BACKGROUND: Charcot-Marie-Tooth is the most common inherited neuromuscular disorder. Rarely, it can be associated with heart failure and various arrhythmic disturbances. This case illustrates the challenges of making decisions to prevent sudden cardiac death in a patient with Charcot-Marie-Tooth disease. CASE SUMMARY: A 69-year-old male with a history of Type 1A Charcot-Marie-Tooth disease was admitted due to repetitive runs of ventricular tachycardia. Twelve-lead electrocardiogram, echocardiography, selective coronary angiography, and cardiac magnetic resonance did not clarify the cause of the electrical storm. As conservative therapy was not successful, radiofrequency ablation was chosen to treat the electrical storm. After this procedure, implantable cardioverter defibrillator (ICD) was implanted. The follow-up revealed severe perforation by the ventricular lead. An extraction was performed with no complications and a new lead was immediately implanted. The patient remains asymptomatic. Three episodes of non-sustained ventricular tachycardia were recorded during the last follow-up. DISCUSSION: This case illustrates the challenges of making decisions to prevent sudden cardiac death in a patient with Charcot-Marie-Tooth disease after successful ablation for electrical storm. Due to a lack of evidence, atypical origin of arrhythmia, and clinical presentation, we did not consider this as idiopathic arrhythmia and decided to implant an ICD, which was complicated by severe perforation by the lead. Specific recommendations for preventing sudden cardiac death in rare cardiac conditions, such as Charcot-Marie-Tooth disease, still need to be refined.

Global obesity rates have risen dramatically, now exceeding deaths from starvation. Metabolic and bariatric surgery (MBS), initially for severe obesity (BMI ≥35 kg/m2), is performed globally over 500 000 times annually, offering significant metabolic benefits beyond weight loss. However, varying eligibility criteria globally impact patient care and healthcare resources. Updated in 2022, ASMBS and IFSO guidelines aim to standardise MBS indications, reflecting current understanding and emphasising comprehensive preoperative assessments. Yet, clinical variability persists, necessitating consensus-based recommendations. This modified Delphi study engaged 45 global experts to establish consensus on perioperative management in MBS. Experts selected from bariatric societies possessed expertise in MBS and participated in a two-round Delphi protocol. Consensus was achieved on 90 of 169 statements (53.3%), encompassing multidisciplinary team composition, patient selection criteria, preoperative testing, and referral pathways. The agreement highlighted the critical role of comprehensive preoperative assessments and the integration of healthcare professionals in MBS. These findings offer essential insights to standardise perioperative practices and advocate for evidence-based guidelines in MBS globally. The study underscores the need for unified protocols to optimise outcomes and guide future research in MBS.
The endorsement of conspiracy theories may be increased by subjectively perceived stress. Yet, it is not known whether this correlation is caused by the effects of the acute stress reaction on the brain or other psychological, social, or methodological factors. The effect of an experimentally induced acute stress reaction on conspiracy thinking was tested on a sample (n = 115) of students of medicine. Although the stress procedure caused a substantial increase in salivary cortisol, there was no significant effect on endorsing conspiracy theories or adopting conspiracy interpretations of novel information. The results confirmed no effect of the acute stress reaction on conspiracy thinking, suggesting it may be absent or weaker than expected. The study demonstrated the viability of psychophysiological experimental design in conspiracy research and may inspire further examination of the physiological mechanisms underlying susceptibility to conspiracy theories.

BACKGROUND: A third of endovascularly treated patients with stroke experience incomplete reperfusion (expanded Thrombolysis in Cerebral Infarction [eTICI] <3), and the natural evolution of this incomplete reperfusion remains unknown. We systematically reviewed the literature and performed a meta-analysis on the natural evolution of incomplete reperfusion after endovascular therapy. METHODS: A systematic review of MEDLINE, Embase, and PubMed up until March 1, 2024, using a predefined strategy. Only full-text English-written articles reporting rates of either favorable (ie, delayed reperfusion (DR) or no new infarct) or unfavorable progression (ie, persistent perfusion deficit or new infarct) of incompletely reperfused tissue were included. The primary outcome was the rate of DR and its association with functional independence (modified Rankin Scale score, 0-2) at 90 days postintervention. Pooled odds ratios with 95% CIs were calculated using a random-effects model. RESULTS: Six studies involving 950 patients (50.7% female; median age, 71 years; interquartile range, 60-79) were included. Four studies assessed the evolution of incomplete reperfusion on magnetic resonance imaging perfusion imaging, while 2 studies used diffusion-weighted imaging and noncontrast computed tomography imaging, where new infarct was used to denote unfavorable progression. Five studies defined incomplete reperfusion as eTICI 2b50 or 2c. DR occurred in 41% (interquartile range, 33%-51%) of cases 24 hours postintervention. Achieving DR was associated with a higher likelihood of functional independence at 90 days (odds ratio, 2.5 [95% CI, 1.9-3.4]). CONCLUSIONS: Nearly half of eTICI <3 patients achieve DR, leading to favorable clinical outcomes. This subgroup may derive limited or potentially harmful effects from pursuing additional reperfusion strategies (eg, intra-arterial lytics or secondary thrombectomy). Accurately predicting the evolution of incomplete reperfusion could optimize patient selection for adjunctive reperfusion strategies at the end of an intervention. BACKGROUND AND OBJECTIVE: Non-muscle-invasive bladder cancer (NMIBC) poses a significant clinical challenge, particularly when failing bacillus Calmette-Guérin (BCG) therapy, necessitating alternative treatments. Despite radical cystectomy being the recommended treatment, many patients are unfit or unwilling to undergo this invasive procedure, highlighting the need for effective bladder-sparing therapies. This review aims to summarize and report the evidence on the efficacy and to estimate the costs of bladder-preserving strategies used in NMIBC recurrence after failure of intravesical BCG therapy. METHODS: We systematically searched online databases for prospective studies investigating intravesical therapy, systemic therapy, or combination of both in patients treated previously with BCG. Owing to significant heterogeneity across the studies, a meta-analysis was inappropriate. A sensitivity analysis was performed in an exploratory manner. We used a decision-analytic Markov model to compare novel U.S. Food and Drug Administration-approved treatments with a 2-yr time horizon. KEY FINDINGS AND LIMITATIONS: A total of 57 studies published between 1998 and 2024, with 68 unique study arms and consisting of 2589 patients, were identified. The 3-mo overall response rate (ORR) across all studies, complete response rate (CRR) in concomitant carcinoma in situ (CIS) or CIS only disease, and recurrence-free rate (RFR) in papillary disease were estimated to be 52.4% (95% confidence interval [CI]: 45.4-59.2), 52.8% (95% CI: 42.9-62.6), and 26.4% (95% CI: 13.3-45.6), respectively. The 12-mo ORR, CRR, and RFR were estimated to be 78% (95% CI: 52.9-91.8), 27.8% (95% CI: 21.3-35.4), and 25.4% (95% CI: 18.2-34.2), respectively. The progression rate was estimated to be 13% (95% CI: 9-18.2). The mean proportion of patients treated with radical cystectomy was estimated to be 24.7 (range 0-85.7). The reported toxicity grades were overall mild, with a median of 3.4% (range 0-33.3%) participants experiencing a dose limiting toxicity. Compared with using radical cystectomy to treat patients failing BCG therapy, at a willingness-to-pay threshold of 100 000 USD, nadofaragene firadenovec was cost effective, with an incremental cost-effectiveness ratio (ICER) of 10 014 USD per quality-adjusted life year (QALY), while nogapendekin alfa inbakicept was less cost effective than nadofaragene firadenovec (ICER of 44 602 USD per QALY). Pembrolizumab, which dominated, was both less costly and more effective than the other strategies. CONCLUSIONS AND CLINICAL IMPLICATIONS: Salvage bladder-sparing therapies show a response rate of around 50% at 3 mo in patients with NMIBC failing BCG. However, long-term data are heterogeneous. Nevertheless, recently developed agents show promising tumor control activity. In the rapidly evolving landscape of urothelial cancer, some of these treatment strategies might be cost effective and improve patients' quality of life. The findings of our review highlight the need for novel, more effective therapeutic strategies. OBJECTIVE: To evaluate the role of clinical exchange programs in postgraduate obgyn training using the International Federation of Gynecology and Obstetrics (FIGO)-World Association of Trainees in Obstetrics and Gynecology (WATOG) One World Exchange (OWE), a clinical exchange program held in France in October 2023, as a case-study. METHODS: This was a cross-sectional study. A 31-item structured questionnaire designed with Google Forms was electronically distributed to the 51 obgyn postgraduate trainees (OWE fellows) who participated in the OWE, to collect information about the exchange. Collected data was analyzed using IBM Statistical Product and Service Solutions (SPSS) Statistics for Windows. RESULTS: The survey response rate was 68.6%. The mean age of the respondents was 33.0 ± 4.0 years. Majority of the them were females (26, 74.3%), married (19, 54.3%), at least in their third year of training (30, 85.7%) and from Africa (11, 31.4%). During the period of the exchange program, fellows observed various obstetric and gynecologic procedures, including open and minimal access procedures, with more than one-fifth (8, 22.9%) of them reporting that they were allowed to assist in some of these procedures. The fellows noted salient differences in practice between their exchange hospitals and their home countries. An overwhelming majority (30, 85.7%) of the fellows believed the OWE was beneficial and would positively impact their clinical practices back in their home countries. CONCLUSION: Clinical exchange programs like the OWE provide valuable benefits in improving the clinical knowledge and skills of postgraduate obgyn trainees.

AIM: Despite the high sensitivity of neonatal screening in detecting the classical form of congenital adrenal hyperplasia due to 21-hydroxylase deficiency, one of the unclear issues is identifying asymptomatic children with late onset forms. The aim of this nationwide study was to analyse the association between genotype and screened level of 17-hydroxyprogesterone in patients with the late onset form of 21-hydroxylase deficiency and to quantify false negativity. METHODS: In the Czech Republic, 1,866,129 neonates were screened (2006-2022). Among this cohort, 159 patients were confirmed to suffer from 21-hydroxylase deficiency, employing the 17-hydroxyprogesterone birthweight/gestational age-adjusted cut-off limits, and followed by the genetic confirmation. The screening prevalence was 1:11,737. Another 57 patients who were false negative in neonatal screening were added to this cohort based on later diagnosis by clinical suspicion. To our knowledge, such a huge nationwide cohort of false negative patients has not been documented before. RESULTS: Overall, 57 patients escaped from neonatal screening in the monitored period. All false negative patients had milder forms. Only one patient had simple virilising form and 56 patients had the late onset form. The probability of false negativity in the late onset form was 76.7%. The difference in 17-hydroxyprogesterone screening values was statistically significant (p<0.001) between severe forms (median 478.8 nmol/L) and milder (36.2 nmol/L) forms. Interestingly, the higher proportion of females with milder forms was statistically significant compared with the general population. CONCLUSIONS: A negative neonatal screening result does not exclude milder forms of 21-hydroxylase deficiency during the differential diagnostic procedure of children with precocious pseudopuberty.

BACKGROUND AND OBJECTIVES: En bloc sacrectomy is associated with sacral root transection causing loss of urinary bladder, rectum, and sexual function. The aim of the study was to determine the position of the pudendal branches (sensorimotor) and pelvic splanchnic nerves (parasympathetic) on the sacral roots relative to the sacrum, and the minimal and maximal defects in the sacral roots that can be reconstructed by grafting after various types of sacrectomy. METHODS: Five cadaveric pelves were dissected bilaterally. The lengths and widths of the S1-S4 roots and their branches were measured. Then, the minimal and maximal defects between the proximal and distal stumps of the sacrificed roots were measured following 3 models of sacrectomy (below S2, below S1, and total sacrectomy). RESULTS: The mean distance of the splanchnic nerves from the S2 and S3 anterior sacral foramina was 17.7 ± 7.3 and 23.6 ± 11.1 mm, respectively, and the mean distance of the pudendal S2 and S3 branches was 36.8 ± 13.7 and 30.2 ± 10.8 mm, respectively. The mean widths of the S2 and S3 roots were 9.3 ± 1.9 and 5.4 ± 1.2 mm, respectively. The mean maximal defects in S2 and S3 roots after various types of sacrectomies were between 61.8 ± 16.3 and 100.7 ± 14.3 mm and between 62.7 ± 20.2 and 84.7 ± 25.1 mm, respectively. There were no statistically significant differences between sides or sexes for all obtained measurements. CONCLUSION: The reconstruction of the S2-S3 roots is anatomically feasible after partial or total sacrectomies in which the resection of the soft tissue does not extend further than approximately 1.5 to 2 cm ventrally from the sacrum.

Cíl: V počtech mechanické trombektomie (MT) symptomatického uzávěru velké mozkové tepny u pacientů s akutní ischemickou cévní moz- kovou příhodou (iCMP) na počet obyvatel se řadí Česká republika na přední místa v Evropě. Cílem práce bylo na základě analýzy dat studie METRICS II (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centers II) zhodnotit aktuální výsledky léčby iCMP a vývoj plnění indikátorů kvality napříč republikou. Sekundárním cílem práce byla subanalýza souboru z hlediska použité techniky MT a jejího vlivu na úspěšnost rekanalizace uzávěru velké mozkové tepny. Metodika: Národní prospektivní observační studie METRICS II probíhala v České republice od 1. ledna 2023 do 31. prosince 2023. Studie se zúčastnilo jedenáct z patnácti center provádějících MT. Všechna technická a klinická data byla sbírána prospektivně. Výsledky léčby iCMP byly následně srovnány s doporučenými hodnotami jednotlivých parametrů multioborového konsenzu a výsledky studie METRICS probíhající v roce 2019. Data vztahující se k vlastní technice MT v předním povodí byla podrobena subanalýze a statisticky zpracována z hlediska vlivu použité techniky na úspěšnost rekanalizace uzávěru velké mozkové tepny. Výsledky: V práci jsou analyzována data z jedenácti center, ve kterých byla mechanická trombektomie provedena celkem 1312 pacientům (51,1 % mužů) průměrného věku 70,9 roků (SD ± 12,7). Intravenózní trombolýzou (IVT) byli před MT léčeni 894 nemocní (68,2 %) a 426 (32,5 %) bylo sekundárně transportováno k MT z jiného centra. Získaná data z národní prospektivní observační studie METRICS II byla hodnocena jako celek i pro jednotlivá centra. Shoda výsledků péče s doporučeními navrženými multioborovým konsenzem byla velmi vysoká. Sedm center splnilo parametry všech doporučení. Ostatní centra nesplnila pouze ojedinělá kritéria a ve většině případů jen hraničně. Statistickým zpracováním souboru MT v přední cirkulaci byla prokázána signifikantní závislost mezi typem MT a dosažením rekanalizace (TICI 2b–3, p = 0,0004, resp. TICI 2c/3, p < 0,0001). Závěr: Výsledky studie METRICS II ukázaly vysokou kvalitu péče o pacienty s uzávěrem velké mozkové tepny léčených MT. Statistickým zpracováním souboru MT v přední cirkulaci byla prokázána signifikantní závislost mezi typem MT a dosažením rekanalizace. Pro skupinu pacientů, u kterých proběhla MT kombinací aspirace a rekanalizace za pomoci stentretrieveru sekundárně, byla nalezena signifikantně nižší úspěšnost rekanalizace uzávěru velké mozkové tepny.
Aim: The Czech Republic is among the leading countries in Europe in the number of mechanical thrombectomy (MT) procedures for symptomatic large vessel occlusion (LVO) in patients with acute ischemic stroke per population. The aim of this study was to evaluate the current outcomes of acute ischemic stroke treatment and the development of treatment quality indicators in the Czech Republic based on the analysis of data from the METRICS II study (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centres II). The secondary aim of the study was to subanalyze the cohort in terms of the MT technique used and its impact on the success of recanalisation of the LVO. Methods: The national prospective observational study METRICS II was conducted in the Czech Republic from 1 January 2023 to 31 December 2023. Eleven of the 15 centres performing MT participated in the study. All technical and clinical data were collected prospectively. The results of ischemic stroke treatment were then compared with the recommended values of the individual parameters of the multidisciplinary consensus and the results of the METRICS study ongoing in 2019. Data related to the actual technique of MT in the anterior circulation were subanalyzed and statistically processed for the effect of the technique used on the success of recanalization of the LVO. Results: Data from 11 centres in which mechanical thrombectomy was performed on a total of 1312 patients (51.1% male) with a mean age of 70.9 years (SD ± 12.7) were analysed. IV thrombolysis prior MT was performed in 894 (68.2%) patients and 426 (32.5%) patients were transferred secondarily for MT from primary stroke centers to dedicated comprehensive centers. Data obtained from the national prospective observational METRICS II study were evaluated as a whole and for individual centers. The concordance of results with the recommendations of multi- society consensus was very high. Seven centers met the parameters of all recommendations. The remaining centers failed to meet only isolated criteria and only marginally. Statistical analysis of the anterior circulation MT cohort showed a significant relationship between MT type and achievement of recanalization (TICI 2b-3, p = 0.0004 and TICI 2c/3, p < 0.0001, respectively). Conclusion: The results of METRICS II showed a high quality of care for patients with large cerebral artery occlusion treated with MT. Statistical analysis of a cohort of anterior circulation MTs showed a significant relation- ship between the type of MT and the achievement of recanalisation. A significantly lower success rate of recanalization of LVO was found for the group of patients who underwent MT by a combination of aspiration and recanalization using a stent retriever secondarily.
Cíl: V retrospektivní studii jsme porovnali efektivitu navigace punkce portální žíly pomocí abdominální ultrasonografie oproti CO2 portografii. Efektivitu navigace jsme posuzovali dle počtu punkcí nutných k dosažení katetrizace větve vena portae, délkou skiaskopie a radiační dávkou charakterizovanou produktem dávkové plochy (DAP). Metodika: Do studie bylo zahrnuto 102 po sobě jdoucích pacientů, kterým byl proveden TIPS v období 3 let (od počátku roku 2022 do konce roku 2024). V souboru bylo 29 žen (28 %) a 73 mužů (72 %), ve věkovém rozmezí 27–81 let. Hlavní indikací k TIPS byl refrakterní ascites u 52 (51 %) a krvácení při portální hypertenzi u 50 pacientů (49 %). TIPS byl proveden jako urgentní výkon u 19 pacientů (v 18,6 %) pro nezastavené variceální krvácení. Childovo-Pughovo skóre A bylo 22 % pacientů, Childovo-Pughovo skóre B 44 % a Childovo-Pughovo skóre C 26 %, u 8 % pacientů nebyla klasifikace zjistitelná. Výsledky: Ze 102 TIPS výkonů nebyl tento zaveden u tří pacientů pro technické selhání (2,9 %). Počet vpichů nutných k dosažení portální žíly se zavedením vodiče byl v průměru 5,6 (od jednoho do 20) u 50 pacientů, jejichž punkce byla naváděna pomocí CO2 portografie. U 28 výkonů, kde byl k navádění použit UZ, bylo průměrně nutných 2,7 vpichů (od jednoho do šesti). Délka skiaskopie byla u 28 výkonů provedených pomocí navigace CO2 v průměru 28 minut (9–50 min), u 28 výkonů s navigací pod UZ to bylo 22,8 min (8–43 min). Ve skupině 19 výkonů před obměnou angiolinky a s CO2 navigací byl v průměru DAP 181 497 mGy/cm2 (27 649–399 657 mGy/cm2 ), ve skupině 17 výkonů po obměně angiolinky a navigovaných pomocí CO2 portografie byl DAP v průměru 159 339 mGy/cm2 (52 613–309 440 mGy/cm2 ) a v časově nejmladší skupině 21 výkonů navigovaných pomocí UZ byl DAP v průměru 120 731 mGy/cm 2 (38 180–315 121 mGy/cm2 ). U sedmi výkonů bylo provedeno měření dávky osobními dozimetry. V části výkonu do zavedení vodiče do portální žíly operatér měl dávku v průměru 6 μSv (0 do 12 μSv), zatímco lékař provádějící UZ navigaci v průměru 4,4 μSv (0 do 9 μSv). Celková dávka pro operatéra byla v průměru 19,5 μSv (8–35 μSv). Závěr: Z naměřených hodnot lze uzavřít, že počet vpichů při navigaci pomocí UZ klesl v průměru z 5,6 na 2,7, tento rozdíl se velmi blíží statistické významnosti (p = 0,052). Skiaskopický čas při výkonu klesl v průměru o 7 minut. Dávka druhého lékaře navigujícího punkci pomocí UZ je do doby katetrizace por- tální žíly srovnatelná s dávkou operatéra. DAP klesl po inovaci přístroje a na novém přístroji a dále ještě více klesl i u výkonů prováděných s navigací punkce pomocí UZ.
Aim: In our retrospective study, we compared the efficacy of portal vein puncture navigation using abdominal ultrasonography versus CO2 portography. We assessed navigation efficiency according to the number of punctures required to achieve vena portae branch catheterization, the length of the fluoroscopy, and the dose area product (DAP). Method: The study included 102 consecutive patients who underwent TIPS over a 3-year period (from the beginning of 2022 to the end of 2024). In this cohort, there were 29 women (28%) and 73 men (72%), ranging in age from 27 to 81 years. The main indications for TIPS were refractory ascites in 52 patients (51%) and bleeding with portal hypertension in 50 patients (49%). TIPS was performed as an emergency procedure in 19 patients (18.6%) for uncontrolled variceal bleeding. Child Pugh A, Child Pugh B and Child Pugh C were 22%, 44% and 26% respectively, with 8% of patients having no detectable classification. Results: Of the 102 TIPS procedures, TIPS was not inserted in 3 patients due to technical failure (2.9%). The number of punctures required to reach the portal vein with guidewire insertion averaged 5.6 (range 1 to 20) in 50 patients whose puncture was guided by CO2 portography. In the 28 procedures where ultrasound was used for guidance, the mean number of punctures required was 2.7 (range, 1 to 6). The mean duration of fluoroscopy was 28 min (9–50 min) in the 28 procedures performed with CO2 guidance and 22.8 min (8–43 min) in the 28 procedures with ultrasound guidance. In the group of 19 procedures before angiomachine exchange and with CO2 navigation, the mean DAP was 181 497 mGy/ cm2 (27 649–399 657 mGy/cm2 ), in the group of 17 procedures after angiomachine exchange and navigated by CO 2 portography, the DAP averaged 159 339 mGy/cm 2 (52 613–309 440 mGy/cm2 ) and in the youngest group of 21 procedures navigated by ultrasound, the DAP averaged 120 731 mGy/cm2 (38 180–315 121 mGy/cm 2 ). Dose measurements with personal dosimeters were performed for 7 procedures. In the part of the procedure until the insertion of the guidewire into the portal vein, the operator had a dose of 6 μSv (0 to 12 μSv) on average, while the physician performing the ultrasound navigation had a dose of 4.4 μSv (0 to 9 μSv) on average. The total dose to the operator averaged 19.5 μSv (8 to 35 μSv). Conclusion: From the measured values it can be concluded that the number of punctures during navigation with US decreased from 5.6 to 2.7 on average, this result is close to statistic significance (p = 0.052). The fluoroscopic time during the procedure decreased by 7 minutes. However, the dose of the second physician navigating the puncture using ultrasound is comparable to that of the main operator. The DAP decreased after the device upgrade, and even more decreased for procedures performed with ultrasound-guided puncture navigation.
