A common problem in management of polytrauma - a simultaneous injury to more than one organ or organ system, at least one of them lethal without intervention - is a discrepancy between a relatively good initial state and a serious subsequent development. Since nitric oxide (NO) is produced in high quantities during tissue injury, we assumed that serum levels of NO (and its oxidation products, NOx) might serve as a prognostic marker of polytrauma severity. However, we found recently that NOx was increased in polytrauma, but not in the most severe cases. The present study was undertaken to test the hypothesis that serum NOx is reduced in severe polytrauma by concomitant overproduction of reactive oxygen species (ROS). Polytrauma was induced in rats under anesthesia by bilateral fracture of femurs and tibiae plus incision of the right liver lobe through laparotomy. Serum NOx was measured by chemiluminescence after hot acidic reduction. The role of ROS was assessed by treatment with an antioxidant, N-acetyl-L-cysteine (NAC). Experimental polytrauma elevated NOx from 11.0+/-0.7 to 23.8+/-4.5 ppb. This was completely prevented by NAC treatment (9.1+/-2.2 ppb). Serum NOx is elevated in severe polytrauma, and this is not reduced by ROS. On the contrary, ROS are necessary for the NOx elevation, probably because ROS produced by inflammatory cells activated by the polytrauma induce massive NO production.

Birth is an event that exposes the newborn baby to a high concentration of free radicals (reactive oxygen species (ROS)/reactive nitrogen species (RNS)), which can contribute to several diseases. The balanced equilibrium between ROS/RNS and the antioxidant system of the newborn is a key factor in preventing a plethora of diseases. The antioxidant system involves endogenous and exogenous molecules, from vitamins (A, E, C) to enzymes (glutathione peroxidase, superoxide dismutase), metals (copper, zinc, and selenium) and other molecules (coenzyme Q10, melatonin) that can act in a synergistic manner to deactivate free radicals. A competent antioxidant system of the baby is strongly dependent on the intake of free radical deactivating molecules from feeding, either maternal or formula milk, with several studies pointing that breast milk has more powerful antioxidant effects on lowering the infant's oxidative status. An improved understanding of the antioxidant molecules, their mechanism of action, and the relationships between them, are key factors to comprehend all the potential benefits of human breastfeeding in this matter. The purpose of this review is to describe different research efforts and methodologies of evaluation of TAS in human milk, and to evaluate and summarize the contribution of different antioxidant molecules.

The present study has examined the effect of free radical spin trap N-tert-butyl-alpha-phenylnitrone (PBN) in the model of seizures induced in immature 12-day-old rats by bilateral intracerebroventricular infusion of dl-homocysteic acid (dl-HCA, 600 nmol/side). PBN was given i.p. in two doses (100 mg/kg each), 30 min prior and 30 min after dl-HCA infusion. PBN did not significantly influence the severity of seizures, evident both from the behavioral symptoms and EEG recordings. PBN normalized decreased ATP levels in the hippocampus, occurring during the acute phase of seizures ( approximately 45-50 min after infusion) and persisting until the end of the 24-h recovery period. PBN also led to normalization of decreased glucose levels and to a significant reduction of lactate accumulation in the cerebral cortex and hippocampus. The neuroprotective effect of PBN was evaluated after 24 h and 6 days of survival following dl-HCA-induced seizures (Nissl and Fluoro-Jade B staining). The administration of PBN resulted in a partial amelioration of severe damage observed in many brain regions following infusion of dl-HCA alone. The data suggest that increased free radical production is apparently occurring during seizures induced in immature rats by homocysteic acid. Free radical scavenger PBN had a clear-cut protective effect, evident as the improved recovery of brain energy status and as a partial, but significant, attenuation of neuronal degeneration associated with this model of seizures.

Z mnoha teorií multifaktoriálního procesu stárnutí patří k nejdůležitějším i volně radikálová teorie. Vysvětluje etiologii a patogenezu některých chorob a poruch během stárnutí. Volné radikály poškozují lipidy, proteiny, glycidy a DNA. Ve stáří klesá antioxidační obrana tvořená zvláště redukovaným glutationem, glutationperoxidázou, katalázou, karotenoidy, estrogeny. Mg, Se, Zn a podobně se snižuje i schopnost opravovat poškozené biomolekuly. Naopak stoupá intracelulární vápník, oxidované nebo modifikované LDL, produkty lipoperoxidace, aldehydy, oxidované proteiny, homocystein a ferritin. Volné radikály se podílejí na vzniku a průběhu mnoha onemocnění, jako jsou např. diabetes mellitus, zhoubné nádory, ateroskleróza, katarakta, makulární degenerace, choroby kůže, zubů, uší, degenerativní choroby mozku - Alzheimerova a Parkinsonova choroba, amyotrofická laterální skleróza, roztroušená skleróza, ale i na revmatoidní artritída a další. Volné radikály mohou mít vliv i na osteoporózu a snižují také imunitu. Ještě není dostatečně prokázáno, zda antioxidanty prodlužují život, ale určité chrání před předčasnou smrtí. Budoucí výzkum různých směsí antioxidantů a jejich preventivní používání pravděpodobně přispěje i k prodloužení života.
There are many theories of the multifactorial processes of aging. One of the most important represents the free radical theory. It explains at least several diseases and disturbances during aging. Free radicals destroy lipids, proteins, sacharides and DNA. Antioxidant defence decreases with age, especially are reduced glutathion, glutathion peroxidase, catalase, carotenoids, estrogens, Mg, Se, Zn and others, as well as the abiUty to repair the injured biomolecules. On the other hand there is increasing of intracellular calcium, oxidized or modified LDL, products of lipoperoxidation, aldehydes, oxidised proteins, homocystein, ferritin. Free radicals take part in the etiology and pathogenesis in many diseases, especially in diabetes mellitus, malignant tumors, atherosclerosis, cataract, macular degeneration, in the diseases of skin, teeth, ears, in degenerative diseases of brain like Alzheimer's and Parkinson's diseases, amyotrophical lateral sclerosis, sclerosis multiplex, in rheumatoid arthritis and the others. Free radicals may take part also in the pathogeny of osteoporosis, they also decrease immunity. It is not yet satisfactory demonstrated, that antioxidants prolong the live, but they protect against premature death. The future research of different mixtures of antioxidants and their preventive use may lead also to the extension of the life span.

UV záření způsobuje v lidské kůži zvýšenou produkci vysoce reaktivních kyslíkových radikálů, které navozují v cílové tkáni oxidační stres a poškozují důležité buněčné struktury. Hlavním ochranným systémem kůže proti oxidačnímu poškození je komplex antioxidantů. Antioxidanty, především vitamin E, mohou být užívány jako fotoprotektiva.
Ultraviolet radiation causes increase of production of the reactive oxygen species in the human skin, which are responsible for oxidative stress and damage of many important cellular components in the target tissue. The complex of antioxidants represents the major protective system against the oxidative damage of the skin. Many antioxidants, namely vitamin E, can be succesfully used in photoprotection.
