IMPORTANCE: The successful implementation of artificial intelligence (AI) in health care depends on its acceptance by key stakeholders, particularly patients, who are the primary beneficiaries of AI-driven outcomes. OBJECTIVES: To survey hospital patients to investigate their trust, concerns, and preferences toward the use of AI in health care and diagnostics and to assess the sociodemographic factors associated with patient attitudes. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study developed and implemented an anonymous quantitative survey between February 1 and November 1, 2023, using a nonprobability sample at 74 hospitals in 43 countries. Participants included hospital patients 18 years of age or older who agreed with voluntary participation in the survey presented in 1 of 26 languages. EXPOSURE: Information sheets and paper surveys handed out by hospital staff and posted in conspicuous hospital locations. MAIN OUTCOMES AND MEASURES: The primary outcome was participant responses to a 26-item instrument containing a general data section (8 items) and 3 dimensions (trust in AI, AI and diagnosis, preferences and concerns toward AI) with 6 items each. Subgroup analyses used cumulative link mixed and binary mixed-effects models. RESULTS: In total, 13 806 patients participated, including 8951 (64.8%) in the Global North and 4855 (35.2%) in the Global South. Their median (IQR) age was 48 (34-62) years, and 6973 (50.5%) were male. The survey results indicated a predominantly favorable general view of AI in health care, with 57.6% of respondents (7775 of 13 502) expressing a positive attitude. However, attitudes exhibited notable variation based on demographic characteristics, health status, and technological literacy. Female respondents (3511 of 6318 [55.6%]) exhibited fewer positive attitudes toward AI use in medicine than male respondents (4057 of 6864 [59.1%]), and participants with poorer health status exhibited fewer positive attitudes toward AI use in medicine (eg, 58 of 199 [29.2%] with rather negative views) than patients with very good health (eg, 134 of 2538 [5.3%] with rather negative views). Conversely, higher levels of AI knowledge and frequent use of technology devices were associated with more positive attitudes. Notably, fewer than half of the participants expressed positive attitudes regarding all items pertaining to trust in AI. The lowest level of trust was observed for the accuracy of AI in providing information regarding treatment responses (5637 of 13 480 respondents [41.8%] trusted AI). Patients preferred explainable AI (8816 of 12 563 [70.2%]) and physician-led decision-making (9222 of 12 652 [72.9%]), even if it meant slightly compromised accuracy. CONCLUSIONS AND RELEVANCE: In this cross-sectional study of patient attitudes toward AI use in health care across 6 continents, findings indicated that tailored AI implementation strategies should take patient demographics, health status, and preferences for explainable AI and physician oversight into account.

INTRODUCTION: Retroperitoneal lymph node dissection (RPLND) is crucial in managing metastatic germ cell tumors (GCTs), particularly post-chemotherapy. Given the long-term survival of these patients, perioperative morbidity is a significant concern. However, data on RPLND morbidity using predefined reporting standards are scarce. This study aims to address this gap by utilizing updated European Association of Urology (EAU) guidelines for standardized complication reporting. PATIENTS AND METHODS: A retrospective analysis was conducted on patients who underwent RPLND for GCTs between 2010 and 2022. 30-day complications were extracted from digital charts using a predefined procedure-specific catalog. Complications were graded using the Clavien-Dindo classification (CDC), and the Comprehensive Complication Index (CCI) was calculated for each patient. RESULTS: Sixty-nine men underwent RPLND at a median age of 32 years (IQR 25-38). Chemotherapy was administered to 64 patients (93 %), with 48 (70 %) having negative tumor markers. Median tumor diameter was 52 mm (IQR 35-83), and median operative time was 197 min (IQR 128-262). Unilateral template removal was performed in 55 patients (80 %). A total of 157 complications were reported in 66 patients (96 %), with anemia (33 %) and gastrointestinal issues (24 %) being the most common. Five patients (7.2 %) had "major" complications (CDC grade ≥ IIIa), and the median CCI was 12 (IQR 9-23). Using the CCI, the proportion of patients with a "major" complication burden increased to 14 %, compared to 8.5 % by CDC alone. The primary limitation of this study is its retrospective design and the limited 30-day follow-up period. CONCLUSION: Most patients experience postoperative complications after RPLND, though severe complications are rare. These findings could improve patient counseling when discussing testicular cancer therapy options.

Úvod: Injekční užívání drog je spojeno se závažnou zdravotní zátěží, a to zejména díky vysokému riziku krví přenosných infekcí, jako jsou virová hepatitida typu C (VHC) nebo infekce virem lidské imunodeficience (HIV). Mezi doporučované intervence v prevenci a kontrole infekcí spojených s užíváním drog patří služby aplikačních místností (AM). AM je odborná adiktologická služba, kde si mohou lidé užívající drogy (PWUDs) aplikovat přinesenou drogu za bezpečnějších a hygienicky nezávadných podmínek. První AM v ČR byla otevřena v Brně v září 2023 v mobilní formě (MAM) v sociálně vyloučené lokalitě (SVL) s vysokým výskytem injekčního užívání heroinu a jiných opioidů, zejména v místní romské komunitě. Její spuštění provázel výzkum zaměřený na analýzu potřeb a nastavení služby. Tato práce představuje analýzu části kvantitativních dat zaměřenou na rizikové chování a další charakteristiky u potenciálních klientů programu. Materiál a metodika: Dvě dotazníkové průřezové studie (n = 131 a n = 135) provedené těsně před spuštěním a po spuštění MAM na souborech PWUDs zařazených do studie namátkovým výběrem prostřednictvím spolupracujících služeb a peer pracovníků v Brně. Analýza byla zaměřena na charakteristiky klientů v SVL, kde MAM působí. Párové souvislosti mezi příslušností k SVL a dalšími proměnnými byly analyzovány pomocí chí-kvadrát testu a t-testu. U statisticky významných prediktorů z párové analýzy byla provedena logistická regresní analýza s adjustací na pohlaví a věk. Výsledky jsou prezentovány jako adjusted odds ratio (AOR). Výsledky: V obou souborech tvořili 2/3 muži, průměrný věk byl kolem 37 let (36,5 let v prvním a 37,5 let ve druhém souboru), většinu tvořili lidé užívající drogy injekčně s poměrně častým výskytem rizikového chování. Ve druhém souboru bylo více osob, které se považují za Romy (50,7 % vs. 20,3 %), a více osob, jejichž primární drogou jsou opioidy (39,7 % vs. 27,5 %). Mezi respondenty žijícími v SVL bylo v prvním souboru statisticky významně častější užívání heroinu (AOR = 8,2) a opioidů (4,7) v posledním roce, aplikace drog ve vnitřním veřejném prostoru v posledních 30 dnech (3,6), častěji byli svědky předávkování (2,5) a byli odvezeni zdravotnickou záchrannou službou (2,6) v posledních 12 měsících. Respondenti ze SVL byli častěji klienty adiktologických služeb v posledním roce (2,8), především opiátové substituční léčby (4,6), ale vykazovali statisticky významně více bariér pro vstup do léčby. V souboru po spuštění MAM se potvrdila vyšší míra užívání opioidů (2,9) a nižší míra užívání pervitinu (0,3) a ukázal se vyšší výskyt VHC diagnostikované někdy v životě (3,0) mezi klienty ze SVL. V souboru před otevřením MAM měli klienti ze SVL menší povědomí o aplikačních místnostech obecně (0,4), ale v souboru po zahájení MAM uváděli větší ochotu ji využít (2,7). Závěr: Spuštění MAM v Brně bylo odůvodněné, neboť výskyt injekčního užívání drog včetně aplikace ve veřejném prostoru a výskyt dopadů s ním spojených je značný. Umístění MAM je vhodné, neboť PWUDs v SVL, kde působí, vykazují vyšší míru rizikového chování a zranitelnosti. Současně vykazují vyšší ochotu službu MAM využít. Nastavení a dopady MAM v Brně na zdravotní a sociální situaci mezi klienty a na komunitu je vhodné dále sledovat.
Background: Injecting drug use is associated with a high disease burden, particularly due to the high risk of blood-borne infections such as viral hepatitis C (HCV) and human immunodeficiency virus (HIV) infection. Interventions recommended for the prevention and control of infections associated with drug use include so-called drug consumption rooms (DCRs). A DCR is a professional addiction service where people who use drugs (PWUD) can consume the drug they bring under safer and hygienic conditions. In September 2023, the first DCR in the Czech Republic was opened in Brno as a mobile setting (MDCR) in a socially excluded location (SEL) with a high prevalence of heroin and other opioids injecting, especially in the local Roma community. Its launch involved mixed methods research aimed at needs analysis and service set-up. This paper presents an analysis of quantitative data focusing on risk behaviours and other characteristics of potential clients of the programme. Material and methods: Two cross-sectional questionnaire surveys (n=131 and n=135) were conducted just before and after the launch of the programme among PWUD recruited through convenience sampling by collaborating services and peer workers in Brno. The analysis addressed the characteristics of clients in the SEL where the MDCR operates. Pairwise associations between the SEL affiliation and other variables were analysed using the chi-square test and t-test. For statistically significant predictors from the pairwise analysis, logistic regression analysis was performed, with adjustment for gender and age. Results are presented as adjusted odds ratios (AOR). Results: In both surveys, two thirds were male, mean age around 37 years (36.5 years in survey 1 and 37.5 years in survey 2). Most of them were people who injected drugs with a relatively high rate of risk behaviours. In survey 2, more people self-reported Roma ethnicity (50.7% versus 20.3%) and opioids as their primary drug (39.7% versus 27.5%). Respondents from survey 1 living in the SEL were more likely to have used heroin (AOR=8.2) and opioids (4.7) in the past year, to have injected drugs in an indoor public space in the past 30 days (3.6), to have witnessed an overdose (2.5), and to have been taken by emergency services (2.6) in the past 12 months. SEL respondents were more likely to have been clients of addiction services in the past year (2.8), especially opioid agonist treatment (4.6), but on the other hand, showed significantly more barriers to treatment. The survey after the MDCR launch confirmed higher rates of opioid use (2.9) and lower rates of methamphetamine use (0.3) and showed a higher prevalence of ever-diagnosed HCV (3.0) among SEL respondents. Prior to the opening of the MDCR, SEL respondents were in general less aware of the DCR (0.4) but reported greater willingness to use it after its launch (2.7). Conclusion: The launch of the mobile DCR in Brno was justified due to the high prevalence of injecting drug use and the presence of associated risks, including injecting in public. The location of the DCR is appropriate as PWUDs in the SEL where it operates exhibit higher levels of risk behaviour and vulnerability. At the same time, they show a higher willingness to use the DCR. The set-up of DCR in Brno and its impacts on the health and social situation of clients and the community should be further monitored.
BACKGROUND AND PURPOSE: CNS embryonal tumor with pleomorphic adenoma gene-like 1 (PLAGL1)/pleomorphic adenoma gene-like 2 (PLAGL2) amplification (ET, PLAGL) is a newly identified, highly malignant pediatric tumor. Systematic MRI descriptions of ET, PLAGL are currently lacking. MATERIALS AND METHODS: MRI data from 19 treatment-naïve patients with confirmed ET, PLAGL were analyzed. Evaluation focused on anatomic involvement, tumor localization, MRI signal characteristics, DWI behavior, and the presence of necrosis and hemorrhage. Descriptive statistics (median, interquartile range, percentage) were assessed. RESULTS: Ten patients had PLAGL1 and nine had PLAGL2 amplifications. The solid components of the tumors were often multinodular with heterogeneous enhancement (mild to intermediate in 47% and intermediate to strong in 47% of cases). Nonsolid components included cysts in 47% and necrosis in 84% of the cases. The tumors showed heterogeneous T2WI hyper- and isointensity (74%), relatively little diffusion restriction (ADC values less than contralateral normal-appearing WM in 36% of cases with available DWI), and tendencies toward hemorrhage/calcification (42%). No reliable distinction was found between PLAGL1- and PLAGL2-amplified tumors or compared with other embryonal CNS tumors. CONCLUSIONS: The study contributes to understanding the imaging characteristics of ET, PLAGL. It underscores the need for collaboration in studying rare pediatric tumors and advocates the use of harmonized imaging protocols for better characterization.

BACKGROUND: Efficacy of tumour necrosis factor inhibitors (TNFi) for peripheral arthritis in patients with psoriatic arthritis (PsA) has been established in randomized clinical trials that have used improvement in summated joint counts as an outcome. Whether joints at different anatomical locations might respond differentially to TNFi remains unknown. The aim of the study was to investigate potential variations in the responsiveness to a first tumour necrosis factor inhibitor (TNFi) among joints at distinct locations in patients with psoriatic arthritis (PsA) treated in routine clinical care. METHODS: Bionaive PsA patients from nine European countries were included in this observational cohort study if ≥ 1 joint was swollen at the initiation of a first TNFi as monotherapy or added to methotrexate. Only the 28-joint count was available without imaging data confirming the presence of synovitis. The primary outcome was time to first resolution of joint swelling at each joint level. Hazard ratios (HR) for resolution comparing different joint locations were estimated using interval-censored mixed-effects Cox proportional hazards models, including a random effect for country and patient, adjusted for age and sex. RESULTS: A total of 1729 patients with 8397 swollen joints at the start of TNFi were included. Considering the upper extremity, a higher rate of resolution of joint swelling (HR, 95% CI) was observed for the shoulder (1.65, 1.16-2.35) and elbow (1.90, 1.38-2.61), while a lower rate was found for the wrist (0.72, 0.62-0.83) compared to the joints of digit 3. Within fingers, and using the same reference, joint swelling resolved fastest in digit 4 (1.77, 1.49-2.11) and digit 5 (1.88, 1.53-2.31). A lower rate of resolution of joint swelling was found for the knee in comparison to the elbow, the corresponding joint on the upper limb (0.56, 0.40-0.78). CONCLUSION: The time to resolution of joint swelling upon treatment with TNFi in patients with PsA seems to depend on the localisation of the affected joints.

The visual evaluation of data derived from screening and optimization experiments in the development of new analytical methods poses a considerable time investment and introduces the risk of subjectivity. This study presents a novel approach to processing such data, based on factor analysis of mixed data and hierarchical clustering - multivariate techniques implemented in the R programming language. The methodology is demonstrated in the early-stage screening and optimization of the chromatographic separation of 15 structurally diverse drugs that affect the central nervous system, using a custom R Language script. The presented explorative approach enabled the identification of key parameters affecting the separation and significantly reduced the time required to evaluate the comprehensive dataset from the screening experiments. Based on the data analysis results, the optimal combination of stationary phase and mobile phase composition was selected, considering retention, overall resolution, and peak shape of compounds. Additionally, compounds vulnerable to changes in selected chromatographic conditions were identified. As a complement to the presented R Language script, a web-based application ChromaFAMDeX has been developed to offer an intuitive interface that enhances the accessibility of the used statistical methods. Accompanying the publication, the R script and the link to the standalone application are provided, enabling replication and adaptation of the methodology.
BACKGROUND AND OBJECTIVE: The role of genetic variants in response to systemic therapy in muscle-invasive bladder cancer (MIBC) is still elusive. We assessed variations in genes involved in DNA damage repair (DDR) before and after cisplatin-based neoadjuvant chemotherapy (NAC) and correlation of alteration patterns with DNA damage and response to therapy. METHODS: Matched tissue from 46 patients with MIBC was investigated via Ion Torrent-based next-generation sequencing using a self-designed panel of 30 DDR genes. Phosphorylation of γ-histone 2A.X (H2AX) was analyzed via immunohistochemistry to evaluate DNA damage. Genetic variants were analyzed along with clinical data and quantitative phospho-H2AX data using the Kaplan-Meier method, Cox regression analysis, and factor analysis of mixed data. Central and Eastern European (CEE) countries have recently implemented reforms to health care provider payment systems, which include changing payment methods and related systems such as contracting, management information systems, and accountability mechanisms. This study examines factors influencing provider payment reforms implemented since 2010 in Bulgaria, Croatia, Czechia, Estonia, Latvia, Lithuania, Hungary, Poland, and Romania. A four-stage mixed methods approach was used: developing a theoretical framework and data collection form using existing literature, mapping payment reforms, consulting with national health policy experts, and conducting a comparative analysis. Qualitative analysis included inductive thematic analysis and deductive approaches based on an existing health policy model, distinguishing context, content, process, and actors. We analyzed 27 payment reforms that focus mainly on hospitals and primary health care. We identified 14 major factor themes influencing those reforms. These factors primarily related to the policy process (pilot study, coordination of implementation systems, availability of funds, IT systems, training for providers, reform management) and content (availability of performance indicators, use of clinical guidelines, favorability of the payment system for providers, tariff valuation). Two factors concerned the reform context (political willingness or support, regulatory framework, and bureaucracy) and two were in the actors' dimension (engagement of stakeholders, capacity of stakeholders). This study highlights that the content and manner of implementation (process) of a reform are crucial. Stakeholder involvement and their capacities could influence every dimension of the reform cycle. The nine countries analyzed share similarities in barriers and facilitators, suggesting the potential for cross-country learning.

BACKGROUND: Constitutional mismatch repair deficiency syndrome (CMMRD) is a rare childhood cancer predisposition syndrome associated with a broad spectrum of malignancies, including non-Hodgkin lymphomas (NHL). Most patients die due to cancer before the age of 20 years. Limited data exist on CMMRD-associated lymphomas and their outcome. METHODS: We conducted a retrospective study including all CMMRD-associated NHL patients registered before 2020 in the European and North American databases or reported by members of the European Intergroup for Childhood Non-Hodgkin Lymphoma (EICNHL). Events considered to define event-free survival included relapse/progression, second malignancy (SML), or death, whichever occurred first. FINDINGS: The analysis included 74 patients, with 20 having multiple metachronous NHL. The median age at diagnosis was 9.4 years. Previous malignancies were reported in 36% of the patients, café au lait spots in 96%, and consanguinity in 54%. The initial lymphoma subtypes were 53 T-cell lymphoblastic lymphomas (T-LBL), four B-lymphoblastic lymphomas, and 17 mature B-cell non-Hodgkin lymphoma (B-NHL). All patients were treated with curative intent, with current chemotherapy regimens adapted to their subtype. The median follow-up was 8.7 years. After the first lymphoma, the 5-year event-free and overall survival rates were, respectively, 23.5% [95% confidence interval (CI): 14.9-35.1] and 61.5% [95% CI: 49.6-72.1]. The 5-year cumulative risk of progression/relapse, SML or death as a first event was 20.8%, 52.9%, and 2.7%. INTERPRETATION: Standard treatments for sporadic NHL are effective in most CMMRD-associated NHL cases, but multiple malignancies, including lymphomas, impair prognosis. Future strategies should evaluate the potential of less genotoxic therapies, including immunotherapy, in preventing SMLs while maintaining effective control of NHL.

BACKGROUND: Face transplantation has emerged as a viable solution for reconstructing the most complex facial injuries. Previous work has demonstrated that surgical revisions are necessary to optimize outcomes. The authors' group has updated the previous report of revisions in their cohort, quantified and described which revisions were performed for functional, aesthetic, or mixed indications, and described the rationale, safety, and long-term outcomes of these revisions. METHODS: A retrospective analysis of the authors' 10 face transplants from April of 2009 to February of 2023 was performed. The patients' medical records, preoperative facial defects, and operative reports (index and secondary revisions) were reviewed. RESULTS: Nine patients were included. One patient underwent irreversible acute-on-chronic allograft rejection and received a second face transplant. The average number of revisions was 5.2 per patient (range, 2 to 11 procedures). The median time interval from transplantation to first revision was 4 months (range, 1 to 21 months). Median follow-up was 106 months (range, 39 to 142 months). Most interventions consisted of debulking the allograft or revising the periorbital tissues. CONCLUSIONS: In the current study, the authors report longer-term data on revision procedures needed in face transplant recipients. Patients should expect to undergo revisions for both functional and aesthetic considerations. Although the majority of revisions are performed within 2 years after transplantation, revisions can be safely performed at any time point. Shared decision-making between the patient and provider team is essential in deciding which revisions are performed and when. CLINICAL QUESTION/LEVEL OF EVIDENCE: Therapeutic, V.
