Práce se zabývá základními nástroji hodnocení efektivity ve zdravotnictví a jejich možnou aplikací do zdravotního systému. Zabývá se definicí efektivnosti a jejím terminologickým odlišením od účinnosti. Stanovuje základní druhy efektivnosti a typy nákladové analýzy, která se v oblasti zdravotnictví používá. Druhá část práce nabízí pohled na jednotlivé hráče zdravotního systému a jejich možnost zapojit analýzu efektivnosti a jejich výstupy do vlastní racionální činnosti. Ukazuje se, že analýza efektivnosti není jen prvkem, který by měl být zaveden do reformy zdravotnictví, ale i samotným nástrojem reformy, který usnadní racionální rozhodování.

This paper treats basic tools of efficiency evaluation and their implementation into health-care system. It presents a definition of "efficiency", distinguished from effectiveness or efficacy. It presents various types of efficiency and efficiency analysis used in health care. Second part of the paper is dedicated to the "market players" of the system and their capability to implement the results of efficiency analysis into their own rational behavior. It shows that the efficiency analysis might not be only part of a health-care system transformation, but even become its basic tool, facilitating the rationale of decision-making.

Efektivnost českého zdravotnictví byla hodnocena indexem efektivnosti, který obsahuje 4 vstupy a 3 výstupy zdravotnického systému. Váhy zdravotnických vstupů a výstupů byly získány na základě dotazníkového šetření mezi experty. Mezinárodní srovnání ukazuje, že efektivnost českého zdravotnictví v období 2010–2021 dlouhodobě klesá. Ovšem stejný trend lze pozorovat i v dalších evropských zemích.

The efficiency of the Czech health care system was evaluated by the efficiency index, which contains 4 inputs and 3 outputs of the health care system. The weights of medical inputs and outputs were obtained on the basis of a questionnaire survey among experts. International comparisons show that the efficiency of the Czech healthcare system has been declining in the long term in the period 2010–2021. However, the same trend can be observed in other European countries.

Healthcare is a highly sophisticated segment of the public sector, which requires not only highly professional and competent staff, but also a properly set ratio of healthcare professionals. In the Czech Republic, the state, as the main guarantor of health care, applied strong control through price and volume control. The aim of the paper is to define the differences in the technical efficiency of public hospitals, with regard to the size of hospitals and partial types of human resources. An input-oriented Data Envelopment Analysis model (DEA model) was chosen for modeling the technical efficiency of 47 public hospitals. The personnel performance concept of the evaluation of technical efficiency was further implemented in eight specific models, from the perspective of individual input variables relative to output variables and according to different assumptions regarding the character of economies of scale. The results of technical efficiency were analyzed using correlation, regression analysis, and the Bootstrap method. The least efficient hospitals in terms of hospital size are large hospitals, and the most balanced results have been achieved by medium-sized hospitals. The average efficiency rate in models that include all selected input and output variables is highest in medium-sized hospitals, with a value of 0.866 for CRS and an efficiency rate of 0.926 for VRS. The rationalization of human resources should be implemented in order not to reduce the quality of care provided.

Cíl práce: Shrnutí přístupů k vykazování a hodnocení výkonů asistované reprodukce.Typ práce: Přehledný článek.Název a sídlo pracoviště: Centrum asistované reprodukce, Gynekologicko-porodnická klinika 1. LF UK a VFN v Praze.Metodika: V databázích pracovišť, v celostátních a mezinárodních registrech sledujeme data o cyklech in vitro fertilizace a příbuzných metodách. Cílem sledování je především zvyšování efektivity.Výsledky: Výstupy z registrů informují o absolutních počtech cyklů, porodů, narozených dětí a o relativních vztazích (implantation rate, pregnancy rate), které vyjadřují úspěšnost léčby.Závěr: Hodnocení výstupů musí brát v úvahu vliv věku žen, počet transferovaných embryí a další faktory.

Objective: Summarize different ways of assisted reproduction performance evaluation.Design: Review.Setting: Centre for Assisted Reproduction, Department of Gynaecology and Obstetrics, 1st Faculty of Medicine, Charles University, Prague.Methods: In databases of assisted reproduction centres, in national and international registers we evaluate data concerning in vitro fertilisation and related methods. Target of this evaluation is, first of all, to increase efficiency.Results: Outputs of all the registers inform about absolute numbers of cycles, deliveries, born babies and about relative parameters (implantation rate, pregnancy rate), describing the efficiency of the treatment.Conclusion: The output evaluation must consider the influence of the age of female, number of transferred embryos and other factors.
