OBJECTIVE: The aim of this study was to analyse the long-term radiation effects on human sperm. METHODS: In total, 104 samples of male donors from 2 regions of Ukraine were tested. Group 1 consisted of 32 donors from the Ivano-Frankivsk region, group 2 included 72 volunteers from the Zhytomyr region. The average age of donors in both groups was 35 ± 6 years (range 24-49). To assess the level of apoptosis, membrane mitochondrial potential, concentration of reactive oxygen species, and ploidy of sperm, flow cytometry was performed. RESULTS: The individual equivalent dose of group 1 was < 0.4 mSv and of group 2 ≥ 0.4 mSv. Live spermatozoa with signs of apoptosis were significantly higher (p < 0.05) in group 2 in comparison to group 1 (15.6% and 11.2%, respectively). Spermatozoa without violating integrity were 73.2% in group 1 and approximately 16% higher than the indices of group 2. The percentage of dead necrotic spermatozoa was twice as high in men with a predicted equivalent dose of ≥ 0.4 mSv than in comparison group. A higher percentage of spermatozoa with low mitochondrial membrane potential, di- and tetraploid was found in group 2. CONCLUSIONS: An equivalent individual dose of ≥ 0.4 mSv can cause a decrease in mitochondrial potential, an increase in the production of spermatozoa with pathological ploidy, as well as to provoke increasing apoptosis in cells.

Arsenic trioxide (As2O3) poisoning and associated potential lesions are of a global concern. Inversely, riboflavin (vitamin B2, VB2) as a component of flavoproteins could play a vital role in the spermatogenic enzymatic reactions. Thus, this research aimed to explore potential beneficial roles of VB2 during As2O3-injured-toxicity. Rats were randomly allocated into 4 groups (n=8/group) and challenged as follows (for 30 days continuously): Group 1 received normal saline; Group 2 was treated with 3 mg As2O3/L; Group 3 received 40 mg VB2/L; Group 4 received 3 mg As2O3/L + 40 mg VB2/L. Both As2O3 and VB2 were dissolved in deionized water. Malondialdehyde (MDA), Glutathione Peroxidase (GSH-Px), Superoxide dismutase (SOD), and Catalase (CAT) were assessed for the oxidative profile, while TAS (Total Antioxidative Status) levels were evaluated for the antioxidant system, in both serum and testicular tissue. P<0.05 was considered statistically significant. The results show that As2O3 significantly decreased the body weight, testicular weight and testis volume, semen quality and testicular cell count (p<0.05). Furthermore, MDA content in the testicular tissue of the As2O3 group rats was significantly higher in comparison to the vehicle group (p<0.05). Likewise, TAS and the activities of GSH-Px, CAT and SOD were reduced (p<0.05) when compared to the control. As(2)O(3) induced testicular damage and seminiferous tubular atrophy. Monodansylcadaverine assays mirrored the histopathology observations. Meanwhile, As2O3 upregulated the expression of mitophagy-related genes including PINK1, Parkin, USP8, LC3-I, Fis1 and Mfn2. The p38 gene, responsible to stress stimuli, was also upregulated by As2O3 administration. Meanwhile, exposure to VB2 led to a significant decrease of the expression levels of mitophagy related genes. Our study revealed that VB2 supplementation protected testicular structures against As2O3-induced injury via a dual inhibition of oxidative changes and a regulation of the PINK1-mediated pathway.

S neplodností, tedy s neschopností otěhotnět v průběhu jednoho roku pravidelného nechráněného pohlavního styku, se potýká cca 15 % párů ve vyspělých zemích. Jelikož se jedná o diagnózu konkrétního páru, a nikoli jedince, je namístě přistupovat k páru jako celku, ne odděleně k ženě či muži. Pro posouzení fertilního potenciálu muže je za zlatý standard již několik desetiletí považován spermiogram, tedy nativní vyšetření ejakulátu. Spermiogram je ve své základní podobě pouze morfologickým, nikoli funkčním vyšetřením, proto nemusí vždy spolehlivě informovat o skutečném fertilním potenciálu daného muže. Z tohoto důvodu jsou do praxe zaváděny nové metody pro zlepšení diagnostiky a následné terapie. Uvádíme možnosti vyšetření ejakulátu průtokovou cytometrií a vliv asymptomatických urogenitálních infekcí na plodnost.
In developed countries, approximately 15% of couples suffer from infertility, i.e. they do not conceive within one year of a regular unprotected sexual intercourse. Since infertility is the only one diagnosis of a couple, and not of an individual, it is essential to examine the couple as the unit. Sperm analysis, i.e. native microscopic evaluation, has been used for decades as a golden standard for male fertile potential assessment. Sperm analysis, in its fundamental form, has been only morphological, and not functional evaluation of ejaculate, thus it might not give us reliable information about actual fertile potential of an individual male. On that account, new methods are being introduced to the clinical practice with a goal to improve diagnostics and subsequent treatment. The article presents these new methods, namely flow cytometry, and the impact of asymptomatic urogenital infections on fertility.

The most common reason for in vitro fertilization (IVF) cycle cancelation is a lack of quality gametes available for intracytoplasmic sperm injection (ICSI). Here we present the successful fertility treatment of the couple affected by obstructive azoospermia combined with suboptimal response to controlled ovarian stimulation. Since the conventional approach appeared ineffective to overcome both partners' specific problems, the targeted interventions, namely, (1) pharmacological enhancement of sperm motility and (2) polarized light microscopy (PLM)-guided optimization of ICSI time, were applied to rescue the cycle with only immature oocytes and immotile testicular sperm retrieved. The treatment with theophylline aided the selection of viable spermatozoa derived from cryopreserved testicular tissue. When the traditional stimulation protocol failed to produce mature eggs, non-invasive spindle imaging was employed to adjust the sperm injection time to the maturational stage of oocytes extruding a polar body in vitro. The fertilization of 12 late-maturing oocytes yielded 5 zygotes, which all developed into blastocysts. One embryo was transferred into the uterus on day 5 post-fertilization, and another 3 good quality blastocysts were vitrified for later use. The pregnancy resulted in a full-term delivery of a healthy child. This case demonstrates that the individualization beyond the standard IVF protocols should be considered to maximize the chance of poor-prognosis patients to achieve pregnancy with their own gametes.

Pro detekci molekulárních změn vyvolaných diabetes mellitus jsme použili panel protilátek proti vybraným proteinům testes a spermií. Detegovali jsme změny v morfologii a histologii varlat a změny v expresi důležitých proteinů, které jsou zapojeny do spermatogeneze a mezibuněčné komunikace. Monoklonální protilátky proti intra -akrozomálním proteinům prokázaly zhoršenou kvalitu spermií u diabetických pacientů i spermií diabetických myší. Panel vybraných protilátek lze použít k testování změn v testes a kvalitě spermií vyvolaných diabetem. Tyto výsledky jasně dokumentují vliv diabetického prostředí na morfologii a histologii varlat a kvalitu spermií.
We used a panel of antibodies against selected testicular and sperm proteins to detect molecular changes induced by diabetes mellitus. We detected changes in morphology and histology of the testes, and changes in the expression and distribution of proteins involved in spermatogenesis and intercellular communication. Monoclonal antibodies against intra -acrosomal proteins demonstrated the impaired quality of sperms from diabetic patients as well as sperms from diabetic mice. In conclusion, the panel of selected antibodies can be used to test diabetes -induced changes in the testes and sperm quality. These results clearly document the effect of the diabetic environment on the morphology and histology of the testes, and sperm quality.
Štúdia popisuje retrospektínu analýzu patológie spermiogramu z pohľadu asistovanej reprodukcie, teda andrologického faktora. Analyzujeme vekové rozpätie pacientov podstupujúcich cyklus in vitro fertilizácie na našom pracovisku za posledných 10 rokov, vplyv veku na patológiu spermiogramu a vplyv veku mužov na úspešnosť cyklov asistovanej reprodukcie. Centrá asistovanej reprodukcie sa neustále snažia zvyšovať úspešnosť cyklov in vitro fertilizácie. V súčasnosti sa pozornosť obracia na podrobnejšiu analýzu vplyvu mužského faktora na úspešnosť asistovanej reprodukcie. Dizajn: retrospektívna analýza andrologického faktora z hľadiska vplyvu veku na parametre spermiogramu a úspešnosť asistovanej reprodukcie. Metódy: porovnali sme vek partnerov pacientok podstupujúcich asistovanú reprodukciu z roku 2007 a 2017. Hodnotili sme jednotlivé parametre spermiogramu našich pacientov vzhľadom na vek. Analyzovali sme celkovú koncentráciu spermií, motilitu a morfológiu spermií. Taktiež sme hodnotili vplyv veku na DNA fragmentáciu spermií. Keďže sme potrebovali minimalizovať vplyv ženského vekového faktoru v ďalšej analýze sme obmedzili výber respondentov len na program darovaných oocytov a hodnotili sme vplyv veku mužov na úspešnosť metódy IVF‑ICSI u párov podstupujúcich asistovanú reprodukciu. Výsledky: Vek sa zdá byť určujúcim faktorom na kvalitu spermiogramu pri hodnotení "normospermikov" klasickými parametrami spermiogramu. Pokles normospermikov vzhľadom na vek je signifikantný. A naopak s vekom rastie kombinácia patológii v spermiograme. Pri hodnotení jednotlivých parametrov ako je celková koncentrácia spermií, motilita a morfológia spermií signifikantný rozdiel nepozorujeme. V hodnotení úspešnosti metódy IVF‑ICSI sú závery kontroverzné, obzvlášť pri vyššie uvedenej selekcii pacientov. Rozdiel v úspešnosti donorského programu, však môžeme pripísať reparačnej schopnosti "mladého" oocytu darkyne.
The study describes a retrospective analysis of spermiogram pathology from assisted reproduction perspective. We analyze the age range of patients undergoing the in vitro fertilization cycle at our workplace over the last 10 years, the effect of age on spermiogram pathology, and the influence of age on the success rate of assisted reproduction cycles. Centres of assisted reproduction are continually trying to increase the success of in vitro fertilization cycles. Attention is now focusing on a more detailed analysis of the influence of male factor on the success of assisted reproduction treatment. Design: The retrospective analysis of the andrological factor in accordance with the age effects on spermiogram parameters and the success rate of assisted reproduction cycles. Methods: We compared the age of patients undergoing assisted reproduction cycles from the years 2007 and 2017. We evaluated the individual sperm parameters of our patients in accordance with their age. The total sperm concentration, motility and sperm morphology as well as the influence of age on sperm DNA fragmentation were analyzed. Because we needed to minimize the influence of female factor, we made a restriction in the consecutive analysis and selected only respondents from the oocyte donation program. We evaluated the influence of age of males on the success of IVF-ICSI method in couples undergoing assisted reproduction. Results: Age seems to be a determining factor of the quality of spermiogram in the group of normospermics evaluated by classical spermiogram parameters. The decrease of normospermics in accordance with the age is significant. On the contrary, combinations of the pathologies in spermiogram increase with age. When assessing individual parameters such as total sperm concentration, motility and sperm morphology, there is no significant difference. The conclusions in the analysis of the success of IVF‑ICSI method are controversial, especially with the selection of respondents. However, the difference in the success of the donor program can be attributed to the reparative ability of the "young" donor oocyte.

Hlavní stanovisko: Vyšetření spermiogramem neposkytuje dostatek informací o kvalitě spermií. Pomocí průtokové cytometrie je možné stanovit zastoupení apoptotických spermií v ejakulátu, fragmentaci jejich DNA a integritu akrozomu. Tyto testy umožňují diagnostikovat pacienty s rizikem nižší fertility, kteří by měli být vyšetřeni andrologem. Cíl: Přehled výsledků vyšetření kvality spermií pomocí průtokové cytometrie Soubor pacientů a metody: Vyšetření ejakulátu bylo provedeno u 800 mužů (ve věku 21-66 let) léčených v Centrech asistované reprodukce GENNET. Soubor zahrnoval 366 normozoospermiků a 434 pacientů s patologickým spermiogramem. Pomocí průtokové cytometrie byl u všech pacientů stanoven počet živých a apoptotických spermií, u 213 pacientů byl stanoven počet spermií s nízkou integritou akrozomu a u 65 pacientů byl stanoven počet spermií s fragmentací DNA metodou TUNEL. Pacienti byli rozděleni do skupiny ApoHigh s vysokým zastoupením apoptotických spermií (>50 % apoptotických spermií) a ApoLow s nízkým zastoupením apoptotických spermií (<50 % apoptotických spermií). Výsledky: U normozoospermiků bylo zjištěno 16,9 % ApoHigh vzorků, u pacientů s patologickým spermiogramem 53,9 % ApoHigh vzorků. Čím více je apoptotických spermií v ejakulátu, tím vyšší je výskyt fragmentace DNA a nízké integrity akrozomu. Průměrná hodnota fragmentace DNA byla vyšší u vzorků ApoHigh než u ApoLow jak u normozoospermiků (24,0 % vs. 13,5 %), tak u pacientů s patologickým spermiogramem (33,3 % vs. 18,0 %). Vyšší průměrný počet spermií s nízkou integritou akrozomu byl zjištěn u ApoHigh vzorků než u ApoLow jak u normozoospermiků (40,2 % vs. 23,9 %), tak u pacientů s patologickým spermiogramem (53,1 % vs. 35,2 %). Závěr: Vyšší zastoupení apoptotických spermií v ejakulátu snižuje jeho kvalitu. Test apoptózy spermií, fragmentace DNA a stanovení integrity akrozomu doplňují informace získané ze spermiogramu a umožňují vytipovat rizikové subfertilní pacienty, pro které je vhodné podrobné andrologické vyšetření.
Semen analysis does not provide enough information about sperm quality. Flow cytometry tests allow us to analyse percentage of apoptotic sperm, DNA fragmentation and acrosome integrity. Sub‑fertile patients are better diagnosed using these tests. Objective: Overview of sperm quality data measured by flow cytometry. Material and method: 800 sub‑fertile men (aged 21–66 years) were treated at GENNET – assisted reproduction centres. 366 patients had normal results and 434 patients had pathological parameters of semen analysis. Flow cytometry analyses of apoptotic sperm were done in all patients. DNA fragmentation rate was measured by TUNEL assay in 65 samples. The acrosome integrity was measured in 213 samples. Two groups of patients were set according to the number of apoptotic sperm in the semen samples – ApoHigh group included samples with > 50 % of apoptotic sperm and ApoLow group included samples with < 50 % of apoptotic sperm. Results: 16.9 % of normal semen samples were ApoHigh as well as 53.9 % pathological semen. Higher average values of DNA fragmentation were detected in the ApoHigh group than in the ApoLow group both in normal semen (24.0 % vs. 13.5 %) and in pathologic semen too (33.3 % vs. 18.0 %). Higher average values of the acrosome integrity were detected in the ApoHigh group than in the ApoLow group both in normal semen (40.2 % vs. 23.9 %) and in the pathologic samples as well (53.1 % vs. 35.2 %). Conclusion: The presence of apoptotic sperm decreases the semen quality. The examination of apoptosis, DNA fragmentation and acrosome integrity together with microscopic semen analyses provide a better diagnostic tool how to identify sub‑fertile men, than simple semen analysis. These tests might be helpful for selection of patients for further uro‑andrological examination and treatment.
BACKGROUND: Huntington disease (HD) is a fatal neurodegenerative disorder involving reduced muscle coordination, mental and behavioral changes, and testicular degeneration. In order to further clarify the decreased fertility and penetration ability of the spermatozoa of transgenic HD minipig boars (TgHD), we applied a set of mitochondrial metabolism (MM) parameter measurements to this promising biological material, which can be collected noninvasively in longitudinal studies. OBJECTIVE: We aimed to optimize methods for MM measurements in spermatozoa and to establish possible biomarkers of HD in TgHD spermatozoa expressing the N-terminal part of mutated human huntingtin. METHODS: Semen samples from 12 TgHD and wild-type animals, aged 12-65 months, were obtained repeatedly during the study. Respiration was measured by polarography, MM was assessed by the detection of oxidation of radiolabeled substrates (mitochondrial energy-generating system; MEGS), and the content of the oxidative phosphorylation system subunits was detected by Western blot. Three possibly interfering factors were statistically analyzed: the effect of HD, generation and aging. RESULTS: We found 5 MM parameters which were significantly diminished in TgHD spermatozoa and propose 3 specific MEGS incubations and complex I-dependent respiration as potential biomarkers of HD in TgHD spermatozoa. CONCLUSIONS: Our results suggest a link between the gain of toxic function of mutated huntingtin in TgHD spermatozoa and the observed MM and/or glycolytic impairment. We determined 4 biomarkers useful for HD phenotyping and experimental therapy monitoring studies in TgHD minipigs.

Cíl studie: Přehled problematiky zhoršování kvality spermií starších mužů a s tím souvisejících rizik početí. Typ studie: Přehledový článek. Název a sídlo pracoviště: Imunologická laboratoř GENNET s.r.o, Praha. Metodika: Sledovanou skupinou jsou muži z párů s poruchou reprodukce. Výsledky: S vyšším věkem u mužů klesá objem ejakulátu, celkový počet spermií, pohyblivost spermií a zhoršuje se jejich morfologie. S věkem se zvyšuje počet de novo vzniklých genových mutací, množství chyb v epigenetické modifikaci DNA a počet apoptotických spermií s fragmentovanou DNA v ejakulátu. Kvalitu spermií ovlivňuje nejen věk, ale také životní styl. Rizikovými faktory jsou kouření, nadváha, nezdravá strava, nedostatek pohybu, zvýšená teplota a onemocnění, jako je varikokéla, infekce močových cest, nádorová onemocnění, hormonální nerovnováha a další. Závěr: Plodnost mužů není časově neomezená. S věkem klesá pravděpodobnost početí a prodlužuje se významně doba snahy o koncepci. Urologické a andrologické vyšetření starších mužů by mělo patřit k základnímu vyšetřovacímu postupu u párů s poruchou reprodukce. Kromě spermiogramu, endokrinologického vyšetření a zobrazovacích metod se doporučuje také použití moderních diagnostických metod – stanovení počtu apoptotických spermií a spermií s fragmentací DNA. I když jsou možnosti léčby poruch plodnosti starších mužů omezené, cílená léčba a zdravý životní styl může přispět ke zlepšení kvality spermií.
Objective: Review the effect of ageing on decreasing quality of sperm and increasing risks of reproductive outcome. Design: Review. Setting: Laboratory of Immunology, GENNET s.r.o, Prague. Methods: Study group – men from sub-fertile couples. Results: Male age is associated with a decrease of semen volume and deterioration of total sperm count, motility and morphology. Number of de novo gene mutations, defects in DNA epigenetic modifications and number of apoptotic sperm with fragmented DNA increase in ageing men. Male age and life-style have an impact on quality of sperm. The risk factors are smoking, overweight, unhealthy food, lack of physical activity, increased body temperature and illnesses e.g. varicocele, urinary tract infections, cancer diseases and hormonal imbalances. Conclusion: Men fertility is not age-unlimited. The probability of conception significantly decreases in older men and the time to conception is significantly prolonged. Urological and andrological examination should be compulsory for older men from subfertile couples. The standard examinations – semen analyses, endocrine examination, ultrasound imaging etc. plus modern methods – such as the examination of sperm DNA fragmentation and the percentage of apoptotic sperm - are recommended. Although the treatment options of male subfertility are limited, the targeted treatment and improvement of patient´s life style can positively influence the sperm quality.

BACKGROUND: Genetic causes of male infertility are hypothesized to involve multiple types of mutations, from single gene defects to complex chromosome rearrangements. Recently, several recurrent X-chromosome microdeletions (located in subtelomeric region of the long arm) were reported to be associated with male infertility in Spanish and Italian males. The aim of our study was to test their prevalence and infertility association in population of men from the Czech Republic. METHODS: 107 males with pathological sperm evaluation resulting in nonobstructive infertility were compared to 131 males with normal fecundity. X-chromosome microdeletions were assessed by +/- PCR with three primer pairs for each region Xcnv64 (Xq27.3), Xcnv67 (Xq28) and Xcnv69 (Xq28). The latter microdeletion was further characterized by amplification across the deleted region, dividing the deletion into three types; A, B and C. RESULTS: We detected presence of isolated Xcnv64 deletion in 3 patients and 14 controls, and Xcnv69 in 3 patients and 6 controls (1 and 1 patient vs.4 and 1 control for types A and B respectively). There was one control with combined Xcnv64 and Xcnv69 type B deletions, and one patient with combination of Xcnv64 and Xcnv69 type C deletions. The frequency of the deletions was thus not higher in patient compared to control group, Xcnv64 was marginally associated with controls (adjusted Fisher´s exact test P = 0.043), Xcnv69 was not associated (P = 0.452). We excluded presence of more extensive rearrangements in two subjects with combined Xcnv64 and Xcnv69 deletions. There was no Xcnv67 deletion in our cohort. CONCLUSION: In conclusion, the two previously reported X-linked microdeletions (Xcnv64 and Xcnv69) do not seem to confer a significant risk to impaired spermatogenesis in the Czech population. The potential clinical role of the previously reported patient-specific Xcnv67 remains to be determined in a larger study population.
