Objectives. The One-minute Sit-To-Stand Test (STST) is a relatively novel physical performance test in the field of pulmonology. It measures the exercise tolerance of the patient. In this pilot study, we evaluated its predictive value for the postoperative course in cardiac surgery patients. Design. This was a prospective observational cohort study of patients undergoing elective aortic valve replacement. STST was applied in the patients, along with pulmonary function testing, a day prior to surgery, on postoperative day 7, and 3 months after surgery. A Short Form-36 health related quality of life survey (SF-36) was also included. The patients were divided into two groups according to the preoperative STST outcome: group A (≤20 repetitions, n = 12), and group B (>20 repetitions, n = 18). Results. We enrolled 30 patients in the study between May 2017 and May 2019. No complication were observed during the STST. The standard perioperative measures were identical for both groups. However, group A exhibited a significantly longer artificial ventilation time, as well as worse pulmonary function (VC, FVC, TLCO), on postoperative day 7. Concerning the SF-36, group A displayed a worse pre- and postoperative evaluation in both physical functioning and vitality. Conclusions. STST is a short and practical physical performance test that could enable a better prediction of the postoperative course after cardiac surgery. Patients with lower preoperative STST performance may require longer postoperative artificial ventilation, as they may present worse in some parameters of pulmonary functions and quality of life. However, these findings must be corroborated in larger studies.
Cíl studie: Charakterizovat pacienty s covidem-19 hospitalizované na naší JIP, zjistit jejich mortalitu a výskyt komorbidit považovaných za rizikové faktory pro těžký průběh nemoci. Metodika: Retrospektivní observační studie na JIP vyššího typu s 5-8 lůžky. Populace 91 dospělých pacientů s covidem-19 vyžadujících intenzivní péči. Výsledky: Průměrný věk pacientů byl 67 let (38-88). Nejčastějšími komorbiditami byly hypertenze (56 pacientů, 61%) a diabetes (35 pacientů, 38%). 24 pacientů (26%) bylo obézních s BMI 30-40, 10 nemocných (11%) s BMI >40. Průměrné SOFA skóre při příjmu bylo 3,5 (1-10). Jako maximální ventilační podpora byla použita HFNO (high flow nasal oxygen) terapie u 14 (15%) pacientů (z nich 9 mělo limitaci terapie ve smyslu nezahajování invazivní plicní ventilace (D.N.I.)), neinvazivní plicní ventilace (NIV) u 17 (18%) pacientů (z nich 9 mělo limitaci péče ve smyslu D.N.I.). Stav 37 (40%) pacientů si vyžádal intubaci a připojení na UPV (umělou plicní ventilaci). Celková mortalita v našem souboru byla 37% (34 pacientů). U pacientů s 2 a více komorbiditami byla mortalita 46%, u nemocných bez komorbidit 44% (jednalo se ale jen o 4 nemocné vysokého věku). Pokud jde o věkové rozložení, nejvyšší mortalita byla ve věkové skupině 80-90 let (89%). Ve skupině pacientů mladších 50 let byla v našem souboru mortalita překvapivě vysoká (27%), jednalo se ale celkem o 3 pacienty. Mortalita pacientů, jejichž stav si vyžádal invazivní umělou plicní ventilaci, byla 43%. Závěr: Mortalita pacientů s covidem-19 na naší JIP za sledované období byla 37%, což je výrazně vyšší než za stejné období v letech 2019-2020 před začátkem pandemie. Mortalita stoupala se stoupajícím věkem. Téměř všichni pacienti měli některou z výše uvedených komorbidit.
Objectives: To determinate characteristics of covid-19 patients in our ICU, to determinate mortality and presence of comorbidities considered as risk factor for severe course of disease. Methods: Retrospective observation study in ICU with 5-8 beds. Population of 91 adults with covid-19 admitted to ICU. Results: Median age was 67 years (38-88). Hypertension (56 patients, 61%) and diabetes (35 patients, 38%) were the most common comorbidities. 24 patients (26%) were obese with BMI 30-40, 10 patients (11%) with BMI >40. Average SOFA score on admission was 3,5 (1-10). HFNO (high flow nasal oxygen) therapy was the highest ventilation support used in 14 (15%) patients (while 9 (64%) of them had limitation of therapy by order D.N.I.), NIV (non-invasive ventilation) in 17 (18%) patients (9 of them (52%) had limitation of therapy with order D.N.I.). Conditions of 37 (40%) patients required intubation and invasive mechanical ventilation. Overall mortality in our cohort was 37%. Mortality of patients with 2 or more comorbidities was 46%, mortality of patients without comorbidities was 44% (in total 4 patients with high age). The highest mortality was in the group of patients 80-90 years (89%). Mortality in the group of patients younger than 50 years was surprisingly high (27%), but these were 3 patients in total. Mortality of patients requiring IPV was 43%. Conclusion: Mortality of covid-19 patients in our ICU was 37% which is much higher than mortality in the same period in 2019 and 2020 before the beginning of pandemic. Mortality increased with higher age. Almost all our patients had at least one of the comorbidities mentioned above.

BACKGROUND: Motility disorders of upper gastrointestinal tract are common in critical illness and associated with significant clinical consequences. However, detailed quantitative and qualitative analyses of esophageal motor functions are lacking. Therefore, we aimed to characterize the key features of esophageal motility functions using high-resolution impedance manometry (HRIM) and to evaluate an objective link between esophageal motor patterns, gastric emptying, and gastroesophageal reflux. We also studied the prokinetic effects of metoclopramide. METHODS: We prospectively performed HRIM for 16 critically ill hemodynamically stable patients. Patients were included if they had low gastric volume (LGV; < 100 mL/24 h, n = 8) or high gastric volume (HGV; > 500 mL/24 h, n = 8). The HRIM data were collected for 5 h with intravenous metoclopramide administration (10 mg) after the first 2 h. RESULTS: The findings were grossly abnormal for all critically ill patients. The esophageal contraction vigor was markedly increased, indicating prevailing hypercontractile esophagus. Ineffective propulsive force was observed for 73% of esophageal activities. Panesophageal pressurization was the most common pressurization pattern (64%). Gastroesophageal reflux predominantly occurred with transient lower esophageal sphincter relaxation. The common features of the LGV group were a hyperreactive pattern, esophagogastric outflow obstruction, and frequent reflux. Ineffective motility with reduced lower esophageal sphincter tone, and paradoxically fewer reflux episodes, was common in the HGV group. Metoclopramide administration reduced the number of esophageal activities but did not affect the number of reflux episodes in either group. CONCLUSION: All critically ill patients had major esophageal motility abnormalities, and motility patterns varied according to gastric emptying status. Well-preserved gastric emptying and maintained esophagogastric barrier functions did not eliminate reflux. Metoclopramide failed to reduce the number of reflux episodes regardless of gastric emptying status. Trial registration ISRCTN, ISRCTN14399966. OBJECTIVE: The association between energy metabolism and prognosis in polytrauma patients has not yet been defined. The aim of this study was to describe energy metabolism and analyze the prognostic value of respiratory quotient (RQ) and nonprotein respiratory quotient (npRQ) in fasting polytrauma patients (fPP) and polytrauma patients with nutritional support (nsPP). METHODS: Twenty-two polytrauma patients (before and after parenteral nutrition administration) and 22 healthy controls (after overnight fasting) were examined on day 4 (median) after admission to the intensive care unit. To evaluate energy expenditure in nsPP and resting energy expenditure in fPP and controls with RQ and npRQ in all groups, we used indirect calorimetry. With regression analysis, the descriptive models of intensive care unit (ICU) length of stay (LOS) and mechanical ventilation time (VT) were derived. RESULTS: RQ and npRQ were significantly lower in fPP than in controls (P < 0.05 and P < 0.01, respectively) and in nsPP (P < 0.05). In nsPP, relationships between RQ or npRQ and the ICU LOS or mechanical VT were demonstrated (P < 0.0001, r = -0.78 for RQ and VT; P < 0.0001, r = -0.78 for npRQ and VT; P < 0.001, r = -0.69 for RQ and LOS; P < 0.001, r = -0.72 for npRQ and LOS). CONCLUSIONS: RQ and npRQ parameters measured by indirect calorimetry in polytrauma patients with parenteral nutrition on the fourth day of ICU stay related to clinical outcomes such as duration of mechanical ventilation and ICU LOS.

Úvod. Klíšťová meningoencefalitida je nejčastějším virovým onemocněním přenášeným klíštětem v České republice. Přestože se jedná o preventabilní infekci, její incidence v posledním desetiletí nikterak významně neklesá. V naší práci byly hodnoceny klinické projevy, závažnost onemocnění a případné dlouhodobé následky onemocnění Soubor pacientů a metody. Do sledovaného souboru byli zařazeni všichni pacienti s laboratorně potvrzenou klíšťovou meningoencefalitidou, hospitalizovaní a sledovaní na Klinice infekčních, parazitárních a tropických nemocí Nemocnice Na Bulovce v letech 2006-2016. Retrospektivně bylo hodnoceno zastoupení jednotlivých forem onemocnění, závažnost a dlouhodobé následky. Výsledky. Celkem bylo zařazeno 582 pacientů. Nejčastější klinickou formou byla meningoencefalitida (74,8 %) a meningitida (16,8 %). Pacientů se závažným průběhem bylo 55 (9,5 %), 22 žen a 33 mužů, nejvíce zastoupena byla věková skupina 60-69 let. Pobyt na jednotce intenzivní péče byl nezbytný u 38 pacientů, 24 pacientům musela být poskytnuta ventilační podpora. Paretické komplikace mělo 45 (7,7 %) pacientů, u 50 % z nich přetrvávají déle než 1 rok od akutního onemocnění, pravděpodobnost trvalých následků je zde vysoká. Závěr. Závažný průběh onemocnění byl pozorován především ve vyšších věkových skupinách, těžké komplikace jsme však zaznamenali i u mladých pacientů. Vzhledem k nedostupnosti kauzální terapie zůstává jedinou ochranou vakcinace.
Introduction: tick-borne meningoencephalitis is the most common tick-borne disease in the Czech Republic. Even though it is a preventable infection, its incidence is not significantly decreasing. Our article evaluates the clinical symptoms, disease severity and possible long-term consequences of this disease. Patients and methods: the patient sample included all patients with laboratory-confirmed tick-borne meningoencephalitis who were hospitalised and monitored at the Infectious, Parasite-borne and tropical disease Clinic at the Bulovka hospital, between the years 2006-2016. The various types of the disease, severity and long-term consequences were retrospectively evaluated. Results: the sample consisted of 582 patients. The most common clinical forms were meningoencephalitis (74.8%) and meningitis (16.8%). There were 55 patients with a severe course of the disease (9.5%), 22 women and 33 men, most commonly in the age group of 60-69 years of age. 38 patients required hospitalisation at an intensive care unit, 24 patients required ventilation. 45 patients experienced complications in the form of paresis (7.7%) - in 50% of these cases, the complications have persisted for more than one year since the acute disease, which means that the likelihood of permanent damage is high. Conclusion: the disease took a severe course mostly in older patients, but serious complications were recorded in young patients as well. Since causal therapy is not available, vaccination remains the only reliable form of protection.

OBJECTIVES: Sarcopenia is associated with a poor prognosis in the ICU. The purpose of this study was to describe a simple sarcopenia index using routinely available renal biomarkers and evaluate its association with muscle mass and patient outcomes. DESIGN: A retrospective cohort study. SETTING: A tertiary-care medical center. PATIENTS: High-risk adult ICU patients from October 2008 to December 2010. INTERVENTIONS: The gold standard for muscle mass was quantified with the paraspinal muscle surface area at the L4 vertebrae in the subset of individuals with an abdominal CT scan. Using Pearson's correlation coefficient, serum creatinine-to-serum cystatin C ratio was found to be the best performer in the estimation of muscle mass. The relationship between sarcopenia index and hospital and 90-day mortality, and the length of mechanical ventilation was evaluated. MEASUREMENTS AND MAIN RESULTS: Out of 226 enrolled patients, 123 (54%) were female, and 198 (87%) were white. Median (interquartile range) age, body mass index, and body surface area were 68 (57-77) years, 28 (24-34) kg/m, and 1.9 (1.7-2.2) m, respectively. The mean (± SD) Acute Physiology and Chronic Health Evaluation III was 70 (± 22). ICU, hospital, and 90-day mortality rates were 5%, 12%, and 20%, respectively. The correlation (r) between sarcopenia index and muscle mass was 0.62 and coefficient of determination (r) was 0.27 (p < 0.0001). After adjustment for Acute Physiology and Chronic Health Evaluation III, body surface area, and age, sarcopenia index was independently predictive of both hospital (p = 0.001) and 90-day mortality (p < 0.0001). Among the 131 patients on mechanical ventilator, the duration of mechanical ventilation was significantly lower on those with higher sarcopenia index (-1 d for each 10 unit of sarcopenia index [95% CI, -1.4 to -0.2; p = 0.006]). CONCLUSIONS: The sarcopenia index is a fair measure for muscle mass estimation among ICU patients and can modestly predict hospital and 90-day mortality among patients who do not have acute kidney injury at the time of measurement.

PURPOSE: Identification of clinical and laboratory predictors related to Infectious Complications (ICs) in patients after Out-of-Hospital Cardiac Arrest (OHCA). METHODS: Patients, aged >18, after an OHCA between 9/2013 and 11/2015, surviving >24h, were studied. RESULTS: Study group consisted of 42 patients (mean age 63.4years, 88.1% men). Forty percent of patients had IC; lower respiratory tract infections were most common (87.5% of cases). ICs were more common in patients receiving Targeted Temperature Management (50% vs. 10%; p=0.032). Antibiotics were used in 85.7% of patients. The mean time to therapy initiation was 9.6 (SD 7.1) hours after admission. The mean course of treatment was 9.0 (SD 6.2) days. Fifty-three percent of patients receiving early antibiotic treatment didn't have IC. Initial antibiotic therapy was changed more often in patients with IC (75% vs. 38.9%; p=0.045). C-Reactive Protein, Procalcitonin, Troponin and White Blood Cell count values were higher in patients with IC. CONCLUSION: Early initiated antibiotic treatment is overused in patients after OHCA. This practice is associated with necessitating antibiotic change in the majority of patients with IC. Assessment of clinical and laboratory parameters in the first days after OHCA increases the likelihood of appropriate ATB therapy.

BACKGROUND AND AIMS: Delirium is an acute brain dysfunction and a frequent complication in critically ill patients. When present it significantly worsens the prognosis of patients. The aim of this study was to evaluate the incidence of delirium and risk factors for delirium in a mixed group of trauma, medical and surgical ICU patients. METHODS: A prospective observational study was conducted in one of the six-bed Intensive Care Units of the University Hospital Ostrava in the Czech Republic during a 12-month period. We evaluated the incidence of delirium and its predisposing and precipitating risk factors. All patients were assessed daily using the Confusion Assessment Method for the ICU (CAM-ICU). RESULTS AND CONCLUSIONS: Of the total of 332 patients with a median APACHE II (the Acute Physiology and Chronic Health Evaluation) score of 12, who were evaluated for delirium, 48 could not be assessed using CAM-ICU (47 due to prolonged coma, 1 due to language barriers). The incidence of delirium was 26.1%, with trauma and medical patients being more likely to develop delirium than surgical patients. Risk of delirium was significantly associated with age ≥ 65 years, and alcohol abuse in their anamnesis, with APACHE II score on admission, and with the use of sedatives and/or vasopressors. Delirious patients who remained in the ICU for a prolonged period showed a greater need for ventilator support and had a greater ICU-mortality.
