) were assessed utilizing the RBANS (Czech Research version) to evaluate cognitive performance and to obtain the Effort Index. The MMPI-2 was used to evaluate personality and psychopathology. RESULTS: Global cognitive performance was 0.92 SD below average (according to the Gaussian distribution) in patients with PNES. The lowest scores in the sample were in the Attention domain (-1.7SD). Insufficient effort was detected in 10% of patients. Education correlated negatively with the Effort index (rs = -0.25, p = 0.01). A mild significant correlation in Scale 7 (rs = 0.21, p = 0.01) and Scale 8 (rs = 0.24, p = 0.01), and a significant correlation between Effort Index and Back F Scale (rs = 0.23, p = 0.01) were noted. CONCLUSIONS: Assessment of cognitive performance and effort is an essential part of the comprehensive evaluation of patients with PNES during their hospitalization at Epilepsy centers. Many aspects of the neuropsychological assessment can offer useful indications for reaching a differential diagnosis, including clinical history, behavioral observations, cognitive and symptom validity testing, and structured psychological inventories.

Při kontaktu s pacientem s epilepsií se často jako lékaři omezujeme na počet záchvatů a efekt úpravy terapie. Někdy nám tak může uniknout to nejpodstatnější, což je vlastní pacient a jeho problémy. Na druhou stranu, pacienti se často rovněž zdráhají svěřit se detailněji "cizí osobě z očí do očí". V současné době se objevuje nový on-line prostor, kde se pacienti svěřují relativně bez obav. Při analýze tohoto "nového světa" se můžeme setkat s názory a postoji, které jsou pro lékaře zajímavé a inspirující, a můžeme se zde rovněž poučit z našich nedostatků. Tento článek byl inspirován analýzou sociálních sítí (Facebook, YouTube, Instagram, Twitter, Pinterest, SnapChat a TikTok). V současnosti máme spíše tendenci sociální sítě přehlížet nebo je vnímat jen jako nutné zlo. Nicméně, vzhledem k jejich širokému rozšíření, s nimi budeme muset v budoucnu počítat a měli bychom se snažit je spíše využít ve prospěch pacienta a nás. Cílem naší práce je podat krátký přehled o tom, co "se děje na sociálních sítích". Vzhledem k dostupné literatuře se opíráme především o zahraniční odkazy, na konci krátce hovoříme rovněž o českých webech. Nemyslíme si, že náplní práce lékaře má být studium internetových odkazů, nicméně je vhodné o tomto "novém prostoru" vědět. Otázka je další využití těchto informací pro klinický výzkum či v rámci epidemiologických studií.
During an encounter with the epilepsy patient, as doctors we often confine ourselves to the number of seizures and effect of treatment adjustment. We may thus sometimes miss the essential, which is the patient himself or herself and his or her problems. On the other hand, patients are often reluctant to confide in "a stranger face to face" in more detail. Recently, a new on-line space has emerged in which patients share their stories relatively fearlessly. When analysing this "new world", we can encounter views and attitudes which are interesting and inspiring for the doctor, as well as learn from our shortcomings here. This article has been inspired by social network analysis (Facebook, YouTube, Instagram, Twitter, Pinterest, SnapChat, and TikTok). Currently, we tend to overlook social networks or to see them only as necessary evil. However, given their widespread use, we will have to reckon with them in the future, and we should better try to use them for the benefit of the patient as well as ourselves. The aim of this article is to present a brief overview of what is going on on social media. Due to the literature available, we rely particularly on foreign references, with Czech websites being mentioned briefly at the end. We do not believe that studying internet links should be in a doctor's job description; however, it is necessary to be aware of this "new space". The question remains of the further use of this information for clinical research or as part of epidemiological studies.

Identification of active electrodes that record task-relevant neurophysiological activity is needed for clinical and industrial applications as well as for investigating brain functions. We developed an unsupervised, fully automated approach to classify active electrodes showing event-related intracranial EEG (iEEG) responses from 115 patients performing a free recall verbal memory task. Our approach employed new interpretable metrics that quantify spectral characteristics of the normalized iEEG signal based on power-in-band and synchrony measures. Unsupervised clustering of the metrics identified distinct sets of active electrodes across different subjects. In the total population of 11,869 electrodes, our method achieved 97% sensitivity and 92.9% specificity with the most efficient metric. We validated our results with anatomical localization revealing significantly greater distribution of active electrodes in brain regions that support verbal memory processing. We propose our machine-learning framework for objective and efficient classification and interpretation of electrophysiological signals of brain activities supporting memory and cognition.

Epilepsie je chronické neurologické onemocnění s řadou negativních dopadů na školní docházku nemocných dětí. Školní prostředí se zásadním způsobem podílí na stigmatizaci doprovázející tuto nemoc. Jednotlivé rizikové faktory související s dětmi s epilepsií samotnými, jejich rodiči, učiteli a spolužáky nebyly dosud dostatečně studovány; komplexní struktura školního života dětí s epilepsií podmíněná vzájemnými vazbami mezi zmíněnými proměnnými tak zůstává nejasná. Prvním cílem naší studie je poskytnout přehled nejdůležitějších současných poznatků o dané problematice. Ve druhé části pak představujeme studii, jejímž výstupem bude detailní strukturální model zohledňující relevantní proměnné vztahující se k životu dětí s epilepsií v prostředí školy, vč. kvantifikace jejich vzájemných vazeb. Tento komplexní model by měl posloužit jako teoretický základ pro plánování cílených intervencí vedoucích ke zvýšení kvality života těchto dětí prostřednictvím redukce stigmatu souvisejícího nejen se samotnou nemocí, ale i se znalostmi a postoji rodičů, učitelů a spolužáků. V závěru studie předkládáme doporučení pro další výzkum i praxi v této oblasti.
Epilepsy is a chronic neurological disorder with many negative sequelae related to the school attendance of affected children. School environment plays a fundamental role in the stigmatization accompanying this disease. Individual risk factors related to children with epilepsy themselves, their parents, teachers and classmates have not been sufficiently studied yet; the complex structure of the school life of children with epilepsy determined by the mutual coupling of the particular variables thus remains unclear. The first objective of this review article is to provide an overview of the most important findings related to this topic. The second objective is to introduce a complex study in which outcome will be a detailed structural model taking into consideration the most important variables related to the life of children with epilepsy in a school and quantifying the links between them. Such a complex model should serve as a theoretical background for planing tailored interventions to improve the quality of life of these children by means of reducing the stigma related not only to the disease itself but also to report the parents‘ teachers‘ and classmaes' knowledge. Recommendations for further research as well as clinical practice in this field are given at the end of the study.

Cíl: Ačkoliv je stigmatu souvisejícímu s poruchami autistického spektra (PAS) věnována poměrně značná pozornost, stále je patrný nedostatek nástrojů umožňujících tento koncept efektivně měřit. Cílem této studie bylo provést na specifické skupině studentů vysokých škol předběžné ověření 24položkového dotazníku adaptovaného ze škály stigmatu u epilepsie a získat tak nástroj měřící míru vnímané stigmatizace související sPAS. Materiál a metoda: Ověření bylo realizováno na vzorku 328 studentů (50 % muži, 50 % ženy) různých oborů dvou českých vysokých škol (40 % humanitní obory, 32 % technické obory, 28 % přírodovědné obory). Výsledky: Bylo zjištěno, že dotazník má v dané skupině velmi dobré psychometrické vlastnosti včetně vysoké vnitřní konzistence (Cronbachovo alfa 0,836). test-retest reliability (Pearsonův korelační koeficient 0,62) a jasně definované třífaktorové struktury. Průměrná míra stigmatizace byla 52 % a nezávisela na oboru studia ani znalosti někoho s PAS (p > 0,05). Závěr: Srovnáním s předchozím výzkumem zaměřeným na epilepsii byla prokázána podstatně větší míra vnímané stigmatizace u PAS, což naznačuje, že by bylo vhodné připravit a realizovat efektivní intervence vedoucí k redukci tohoto stigmatu.
Objective: Although significant attention has been devoted to considerable stigma associated with autism spectrum disorders (ASD), there is still a significant lack of instruments enabling a valid measurement of this concept. We aimed to carry out a preliminary verification of the 24-item questionnaire based on a stigma scale of epilepsy on a specific group of Czech university students, and to obtain an instrument for evaluation of perceived ASD-related stigma. Methods: The research was carried out in the group of 328 university students (50% of them were women and 50% men) of different specializations (40 % of respondents studied social science and humanities, 32% engineering and 28% natural sciences). Results: We found that the instrument exhibited very good psychometric properties including high internal consistency (Cronbachs alpha of 0.836) and test-retest reliability (Pearson correlation coefficient of 0.62) and well-defined three-factor structure. The average level of stigmatization was 52% and no effect of field of study and knowledge of someone having ASD was observed here. Conclusions: Comparison with our previous research focused on epilepsy revealed higher level of stigmatization related to ASD suggesting that an effective intervention aiming on reduction of the perceived ASD-related stigma would be highly needed.
